Today's guest has a unique perspective as she's both a high performance athlete AND a pelvic physio who is passionate about keeping girls and women involved in sport.Clodagh McIntyre is a rising star in physiotherapy who is both an inter-county camogie player for Tipperary and a pelvic health physiotherapist. Clodagh discussed her journey from UCD graduate to working in private practice, her experiences with stress incontinence as an athlete, and the lack of pelvic health education in physio training programs. We explored how pelvic health issues affect female athletes, with statistics showing 7 out of 10 athletes leaving sport due to bladder leakage, and discussed the taboo nature of discussing these topics within sporting teams. We discussed the importance of education and support around pelvic health issues for female athletes, highlighting the lack of standardization in GAA and potentially LGFA, as well as emphasising the need to normalize conversations about these issues and prevent them from deterring athletes from participating in sports.Clodagh shared her upcoming trip to Kenya with Warriors for Humanity and Self-Help Africa, where she plans to plant trees and help communities, for which she is fundraising. The first half of the WorldSBK season is in the books and Steve and Gordo get you up to date with the latest news from inside the paddock as Nicolo Bulega continues his relentless drive to a first Superbike title. The Italian is still perfect for 2026 with 18 wins from 18 starts but Sunday's Superpole Race saw him pushed harder than at any point thus far. His harder rear tyre proved the difference maker but, as ever, Bulega found a way to win. What must Iker Lecuona be thinking? The Spainard, 15 times a runner-up finisher this year, is hunting for his first WorldSBK victory. He's hurting too because he knows how close he keeps coming to that success. His reward for his hard work will be seen this weekend in MotoGP where he'll replace Alex Marquez. Outside of the Aruba.it Ducati riders there was plenty to talk about but Gordo's conversations with Tommy Bridewell have certainly left an impression. The Englishman had his best finish of the season, fifth position in Race 1, and is making the most out of his Ducati V4R to reap some good results.

Hello and welcome to today's episode, where I'm discussing neuropathic pain, particularly the Sciatic & Pudendal nerves, with Thomas Dekkers - Tom's background includes his 18-year clinical experience as a spine specialist physiotherapist, his PhD research on athletic back pain, and current role as a clinical specialist in neurosurgery at Cork University Hospital.In our conversation today, we explored how dermatomes can be unreliable for diagnosis, particularly with pelvic pain conditions, and discussed the role of neuroinflammation in persistent pain states. Tom shared his approach to managing neuropathic pain, emphasizing the importance of first avoiding activities that aggravate symptoms before gradually reintroducing movement and exercise. We also discussed the problematic nature of the "core stability" terminology in physiotherapy and the need for more biopsychosocial approaches to patient care. The conversation highlighted the challenges of managing chronic pain conditions and the importance of proper patient education and goal-setting in treatment plans, including neuroinflammation, when compression can lead to inflammation throughout the nervous system, including glial cell activation in both the dorsal horn and thoracic spinal cord. We talked about that when patients completely color in body maps, it raises concerns about nociplastic pain, particularly given the anatomical proximity of nerve roots and the potential for irritation to spread. Hello & Welcome to today's episode where we are focusing on new research that really helps us change the conversation about bladder leakage in runners, and why we need to move beyond isolated pelvic floor muscle exercises...I'm in conversation with Natalia Cardoso Campos, who published a really interesting paper in 2025 'Inefficient impact absorption and reduced shock attenuation in female runners with stress urinary incontinence'Natália is a physical therapist from Brazil, and in our chat we presented findings from her master's thesis showing that female runners with urinary incontinence have poor shock absorption capacity at the ankle and significantly reduced shock attenuation compared to continent runners. Her research revealed that while continent runners had shock attenuation values of around -29, incontinent runners had values of only -6, indicating much more energy reaching the pelvic region. We discussed how this biomechanical imbalance could explain why some runners experience urinary leakage despite having normal pelvic floor strength, suggesting that the problem originates in the lower kinetic chain rather than the pelvic floor itself. We explored:urinary incontinence research in runnersshock absorption & shock attenuation in runnersfoot strike patterns, ankle injuries and asymmetry & how conditions like REDS (Relative Energy Deficiency in Sport) and bone stress injuries can exacerbate these issuesSome key takeaways for me were the importance of looking beyond the pelvic floor muscles when we are working with women, especially athletes, with pelvic health issues - considering the ankle, knee and hip as well as systemic issues like hypermobility and REDs.I can't wait to see what Natalia's PhD research on this topic reveals!We also talked about a new paper she was a co-author on, 'Sexual Trauma and Pain: A Study of Women's Experiences With Vaginal Dilators and Perineal Massage in Trauma-Informed Pelvic Floor Physical Therapy' (Modesto et al 2026)Natália shared insights from this qualitative study on women's experiences with vaginal dilators and perineal massage in trauma-informed pelvic floor physical therapy,   underscoring the significance of patient rapport and validation in treatment.Want to learn more? Hello & Welcome to the CelebrateMuliebrity Podcast!Today's episode is all about strength & resistance training at menopause - why are only 30% of women engaging in resistance training at menopause?(and sorry, my sound is a bit wonky at the start)My guest today is Chloe Lewis, who had a non-linear career into women's health, to say the least!Chloe is a physiotherapist conducting PhD research on optimising resistance training at menopause while incorporating pelvic health concerns and tendon issues. Chloe shared her personal journey into physiotherapy after experiencing a heart condition that led to heart surgeryWe discussed how menopausal women face barriers to resistance training due to generational messaging about smaller bodies and fear of bulking up. We explored how resistance training can benefit bone health, muscle maintenance, and mental well-being, while addressing specific concerns about tendinopathy in areas like the rotator cuff and gluteals. Chloe explained her research approach, which includes studying barriers to resistance training in menopausal women and potentially examining how effective resistance training protocols might impact pelvic floor strength. This conversation emphasised the importance of individualised approaches, starting with bodyweight exercises, and the need for healthcare professionals to prescribe resistance training as part of rehabilitation programs for women.We also discussed the similarities and differences in resistance training protocols for men and women, emphasising that proportionate muscle growth is similar despite men starting with more muscle mass. Chloe highlighted that social context and barriers, rather than physiological differences, are the main obstacles for women engaging in resistance training. We definitely agreed on the importance of understanding and addressing these barriers to encourage women's participation in resistance training....and much much more, including her PhD research focusing on the impact of resistance training on pelvic floor health in menopausal women. She plans to conduct a four-armed study comparing the effects of resistance training alone, resistance training plus pelvic floor exercises, pelvic floor exercises alone, and a control group. Chloe is currently in the ethics approval stage for qualitative interviews with menopausal women and plans to survey healthcare professionals about resistance training practices for menopause. She aims to develop a co-produced exercise program that is accessible and relevant to the participants - I can't wait to see the outcomes!If you'd like to learn more about menopausal health, be sure to check out my comprehensive online course, 'Flourish! Let's go!As you probably know, March is Endometriosis Awareness Month and although we are only a quarter of the way through the year, there are already some noteworthy papers out there, from acknowledging the many issues people with endometriosis face, from dismissal and gaslighting to clinical diagnosis, assessment and pelvic rehab strategies - what the evidence actually is showing (some of it may surprise you!I'm going to talk through what these research papers are saying, why they matter and how we can use their findings to improve the care of people dealing with pain, fatigue, GI, MSK, urologic and neurological issues, as well as including nociplastic changes - and how the evidence is really supportive of a multimodal pelvic rehab approach, which we're talking about pelvic floor therapy, tens, movement or manual therapy - there's also some interesting implications for including telehealth options as part of our psychologically informed care...I'm in the middle of doing my regular research updates for my online Female Pelvic Pain Rehab course - I'm starting with the Endometriosis module but expect updates on vulvodynia, pudendal neuralgia and bladder pain syndrome...as well as how we can include a bio psychosocial approach to assessment and differential diagnosis, as well as an integrated treatment toolbox - if you're already enrolled, you'll get all the updates as they are released and if you're not...come and join the pelvic pain rehab conversation! His mission was to bring her back for his brother Euron, but he plans to claim her as his own bride. One ship brings a prisoner to Victarion - Moqorro the priest of R'Hllor. Greyjoy is tempted to sacrifice him to the Drowned God, but the man has knowledge that he shouldn't. Hello and Welcome! In today's episode, I had the pleasure of chatting with Dr Sula Windgassen, health psychologist, psychotherapist and author of one of my favourite new books, 'It's All In Your Body', which explores the conversation between mind, body and environment and how that can influence and sustain pain and poor healthIn today's conversation, we discussed lots of different threads, including the intersection of psychology and physiology in health care, particularly for pelvic pain and bladder/bowel conditions. We explored the Power Threat Meaning Framework, stress management, and goal-setting approaches, emphasising the importance of process goals rather than solely focusing on pain elimination. Sula shared her personal health journey and explained how health psychology differs from clinical psychology, and we both highlighted the need for integrated care between psychologists and physiotherapists. We discussed the complexities of stress, particularly how it relates to endometriosis and the challenges faced by those with persistent pelvic pain, and explored how stress can be a stressor, an automatic response, and an action, emphasising the need to differentiate between these concepts to avoid self-blame and stigma. Sula explained that stressors can be external or internal, and automatic stress responses can be psychological or physiological, highlighting the importance of understanding these distinctions to better support individuals experiencing stress-related issues.Sula's book explores the intersection of biology and psychology in pain management, particularly relevant to women's health issues, such as bladder dysfunction, IBS and persistent pelvic pain. I'm so pleased to share this conversation with high performance rehab specialist, SJ McDonnell, who is leading the charge to integrate pelvic health into sports medicine at Sport Ireland.Our paths crossed many (many) years ago but we reconnected when SJ and I started having conversations about how to bring pelvic health into high performance sports - how to bring expertise into this field in a thoughtful way.We discussed lots of different ways to empower and educate athletes, coaches & medical staff about assessing and managing pelvic health conditions, including:the importance of the MC in sports & the challenges of bringing a nuanced approach to menstrual healththe level of information about pelvic health that is available to athletesthe difficulty with the diagnosis and management of LEA or dysmenorrhea in athletes who have limited access to medical appointmentsthat athletes may not seek help for a variety of reasonsand how a one size fits all approach to menstrual health is not a good strategy & how more training & understanding are required to improve the skills practitioners need to better support female athletesUltimately, there is a definite need to do a better job at integrating pelvic health into sport at every level, with the inclusion of psychological health into this domain as well, as there is a complex interplay between physical performance and mental wellbeing. SJ is leading a great team at Sport Ireland and is driving the conversation about creating a safe space for athletes to discuss menstrual & pelvic health. I had the pleasure of seeing Ros present on this topic at the POGP conference in Edinburgh last October.In today's episode, we discussed pelvic health in sportsthe 6Rs framework for postpartum athletes and its application to broader pelvic health considerations. (Donnelly et al 2022 Reframing return-to-sport postpartum: the 6 Rs framework)we also explored how pelvic floor dysfunction can contribute to groin pain in athletes and emphasized the importance of integrating pelvic health into mainstream sports medicine practice. The conversation also covered ACL injury prevention in female athletes, with Ros highlighting that while hormonal factors may play a role, they are just one aspect among many including biomechanics, training environment, and overall athlete resilience. Hello & Welcome to today's episode, where I'm chatting with Dr Melissa Davidson about prolapse, avulsions and pessaries!My guest today is a specialist physiotherapist in pelvic health, and we discussed her career journey, including her specialization in pelvic health in New Zealand, where she is the only registered specialist. She highlighted her expertise in advanced clinical practice, research, and leadership within the profession. Melissa also shared her experience conducting research in bioengineering at Auckland University, which revealed that physiotherapists' assessments of muscle tone and stiffness lack objective measurement methods, challenging traditional practices in the field...We discussed the importance of evidence-based practice in pelvic health physiotherapy, particularly regarding prolapse and pain management. Melissa shared her experience conducting a PhD in bioengineering, which challenged many accepted beliefs in physiotherapy and highlighted the need to think outside traditional silos. Melissa shared her experience developing and using a 3D model named Lily for patient education, discovering its effectiveness in helping patients understand complex medical concepts. She discussed her collaboration with bioengineers, noting that while the engineers were initially skeptical about her approach, they eventually recognized the value of her clinical perspectiveWe explored the diagnosis of levator avulsion and emphasized that a definitive diagnosis should not be made before 6-12 months postpartum, and we highlighted the importance of careful wording when communicating with patients about this condition, as the diagnosis can be devastating and there is currently no surgical fix.Melissa discussed her approach to pessary management and training for physiotherapists, emphasizing the importance of medical clearance and speculum exams for assessing mucosal integrity. She explained the assessment process for avulsion injuries, including the use of a training model and peer-to-peer learning. Melissa also highlighted the need for informed decision-making during pregnancy regarding delivery options, advocating for patient autonomy and open discussions about birth plans.We definitely agreed on the importance of using validated research and scientific terminology, rather than relying on subjective assessments or unproven treatments.This was definitely a deep and rich conversation, that I really enjoyed...and I hope you will too!Want to learn more about prolapse and other perinatal pelvic health issues, from a whole woman, evidence based, clinical reasoning from assessment to management perspective? Winter is here in the UK, so while Saul and Lucy hunker down, don layers and fire up the kettle more frequently than usual, what do their minds turn to in the garden? This is the perfect season for clearance of 2025 growth, for establishing new designs and for de-cluttering stores and greenhouses. Any hours that you can put into the garden now, will reward you hugely come spring and summer, when the jobs mount up. Quick - more tea and biscuits!And they're off! While Saul enjoys his new life in a warm, dry office, Lucy is dodging the weather whilst still attempting to earn a crust (thank goodness for gardening radio and magazines :-). Snowfall and storm Goretti force her to dance the merry winter waltz, as she sidesteps sleet and soggy spells. Her body also reminds her that two weeks of doing very little physically, followed by intense compost hurling then a rapid cooldown, spells a date with Mr Nurophen and Master Wheatbag. It is indisputably history's greatest crime against humanity that some 380 million Christians around the world are being heavily persecuted.  Events of the past few months offer hope that leaders of the civilized world are, at last, going to defend those persecuted simply for following Jesus Christ. Notably, President Trump warned jihadists in Nigeria, including its government, to stop such attacks or face U.S. military action – which he unleashed shortly before Christmas. Some Syrian jihadis butchering Christians and other minorities also faced American military retribution.  And Israeli Prime Minister Benjamin Netanyahu recently declared that, “We are joining an effort to have basically a united nations of countries that support Christian communities around the world. This is…a main part of our agenda and it's going to continue with greater force and greater might in this coming year.” Onwards Christian – and Jewish – soldiers! This is Frank Gaffney.
