These two issues are in addition to the potential crisis of a physician anesthesiologist shortage in the coming years. Ampisilin-sulbaktam enjeksiyonluk steril su veya uyumlu olduğu bir solüsyonla sulandırılmalıdır. Oluşabilecek köpüklerin kaybolması ve tam çözündüğünün gözle tetkiki için bir süre bırakılmalıdır. In Season 2, we dive into the future of the specialty of anesthesiology and the looming physician shortage that could threaten the field as we know it. We start by looking into a recent report from the Association of American Medical Colleges (AAMC) and its projections of health care–wide physician shortages expected by 2033. In fact, the AAMC report projects that the United States will experience a shortage of 139,000 physicians, and that anesthesiology—along with specialties like pathology and emergency medicine—will have a shortage between 17,000 and 42,000 physicians. Kullanıma hazır çözelti 10 mL %0,9 sodyum klorür çözeltisinin kuru toz içeren flakona enjeksiyonu ile hazırlanır. Bu çözelti doğrudan uygulanabilir veya 100 mL %0,9 sodyum klorür enjeksiyonu veya %5 glukoz enjeksiyonu ile karıştırıldıktan sonra da uygulanabilir. Cam veya plastik kaplar dilüsyon için kullanılabilir. PANTO®, burada belirtilen çözeltiler dışındaki çözeltiler ile karıştırılmamalıdır. Damar içine uygulama 2-15 dakikada yapılmalıdır. İntravenöz kullanım oral yolla uygulama mümkün olmadığı durumda önerilir. İntravenöz kullanımda 7 güne kadar veriler mevcuttur. (DİLTİZEM-L) intravenöz (İ.V.) kullanım içindir. %5 dekstroz ve Serum Fizyolojik çözeltileri içinde infüzyon şeklinde de uygulanabilir. Enjeksiyonluk su veya uygun diğer çözücüler ile kullanıma hazır hale getirildikten sonra bolus veya sürekli infüzyon olarak damar içine uygulanır​1​. Direkt İntravenöz Tekli Enjeksiyonlar (Bolus) Diltiazem hidroklorür enjeksiyonunun başlangıç dozu, 2 dakika boyunca uygulanan bir bolus olarak 0.25 mg/kg gerçek vücut ağırlığı olmalıdır (20 mg, ortalama hasta için makul bir dozdur). Sürekli İntravenöz İnfüzyon Diltiazem hidroklorürün önerilen başlangıç infüzyon hızı 10 mg/saattir. Bazı hastalar 5 mg/saat'lik bir başlangıç hızına yanıtı sürdürebilir. Kalp atış hızında daha fazla azalma gerekirse, infüzyon hızı 5 mg/saatlik artışlarla 15 mg / saate kadar artırılabilir. İnfüzyon 24 saate kadar muhafaza edilebilir. Diltiazem, doza bağlı, doğrusal olmayan farmakokinetik gösterir. 24 saatten uzun infüzyon süresi ve 15 mg/saatten daha yüksek infüzyon hızları çalışılmamıştır. Join the EMGuideWire Crew as they continue to explore the High Yield management points for Sepsis!  This week's Episode's Pearls: Early fluids save lives. Give 40cc/kg bolus in first 3 hours. But don’t fluid overload the patient! U/S the heart and lungs: A plethoric IVC, immobile mitral valve, and B lines on the lungs should urge you to be more cautious with fluids. Goal in all patients is to establish an adequate MAP ASAP! Fluids + Vasopressors! Vasopressor titration algorithm: First low dose NE (10mcg/min); if still in shock, initiate vasopressin (0.04 units/min); do not wait on providing vasopressin if EPI is readily available (establish MAP ASAP!).  Initiate vasopressors early with fluids! NE can be initiated peripherally, so don’t wait for a central line! Only consider dopamine for absolute bradycardia.  Methylene blue is a last resort consideration.  Vasopressors are commonly needed at high doses (i.e., 1mcg/kg/min EPI).  Hydrocortisone 50-100mg for patients with septic SHOCK, not sepsis alone.  Summarized by: Travis Barlock, MD PGY-1 References: Farkas J. They opened fire into crowds of innocent people, burned homes and businesses to the ground, and forced countless others to flee.
