The goal is not perfection by the first milestone, but real-world minimum viable functionality that can iteratively improve.Alix notes that from the standards community perspective, this approach feels aligned with long-standing calls for industry-driven collaboration, though it remains early to measure widespread impact.Carrots, Sticks and Rural HealthThe discussion turns to incentives.Amy outlines the administration's carrots and sticks strategy:Stick: Enforcement of information blocking, with penalties up to $2 million per occurrenceCarrots: Financial incentives such as the $50 billion Rural Health Transformation Program and the CMS ACCESS Model, which pays for technology-enabled outcomesThe Rural Health Transformation Program directs money to states with expectations that ecosystem-aligned interoperability and app participation be incorporated into funding proposals. CMS retains oversight and clawback authority to ensure funds support rural providers.The ACCESS Model represents a significant shift. Standards participation, implementation and technology upgrades require resources that are often scarce. The success of these incentives will depend on whether they reduce burden rather than add to it.AI: Evolution, Risk and RealityAI becomes a central thread of the episode.Amy compares AI adoption to autonomous vehicle models. Some scenarios allow tightly controlled automation, such as medication refills, while others require a human in the loop for higher-risk decisions. A 75 year-old man with severe aortic stenosis is deemed unsuitable for surgery on the basis of a porcelain aorta detected with cross-sectional imaging. The patient had, a decade earlier, been diagnosed with hypertrophic cardiomyopathy after presenting with cardiac arrhythmia. The final recorded phone call between Keith Caneiro and his older brother Paul was played for jurors this week—and it captures a man demanding answers hours before his murder. "Give me the f***ing login, Paul!" Keith is heard saying, giving his brother a deadline of 8 p.m. to provide access to a trust account. By the next afternoon, Keith, his wife Jennifer, and their two children—11-year-old Jesse and 8-year-old Sophia—were dead. Prosecutors say Keith had discovered Paul was stealing from their shared businesses and was cutting him off from a $225,000 salary. A neighbor put a timestamp on the killing. Dennis Corpora testified he woke up around 3:20 AM on November 20th, 2018. "I heard the shots, and I said, 'Someone just got whacked.'" He told jurors it was a pistol. Prosecutors say Paul shot Keith four times in the head and once in the back outside the Colts Neck mansion, then went inside and stabbed Jennifer and the children.A detective testified that Sophia had a stab wound to her left eye. Trust account expert Lazaro Cardenas delivered testimony that changes everything: Keith's $3 million life insurance policy was structured so Paul would only benefit if all four family members were dead. According to prosecutors, this is why the children weren't spared. They were allegedly necessary casualties. Hours before the mansion fire, Paul allegedly set fire to his own Ocean Township home while his wife and daughters slept inside. The defense points to a third brother, Corey, claiming police never investigated him. But the murder weapon, silencer, night-vision scope, bloody jeans with the children's DNA, and a go-bag with a passport were all found at Paul's house. In 2027, NASA's is planning to land astronauts on the moon for the first time in 53 years with the expectation that there will be a permanent base there by the early 2030s. And the ever-humble Elon Musk reckons he'll be sending people to Mars by then too. This has prompted a renewed interest in the prolonged effects of space travel on the human body, and a lot of fascinating research has been conducted aboard the International Space Station over the last two decades. The main objective risk to astronauts is exposure to galactic cosmic radiation. This can be reduced to some degree by shielding of space habitats, however, the impacts of microgravity are much harder to engineer away. In part 1 we'll discuss spaceflight associated neuro-ocular syndrome and other sensorimotor impacts. In part 2 we'll focus on the effects of microgravity on bone density and the circulatory system. We'll also talk through the management of a cases of suspected thrombosis on the international space station from a few years ago. The record for the longest space-flight is held by physician-cosmonaut Valeri Polyakov. Back in 1994-95, he spent 437 days on the Mir space station and grew 4 centimetres in height through elongation of his spine in micro-gravity. Polyakov had an uncomfortable ride back to Earth in the very precisely customised descent module. Microgravity also causes demineralization of weight-bearing bones that is faster than age-related decline. But the cosmonaut had worked out religiously for the entire mission and after his capsule parachuted to the ground he made a point of walking from it relatively unassisted. One of the main objectives of the marathon flight had been to prove that walking proudly onto the Martian surface after a 9-month journey might be possible, given it only has 37 percent the gravitational force that Earth does. Microgravity additionally results in adaptive plasticity of the vestibular and sensorimotor networks and deconditioning of the cardiovascular system. Indeed, several years ago there was a medical emergency aboard the international space station when an ultrasound investigation revealed thrombosis of the internal jugular vein in one astronaut. In this podcast we discuss how management of cases like this has many parallels with remote medicine on earth. Prosecutors say greed drove Paul to slaughter his brother's family after Keith discovered he was stealing from their shared businesses. Hours before Keith Caneiro and his entire family were killed, he made two phone calls to his brother Paul demanding answers about missing money. The jury heard those calls this week. Keith's voice is furious: "I need to know where that money went." And then: "Give me the f---ing login, Paul!" He gave Paul a deadline — two hours. Paul never responded. By morning, prosecutors say, Keith was dead on his front lawn with five gunshot wounds. His wife was dead inside, shot and stabbed. His two children — ages 8 and 11 — had been stabbed repeatedly and left to die of smoke inhalation as the house burned around them.The Paul Caneiro trial is now in its fourth day in Monmouth County, and the picture emerging is devastating. Prosecutors have presented evidence of a man allegedly living a fantasy — three Porsches, Tiffany's shopping sprees, an Audi for a mistress — while overdrafting his bank accounts and draining his brother's life insurance trust. Detective testimony revealed that bank statements Paul provided to the family accountant had been altered to hide the theft.Witnesses have described the scene at Paul's own house fire that morning: a gas can in the driveway, burn marks on his Porsche, the family sitting calmly in a car while firefighters worked. When police asked Paul where his security DVR was, testimony says he didn't answer — he looked to his daughter. That footage, showing Paul disconnecting his cameras at 1:28 AM, is now in evidence after the New Jersey Supreme Court ruled it admissible.Paul Caneiro maintains his innocence. His defense points to a third brother.
