In this conversation, we will examine the complex and challenging environment of toxic drug and substance misuse on the streets of Vancouver and across British Columbia. We will explore the issues of concomitant drug ingestion, the use of fentanyl, the complications of mental health and the cyclical pattern of pre-hospital presentation. We will also explore the challenges of bias towards this patient group and how that can both affect care and outcomes.    Jen Bolster is an Advanced Care Paramedic in British Columbia. She is also a Paramedic Practice Leader (PPL) within BCEHS. She has a portfolio which examines and engages with paramedic-led research into mental health, substance use and the toxic drug crisis. She advocates that paramedics are uniquely positioned to offer patients alternatives to the emergency department. Jen's research interests include paramedic mental health and resilience, contemporary vs non contemporary leadership styles, women studies, and non-technical skills.

In this episode, we sit down with Jacob Simonini, Osteopath and Integration 2023 Keynote Speaker. Jacob is incredibly passionate about osteopathy and shares about his 4:30 am wake-ups and what he does in his routine to be a better person for his family and his clients.   ABOUT JACOB Jacob is an Osteopath whose career began as an Advanced Care Paramedic with many other focuses in the medical field. Jacob was a clinical instructor in the Advanced Care Paramedic program, a preceptor for Advanced Care Paramedic students, and studied and researched alongside a team of physicians involved in prehospital trauma and cardiac arrest examination. Looking for a new challenge and a more in-depth understanding of human bodies, Jacob began his osteopathic career at the Canadian Academy of Osteopathy (CAO). Later, Jacob received a Master of Osteopathic Manual Science (MOMSc) and pursued a professional position as a member of the Ontario Osteopathic Association (OOA). Jacob betters his patients to the utmost ability using osteopathic skills and reasoning. The central idea Osteopathy teaches and what Jacob tries to preach and apply during treatments is that the practice seeks to free the natural healing capabilities of the body. Moreover, each treatment Jacob gives is tailored to every individual and the condition of their body. In this episode of Better Thinking, Nesh Nikolic speaks with Richard Hill and Matthew Dahlitz about their new book, The Practitioner's Guide to the Science of Psychotherapy. Richard Hill, MA, MEd, MBMSc, DPC, has emerged from a diverse and fascinating journey to become an innovative speaker on the mind, brain and the human condition. From an early career in the creative arts, Richard returned to intellectual studies at 42 (1996) beginning with a Bachelor of Arts majoring in linguistics. This triggered a curiosity that led to a diploma in counselling and a new career in psychotherapy. Studying continued and he has added three Masters degrees – an MA in Social Ecology; an MEd in Social Ecology; and a Masters in Brain and Mind Sciences (MBMSc) from Sydney University. Richard is also fortunate to be mentored by the esteemed Ernest Rossi PhD who has invited Richard into the International Psychosocial Genomics Research Team to study the impact of therapeutic practice on the genetic level. Matthew Dahlitz is both university trained and an autodidactic, whose knowledge spans across the arts, technology, psychology, neuroscience, emergency medicine, and business. Studied psychology at the University of Queensland and gained a Master of Counselling postgraduate degree – now a specialist in neuropsychotherapy and the Editor-in-Chief of The Neuropsychotherapist, has taught post-graduate courses in neuropsychotherapy, and is author of the book The Psychotherapists's Essential Guide to the Brain. Matthew, like Richard, has a musical bent, having studied at the Queensland Conservatorium of Music in both music technology and master of composition – having scored for film and produced a number of albums of original music he has also been a producer in a film production company. Matthew has also been an Advanced Care Paramedic for a decade, managed commercial property and property development projects in the tens of millions of dollars, is the Editor-in-Chief of the technology magazine Age of Robots, covering the technology and the social and psychological impacts of this second machine age, and runs a media and publishing company with an emphasis on cutting edge science and technology. Guest 1: Nicholas Hennink Nicholas Hennink ('Henn-ink') is an Advanced Care Paramedic and Singer/Songwriter by both passion and vocation, known for his inspirational music that gives hope to those who need it the most. Nicholas also served as an advanced care paramedic for over a decade. His love of music began during his childhood, playing in bands and creating songs with his acoustic guitar in Cape Town, South Africa, where he was born and raised. Serving as an Advanced Care Paramedic Nicholas was diagnosed with severe PTSD, seeing things that can't be unseen, he used alcohol as a coping mechanism and was afflicted with alcohol addiction. Stemming over 6 years of relapses, rehab three times, and watching his life collapse around him, suicide was not far off. Keeping his daughter and fiancé front and center in his mind pushed him to heal, and use his gifts to help other people that still struggle. Born in Ottawa, raised an anglophone in Montreal, I have been out on Vancouver Island for 34 of my 60 years on the planet. I'm a father, a stepfather, a partner, a businessman and a man in long term recovery from substance addiction. I'm over 31 years clean today. I have solid Newfoundland and Australian ancestry, so I'm pretty complicated. Stubborn, silly, patriotic and non-judgmental most of the time. I can be pretty skeptical and sarcastic other times. I am a fledgling golfer, guitarist and bass player. I'm also a history buff, a voracious reader, and a pretty good cook. I am a political buff, love cats, omelets and the Boston Bruins. Pete Tripp is an Advanced Care Paramedic with experience in many fields of paramedicine including as a flight medic.  Pete's journey is a powerful story from his start as a Janitor at his local emergency hospital to his spectrum of expereince he built as a paramedic.  He is open about his struggles with childhood trauma and finding his way as a troubled youth.  Through building up personal resiliency and never giving up, Pete was able to become an excellent paramedic and carry on through an incredible career.  He uses what he has learned over these years of experience to shed light on some of the dark sides of mental health stigma in emergency services, along with ways we can improve the atmosphere to make it safer to address these issues in the workplace.  If every first responder knew how to cry, knew how to be mindful, and knew how to connect with compassion things would become much easier over time.    Pete now has been focusing on helping first responders recognize signs of post-traumatic stress and acheiving post-traumatic growth.  He has been involved in his own initiatives like Navigate Change, an initiative to raise awareness and support for first responders struggling with PTSD.  He feels the work he was able to do with Project Trauma Support was a major factor in his resiliency and post-traumatic growth.  It was a pleasure hearing his story and his valuable perspectives on the state of emergency services and mental health stigma.  Together, we discuss our own experiences with mental health struggles as paramedics and where we hope to see changes to improve the longevity of this wonderful career.  We will be working together on initiatives to train new and current first responders in addictions/mental health awareness, with tools and knowledge to prepare them for a career that inevitably exposes you to major psycological trauma. Episode 14 features Derek Davis, an Advanced Care Paramedic who is hitting 15 years of experience in EMS. He takes us through several topics that all connect to mental & physical health and wellness. * Derek elaborates on the struggles he has faced as a paramedic and his journey to overcoming them; which many listeners will empathize with and learn a lot about themselves from.  We shed more light on how common mental health problems are among first responders and why we tend to hide them. More paramedics die by suicide than in any other profession in Canada and very few effective supports are available. Workplaces still discriminate against workers who are struggling from a condition that we are all statistically expected to have. * He has now been a major advocate of mental health care and education, self-care, and increased peer-support. It is important to be aware of our emotions and identify when we are feeling fear, confusion, hopelessness, or sadness from our experiences before they come out at anger and pain. We go into the signs and symptoms of Post-Traumatic Stress that can range on a complex spectrum.
