Clair Obscur – Expedition 33 !
In this episode, I speak with Ian Mitchell, the lead author of the 2025 Commitment to Development Index (CDI), about what this year's results reveal. We discuss why many countries have slipped in their overall contributions to global development, where progress is emerging, and how the CDI can guide smarter, more coherent policymaking across trade, migration, climate, technology, and development finance at a time of increasingly constrained budgets. Ian also reflects on the major trends shaping this year's index and urges policymakers and advocates to look beyond aid when thinking about how to drive real impact for the world's poorest countries.
In this episode of the AgNet News Hour, commodity expert and fourth-generation farmer Nick Foglio of Foglio Commodities delivers one of the most insightful breakdowns of California agriculture heading into 2025. From hay and forage markets to dairy economics, water uncertainty, and the global forces shaping local prices, Foglio explains why producers across the Central Valley and Imperial Valley are facing unprecedented volatility. He covers early-season pricing standstills, the impact of the latest CDI milk price drop, and the ripple effects that global commodity shifts have on feed markets. Foglio also provides a grounded outlook on the upcoming almond season, forecasting a more realistic 2.5–2.75 billion pounds—far below early, panic-inducing estimates. He discusses land values, long-term crop investment strategy, and the pressures younger farmers face in today's ag economy. The conversation expands into broader issues affecting California's future, including water planning, housing affordability, fire prevention, and the need for stronger leadership. Packed with real-world experience and no-nonsense analysis, this episode is essential listening for farmers, dairymen, ag professionals, and anyone who wants to understand the fast-changing landscape of California agriculture.

Since 80 percent of a person's health is influenced by factors outside of medical care, it is critical that a healthcare system has an understanding and appreciation for the circumstances of patients' daily lives that impact their health outcomes, referred to as the social determinants of health (SDoH). Linda Schatz, Director of AKASA, explains the role of Clinical Documentation Integrity (CDI) specialists in ensuring accurate coding and bridging the gap between clinical documentation and specific, accurate codes to ensure proper reimbursement. The complexity of medical coding often leads to errors, which can be nearly eliminated by using AI to review 100% of patient encounters to identify inconsistencies and help CDI and coding professionals process more accurate claims quickly. Accurate documentation is important for hospital revenue, patient care quality, and perception of the hospital's performance. Linda explains, "Well, the old adage, if it isn't documented, it wasn't done. If the doctor uses incorrect or perfectly acceptable medical terminology, it doesn't translate into an appropriate code. You've heard the term UIs, this is years ago, right? Grandma had UTIs and died. In the coding world, that used to code for a simple UTI. So the hospitals are getting paid for a patient that took care of a UTI, when in reality that patient was septic. To the outside world, it looks like Grandma came to the hospital, something that could have been treated outpatient, and she died. So the public perception of quality is less. So not only is it revenue, it's quality, but ultimately it's delivering patient care." "I'm an old nurse. I've been in this field for over 40 years. I've worked across the NICU, PICU, and adult ICU. I've worked in access hospitals to large academics and all the way through hospice. That's pretty unique as a nurse to have that big of a background. Then I became a CDS, or clinical documentation specialist, or integrity specialist, and learned the documentation and coding aspect." "Then I moved into the consulting role and worked with organizations and physicians all across this nation, helping them learn how to do this. And so you've got the clinical background, the coding background, and now I understand how generative AI works. And so while you're a new nurse, you're a horse, right? When we hear a heartbeat, we think of a horse, and after years, you earn your stripes and you become a zebra, and then you add all of these multiple areas of expertise, you become uniquely valuable as a pink zebra." #AKASA #GenAI #CDI #RevenueCycleManagement akasa.com Download the transcript here

It's often said that “words matter.”And hospital patient status assignment is no exception. What do your teams say when a patient is in inpatient status which isn't supported? How about the reverse: when a patient is in outpatient status with observation services. Can your team change to inpatient? Words matter, and Dr. Juliet Ugarte Hopkins, chief medical officer for Phoenix Medical Management, Inc., will explain precisely how impactful your choices can be.During the next live edition of Talk Ten Tuesday, Dr. Juliet Ugarte Hopkins will report on this timely topic. The broadcast will also feature these instantly recognizable panelists, who will report more news during their segments:Social Determinants of Health: Tiffany Ferguson, CEO for Phoenix Medical Management, Inc., will report on the news that is happening at the intersection of medical record auditing and the SDoH.CDI Report: Cheryl Ericson, Senior Director of Clinical Policy and Education for the vaunted Brundage Group, will have the latest CDI updates. The Coding Report: Christine Geiger, Assistant Vice President of Acute and Post-Acute Coding Services for First Class Solutions, will report on the latest coding news.News Desk: Timothy Powell, ICD10monitor national correspondent, will anchor the Talk Ten Tuesdays News Desk.
No matter how great your CDI program is—its spectacular workflow, outstanding chart review team, on-point KPIs, shiny new AI tools—nothing works if you don't have engaged physicians. It still all comes down to provider engagement. Without a physician staff who is bought in and willing to participate and document with specificity in the health record, all these efforts are for naught. But with great engagement, great things are possible. So where are we today with the big daddy of all CDI topics? Joining me on this episode of #OTR are two physicians with considerable experience and plenty of war stories. Trey LaCharite, Medical Director for CDI and Coding and Clinical Associate Professor for University of Tennessee Medical Center, and Vaughn Matacale, director of the physician advisor group for ECU Health in North Carolina, open up for a frank, no-holds barred discussion on the following topics: What is overrated when it comes to provider engagement--and what is underrated? The best high-tech solution each recommends, and a great low-tech solution that stands the test of time. An ultimate success story winning over a difficult provider or service line. Notable failures others can learn from. RUSH reunion tour in 2026—yay or nay? Spoiler alert: Of course the answer is yes... And other fun stuff you really shouldn't miss...

Welcome to “Is Ambient Listening Right for You?” Or, in the alternative, “Free AI Tools and Other Expensive Mistakes.”Nick van Terheyden, MD, the special guest during the next live edition of Talk Ten Tuesdays, says he has seen enough of artificial intelligence (AI) “helpers” turn doctors into unpaid editors and part-time exorcists for hallucinating software.“Clinicians signed up to heal humans, not debug bots,” Dr. Nick said. “Yet here we are, creating ‘AI slop' so thick it needs its own billing code.”Dr. Nick also references a Harvard study that says your coworkers think using AI makes you less competent; meanwhile, MIT says you're probably not getting a dime's worth of value. But don't worry, you're getting your AI tool for free! Which, like the “free drinks” at an all-inclusive resort, often tastes mostly like disappointment. Finally, Dr. Nick offers this parting shot: “Remember: your medical records can't be ‘vibe-coded' clean. And while lawyers blame the AI for their made-up filings, you don't get that luxury. Choose your AI tool wisely, because ‘no hallucinations added' shouldn't have to be in your note.”The broadcast will also feature these instantly recognizable panelists, who will report more news during their segments:CDI Report: Cheryl Ericson, Senior Director of Clinical Policy and Education for the vaunted Brundage Group, will have the latest CDI updates.Social Determinants of Health: Tiffany Ferguson, CEO for Phoenix Medical Management, Inc., will report on the news that is happening at the intersection of medical record auditing and the SDoH.The Coding Report: Christine Geiger, Assistant Vice President of Acute and Post-Acute Coding Services for First Class Solutions, will report on the latest coding news.News Desk: Timothy Powell, ICD10monitor national correspondent, will anchor the Talk Ten Tuesdays News Desk.
Whistleblower lawsuits are pulling back the curtain on risky coding practices and costly compliance gaps in risk adjustment. From inflated RAF scores to vendor oversight failures, the consequences are real and growing. This episode unpacks several DOJ cases that reveal how coding shortcuts, unsupported diagnoses, and non-compliant queries can escalate. From internal governance gaps to coding-policy breakdowns, the message is clear: vendor oversight is a necessity.Listen now to uncover what these cases reveal and what your plan can learn from these whistleblower lawsuits.ABOUT OUR GUEST: Kristi Reyes is the Director of Risk Adjustment Coding Operations. She leads a team of 250 medical coders from various aspects of risk adjustment—including retrospective, prospective, CDI, QA, risk mitigation, and RADV. Kristi holds Certified Professional and Outpatient Coding, Risk Adjustment Coding, and Auditing certifications.
As centres of learning, universities should be places where ideas, opinions and beliefs can be openly discussed, challenged and interrogated. They also have a duty of care to ensure their diverse community students and staff feel safe, welcome and free from discrimination. But some claim that an over-zealous focus on inclusion and appeasing students has led to an erosion of academic freedom and allowed a ‘cancel culture' to dominate higher education, leading to a worrying expected conformity of opinion on important contemporary issues. This is all playing out against the wider backdrop of growing polarisation and identity politics. For this episode, we speak to two experts in this space to find out what practical steps universities can take to encourage more constructive disagreement and engagement with differing viewpoints among students as part of their learning. And how institutions can uphold the requirements of free speech and nurture plurality across campus, while protecting those most affected by contentious issues. You will hear from: Caroline Mehl, the co-founder and executive director of the Constructive Dialogue Institute, a non-profit organisation that builds educational tools to equip US higher education institutions and other organisations with skills to communicate and collaborate across differences. She founded the CDI with psychologist and author Jonathan Haidt in 2017 having previously worked as an associate research scholar and visiting scholar at New York University's Stern School of Business. Abhishek Saha, a professor of mathematics at Queen Mary University of London and co-founder of the London Universities Council for Academic Freedom. Abhishek was heavily engaged in lobbying the UK government over key details of the Higher Education (Freedom of Speech) Act 2023 which came into force this year. You can find more insight and advice on how universities can encourage respectful disagreement, while handling sensitive topics with care and protecting academic freedom in our latest spotlight guide: Dealing with division: the polarised university.
H.R. 1, known as the One Big Beautiful Bill Act (OBBBA), is set to reshape hospital finances by cutting an estimated $840 billion from Medicaid and Patient Protection and Affordable Care Act (PPACA) funding. As hospitals absorb these losses, many are tightening budgets, reducing staff, and facing renewed labor tensions. For the first time, clinical documentation integrity (CDI) teams are part of this unionized landscape, connecting frontline workforce issues directly to reimbursement and data accuracy. When strikes or slowdowns occur, they can disrupt queries, coding, and claims, creating both financial and compliance risks. Together, these forces reveal how economic and workforce realities are now intertwined across every level of healthcare.Reporting the lead story during the next live edition of Talk Ten Tuesdays, the popular Internet broadcast produced by ICD10monitor, will be Penny Jefferson, CDI Manager for UC Davis Health. The broadcast will also feature these instantly recognizable panelists, who will report more news during their segments:CDI Report: Cheryl Ericson, Senior Director of Clinical Policy and Education for the vaunted Brundage Group, will have the latest CDI updates.Social Determinants of Health: Tiffany Ferguson, CEO for Phoenix Medical Management, Inc., will report on the news that is happening at the intersection of medical record auditing and the SDoH.The Coding Report: Christine Geiger, Assistant Vice President of Acute and Post-Acute Coding Services for First Class Solutions, will report on the latest coding news.News Desk: Timothy Powell, ICD10monitor national correspondent, will anchor the Talk Ten Tuesdays News Desk.

Have you been told that Medicare and Medicare Advantage (MA) patients who have difficulties with activities of daily living (ADLs) are appropriate for inpatient status if their hospital stay crosses a second midnight? What direction have you received about “discharge effectuation?” Are your physicians keen on placing the majority of their patients into inpatient status because they want to prevent delivery of an expensive hospital bill? Erin Kreider gives me hope for the future of the CDI profession. That's a pretty positive first impression for someone I recently met. Why do I say this? Erin is a newly-minted clinician who returned to medical school after nursing and CDI to become a nurse practitioner (NP), and now sees patients once a week. Her “welcome mat” on LinkedIn lists her core passions, which include clinical honesty and ethical integrity in the final code set, advocating for patients' rights and best interests, and education and thought leadership. Today the majority of her work is with a technology company, Ambience, where she is helping build out ambient listening technology that assists with documentation and coding. Prior to that she had lengthy stop at Kettering where she was a CDI professional and a case manager. How's that for achievement—and purpose? We get into all this on this episode of Off the Record, covering: Erin's first day on the job as an NP and what becoming a clinician taught her about documentation/coding/the mid-revenue cycle that she didn't know or fully appreciate prior. CDI in the ED: Missed opportunity to improve documentation for admitted patients and strengthen Medical Decision Making (MDM) Best practices for leveraging APPs: Support system for MD physician advisors, second level chart reviews, educators, and more—a topic she recently wrote about for ACDIS' CDI Journal UR/UM synergies with CDI and coding CDI pet peeves: Queries for the sake of metrics, and perception of CDI as an easy retirement job for fatigued bedside nurses How ambient listening can help with documentation, provider burnout, and surface additional missed diagnoses/HCCs

The revenue cycle helps keep hospital revenue flowing. But sometimes, it seems to sputter out, as if not all the cylinders are firing. And that is why the producers of Talk Ten Tuesdays have invited longtime ICD10monitor editorial consultant Dennis Jones, senior director of revenue cycle at the famed Jefferson Health, to be the special guest during the next live edition of the weekly Internet radio broadcast. The popular broadcast will also feature these instantly recognizable panelists, who will report more news during their segments:• CDI Report: Cheryl Ericson, Senior Director of Clinical Policy and Education for the vaunted Brundage Group, will have the latest clinical documentation integrity (CDI) updates.• Social Determinants of Health: Tiffany Ferguson, CEO for Phoenix Medical Management, Inc., will report on the news that is happening at the intersection of medical record auditing and the SDoH.• The Coding Report: Christine Geiger, Assistant Vice President of Acute and Post-Acute Coding Services for First Class Solutions, will report on the latest coding news.• News Desk: Timothy Powell, ICD10monitor national correspondent, will anchor the Talk Ten Tuesdays News Desk.
Many healthcare organizations know they need an outpatient CDI program, but are tripped up by the most fundamental question: What is the expected return on investment after a considerable upfront investment of time, human capital, training, and technology? Discover the tangible impact of outpatient CDI on revenue, risk capture, provider alignment, and compliance in this special session of Off the Record. Jason Jobes, Senior Vice President, Solutions, Norwood, and Carol Ann Hudson, AVP of Quality and Clinical Operations and Population Health for Lifepoint Health first presented this popular session at the ACDIS 2026 national conference and later for a select virtual audience, and I'm pleased to bring it to my listeners. You'll walk away with a big picture understanding of OP CDI and concrete, operational strategies for transforming your program. If you haven't started yet, consider this your blueprint. Note that “ROI” doesn't only equate to revenue. Return on investment can also come in the form of denials resistant documentation, coding compliance, and improved patient care and value based care alignment through a greater emphasis on patient scheduling. All of which Lifepoint Health experienced. Note: Jason and Carol Ann refer to slides at points of this session; the audio can stand alone but if you prefer you can also download them on the Norwood website (free with registration): https://www.norwood.com/resource/the-roi-of-outpatient-cdi-slide-deck/
