Some background, three and a half years ago I got pregnant and early in the pregnancy I identified that I needed to manage my blood sugar with diet, which I successfully was able to do. About a year after having the baby I was diagnosed with methane SIBO, and went through a long treatment, which was successful and improved my gut symptoms drastically. After this, my cholesterol levels and insulin resistance score improved quite a bit. The past year and a half , I have been wearing a continuous glucose monitor, knowing that I am vulnerable to type II diabetes, and in hopes that I will be able to improve my blood sugar and regulate my menstrual cycle. I think it is also be important to mention that I have had poor gut health since I was a child, I suspect I am on the spectrum of PCOS - as I have an irregular cycle which is consistent with those patterns, and I have high cholesterol and have since I was 18 years old, I tend towards inflammation, and I would say I have about 15# to lose. Despite all my efforts by ensuring quality sleep, eating low carb - unprocessed and high quality foods, weight training 3x per week, managing my stress with meditation, and walking 5-7000 steps per day, things are not improving. I have also tried berberine and various other recommended supplements and there is ZERO improvement. My A1C three years ago was 5.3 and most recently it is 5.7. The week before Christmas I had the flu, 104*F fever for four days, congestion and a cough. A week after having the flu, I noticed that my head, shoulders, chest and other parts of my body seemed to be getting hot and itchy like hives, plus I was having bloating and constipation/diarrhea, and physical anxiety (like tight chest, pulse in the 90s). Plus, my blood sugar was spiking after eating a low carb meals and in other situations that didn't make sense. At a certain point I tried to help myself by taking electrolytes and extra whole food vit C. and things continued to get worse. A couple weeks ago my blood sugar was going up to 125-130 and sitting there all day, even if I walked or exercised, and then it would go down to 90-100 after I went to bed. This has happened a couple other times before, and I could not identify why this was the case. All this time I was eating under 40g or less carbs, around 140g protein, and 100g+ fat. As my symptoms got worse, I was led down the podcast rabbit hole of Mast Cell Activation Syndrome, and histamine intolerance, which I had learned about years ago but didn't necessarily think I fit into this category. In hindsight, I am realizing that Mast Cell Activation Syndrome and or having high histamine loads in the body, may explain some of the very unexplained and inconsistent patterns I have observed while using the CGM over the past year and a half. Some examples are, blood sugar spikes up to 130 after taking supplements (!), blood sugar spikes up to 160 after eating low carb soup with bone broth and vegetables, spikes after low carb meals that I would include ferments or drinking hot chocolate (made with collagen, coconut milk, salt, cacao and stevia). And maybe, it explains why my blood sugar will go up to 170-200 with any intense exercise, and my inflammation takes days to recover from. And I am wondering now, if it is why I am seeing no improvement with my insulin resistance, even after consistently (not perfectly) doing all the things that are supposed to help with insulin resistance. Have you explored the relationship between insulin resistance/type II diabetes and Mast Cell Activation Syndrome? As I look back at my health journey this is starting to look very possible to me, but I am not a scientist or medical professional, and know that there is so much I do not understand . And, I am wondering if you have explored this for yourself at all? But then still my question is! What is to be done? Lymbic system retraining? Somatic experiencing? Making sure the body has balanced and adequate minerals? Low Dose Naltrexone? At this point I have tried many different things, and man, if I knew what to do I would do it. I am so curious what you guys think, and would be so grateful to hear from you. Angie MCAS and histamine after exposure Jacob writes: Hi Robb and Nicky, First thank you very much for the podcast. It's really helpful to hear all the different approaches to the health questions people raise. On last episode you mentioned you might do the next episode on MCAS. Personally I have post viral POTS and some sort of MCAS after my 2020 COVID infection. LMNT helps me avoid POTS symptoms and if I completely avoid histamine from food I do great, with basically zero issues. I found that I can even take grass fed beef liver pills before I have a meal with histamine and it seems to help me avoid the issues. I have two questions. First, do you have any experience on how the liver pills help me to not get beat up when I take them prior? It seems to have something with natural DAO but to be honest it's sortof a black box. I have basically no clue how much I'm ingesting from a DAO standpoint. I was taking the Ancestral Health beef liver pills and taking 6 capsules before the meal. That seems to help. My second question is the more important one. How the heck do I deal with a histamine exposure. If I get got, I have anywhere from 24-48 hours of brain fog, extreme fatigue, malaise, and depressed/depression feelings. If there were something that I could do post exposure to abate this while I recover I would be extremely grateful. I found that if I take antihistamines (my rheumatologist recommended citrizine) that it helps *some* but not really enough. Thank you! Jade from South Africa talks about dealing with Hashimoto'sand her journey with Low Dose Naltrexone (LDN). Jade was diagnosed withHashimoto's in 2010, but she only discovered LDN in 2019 after stumbling uponit in a Hashimoto's Facebook group. She consulted with a functional doctor inSouth Africa who prescribed LDN for her.Before receiving her Hashimoto's diagnosis, Jade experiencedsevere hair loss, which prompted her to seek medical attention. Although shedidn't exhibit typical symptoms such as weight gain, she suffered from fatigue,which she later learned was related to Hashimoto's. Initially diagnosed with anunderactive thyroid, it took years before she was informed of her Hashimoto'scondition.After moving to Germany in 2016, Jade's symptoms worsened,leading her to research and seek out support groups to manage her conditionbetter. Despite making dietary changes, she found that her antibodies remainedhigh, which motivated her to explore LDN as a potential solution. Since then,she has seen significant improvement, with her antibodies dropping to remissionlevels.Obtaining LDN in Germany proved to be challenging, as manydoctors were unfamiliar with the medication. Ultimately, Jade continues tosource her LDN from her doctor in South Africa and relies on family members tobring it to her when they visit.Jade started LDN at a low dosage of 0.5mg, graduallyincreasing the dose each week until she reached 4.5mg. Initially, sheexperienced vivid dreams and fatigue for the first few weeks.Overall, Jade's journey with LDN and Hashimoto's highlightsthe challenges of accessing specialized treatments and the importance ofadvocating for oneself in managing chronic conditions.In addition to managing her Hashimoto's with LDN, Jadecontinued taking LDN throughout her pregnancy. Despite some doctors advising againstit during pregnancy, she did her own research and found a supportive community.She believed LDN potentially helped her from having a miscarriage and had apositive impact during her pregnancy. She had a healthy pregnancy and a smoothbirth, and her baby is now three months old and healthy. She values theimportance of doing her own research and seeking second opinions from doctorsand is grateful for the positive experience she had during her pregnancy.
Today, we had the opportunity to hear Debbie's Chronic Pain story, a patient from the United States who is using Low Dose Naltrexone (LDN) for chronic regional pain syndrome (CRPS). Debbie recounted going back to when she was seven years old and suffered a knee injury, leading to a series of medical interventions. Despite numerous surgical procedures, the chronic pain persisted, and she endured multiple operations without finding relief. Eventually, Debbie was diagnosed with CRPS, marking the beginning of her battle with persistent pain. Teresa Kenney NP here! For many years, I have been able to see patients benefit from this medication called low dose naltrexone. But is it more hype than for real? It has been known to be a treatment for an incredible amount of conditions including; autoimmune diseases of every sort, chronic pain, PMS, infertility, pyschiatric disorders, and more. That is what it makes it seem unbelievable, a drug that can help such a wide complex of symptoms and conditions. But I am here to tell you that it may be true. We actually have decent research that proves low dose naltrexone, with its immune system modulation powers, and endorphin boosting capability, can truly treat diseases. What I love about LDN is that it is a more root cause approach than most treatments. Through its ability to help our own immune system reduce inflammation markers, and target tissues to gain cellular health, it truly can allow restoration through an underlying root cause way. Now in certain areas, we have a long way to go in terms of the research, and that includes the area of women's health, especially fertility. Emily, is working on her Bachelor's Degree in Nutritional Science and is looking forward to completing a PhD in the same field a few years after graduation. Emily has been sensitive to metals her whole life, starting with a severe allergy to earrings and other jewelry as a child. In early 2019 she was talked into getting her nose pierced by a friend and was SUPER excited. Even picking out cute nose rings. Within months of that nose piercing Emily developed multiple autoimmune diseases and suffered with the debilitating effects of Trigeminal Neuralgia, a syndrome nicknamed “The Suicide Disease” creating painful nerve sensations on the face. She also suffered from an onset of joint pain, repeated sore throats and swelling of her lymph nodes, as well as all kinds of tenderness surrounding the area. Around the same time frame Emily started noticing increase symptoms from several mercury fillings in her mouth, experiencing a large “flair” when the fillings were removed and likely swallowing a small piece of the extracted filling during removal. Symptoms from her fillings were most notably a metallic taste in her mouth, among others. Emily also shares experiences where her body actually pushed out an IUD device, and relays stories of her throat becoming sore when drinking from stainless steel water bottles and starting to feel unwell from a gifted set of new stainless steel cookware. She is still trying to discern whether or not a surgical clip was left in place during a 2017 appendectomy. She experienced a substantial, indisputable relief of her Trigeminal Neuralgia and other symptoms upon removing the nose ring for an unrelated surgical procedure, along with having her metal fillings removed. Why add in Low Dose Naltrexone? LDN - Naltrexone is used for opioid withdrawal or in certain cases, alcohol addiction. It will block the receptor sites so the addict will not get the high they are expecting from using. But in low doses, like really low doses, Naltrexone is found to modulate the immune system and lower autoimmune antibodies. And this is true for any autoimmune condition. Anytime I see a patient that is literally an autoimmune ball of fire, we want to incorporate LDN. This will help lower those antibodies in autoimmune cases. Naltrexone is a medication approved for use by the FDA since 1984 for the treatment of opioid and alcohol addiction. It is usually used at a dosage of around 50-150 mg for that indication, however when used at a Low Dose such as 0.5 mg to 4.5 mg, it can be very effective in treating other conditions such as autoimmune disorders, chronic pain and even certain cancers. Low Dose Naltrexone (LDN) is currently more popularly used in the alternative/functional medicine world, however given its cost-effectiveness, safety and science based evidence, it should be utilized more frequently in more mainstream medicine. Today's interview with Ginny Isbell, Pharm D, will bring clarity and insight on the many benefits of LDN. Ginny decided to pursue pharmacy while still in high school where she spent her summers working at the local hospital pharmacy. This led her to the McWhorter School of Pharmacy at Samford University where she graduated with a Doctor of Pharmacy. She worked in Long-Term Care Pharmacy before she discovered her appreciation for pharmacy compounding. In 2006 she joined Madison Drugs diving into the world of compounding and nutrition. She started attending numerous compounding, hormone and functional medicine conferences each year and quickly realized this was a place she were she would never tire of learning in order to help others. Ginny has now completed over 150 hours of live CE on Pharmacy Compounding and over 120 on hormone replacement and functional endocrinology. Ginny is a member of A4M and IFM. Ginny completed the LDN Masterclass and is now certified as an LDN Specialist. The Masterclass program included over 35 hours of continuing education and the specialist certificate exam.
