En tiempos en los que se repite que el intestino es el segundo cerebro del cuerpo, varios estudios científicos han señalado que personas con autismo pueden sufrir problemas gastrointestinales con más frecuencia. Por ejemplo, mayor permeabilidad intestinal, lo que puede permitir que toxinas, bacterias u hongos ingresen al torrente sanguíneo y afecten el cerebro. En este contexto, un grupo interdisciplinario del Instituto Pasteur, la Escuela de Nutrición de la Udelar y el Centro Hospitalario Pereira Rosell trabaja desde 2023 en un proyecto financiado por ANII para desarrollar herramientas más precisas para diagnosticar el trastorno, idealmente mediante “biomarcadores de salud intestinal”. Y, a partir de eso, poder pensar en dietas que puedan contribuir a mejorar la calidad de vida de niños con autismo. ¿Qué tan relevante es la microbiota en las funciones del cuerpo humano? ¿Por qué la alimentación puede ser importante en enfermedades que tienen que ver con el cerebro? ¿Qué diferencia encuentran en la microbiota de niños con autismo y niños sin autismo? Hoy hablamos de tu microbiota intestinal, ese ejército de bacterias, virus y hongos que conviven contigo —y que a veces, cuando tú estás estresado, también lo están. Nuestra invitada es la doctora María Dolores de la Puerta, quien se licenció en Medicina y Cirugía en la Universidad de Murcia en 1991. Inicialmente formada como cirujana plástica, su inquietud personal y espíritu curioso la llevaron hacia el fascinante mundo de la microbiota intestinal alrededor del año 2000. Desde entonces, ha dedicado su carrera a ayudar a sus pacientes a restaurar la homeostasis intestinal y mejorar su salud, especializándose en el tratamiento de patologías asociadas a la disbiosis intestinal. A lo largo de su carrera, la Dra. De la Puerta ha evolucionado su práctica médica para concentrarse en tratar a pacientes con patologías relacionadas con la disbiosis intestinal. La Escala de Bristol es una herramienta visual y descriptiva que clasifica las heces en 7 tipos diferentes, según su forma y consistencia. Fue desarrollada en la Universidad de Bristol en los años 90, y hoy en día se utiliza en consultas de médicos, nutricionistas y gastroenterólogos para evaluar la salud intestinal.Te explico brevemente los 7 tipos:Tipo 1: Heces separadas en bolitas duras, como nueces. Esto suele indicar estreñimiento severo.Tipo 2: En forma de una salchicha grumosa. También asociado al estreñimiento.Tipo 3: Salchicha con pequeñas grietas en la superficie. Considerada casi normal, pero un poco seca.Tipo 4: Forma de salchicha o serpiente, lisa y blanda. Este es el tipo considerado ideal o 'saludable'.Tipo 5: Pequeños fragmentos blandos con bordes definidos. Puede indicar una dieta baja en fibra.Tipo 6: Heces blandas, con bordes irregulares, casi pastosas. Sugiere diarrea leve o tránsito rápido.Tipo 7: Totalmente líquidas, sin partes sólidas. Esto indica diarrea severa.La escala no es solo una curiosidad médica; es una forma concreta de entender cómo está funcionando tu digestión. In this episode, we explore the intricate role of the vagus nerve as a central regulator within the microbiota-gut-brain (MGB) axis, examining its neuroanatomical structure, signaling mechanisms, and interactions with microbial metabolites and immune pathways. We discuss how vagal afferent fibers relay sensory input from the gut to the brain, including signals mediated by short-chain fatty acids (SCFAs) and gut-derived hormones, and how efferent fibers modulate gut motility, intestinal barrier integrity, and inflammation through the cholinergic anti-inflammatory pathway. Finally, we explore vagal dysfunction as well as associated conditions and symptoms, and we touch on just a few potential root causes. Topics: 1. Introduction Focus on the vagus nerve's role in the microbiota-gut-brain (MGB) axis. Bidirectional communication between the brain and microbiota. Overview of communication pathways: neural (e.g., vagus nerve), endocrine (e.g., HPA axis), immune (e.g., cytokines), and metabolic (e.g., SCFAs). 2. Overview of the Nervous System The CNS includes the brain and spinal cord - control centers for the body. Interaction with Intestinal Cells Enteroendocrine cells (EECs) release gut hormones in response to microbial metabolites. SCFAs, such as butyrate, activate free fatty acid receptors on EECs, stimulating vagal afferents. Immune cells within the gut wall modulate vagal signals during inflammatory responses. 7. Review of Functions Sensory input (afferent fibers): Detects gut-derived signals like microbial metabolites and mechanical stretch. Motor output (efferent fibers): Regulates gut motility, secretion, immune responses, and more. 8. Impact of a Diverse Microbiome on Vagal Activity Enhanced SCFA production boosts vagal activity. SCFAs improve gut barrier integrity, reduce systemic inflammation, and assist in regulating stress responses. 9. Examples: Intestinal Barrier Function Releases acetylcholine (ACh) to modulate inflammatory pathways. Helps enhance tight junction protein expression, preserving gut barrier integrity. Helps prevent the translocation of microbial endotoxins like LPS into systemic circulation. 10. Dysfunction of the Vagus Nerve Reduced vagal tone disrupts gut homeostasis. Conditions such as IBS, IBD, chronic fatigue syndrome, anxiety, depression, and POTS. Chronic stress, infections, and dysbiosis are common contributors. 11. Root Causes 12.
