There is evidence that six months or more off the job leads to some loss of practical skills and knowledge and certainly, many doctors a loss of self-confidence. People take time out from medical practice for many different reasons but career breaks to raise children are more common than ever before. Paediatrics is one specialty where female representation exceeds 70 percent and it is also becoming more common for new fathers to take leave as primary carers. Senior staff at Sydney Children's Hospital have developed a day-long workshop to help medics brush off the cobwebs before they return to practice. It involves rehearsal of specific skills, refreshers on calculation and interpretation tasks and a high-fidelity critical care simulation. Just as importantly, there is open discussion and mentorship to support the transition back to work. DOD aims to rein in spending on IT services contracts; GSA tech arm faces more workforce cuts

Secretary of Defense Pete Hegseth signed a memo Thursday ordering the termination of several IT services contracts and directing the Pentagon's chief information officer to draw up plans for in-sourcing, among other measures. The aim is to “cut wasteful spending” and “support the continued rationalization” of the Defense Department's IT enterprise, Hegseth wrote. The move comes amid a broader push by the Trump administration to implement Department of Government Efficiency (DOGE) initiatives across federal agencies. Hegseth's new memo to senior Pentagon leadership ordered the termination of contracts affecting a variety of DOD components, including a Defense Health Agency contract for consulting services; an Air Force contract to re-sell third party enterprise cloud IT services; a Navy contract for business process consulting services; and a Defense Advanced Research Projects Agency (DARPA) contract for IT helpdesk services. In a video released on social media touting these DOGE-related efforts, Hegseth estimated that those contract terminations would save the Pentagon approximately $1.8 billion, $1.4 billion, $500 million and $500 million, respectively. Another round of General Services Administration workforce cuts is hitting Technology Transformation Services, specifically within its Integrated Award Environment (IAE), Solutions, and Office of Regulatory and Oversight Systems (OROS) programs, sources confirmed to FedScoop. Under TTS, the Solutions platforms and services, front office, public experience and accelerators teams were all affected by the reductions, according to a source with knowledge of the situation. However, programs that are safe from the current — and widespread — reductions in force include FedRAMP, Login.gov and Cloud.gov, sources said. In Aotearoa-New Zealand, the proportion of doctors identifying as Māori has doubled from where it was a decade ago to over 5 percent. But there is still a long way to go before the workforce is representative of the broader population which is 17 percent Māori.The Auckland and Otago Medical Schools have in recent years turbocharged their intake of Māori and Pasifika students but these graduates don't seem to have trickled through to the RACP in great numbers. Just 3.5 percent of general physicians and 4.8 percent of paediatricians identify as Māori, and Pasifika doctors make up a further 1 and 2 percent respectively.In this podcast, three Māori medics discuss how the culture of training environments can be made more welcoming to junior doctors with diverse ethnic backgrounds. Wer sich vor 2025 freiwillig für die elektronische Patientenakte (ePA) entschieden hat, war Teil einer digitalen Minderheit. "Das haben tatsächlich nur sehr wenige gesetzlich Versicherte gemacht", berichtet Andreas Straußfeld. Gerade einmal ein Prozent der Versicherten ließ sich ihm zufolge proaktiv freischalten.Straußfeld ist CEO von Bitmarck. Das Unternehmen ist neben IBM einer der beiden technologischen Anbieter für die elektronische Patientenakte. Diese wird wiederum seit Januar 2025 für alle gesetzlich Versicherten eingeführt. Das passiert in Form des Opt-out-Modells. Wer nicht widerspricht, bekommt automatisch eine. Bitmarck arbeitet für die elektronische Patientenakte ePA mit der österreichischen Firma Rise zusammen. Gemeinsam beliefern sie rund 80 gesetzliche Krankenkassen und 14 private Versicherer. Technisch basiert alles auf einem Standard. Unterschiede sieht man höchstens im Logo im App-Store. Doch obwohl die Datenbasis täglich wächst, läuft nicht alles glatt. Der erste Zugang zur ePA-App ist für viele Nutzerinnen und Nutzer ein echter Hürdenlauf. "Das ist ein sehr komplizierter Prozess, mit Postident und was man dann alles tun muss", gibt Strausfeld zu. "Ich bin auch am Anmeldeprozess gescheitert, mit meiner Krankenkasse." Besser werde es erst nach dem ersten Login, sagt der Bitmarck-CEO.Dabei wurde das System von Grund auf neu entwickelt – mit Cloudtechnologie, standardisierten Schnittstellen und Zertifizierung durch die Gematik, der Gesellschaft für Telematikanwendungen der Gesundheitskarte, die im deutschen Gesundheitswesen für die technologische Infrastruktur verantwortlich ist.Bei sensiblen Gesundheitsdaten ist Sicherheit das A und O. "Die Daten liegen auf Bitmarck-eigenen Systemen in eigenen Rechenzentren in Deutschland", sagt Strausfeld. In den Pilotregionen funktioniere das System bereits überraschend gut. "Täglich wandern 1600 bis 2000 Dokumente in die Akten, wir sehen 60.000 Logins pro Tag." Trotzdem sind bisher nicht alle Praxen bereit, der bundesweite Rollout wurde deshalb auf April verschoben.Strausfeld sieht in der elektronischen Patientenakte auf mittlere Sicht das Fundament einer künftigen digitalen Gesundheitsversorgung. Ziel sei nicht nur mehr Transparenz: "Je größer die Datenmengen, desto klarer die Erkenntnisse - für die Gesamtheit, aber auch für den einzelnen Versicherten." This case report comes to you from Brigham and Women's Hospital in Boston, a huge teaching hospital that serves the Harvad Medical School. The 52-year-old female presented with clumsiness and paresthesia of the right hand that had persisted for several days. She also had a headache and three weeks prior to presentation had undergone a suboccipital craniotomy for a Chiari I malformation. To complicate things, there was a past medical history of migraines and a family history of a Factor V Leiden mutation. All non-critical and non-statutorily required work will cease at the General Services Administration's Technology Transformation Services as part of a 50% reduction of the office, according to Director Thomas Shedd. In his prepared remarks for a Thursday afternoon town hall, which were obtained by FedScoop, Shedd said that to deliver technology at GSA in a “more focused and streamlined way,” moving forward TTS will support only work that is required by statute and policy, fits into the Trump administration's definition of critical, and is prioritized by the leadership at GSA “in accordance with the priorities of the administration.” Everything else will be eliminated, per Shedd, who said in his remarks that TTS will be smaller in size – at least 50% smaller. Additionally, any contracts that support the work that falls outside of the established bounds “will be terminated” and any job functions that are deemed non-essential will be cut. The prioritized and remaining TTS programs include Login.gov, FedRAMP, Cloud.gov, statutorily required websites, the Integrated Award Environment, the Office of Regulatory Oversight, the Centers of Excellence, the Presidential Innovation Fellowship Program, the U.S. Digital Corps, operations and other “special projects.” Australian-based software company Atlassian has tapped Matthew Graviss to be its first public sector chief technology officer following his recent departure as the State Department's top data and AI official. Although the role starts a new private sector chapter in Graviss's career, being the first person to establish a newly created position is familiar ground. During his time in the federal government, Graviss was the first-ever chief data officer at both the State Department and the Department of Homeland Security's U.S. Citizenship and Immigration Services. In an interview with FedScoop, Graviss said his role at Atlassian is an extension of that experience in that he'll again be codifying the responsibilities of the job, showing value and solving customer problems. Professor Gary Lee established the first dedicated pleural service in the southern hemisphere in 2009, at the Sir Charles Gairdner Hospital in Perth. He says that pleural disease has finally come to be regarded as an area of subspeciality interest in its own right, not just a complication of other comorbidities. In this podcast he presents a potted history of key developments in the management of pleural effusion in particular. This is diagnosed in about 60,000 people every year in Australia, mainly as a result of infection or malignancy. With mentors in the UK, Professor Lee conducted some of the earliest trials on fibrinolytics and DNAses to break down purulent effusions. They also put to the test protocols for pleurodesis via talcum insufflation that date back to the 1930s. Professor Lee's more recent clinical research has focused on the use of indwelling pleural catheters that a patient can use to drain pleural effusate when feeling breathless. He has also a made an important contribution to conservative management guidelines for primary spontaneous pneumothorax. The genomic understanding of cancer has transformed a tissue-based classification model that had been dominant for 150 years or more. The last three decades have seen highly targeted therapies developed at blistering pace, and unprecedented improvements in patient outcomes. To date, these advances have been focused on more common cancers. The financing model for drug development means that rare cancers get overlooked, given the small pool of potential buyers relative to the costs and risks of investment. However, the molecular targets characterised in more common cancers are often found in cancers of a different histotype. As such, precision therapies will sometimes have tissue-agnostic efficacy and offer a lifeline for patients with neglected diseases or cancers of unknown origin. Professor David Thomas has founded an NGO called Omico to enable such patients to undergo profiling for hundreds of potential molecular targets. After serving as the head of the Technology Transformation Services at GSA for the past two years, Ann Lewis announced recently she is stepping down from the role. Lewis led the TTS organization during a time of major AI adoption across government, workforce transformation and a greater focus on spreading use of the agency's Login.gov single sign on platform. In a final interview with FedScoop, Lewis discusses those priorities for TTS going forward, as well as her journey into government, now that she's stepping away. The new director of the Defense Department's All-Domain Anomaly Resolution Office (AARO) is scheduled to meet with lawmakers in closed-door and open sessions Tuesday to discuss his organization's activities investigating “unidentified anomalous phenomena” that have raised national security concerns. The hearing with the Senate Armed Services Subcommittee on Emerging Threats and Capabilities comes on the heels of the release of the Pentagon's fiscal 2024 consolidated annual report on UAP — the modern term for UFOs and mysterious Restrictive software licensing practices are impacting multiple federal agencies in the form of cost increases and limited choices of cloud service providers (CSP) or cloud architecture, according to the Government Accountability Office. The Department of Justice, NASA, Department of Transportation and Department of Veterans all told the GAO that as result of restrictive practices in software licensing, the agencies either experienced or expected cost increases in acquisition, infrastructure and licensing.
