Steve Irsfeld is the owner of Irsfeld Pharmacy PC in Dickinson. Steve graduated from NDSU College of Pharmacy in 1988. After working in Fargo as an intern and staff pharmacist he returned to Dickinson to work alongside his dad and has been there for the last 28 years. Pharmacy is a family affair for Steve; NDSU School of Pharmacy has graduated his father, Jim, his daughter, Molly , and 3 uncles (one misguided cousin graduated from some other school)Irsfeld Pharmacy has been a Professional Compounding Centers of America (PCCA) member since 1998 and has incorporated nutrition into the practice in 2005. at a time when we began receiving more questions on how supplements interact with prescription medications. The pharmacy is a hybrid pharmacy with a focus on compounded medication including hormone replacement, pain management, and low dose naltrexone. The pharmacy goal is to help as may patients as possible. By having tools such as compounding and nutrition as well as having relationships with alternative medicine practitioners, they are able to help our patients seek wellness with non-traditional methods. “It has been a joy to hear patients come in and tell us that we have greatly improved their quality of life. That is the big reason I come to work each and every day”, Steve said.Steve was appointed to the ND Board of Pharmacy in April of 2014 and is a current member. He has been an active member of District 5, a local pharmacy organization, having held several offices and serving as the District 5 representative on the ND Pharmacists Association Board. He also served as the chair of the Associations Government Affairs Committee. He was a member of the Heartland Hospice IDG team for the last 24 years. He is a member of North Dakota Pharmacists Association, International Association of Compounding Pharmacists and National Community Pharmacists Association. Steve was the recipient of the Marion-Merrill Dow Distinguished Young Pharmacist Award for 1993 and the Dupont Innovative Pharmacy Practice Award in 2001. Steve is co-owner of Healthy Profits LLC, a business that helps pharmacies grow their supplement sales. Steve routinely speaks publicly and to professionals about the merits of compounding and nutrition. He has writen a weekly article on a health-related topic in his local newspaper since 2016 which is also published in several small papers in the surrounding communities. His most recent endeavor is the Irsfeld Pharmacy Optimal You Podcast where he visits with area providers and health influencers.Steve enjoys spending time with his 3 daughters, their families, including his 2 granddaughters, and his wife of 35 years, Carolyn. According to the American Association of Clinical Endocrinologists, an estimated 27 million Americans have thyroid disease, and more than half of those individuals are undiagnosed and unaware that they have a thyroid condition.However, it's important to understand that if more comprehensive thyroid testing was used by doctors and functional reference ranges were used to interpret that testing instead of the overly lax lab reference ranges, a much larger number of people would be classified as having some type of thyroid dysfunction.Most doctors rely almost exclusively on the thyroid stimulating hormone or TSH test to detect thyroid dysfunction, yet TSH is not even technically a marker of thyroid function, it's actually a marker of pituitary function. Even worse, the medical community can't even agree on what the normal reference range is for the TSH test.My other issue with conventional medicine is its refusal to actually look at underlying causes of thyroid dysfunction including but not limited to gut inflammation, chronic stress, adrenal issues, nutrient deficiencies, toxins in our food, air, and water, and chronic infections such as periodontal disease and h. Pylori.The thyroid's primary function is to control the body's metabolism – the rate at which cells perform duties essential to living. It manages how we turn our food into energy by facilitating the processes of energy production in the mitochondria. I want you to think of your thyroid as your body's “engine,” which sets the pace at which the body operates. Too little thyroid hormone can cause the body's systems to slow down, and too much thyroid hormone can cause the body's systems to speed up.In today's episode, I've brought back my friend and pharmacist Jim Paoletti to talk about this amazing gland that does so much for us. In fact, there's not a single cell in the body that doesn't depend on thyroid hormone in some way.Jim Paoletti, B.S. I have known Rob Kress RPh, since 2014 and he was the first pharmacist who was talking about functional pharmacy and the opportunity for pharmacists in functional medicine. Rob, graduated from Temple University School of Pharmacy in Philadelphia PA in 1994. After becoming disenchanted with chemical dominant based medical system, Robert took his passion for nutrition and became board certified in clinical nutrition through the Clinical Nutrition Certification Board, as well as trained as a compounding pharmacist through the Professional Compounding Centers of America.   During this time, he opened his own compounding and nutritional clinic, which blossomed into a complete anti-aging clinic housing other integrative practitioners. Robert became a Certified Clinical Nutritionist, certified in Quantum Reflex Analysis, a practice of kinesiology, as well as have trained and certified in other modalities such as Reiki and auricular acupuncture, a primal health coach, a licensed yoga instructor, and most recently a certified @movnat instructor.   Currently, Robert consults with both with patients to enhance their health through lifestyle medicine as well other practitioners to help integrate, promote and implement natural medicine in their practices. I had a blast recording this latest episode, featuring a multi-passionate pharmacist and entrepreneur, who boldly went after his dreams and never looked back. Robert Kress, RPh, graduated from Temple University School of Pharmacy in Philadelphia PA in 1994. After becoming disenchanted with the chemical dominant based medical system, Robert took his passion for nutrition and became board certified in clinical nutrition through the Clinical Nutrition Certification Board, as well as trained as a compounding pharmacist through the Professional Compounding Centers of America. During this time, Robert opened his own compounding and nutritional clinic, which blossomed into a complete longevity clinic housing other integrative practitioners. Today's episode of Redefining Medicine features Lynn and Michell Crouse. Lynn Crouse, Pharm D. of Lake Village Arkansas is the CEO of My Village Pharmacy and owner of 7 community retail pharmacies in Southeast Arkansas and 2 community retail pharmacies in Greenville Mississippi. In July 2018, Governor Asa Hutchinson appointed Crouse as a Board Member to the Arkansas State Board of Pharmacy.  He is serving a six-year term that will conclude on June 30, 2024.   Crouse previously served as the Arkansas Pharmacist Association (APA) 2017-2018 President, APA District President, APA Area 4 Representative, and has served on various APA committees.  He is also a member of the National Community Retail Association (NCPA), the Professional Compounding Centers of America (PCCA), and the International Academy Of Compounding Pharmacists. Crouse earned a Bachelor's degree in Microbiology from the University of Arkansas at Fayetteville (1989), a Bachelor's degree in Medical Technology from Louisiana State University (1990), and a Doctor of Pharmacy degree from the University of Arkansas for Medical Sciences.  He also completed an Advanced Fellowship with A4M in December 2019.   Dr. Michelle Crouse is a clinical consultant pharmacist, compounding pharmacist and community retail pharmacy business owner. Dr. Crouse received her Doctor of Pharmacy degree from University of Arkansas College of Pharmacy in 1996. She and her husband, Lynn, purchased Hunter’s Pharmacy and completed compounding training with Professional Compounding Centers of America (PCCA) in 2003. Since that time, she has purchased 8 other pharmacies. Dr. Crouse has been an instructor for the Diabetes Self Management Education (DSME) program through the American Diabetes Association. She also teaches at UAMS College of Pharmacy and lectures at Harding College of Pharmacy. Professional associations include Arkansas Pharmacist Association, National Community Pharmacist Association (NCPA), American Academy of Anti-Aging and Functional Medicine (A4M), and American College of Nutrition. Having a passion for Functional Medicine, she completed the Advanced Fellowship program with A4M in December 2019. She was voted “Pharmacist of the Year” by the Arkansas Pharmacist Association in 2014. She also serves as a luminary for Arkansas CPESN (Community Pharmacy Enhanced Services Network) and is a member of Arkansas Rural Health Opioid Task Force. Dr. Crouse lives in Lake Village, Arkansas. Compounding pharmacies take prescriptions from physicians and make products that are not commercially available by using the active pharmaceutical ingredient. These prescriptions are customized specifically to what the patient needs.   What is a compounding pharmacy? Robert Seik, PharmD, is trained in functional and rejuvenative medicine and owns Partell Specialty Pharmacy, which is a compounding pharmacy in Las Vegas. He explains exactly what compounding pharmacies are and what they do.   The original way medications were prepared were with a mortar and pestle and specific ingredients were made for each individual. The ingredients included were only what the person needed and designed specifically for that person's needs. It's only been in recent history where manufactured drugs have become the primary therapy. Compounding pharmacies have always been there.   Compounding pharmacies take prescriptions from physicians and make products that are often not commercially available by using the active pharmaceutical ingredient. These prescriptions are customized specifically to what the patient needs.   For example, a physician can write for a multi-ingredient cream that is applied to the skin to treat for an area of pain. Perhaps this patient is unable to tolerate oral medication, so a topical cream can be made with superb absorption, using only the necessary ingredients, without added toxic chemicals and fillers and with the person's specific allergies in mind.   You can also have compound hormone therapy that may not be offered commercially like bio-identical hormone therapy with the exact dosages needed for that particular person. So, customized hormone therapy to order; exactly what the patient's blood or saliva test showed the deficiency to be.   Creams, capsules, gels, sublingual troche, specialized eardrops and eyedrops can all be made. They can also make sterilized products with their sophisticated clean room that allows products to be sterilized, which is highly regulated. Sterilized injectables such as nutrients, Vitamin D, CoQ10, high dose B12 as methylcobalamin, which is not commercially available, can all be made at a compounding pharmacy. Certain products can be made preservative-free or gluten-free. It is important to know that many commercially available products contain preservatives, gluten and other added fillers that may or may not be safe for the patient. PCCA, Professional Compounding Centers of America, is where Partell Pharmacy purchases many of their active ingredients. It is highly specialized and many compounds are made on site with highly sophisticated equipment, so it takes many more personnel to run a compounding pharmacy.
