Medicus Pharma CEO Dr Raza Bokhari joined Steve Darling from Proactive to announce that the company has received “study may proceed” clearance from the U.S. Food and Drug Administration to initiate its Phase 2b dose-optimization study of Teverelix®, an investigational next-generation long-acting GnRH antagonist, in men with advanced prostate cancer (APC). Bokhari told Proactive that the Teverelix trial will be conducted as a Phase 2b open-label study enrolling approximately 40 men with advanced prostate cancer who are appropriate candidates for androgen deprivation therapy (ADT). Patients will undergo treatment for approximately 22 weeks. The study's primary endpoint is confirmation of medical castration by Day 29, with sustained testosterone suppression maintained through Day 155. The company is targeting a probability of success exceeding 90% for achieving this endpoint. Teverelix trifluoroacetate is formulated as a long-acting injectable GnRH antagonist delivered as a microcrystalline suspension. Unlike GnRH agonists—which typically cause an initial surge in testosterone levels before suppression—Teverelix works through immediate receptor antagonism. This mechanism enables rapid suppression of luteinizing hormone (LH), follicle-stimulating hormone (FSH), and downstream sex hormones without the testosterone “flare” associated with agonist therapies. This rapid suppression profile may hold particular clinical relevance for patients with advanced prostate cancer who are at elevated cardiovascular risk. Emerging evidence suggests that persistent FSH exposure in patients treated with GnRH agonists may contribute to adverse cardiovascular outcomes. While further clinical validation is required, prior Teverelix studies have not demonstrated significant cardiovascular safety signals to date. Cardiovascular disease remains one of the leading causes of non-cancer mortality in men with prostate cancer, accounting for approximately 30% of deaths. The risk can be further amplified during androgen deprivation therapy, particularly among patients with pre-existing cardiovascular disease. Clinical and observational data indicate that such patients may face a five- to six-fold higher incidence of major adverse cardiovascular events (MACE) when treated with GnRH agonists compared to GnRH antagonists. The teaching on the laying on of hands to minister is one of the foundational, elementary principles of Jesus Christ. Our hands are normal, mortal hands. Nothing special. But when we lay holy hands, with a heart full of faith to minister to people - God does wonderful things! In this sermon we highlight six areas of God's workings through the laying on of hands as seen in Scripture. #1, The laying on of hands to bless #2, The laying on of hands to minister healing #3, The laying on of hands to help people receive the baptism in the holy spirit #4, The laying on of hands to anoint and appoint #5, The laying on of hands to impart spiritual gifts and anointing #6, The laying on of hands to work miracles God is still working in this manner and we must practice this with boldness, confidence and expectation, as we minster to one another. God can also work through the "indirect touch". The apostle Paul laid hands on cloth and people received healing and deliverance when these cloths were laid on people. So today people can lay hands over a video stream, or over a text message, or over a picture of another person, etc. to minister to them. As God works through the laying on of our hands and does wonderful things, we must ensure that people don't get the wrong impression that there is something “special” about our hands or about us. We must instruct people keep their focus on God and His Word rather than us becoming the “center of attraction”. In my former life as a reporter, I eventually started neglecting my body armor. And that's easy to explain: it is heavy, takes up a lot of space, it is hot inside it, and it's difficult to move. I always took a helmet with me to every dangerous assignment, Syria, Iraq, Gaza, and so on. But the west I carried far from always did I ever regret, not taking it with me? No, at least I don't remember ever regretting it. Did I regret it when I did bring the vest? Almost every single time. I had to look for a bag big enough to fit it, pay huge extra fees for its excessive weight at airports, and once the border guards confiscated it from me, right upon arrival for the Arab Spring in Cairo- it just sat in the warehouse for two weeks until I flew back. Essentially, I just took the vest on a round trip to Africa at my own expense. And it is very hard to run in it, especially with asthma: getting into a car or an APC is a struggle and clothing under it gets ruined very quickly from friction and sweat. Long story short, at some point I simply decided I was not going to take it anymore -that a helmet alone was really enough for me. I even justified that decision with Vietnam War era statistics claiming what the likelihood of suffering a severe head injury was much higher than getting hit in the body by a bullet or shrapnel. I did not know how accurate those numbers really were, but they suited me, so I clung to them. The helmet did save me a few times. It took hits from the stones and tear gas grenades, it served as my pillow, my chair, and my hammer. At one point, I just gave my body armor to a friend who was heading into a dangerous place and told him he could keep it. I said that it only collected dust and took up space. So when the full scale war came to my own country, I ended up having no armor at all. In the first weeks of the war, they gave me a strange old plate carrier with large, heavy plates. It was poorly sewn and fell apart very quickly. I tried to find a new carrier for the plates that remained, but it turned out those plates were completely non-standard and simply did not fit into normal carriers. I had to buy new plates- with the money raised through this podcast -ordered from Austria, they arrived very quickly and fit perfectly into standard vest. Honestly, I don't remember whether I read the markings on those plates at that time or not, but recently I took them out of the vest and accidentally noticed that they have an expiration date. They are not eternal and in fact not very long lasting. Mine are good until June 2027. What happens after that is hard to say. As far as I understand, some kind of composite material, which makes the plates somewhat lighter than the solid metal ones, will start losing its properties and won't be as strong as before. Of course, they won't crumble into dust in July 2027, but they won't be as reliable as we are now. I hope I won't be serving in the army long enough for that date to matter, that the war will be over before then. But there is no guarantee at all. In the winter of 2022. When I went into the Army, I thought it would be for a few weeks, maybe a couple of months, but I've been serving for almost four years now. June 2027 is already much closer than February 2022.

In this episode of JCO PO Article Insights, host Dr. Jiasen He summarizes the article, "Somatic Mutation Profiles of Colorectal Cancer by Birth Cohort" by Gilad, et al published October 11, 2025. TRANSCRIPT Jiasen He: Hello, and welcome to the JCO Precision Oncology Article Insights. I am your host, Jiasen He, and today, we will be discussing the JCO Precision Oncology article, "Somatic Mutation Profiles of Colorectal Cancer by Birth Cohort," by Dr. Gilad and colleagues. Early-onset colorectal cancer is defined as colorectal cancer diagnosed before the age of 50. Several reports have suggested that early-onset colorectal cancer has unique characteristics. Compared with late-onset colorectal cancer, early-onset colorectal cancer cases are more commonly found in the distal colon or rectum, tend to be diagnosed at more advanced stages, and may display unfavorable histologic features. Although the overall incidence of colorectal cancer has declined in recent decades, the incidence of early-onset colorectal cancer continues to rise. This increase appears to be driven by birth cohort effects. The reasons behind this rise remain unclear but are likely multifactorial, involving changes in demographics, diet, lifestyle, environmental exposures, and genetic predisposition. At the same time, studies have shown conflicting results regarding whether there are differences in the mutation profiles between early-onset and late-onset colorectal cancer. Therefore, it is crucial to explore whether colorectal cancer somatic mutational landscape differs across birth cohorts, as this could provide important insight into generational shifts in colorectal cancer incidence. To address this question, the authors conducted a retrospective study to characterize the mutation spectrum of colorectal cancer across different birth cohorts. Consecutive colorectal cancer patients who underwent somatic next-generation sequencing at the University of Chicago pathology laboratory between 2015 and 2022 were retrospectively identified. Tumors were tested for 154 to 168 genes and categorized as either microsatellite stable or high according to established thresholds. Patients with hereditary cancer syndromes or inflammatory bowel disease were excluded. Participants were then grouped into birth cohorts by decades, as well as into two major groups: those born before 1960 and after 1960. Genes that were identified in at least 5% of the sample were selected and grouped into 10 canonical cancer signaling pathways. These genes and pathways were then included in the analysis to explore their association with colorectal cancer across different birth cohorts and age groups. A total of 369 patients were included in the study, with a median birth year of 1955 and a median age at colorectal cancer diagnosis of 62.9 years. 5.4% were identified as having microsatellite-high tumors. The median tumor mutational burden was 5 mutations per megabase for microsatellite-stable tumors and 57.7 mutations per megabase for microsatellite-high tumors. Patients with microsatellite-high tumors tended to have earlier birth years and were diagnosed at an older age. However, after adjusting for potential confounders, neither birth year nor age remained statistically significant. Similarly, after controlling for confounders, no significant associations were observed between birth year or age and mutation burden. In this cohort, APC, TP53, and KRAS were the most frequently mutated genes. No statistically significant differences in the prevalence of gene mutations were observed across birth cohorts. Correspondingly, the most affected signaling pathways were the Wnt, TP53, and (RTK)/RAS pathways. Similar to the gene-level finding, no significant differences in the prevalence of these pathways were identified among birth cohorts. When examining patients born before and after 1960, the authors found that the older birth cohorts were diagnosed at an older age and had higher tumor mutational burden. However, no significant differences were observed in any of the genes or pathways analyzed. Among microsatellite-stable tumors, 18.3% were classified as early-onset colorectal cancer, while 81.1% were late-onset colorectal cancer. Consistent with previous reports, early-onset colorectal cancers in this cohort were more likely to be left-sided and more common among more recent birth cohorts. However, no significant differences were identified in any of the examined genes or pathways when comparing early-onset to late-onset colorectal cancer. In this cohort, a higher prevalence of early-onset colorectal cancer was observed among more recent birth cohorts, consistent with previous reports. Still, no distinct mutational signature was identified between the early and late birth cohorts. The authors proposed that the lack of distinct mutational profile by age or birth cohort may be due to the limited number of key molecular pathways driving colorectal cancer. Although environmental exposures likely differ across generations, the downstream effects may have converged on similar biological mechanisms, leading to comparable somatic mutations across cohorts. Alternately, they proposed that the observed birth cohort differences in colorectal incidence may be driven by distinct mutation signatures, epigenetic alterations, or changes in the immune microenvironment rather than variations in canonical gene mutations. As the authors noted, given the retrospective nature of this study, its modest sample size, and the predominance of advanced-stage tumors, larger prospective studies are needed to validate these findings. In summary, this study found no significant differences in the mutational landscape of colorectal cancer across birth cohorts or age groups. The authors proposed that the generational shift in colorectal cancer incidence is unlikely to be driven by changes in the underlying tumor genomics.
