Unfortunately, there simply isn't a one-size-fits-all treatment protocol for patients infected with Lyme disease and/or co-infections. This is why it's critical for physicians treating Lyme disease to invest time with patients, thoroughly understand their medical history, and closely monitor symptoms and treatment response. With that in mind, there are currently two different treatment approaches for Lyme disease. The Infectious Disease Society of America (IDSA) and the International Lyme and Associated Diseases Society (ILADS) have each published their own set of evidence-based treatment guidelines. IDSA guidelines recommend a short course of antibiotics, typically 14 to 30 days. IDSA argues that the Borrelia burgdorferi bacteria do not persist in a patient beyond this timeframe and that lingering symptoms are the result of an ongoing immune response and not an active infection. It also cites scientific evidence claiming treatments beyond 30 days are ineffective, unnecessary, and even dangerous. IDSA physicians will stop treatment after 30 days, even if symptoms remain. They advise an additional 30 days of treatment recommended for patients with Lyme arthritis. On the contrary, ILADS offers its own scientific data to show that a additional treatment with antibiotics is required to eradicate the bacteria. ILADS recognizes that a month of treatment may be sufficient for patients in the acute stage of Lyme disease, but in cases where the spirochete has disseminated and the disease has advanced, a 30-day treatment regimen is inadequate. ILADS guidelines recommend additional antibiotics until a patient's symptoms have been resolved. Treating Lyme disease in its advanced stage can be complicated based on the complexity of the organism itself, differences in each patient's immune system, the length of time infected, and the possible presence of other co-infections transmitted by the same tick. There are several choices in treating Lyme disease, which include oral, intravenous, and intramuscular antibiotic options. Other options may include sequential antibiotic therapy, higher doses of antibiotics, taking antibiotics for a longer period of time, a combination of antibiotics, retreatment, as well as diagnosing and treating co-infections. Some specific antibiotics used in treating Lyme disease are doxycycline, minocycline, amoxicillin, cefuroxime, azithromycin, and clarithromycin. Other tests include measures of blood counts, chemistries, liver function tests, ANA, dsDNA, RF, TSH, free T3, free T4, ESR may be helpful at ruling out other conditions. Referral to specialist might help to rule out other conditions. I find shared decision with my patient helpful. I also find follow-up helpful to assess my patient's response to treatment to rule out other conditions. There are additional protocols that may also aid in treating Lyme disease, such as avoiding alcohol, simple and processed sugars, exercising as tolerated, counseling for a Jarisch-Herxheimer reaction, managing symptoms, monitoring and reducing the risk of an adverse event, and reducing stress. However, there is a chance of side effects such as Clostridium difficile-associated diarrhea (CDAD). Probiotic have been prescribed with the hope of reducing the risk of developing CDAD.

In episode 57, we had in depth discussion of safety issues, general use and recommendations for PPI therapy. In this episode, we briefly review PPI safety concerns and focus on strategies to deprescribe proton pump inhibitors. Key Concepts PPIs are approved as acid-reducing therapies to treat various conditions related to decreased gastric pH, however, undocumented use of PPI has increased over the years. For indicated conditions, PPI should be used at lowest dose for shortest duration possible. When used for longer than intended duration, PPIs may cause long-term issues such as hypomagnesemia, low vitamin B12 levels, low iron levels, anemia, C. difficile-associated diarrhea (CDAD), chronic kidney disease (CKD), and hypergastrinemia causing rebound acid hypersecretion. Options for limiting PPI use are: abrupt discontinuation, dose reduction, on-demand dosing, slow deprescribing, or switching to alternate therapy. Abrupt discontinuation leads to rebound hypersecretion due to high gastrin levels. There is lack of substantial evidence for what deprescribing strategy is better, but decreasing from multiple times a day to single daily dose, reducing the single daily dose, and switching to every other day are all appropriate strategies. Clostridium difficile (klos-TRID-e-um dif-uh-SEEL), often called C. difficile or C. diff impacts 453,000 people every year. And with 29,000 associated deaths, it takes more lives than AIDS and drunk-driving combined. Yet, most people have never even heard of it. That's a big problem because you can't protect yourself from a threat when you don't even know it exists. The impact on the business of healthcare is significant too. A study published in the American Journal of Infection Control found that C. diff-associated diarrhea (CDAD) increases hospital costs by 40% per case (an average of $7,285 ) and puts those infected at high risk for longer hospital stays and readmissions. Some even believe those numbers are likely underestimated. C. diff presents us with an interesting problem at the cross-section of Infection Prevention and Control (IPAC). It's an environmental bacterium that's found pretty much everywhere and is difficult to kill, but it's usually held in check by the good bacteria in our bodies. The problem usually occurs when a patient is in a weakened state from some other healthcare intervention. That may be an antibiotic treatment for another healthcare-associated infection or chemotherapy, etc. With our bodies in a weakened state and our good bacteria depleted by antibiotics, we become susceptible to C. diff. So, it's important that we avoid unnecessary antibiotics and execute on all the other IPAC practices like proper hand hygiene and surface cleaning in the hospital so that, as our guest puts it, we can disrupt the chain of events that allow to C. diff to proliferate. PLF is building a nationwide C. diff awareness movement by educating the public, empowering advocates and shaping policy. He was previously managing director of prospect strategy & research at Teach For America. With more than 15 years' experience as a frontline fundraiser and behind-the-scenes strategist for healthcare, LGBT rights, and education organizations, he led prospect identification and research programs that increased resource development both nationally and regionally. Prior to joining Teach For American in November 2009, Christian served in a variety of roles for a diverse group of nonprofits, including as Director of Development for In The Life Media, Director of Major Gifts for the National LGBTQ Task Force, and Associate Director of Development Research for NYU Langone Medical Center. Throughout his career, Christian has been a frequent speaker at conferences including the Mid-Atlantic Researchers Conference and Creating Change: The National Conference on LGBT Equality. He was also a contributor to Prospect Research: A Primer for Growing Nonprofits by Cecilia Hogan.
