You receive a page from labour ward. A woman at 35/40 weeks gestation has just arrived in the hospital very distressed in a lot of pain. A quick bedside ultrasound by the obstetric team has unfortunately demonstrated a large abruption and fetal death in utero. She is contracting strongly and beside herself in pain, the team would like you to come down and place an epidural for analgesia. The team are hoping she will deliver vaginally in the next few hours. What is your approach in this situation? Join Graeme and I as we discuss this complex and challenging clinical condition and the coagulopathy that can occasionally occur. Meghan Murray and her husband decided to have one more baby. She'd already experienced beautiful vaginal births with her first two boys and, like so many women, she mentally went through all the things she could do this time around to get things just right. In preparing for her dream birth, Meghan hired new midwives who practiced outside of a hospital setting and seemed perfectly aligned with her wishes. She remained fit, practiced Spinning Babies and learned relaxation techniques for a comfortable physiologic birth. She savored her pregnancy knowing it was her last, and excitedly planned her first water birth. At 36 weeks, Meghan suddenly heard a pop and assumed her membranes had released. But instead of amniotic fluid, she saw blood -- and lots of it. Eir got pregnant unexpectedly after years of suffering from endometriosis. During her pregnancy, she had two partial placental abruptions and was put on bed rest. At about six months pregnant, Eir began to have severe pain in her abdomen. She tried multiple times to call her doctor, but was written off as a first-time mom who was experiencing ligament pain. Even with her history of TWO partial abruptions, her pain was not taken seriously. When Eir went into labor, her water broke first. Contractions never started, even with Pitocin. But the pain in her abdomen got worse. When they checked her baby's heart rate, they knew something was wrong. Eir was rushed to the OR for an emergency c-section, where they discovered she had a severe infection. She went septic, clinically died, and was brought back. Due to hospital negligence, her daughter almost died as well. Eir shares how crucial it is to stand up for yourself and your baby, even to medical professionals who think they know your body better than you do. She stresses the importance of having a great support team to help advocate for you, and how you can still create a life that you will love, despite past trauma. On today's show we hear from Northern River's artist and mother of two, Jo. Being a yoga teacher when she fell pregnant with her first baby at 21, helped Jo connect with her innate instincts and trust in physiological birth. She loved the continuity of care she received from the midwives at her local birth centre where she was planning to birth her baby, but at her 34 weeks scan concerns were raised about her high blood pressure. A preeclampsia diagnosis and placenta abruption sent her straight into theatre for an emergency c-section at 34+2 weeks. After that experience she dove deeply into research, educating herself to safely navigate her second pregnancy and avoid preeclampsia, or any other complications. Although she desired a homebirth, she needed to regain the dimmed trust in herself; choosing to birth in hospital, armed with the knowledge she gained to fight for the VBAC she knew was possible. This week Amy and her mum Janene a midwife join us to share her extraordinary three birth stories. Her first a vaginal birth, her second a caesarean birth, and her third a VBAC with twins.This episode has a trigger warning as baby loss is discussed.Amy's first pregnancy was thankfully straightforward with a positive induction experience followed by a vaginal birth under Midwifery Group Practice at her local hospital. Having her mum's guidance she was well informed ahead of time, however she was faced with breastfeeding struggles following her first birth and the positive birth experience was marred by the passing of her father from Motor neurone disease, a battle he had fought for many years.For her second pregnancy Amy was planning to have a homebirth through her hospital's birth center homebirth program and looked forward to this experience. However at 36 weeks her pregnancy took an unexpected turn. Amy experienced a concealed placental abruption and she was taken for an emergency caesarean.Her son Arlo was born premature but healthy and she was discharged from hospital, but at five days of age they returned to hospital as his health began to deteriorate. It was found he had contracted a common cold virus. Arlo was airlifted to Westmead Hospital in Sydney from Newcastle and in the coming weeks Amy and her husband would stay by his side while he was fighting for his life.Amy tells of the unspeakable grief of losing Arlo and the 29 days of his life which was cut short far too soon. Symptoms include vaginal bleeding, belly pain, and back pain in the last 12 weeks of pregnancy. Depending on the degree of placental separation and how close the baby is to full-term, treatment may include bed rest or a cesarean section. Raquel Caffet, from Spiritwood, Saskatchewan, joins me on this episode of Moony Birth Stories to share her four pregnancy and birth stories.  Her first pregnancy was a surprise that happened early on in her relationship with her now husband. It was a smooth pregnancy but Raquel was very fearful of pain as labour approached.  After reaching 10 days overdue, she was induced and delivered her first daughter after epidural complications and a long period of pushing.  Baby number two was also a surprise for the couple and Raquel had another smooth pregnancy.  Again, overdue, she was induced and delivered their second child but had a placental abruption.  After this birth, Raquel struggled with a period of postpartum depression and was able to work through it with the support of a councilor.  With baby number 3, Raquel had a much more difficult pregnancy suffering many more symptoms and the baby remained breach until the end of pregnancy.  Raquel was again induced and had a successful epidural for this birth.  Her final pregnancy and birth happened during the Covid-19 pandemic which made for a much different experience as she wasn't able to have many in-person visits with her healthcare providers.  She developed gestational hypertension later in the pregnancy and was ultimately induced before her due date.

This is the first partner perspective episode of Moony Birth Stories!  It features Rob Horsely, sharing his wife's pregnancy and birth stories of his three children from his perspective.  The pregnancy with their first, Charlotte, was pretty smooth and they had a relatively uncomplicated vaginal birth in hospital but the epidural didn't work.  Sadly, after their first daughter, they suffered two miscarriages before becoming pregnant with their twin sons.  This pregnancy was much more challenging and they encountered a few scares along the way, including two partial placental abruptions during pregnancy.  The boys, Sebastian and Connor ended up being born in hospital at 34 weeks and required some time in the NICU.  Rob shares his experiences navigating this journey with his wife Courtney.

The condition can deprive the baby of oxygen and nutrients. Symptoms include vaginal bleeding, belly pain, and back pain in the last 12 weeks of pregnancy. Depending on the degree of placental separation and how close the baby is to full-term, treatment may include bed rest or a cesarean section. According to the American Pregnancy Association (APA), if severe abruption occurs, 15% ends in fetal death. Infants who survive have a 40-50% chance of developing long-term health complications. Today’s guest had a placenta abruption at 36 weeks gestation due to preeclampsia. Vanessa Robinson experienced severe bleeding losing 4 liters of blood and needed blood transfusions to survive. Her son died 4 minutes into being rushed to the hospital from severe bleeding. In this episode Vanessa takes us back to the moment she knew something was wrong, how she grieved her loss while fighting for her life, and how depression post loss lead her to research and start her own nonprofit to bring awareness, support, and education about the racial disparities against women color in the medical field. In this episode we meet Ashleigh who experienced three very different births in a Sydney public hospital under the caseload midwifery care model. Ashleigh suffered from Hyperemesis Gravidarum in her first pregnancy only. She went into preterm labour in her first birth at 36 weeks and was prepped for a caesarean after her son was showing unexplained foetal distress. She was taken into theatre where it was found that she had suffered from placental abruption during the labour. This occurs when the placenta detaches from the uterus.Following this experience Ashleigh was steered towards the repeat caesarean path by her hospital during her second pregnancy but decided that caesarean recovery and surgery was not what she wanted. Ashleigh tested Group B strep (streptococcus) (GBS) positive in this pregnancy.It was her caseload midwife who supported her desire to achieve a natural birth and at 39 weeks she delivered her second baby via a successful VBAC. This birth left Ashleigh on cloud nine, and when she fell pregnant two years later with her third baby she assumed the same kind of birth would unfold. But her third birth was very different to what she had anticipated.Ashleigh’s story isn’t your usual VBAC birth story, it’s so much more. It’s about how Ashleigh confronts the trauma from her third birth where these factors were truly out of her control and how she reconciles this with the feeling of complete control that she had during her VBAC. In today’s episode I chat to Gemma Pranita about her second pregnancy and birth with her baby boy, Iggy. Gemma is the first to admit that pregnancy is not her friend and while she had a challenging time in her first pregnancy (listen to the story in ) her most recent experience brought her to her knees. She was overwhelmed with severe nausea and vomiting till 18 weeks, she then developed early onset PUPPPs rash which resulted in weeks and weeks of severe itching and subsequent sleeplessness and in her third trimester pubic symphysis and pelvic separation made walking very painful. The start of Gemma’s pregnancy was an entirely different story. With her daughter, Raffaella, she spent close to two years trying to conceive, before she resorted to Intrauterine Insemination IUI (she describes it as a beautiful stepping stone to IVF). Gemma admits that unexplained infertility is a very frustrating journey and not one she would wish upon anyone: “You want to know what the problem is so you can find the solution and when you’re told there’s nothing wrong and you’re just in the waiting zone…you just want to bang your head against the wall.”
