Dr. Stephen Gamboa is an Adjunct Associate Professor at UNC and medical director of UNC Health Pardee's emergency department in Hendersonville, North Carolina. In today's GEMCast episode, he shares his first-hand experiences from two major natural disasters: the 2017 Tubbs Fire in Northern California and Hurricane Helene, which hit Western North Carolina in 2024. As he joins host Dr. Christina Shenvi, the two talk about how these events have a disproportionate impact amongst our frail older patients, which can be attributed to age-related comorbidities, reduced mobility, and higher care needs. Tune in and hear a variety of innovative solutions brought about by the various challenges that these major events can pose. The immediate impacts of natural disasters, such as smoke inhalation and needing to rapidly evacuate homes and skilled nursing facilities (SNFs), are just the tip of the iceberg. Older patients with decompensation or exacerbations of chronic conditions made up a large proportion of the increased demand on EDs following these disasters. Coupled with power outages, electricity shortages, no internet connectivity, and pharmacies closing, Dr. Gamboa describes how his team had to adapt quickly and how system-level support from UNC during Hurricane Helene, including staff, equipment and transport, was crucial to providing care to these vulnerable patients. Yeah. so uh and guess what? They say, well, well, the when I called the county, I said, who's doing this? Well, they've 00:59 they've approved it. I'm right. guess what? uh We pay in taxes on property uh that they come over and take over and do this to our yard. Yeah, unfortunately, they do have that that utility easement and they can come in and do that. I mean, I know and I know exactly what you're talking about, Steve. We had a company come through our yard and 01:27 Our neighborhood was none too happy. So best thing I would do is I'd call your councilman. I'd call your representative in Columbia. I appreciate it, buddy. So yeah, they come in and I won't say the company's name. They came in and destroyed our neighborhood. But when you saw the job they did, you went, wow. uh 01:47 So, and now it was so bad. It was so bad that they literally, it was about a month and a half ago, they came through our neighborhood door to door. This is no joke. They came through our neighborhood door to door and they said, we just want to apologize for the job that our company did in your neighborhood. We are not hiring that crew again. 02:14 just and they did. mean, they were very, they're very sorry. mean, they even they damaged the sign to our neighborhood. They cut the power line to that. um did. Yeah, that was nice. um And then they they just came in and destroyed people's yards. It was oh, it was a mess. And then they one of my neighbors, they actually left trash in her yard. But I want to go back real quick. um Before we get to Joe in Hendersonville and talk about the difference 02:45 in Charleston from that caller's perspective and my perspective. He was working in new construction. So the people that he was meeting on a day-to-day basis, yeah, they were the ones coming in for Boeing. They were the ones coming in for Sprint, the Sprint vans with the Mercedes. They were the ones that were coming in with the corporation. 03:12 They were the ones that were spending what, $150 to $250 a square foot on their new home construction? That's a lot of money. So yeah, those people are probably very happy to be there and not running away from something. But the people that I knew, the renter class in Charleston, those were the runaways. By and large, not all of them. But those, and when I was there in Charleston, I think, 03:41 of everybody that I knew, I would say maybe 10 were originally from Charleston. Maybe 10. But I'll tell you something, when I was in engineering school in New Jersey, we were given grants to find out how to desalinate water. of course we looked at everything to make the electricity to use electricity. 02:56 to split the water to make clean water. But what happened was somebody came up with the idea that while we use the spin of the earth and the heat in the earth to make the electricity, well, we had people on the board of NASA that were working on the moon shots when I was going there. And they said, you can't do that. 03:25 If you take the energy out of the earth, the earth has to slow down. That means our day would get shorter and all the plant life would take a bath from not doing its thing. Cause there's no energy coming from the sun. The sun can't replenish it as fast as you can take it out. don't even understand how they would do it. I don't know Ray, you're way over my head with all of that engineering stuff.
