The difference between forceps (which look like fancy salad tongs) and vacuum devices (like the KiwiVac) was explained—with forceps generally having a higher risk for maternal tearing, while vacuums can be safer for the mother but have their own set of risks for baby. Forceps use is becoming rare in the US, and many younger doctors are not trained in both tools; most providers specialize in one over the other. The vacuum method, such as the KiwiVac, doesn't just involve pulling—the device helps rotate and flex the baby's head to ease passage through the pelvis, working in tandem with the mother's pushing effort. Not every provider can use both vacuums and forceps, and midwives in the US generally use vacuums regionally; outside the US, like in Europe, midwives may use vacuums more routinely. Assisted deliveries make up less than 5% of births, so most people will not need them, but knowing about the process can reduce fear if the situation arises. Before offering assisted delivery, doctors must ensure the baby is low enough, the mother's pelvis is adequate, and water is broken—these tools can't compensate for a truly “stuck” baby or incomplete dilation. Vacuums and sometimes forceps can even be used during C-sections if the baby is deeply engaged or in an awkward position, to help bring the baby up through the uterine incision. Consent and clear communication are vital—sometimes decisions have to be made quickly, so it helps when patients have discussed these possibilities in advance with their providers. I remember hitting peri-menopause in my 40's and having no clue what was happening and that there was help! Fast forward a little bit from there and I met Dr. Mary Claire Haver who is not only an expert but also on a mission to help women through all phases of menopause. Let's meet Dr. Mary Claire Haver:As one third of women are currently in some phase of menopause, it is an essential aspect of women's lives, yet so stigmatized and shrouded with shame and steeped in anxiety over aging that many women still don't feel comfortable talking about their menopause journey with friends, family, and most crucially, their doctors. Kept in the dark, menopause becomes more difficult to understand and control, leaving many women to suffer in silence. Dr. Mary Claire Haver is here to change all of that. Recently the Board Certified OB-GYN has set TikTok alight with an unlikely subject---menopause---amassing nearly 2 million followers by posting advice for women going through it. Dr. Haver understands that women's healthcare is indire need of a change. So often, when women talk to their healthcare providers about issues such as weight gain, sleep changes, hot flashes, and mental health issues, they are dismissed with platitudes like "it's just your hormones" or it's "just your age," and given the advice to "eat less and exercise more."The problem has led Dr. Haver to develop The Galveston Diet, a three-pronged lifestyle plan that encourages fuel refocusing, intermittent fasting, and anti-inflammatory nutrition to manage hormonal symptoms, stabilize weight, and revitalize the body as it ages to provide benefits that will last a lifetime.Dr. Haver has always had a passion for learning and a strong work ethic. She also went on to get a Masters of Public Health degree from UCLA in Community Health Education. Dr. Liz is also a hormone consultant/midlife health expert. She has helped women and men increase energy, reignite their sex drive, clear up hormonal imbalance, and lose hundreds of pounds. She worked with adults and children to manage chronic health conditions and obesity. She developed curricula for group nutrition interventions for children, pregnant women and adults. She is the founder of PollyPrep, a company aimed at educating and treating people with PCOS using an evidence-based mix of Culinary Medicine and Gynecology.She recently joined The Mount Sinai Health System in New York where she holds the position of Assistant Professor in Obstetrics, Gynecology and Reproductive Science. Previously she practiced at Weill Cornell Medicine, and NYU Langone Brooklyn where she served as the medical student Clerkship Director, as well as Associate Residency Program Director and she led the peer review committee for the department.Dr. Faris practices General Obstetrics and Gynecology and is a fellow of the American College of Obstetrics and Gynecology.
