Dr. Schacter's story is personal, powerful, and eye-opening — especially for women dealing with joint pain and osteoporosis. What I love about his approach is that it aligns so closely with what I teach: bone loss is not just a number on a scan. If you are over the age of 35 or within a few years of 35, this episode is for you! Dr. Stephanie Cowie is back on the podcast today for a deep-dive conversation on Bioidentical Hormone Replacement Therapy (BHRT) and its powerful connection to women's mental health, aging, and overall wellness during perimenopause and menopause. In this episode, we unpack the difference between BHRT and synthetic hormone replacement therapy (HRT), and why that distinction really matters. We also discuss the major gap in traditional medical training when it comes to supporting women through perimenopause and menopause, and how that impacts the kind of care many women receive. Hormonal shifts can begin as early as age 35, with symptoms often peaking in the late 30s to early 50s. Dr. Cowie emphasizes the importance of starting BHRT at the right time, and breaks down what to expect from a thorough intake, lab testing, and symptom monitoring process when exploring your options. Did you know that even on hormone replacement therapy, many women are still losing bone? I hear it all the time—“But I'm on BHRT…why are my bones still getting weaker?” And that's exactly what we're diving into today on the Stronger Bones Lifestyle podcast. I'm so excited to share this powerful episode with you because we're pulling back the curtain on hormone therapy and giving you real, practical answers.I'm joined by Dr. Nayan Patel, a compounding pharmacist who's trained over 5,000 physicians across the country on how to properly prescribe bioidentical hormones. We break down what most doctors get wrong about estrogen, progesterone, and testosterone—and how the way you take your hormones matters just as much as what you take. You'll learn why the right dose, delivery method, and hormone balance are essential for bone protection, and how lifestyle is still the foundation no matter what you're prescribed. Dr. Patel shares so much wisdom in this conversation—so grab a cup of tea and settle in, you're going to want to take notes!Key TakeawaysThe Role of Estrogen – Estrogen levels must be between 60–110 to effectively protect bones and prevent fractures.Progesterone's Dual Benefit – Oral progesterone supports bone formation and improves sleep and emotional balance.Testosterone's Impact – Testosterone is key for building muscle mass that supports and stabilizes aging bones. Why Hormone Delivery Matters – Applying creams to the inner labia ensures better absorption and fewer side effects than wrist or oral routes.Delayed BHRT Can Still Help – Women 10+ years post-menopause can start BHRT safely when guided by a knowledgeable provider.Understanding Hormone Balance – Feeling your best is the true marker of hormonal balance—not just lab numbers.Cancer History and Hormones – BHRT may not be safe for those with recent estrogen-sensitive cancers; timing and monitoring are critical.Lifestyle is 70% of the Solution – Managing stress, eating nutrient-dense foods, sleeping well, and moving intentionally are the true drivers of bone strength.Where to Find Our GuestAuro Wellness: https://aurowellness.com/Auro Wellness YouTube: https://www.youtube.com/@aurowellnessAuro Wellness IG: https://www.instagram.com/AurowellnessLinks & ResourcesStronger Bones Lifestyle CommunityJoin a supportive space to strengthen your bones naturally with live sessions and masterclasses. Cortisol plays a critical role in how your body handles stress, energy, and metabolism—but in perimenopause, it can easily become imbalanced, leading to fatigue, weight gain, anxiety, and disrupted sleep. In this episode, Tara unpacks everything you need to know about cortisol, including why adrenal testing often isn't necessary and what really works to regulate stress hormones. It remained in first or second place for the rest of the century. Until 2002 when the media got a hold of the results of the Women's Health Initiative study. Doctors around the world were warned not prescribe it and women were told that it was dangerous and shouldn't be on it. About Donna White: Donna White is a seasoned expert in bioidentical hormone replacement therapy with over 30 years of experience. As the founder of the BHRT Training Academy, author of "The Hormone Makeover," and a pioneer in establishing BHRT programs, she has played a pivotal role in advancing hormone health. Her expertise is augmented by her personal journey with hormonal imbalance, rendering her a compassionate authority in this field. Donna's unique approach lies in her ability to translate complex medical information into digestible content, making her a sought-after speaker, educator, and consultant. Her training academy has revolutionized the way medical practitioners approach hormone therapy, leading to significant improvements in patient care and practice growth. What We Discuss In this Episode: In this episode, Donna opens up about her struggle with severe PMS symptoms in her 20s and how it motivated her to find answers, ultimately transforming her life with bioidentical hormone therapy. She sheds light on the widespread confusion surrounding hormones, debunking myths like the misconception that they cause health issues such as cancer. Donna clarifies the critical differences between bioidentical, synthetic, and animal-derived hormones, explaining why bioidentical options—molecularly identical to those produced by the body—are a safer and more effective choice. Donna dives deep into the common signs of hormone imbalance, including fatigue, sleep issues, mood swings, weight gain, and skin changes, and stresses the importance of proper testing to uncover root causes. She explains the pros and cons of various hormone testing methods, such as saliva, dried urine, serum, and capillary blood spot, emphasizing the need to monitor cortisol levels accurately. Donna also highlights how hormone balance is essential at every stage of life, showing how declining hormone levels can contribute to age-related diseases and sharing research that supports hormone replacement therapy for improved longevity and health. Additionally, Donna addresses fears about hormones and breast cancer, citing evidence that hormones like estrogen, progesterone, and testosterone can be protective and even improve outcomes for those diagnosed. She offers practical advice for finding qualified healthcare providers, from evaluating testing protocols to consulting compounding pharmacists and hormone testing labs. If you're ready to cut through the misinformation and take control of your hormone health, this episode is packed with valuable insights you won't want to miss! Mara is a licensed marriage and family therapist She first found fasting in 2019, and tried extended fasting, but had limited success. Her frustration eventually led her to try the Galveston diet by Dr. Mary Claire Haver in June of 2019, which not only helped her shed pounds but also introduced her to the benefits of fasting. She started out with macro tracking, which helped her adopt a more balanced approach to eating which has stuck with her. However, the initial weight loss journey had its ups and downs, especially during menopause.Mara experienced a mental funk by January of 2020 due to the weight gain of menopause, and planned a yoga retreat for self-care, but the COVID-19 pandemic forced her to cancel the retreat. Despite the challenges, the lockdown brought precious family bonding time as her daughters returned home from college. This period, which Mara describes as "stolen time," allowed for unexpected moments of joy and indulgence. During the lockdown, Mara also explored hypnotherapy for weight loss, unearthing deeper emotional issues related to self-imposed guilt, and learning the importance of forgiveness and implementing personal rules that provided the discipline she needed.Mara achieved her weight goal of 145 pounds in six months. In 2021, she discovered Gin's work and delved deeper into intermittent fasting. Initially hesitant to change her habits, Mara eventually embraced a clean fast with positive results. Gin and Mara also discuss Dr. Mary Claire Haver's insights on perimenopause and the importance of proper education in women's health, as well as Mara's and Gin's positive experiences with bioidentical hormone replacement therapy (BHRT).Mara shares her personal triumphs over insomnia, brain fog, and the incredible feeling of restful sleep after starting BHRT. She has also experienced powerful mindset transformations along the way. In closing, Mara emphasizes the importance of finding personal solutions that work for you, highlighting the necessity of balancing discipline with freedom.
