Sleep breaks down, pain turns constant, and the mind keeps running like it never got the memo that the mission is over. This conversation follows what it looks like to claw your way back when the body is hurting, the nights are loud, and isolation starts to feel normal. Rowdie McMahon shares her experience as an Air Force nurse deployed to Afghanistan, including the relentless pace and mass casualty reality, and how that pressure followed her home. She opens up about chronic pain, years on heavy medications, and the slow work of tapering off while staying engaged with mental health support. From there, the story shifts to what finally helped: Wounded Warrior Project programs, small steps back into community, and a surprising turning point through racing, building cars with other veterans, and putting 988 and the Veterans Crisis Line on the car as part of the mission. Dr. Foreman pulls back the curtain on the sobering reality: the demand for evidence-based suicide prevention care is simply too vast for traditional methods to meet. Discover the shocking "sin" of EBP practice we might all be committing, learn how simple AI tools (like automated scribing) can boost your clinical fidelity, and find out what happens when a clinical expert "red-teams" popular chatbots for suicidality. Get the insights you need to confidently apply your ethical framework to the future of care and conquer your fear of the algorithmic boogeyman!April C. Foreman, Ph.D., is a Licensed Psychologist serving Veterans as Director of Technology and Innovations for the Veterans Crisis Line. She is a member of the team that launched OurDataHelps.org, a recognized innovation in data donation for ground-breaking suicide research. She is passionate about helping people with severe (sometimes lethal) emotional pain, and in particular advocates for people with Borderline Personality Disorder, which has one of the highest mortality rates of all mental illnesses. She is known for her work at the intersection of technology, social media, and mental health, with nationally recognized implementations of innovations in the use of technology and mood tracking. She is the 2015 recipient of the Roger J. Tierney Award for her work as a founder and moderator of the first sponsored regular mental health chat on Twitter, the weekly Suicide Prevention Social Media chat (#SPSM, sponsored by the American Association of Suicidology, AAS). Over 22 veterans choose to end their lives every single day. The system meant to support them is broken and the U.S. Secretary of Veteran Affairs Doug Collins knows it.In this episode, VA Secretary Doug Collins joins Chad Robichaux to explain how he is overhauling bureaucracy, slashing red tape and putting veterans first. Collins describes why the VA's 170 hospitals must function as one integrated network and how he will steward a $400 billion budget and 460,000 employees for maximum impact. The conversation covers efforts to accelerate claims, cut wait-times, and create seamless hand-offs from DoD service to VA care for the nine million veterans currently enrolled. Collins also shares initiatives in suicide prevention including the 988 Veterans Crisis Line, expanded Vet Centers, and partnerships with faith-based organizations, along with community-care upgrades, stronger accountability for under-performers, and clinical trials of psychedelic and stem-cell therapies to treat PTSD and chronic pain. If you or someone you know is struggling and needs help the Veteran's Crisis Line is 24/7, confidential crisis support for Veterans and their loved ones.
