I'm recently back from the 2025 ACDIS national conference, and as usual flush with the latest in CDI trends, education, and breaking news. And right at the top of my takeaways is a seemingly innocuous classification with big ramifications: admit type. What makes this a big deal? Some hospitals appear to be playing a bit fast and loose with guidance from the National Uniform Billing Committee (NUBC) in order to classify surgical admits as “urgent” rather than the more accurate “elective.” Doing so removes them from certain PSIs that negatively impact quality metrics and indirect revenue. My guest is Penny Jefferson, manager of clinical documentation integrity at UC Davis Health. Penny co-presented the session with Cheryl Ericson at the ACDIS conference. On this show we discuss: What is admit type, why is it important, and common misunderstandings (admit type is very different than admit status--IP/OP/observation) Who is the NUBC, and what are the current rules as they stand around elective, urgent, and emergent? Compelling data—deidentified, but real—presented at ACDIS that shows what appears to be clear gaming by some healthcare organizations of the assignment of admit type, specifically opting for urgent over elective The dramatic impact this seemingly small change can have on quality scores: Reclassifying an elective procedure as urgent effectively circumvents PSI exclusions, allowing the case to be excluded from elective-only quality measures, such as PSI 10, 11, or 13. En France, en 2024, plus de six millions de CDI ont été signés au premier trimestre de l'année , c'est 845 000 de plus qu'en 2019 selon l'URSSAF. Pourtant, selon une étude du Centre de recherche pour l'étude et l'observation des conditions de vie, 45% des 18-24 ans ne voudraient plus de ce graal et préféreraient travailler à leur compte. Ancien éducateur spécialisé, formé à la gestion des ressources humaines et titulaire du CAFDES, il partage une vision engagée et profondément humaine du management en EHPAD.Nous parlons d'abord de gestion des ressources humaines, non pas comme une suite de contraintes administratives, mais comme la base du fonctionnement d'un établissement et de son équilibre budgétaire. Pascal évoque l'influence des écoles de relations humaines, et notamment l'idée qu'un salarié écouté et responsabilisé est un salarié plus engagé.L'organisation du travail fait l'objet d'un soin tout particulier : chaque professionnel peut choisir son jour de repos fixe et ses horaires selon son rythme personnel. Le planning tourne sur deux semaines pour favoriser l'équilibre vie pro/vie perso. Résultat : un turnover réduit de 24 % à 4,55 %. Pascal insiste sur la confiance accordée aux équipes. A flurry of national news reports continues to document the large-scale restructuring taking place at the U.S. Department of Health and Human Services (HHS), where 10,000 jobs were reportedly slashed earlier this month, a move also impacting the workforce at the Centers for Medicare & Medicaid Services (CMS). En France, la durée moyenne des arrêts de travail connaît une hausse notable, atteignant en 2024 un niveau record depuis la crise sanitaire liée à la Covid-19. Selon une étude publiée par le groupe Diot-Siaci, cette durée moyenne s'élève à 21,5 jours pour les salariés en contrat à durée indéterminée (CDI), soit trois jours de plus qu'en 2022. Cette tendance s'inscrit dans une dynamique globale d'augmentation de l'absentéisme dans les entreprises, qui se manifeste aussi bien par des arrêts courts et fréquents que par des arrêts de longue durée.Les arrêts longs, c'est-à-dire supérieurs à 90 jours, représentent désormais plus de la moitié des absences. Cette situation est en partie liée à la montée des risques psychosociaux, comme le burn-out ou la dépression, souvent causés par le manque de reconnaissance, un sentiment d'injustice ou encore un déséquilibre entre vie professionnelle et personnelle. Ces facteurs touchent particulièrement les jeunes salariés, notamment ceux de moins de 35 ans, qui sont trois fois plus concernés par l'absentéisme perlé (arrêts courts mais répétés) que leurs collègues plus âgés.Les causes des arrêts de travail sont multiples. La maladie ordinaire (grippe, bronchite, gastro-entérite, etc.) reste la principale, invoquée par 54% des salariés arrêtés. Cependant, d'autres facteurs prennent de plus en plus d'importance. La fatigue est mentionnée par 37% des salariés, un chiffre qui grimpe à 48% chez les moins de 25 ans. Les troubles musculo-squelettiques et les troubles liés à la santé mentale jouent également un rôle grandissant dans les arrêts maladie.On observe également des disparités selon le genre : en 2024, les femmes présentent un taux d'absentéisme plus élevé que les hommes. Ce phénomène s'explique notamment par la nature des postes occupés, qui peuvent avoir un impact plus fort sur leur santé physique et mentale.Enfin, le télétravail apparaît comme un levier efficace pour limiter les arrêts. En effet, 67% des salariés en télétravail déclarent que cette possibilité leur a permis d'éviter un arrêt maladie. Toutefois, cette solution n'est pas accessible à tous, en raison de fortes inégalités socioprofessionnelles.En somme, les arrêts de travail en France s'allongent en durée, sous l'effet conjugué de problèmes de santé classiques, de troubles psychosociaux croissants, et d'une organisation du travail parfois mal adaptée aux attentes des salariés. I've been in the coding and CDI worlds since 2004, and in that time watched the rise of Facebook, YouTube and other social media.
