Alexis Sweda Rose is on the podcast to share her heartbreaking story of the loss of her beautiful baby, Brock Tyler, through stillbirth at 29 weeks. Honestly, this is a very hard episode to listen to or watch but I cannot commend Alexis enough for her bravery and vulnerability in sharing her story. It is her goal to spread awareness of stillbirths, the importance of keeping these babies names alive in support of your friends or family who have gone through this and will go through it the rest of their lives, the importance of having full confidence in your medical team, the confidence in knowing if you call your doctor with a medical question they may find silly, that they fully support you and give you the advice and comfort you need. I can't recommend my OGYN enough as you hear on my previous episodes. I know from personal experience, if you are pregnant this can be a hard listen, but Alexis does give advice on things that she was told were “normal” in which in fact they were not normal. She also openly shares about her marriage and how this loss has made them stronger than ever and gives great advice to those who are also going through this. We talk about Alexis's wedding day and the crazy windstorm that came through and a sign that was given from God, her pregnancy, her prenatal care, the whole story of losing her dear baby, Brock Tyler, at 29 weeks, how the hospital handled the death of her child moments after he died, and more. There are moments of laughter and happiness because Alexis is a bright light and her husband Matt is a hoot. One in five women in the U.S. have a BMI of 30 or more at the START of pregnancy. Around 1 in 5 women gain more than 40 pounds during pregnancy, which is more than any woman should gain. Only about one-third of women gain the recommended amount of weight during pregnancy. Gaining too much weight during pregnancy can increase the risk of HDP, GDM, fetal macrosomia, and can cause complications of birth, such as shoulder dystocia or preterm birth. Excessive weight gain during pregnancy can also increase the likelihood of postpartum weight retention. But what about stillbirth risk? Does excessive maternal weight gain during pregnancy increase still birth risk? The ACOG recommends antepartum fetal surveillance based on pre-pregnancy BMI. Why is maternal weight during pregnancy not an indication for an antepartum fetal surveillance? The data may surprise you! Listen in for details.

Few knew that Fred and Krystal were expecting their seventh child. At 24 weeks, their beautiful daughter's heartbeat could no longer be detected. On January 21, 2025, with heavy hearts, they said goodbye as Krystal delivered her in the hospital. Around a third of a person's life is spent asleep. Previous studies have reported an association between sleep disordered breathing, like OSA, and pregnancy complications such as pre-eclampsia, gestational diabetes, and preterm birth. We recently discussed a stillbirth bundle from Australia which includes patient education on sleeping practices and stillbirth risk. Does maternal sleeping position ready influence stillbirth risk? Some data says NO, but there's a catch to those. Other population level data says YES. Why the difference? And why is the position the mother STARTS to sleep very important here? The stillbirth rate in the US is considerably higher than in many countries with similar resources. In 2021, the United States stillbirth (loss before birth at ≥20 weeks' gestation) ratio was 5.73 per 1000 births or 1 in 175 pregnancies with 21,000 stillbirths occurring annually. Contrast that to the rate in Japan which is 1.6/1000! The UK and Australia have both implemented stillbirth prevention bundles which have proved worthwhile. It is long past due for the US to have its won national stillbirth prevention bundle. In this episode we will review a publication from Aug 2024 (AJOG) describing this bundle proposal and highlight a letter to the editor from January 9, 2025 in the AJOG in response to that August publication.
