The new Lunder CareForce Institute makes no small plans: It aims to be a driving force behind solving the nation's frontline healthcare worker shortage. Established by Hebrew SeniorLife, a Harvard Medical School-affiliate, the institute opened for business last week with sector veteran Tammy Retalic at the helm. It will employ a mix of technology and elite caregiving expertise, as well as online modules that inject experts into training and education cycles. The first nurse-aide students should be enrolled by mid-summer, Retalic told McKnight's Executive Editor James M. Berklan in this McKnight's Newsmakers podcast. An LPN track will be added within 18 months, she added. The concept uses a “hub” model that extends into the local greater Boston community. If that can be successfully grown around the state, Retalic is confident its influence can blossom further from there. “If that is something we can accomplish, which we are confident we can … we'll have the ability to go throughout the country and replicate this type of model, and that's really our big vision,” said Relatic, who switched from Hebrew Senior Life's chief nursing officer and vice president of patient care services to the Lunder CareForce Institute's executive vice president last week. “We don't want to just solve it just for our own needs.” The secret, she says, is finding individuals who are motivated and are filled with compassion. Students will undergo paid six-week nurse-aide training periods, and might not necessarily think of themselves as aide-candidates when first approached. They could be high school students, under- or unemployed individuals, military veterans, parents returning to the workforce, or many others who may thrive with steady caregiving employment, noted Retalic, an award-winning longtime educator. “If we can create some kind of training option that produces the most amazing students when they are done, students who are motivated and accountable, who really want to make a difference, then we need to be able to lead that way nationally,” she said. “You can't teach compassion. You can't teach motivation. But when you have those two things, you pretty much can teach any other skill set,” she added. “We believe that. We've seen that time and time again … They flourish. High readmission rates can result in financial penalties and are often caused by the complexity of the patient's condition and social determinants of health. Puzzle is working with health systems and SNFs to implement a readmission prevention program that includes physiatry and care coordination 90 days after discharge.  Ahzam explains, "Hospital readmissions have become a focal point for our healthcare systems for several reasons. First and foremost, they are a key indicator of the quality of care being provided by these facilities. High readmission rates often suggest that patients may not be receiving the comprehensive care they need during their hospital stay or the necessary support post-discharge. This could be due to inadequate discharge planning, insufficient patient education on their conditions, or a lack of effective follow-up care."   "Additionally, readmissions place a substantial financial burden on healthcare facilities. I sat down with Emily Gavin who decided to take the leap into private practice with encouragement from her peers and spouse after being laid off from her job.In this episode, she talks about her journey from SNFs and inpatient rehab to owning her own dementia-focused business. Emily shares how caring for her mother-in-law with dementia inspired her special interest. Emily S. Gavin, MS, OTR/L, came to occupational therapy as a second career in 2012 at the age of 41. After a few years of clinical work in geriatrics, she transitioned into a management position at a Program of All-Inclusive Care for the Elderly (PACE) in Philadelphia, PA. This non-traditional setting allowed her the freedom to develop specialized knowledge in dementia care training, though she always felt the vulnerability of being a middle manager. Being laid off in May of 2023 seemed like the perfect time to start PhillyCare Coaching LLC, the private practice she'd always thought of as the next thing.Emily hopes to complete her Post-Professional OTD at Thomas Jefferson University in late 2025. She has also earned an Advanced Practice Certificate in Coaching, and will soon be a certified practitioner of Skills2Care®, an evidence-based program supporting caregivers of people living with dementia. There are substantial new changes to the minimum data set (MDS) assessment, including how functional abilities and goals are scored, the introduction of tracking of indications for medication use, and the addition of new measures for tracking communication of transfer-of-health information. The recent changes became effective Oct. 1, the start of the government's fiscal year.The MDS assessment is a standardized assessment used in skilled nursing facilities (SNFs) and some swing beds to capture administrative and clinical data for residents. The information compiled in the MDS assessment is used to create resident care plans and determine reimbursement, and it impacts quality measures.As a warning, SNF personnel should be on the lookout for any denials related to the new ICD-10 codes, according to Deanna Peterson, the special guest during the next live edition of Talk Ten Tuesdays. At Viviant Healthcare of Murfreesboro, located in rural TN, 25% of residents are short stay. When they leave, Lisa Lugo, Director of Social Services, worries. Lacking transportation makes it hard for many to fill prescriptions. Some in need of pain medications can't get an appointment with a pain clinic for days. Others return home and don't follow up with their healthcare providers. Without support, residents may experience health mishaps that land them back in the hospital. To avoid these complications, Lugo opted to take part in a pilot program for Continue Care, a suite of services from PharMerica designed to help keep patients healthy at home. It worked: the rehospitalization rate for Viviant during the pilot was 10%, a significant improvement over the average rate among SNFs of 23%-31%. And residents weren't the only ones who benefitted. By eliminating manual processes and calls or faxes to the pharmacy to get prescriptions filled, Continue Care saves a lot of staff time and frustration and frees caregivers up to spend more time with residents. Plus, with a three on/three off schedule, there's no risk of orders falling through the cracks between shifts. Hear firsthand from Lugo when she joins hosts John O'Connor and TJ Griffin along with PharMerica's Bill Deane, whose team operationalized the program, to discuss her facility's previous processes that prompted her to adopt Continue Care and the positive impacts it has had on outcomes and her team alike.

Amber graduated from Ithaca College from the BS/MS Occupational Therapy program in 2018 and 2019, respectively. Currently, she is a Travel Occupational Therapist in NYC working with the geriatric population in SNFs. In addition to being an OT, Amber is also passionate about fitness. At Ithaca College, Amber was a 2x All-American hurdler and captain on the Women's Track and Field team and currently enjoys strength training with kettlebells as a Certified Russian Kettlebell Coach (RKC) and Dynamic Sports Performance Certified Coach (DSPCC). In this episode, we will learn about two occupational therapists who are Captains in the United States Air Force, Capt. Andrew Antonio, OTR/L, CHT, and Capt. Alain Pierre Eloi, OTD, OTR/L. Capt. Antonio is an occupational therapist at the Special Warfare Human Performance Squadron, Kirtland Air Force Base, N.M. He leads a multidisciplinary team of military and civilian personnel across six functional areas in support of the sole Pararescue pipeline for six commands. He supports the Human Performance Optimization Program, providing sports medicine consultation to providers, independent medical technicians, athletic trainers and embedded Squadron leadership. He runs the Special Operations Forces-centric Training Squadron Occupational Therapy Clinic, conducting same-day evaluations and treatment in support of 138 staff, 135 trainees. He also integrates performance and injury data to optimize training, and develops and executes policies with coordination for two Wings and three medical facilities. He is also the Occupational Therapy Career Recruitment Officer responsible for inspiring, engaging, and recruiting diverse, highly qualified occupational therapists to serve as officer candidates to sustain the combat capability of the United States Air Force. Capt. Antonio received his postgraduate degree in occupational therapy at the University of St. Augustine in April 2015 and entered the Air Force in April 2017 through direct commission as a distinguished graduate. He also earned a board specialty certification as a Certified Hand Therapist in December 2020. Since then he has held a variety of positions within the medical treatment facility and line operations including directing the first Occupational Therapy/Orthopedics Post-operation Hand Clinic, leading the first KC-10 Pilot Ergonomic Program for the 6 Air Refueling Squadron, and drafting the inaugural Medical Group Instruction for Management of Emergent Suicide Risk Patients as the Group National Patient Safety Chair at Travis, AFB, Calif. He also served as the sole Surgical and Hand Therapy Fellow serving the Greater Philadelphia, Pa. area into and post-COVID-19 shutdown. Prior to his current position, Capt. Antonio was a Surgical and Hand Therapy Fellow at the Philadelphia Hand to Shoulder Center, in Philadelphia, Pennsylvania. Capt. Eloi is a native of Montreal, Canada who moved to California to study Occupational Therapy at Loma Linda University. After several years of working in various settings such as outpatient hand, home health, and SNFs, Capt. Eloi made the decision to go back to school in 2018 at Rocky Mountain University, where he obtained his OTD in the hand therapy track. During the pandemic, he made yet another life-changing decision; he decided to commission into the United States Air Force as an active-duty officer. Capt. Eloi is now stationed at Wright-Patterson Air Force Base since March 2022 as he currently sees a varied population of active-duty members, family members, and retirees.Show Key Points:· Capt. Dr. Cardo joined CDC in 1993 as a medical epidemiologist in the Hospital Infections Program (later named as Division of Healthcare Quality Promotion). After holding several leadership positions in DHQP, she was selected as division director in 2003. Spending time in research can deliver the humbling, but needed, reminder that not all therapy leads to clear value for our clients. For example, in the United States, we can now see that in the 2010s therapy was being delivered in skilled nursing facilities (SNFs)—beyond what was helpful for clients. Part of the problem was the fee-for-service payment model that incentivized skilled nursing facilities to deliver high quantities of therapy minutes to as many patients as possible. (More therapy equaled more money.) The research article that we will explore in this one hour course, shows us the humbling data behind this problem. And, it reminds us of why the transition to value-based care continues to happen. Marie is a licensed speech-language pathologist with a graduate degree in speech-language pathology from the University of North Carolina- Greensboro. She also possesses the national accreditation, certificate of clinical competency, from the American Speech and Hearing Association. She is a recent graduate of The Myo Method, an orofacial myofunctional therapy course and is a private practice owner currently going through the credentialing process. She has also created “My Speech Sounds” which are a line of coloring books designed to accompany therapy by providing simple  practice steps at the word, phrase and sentence levels. Myofunctional therapy, early intervention, pediatrics and multicultural aspects (since she is half Asian and grew up in a bilingual home) are just a few of her clinical interests. Prior to becoming an SLP, Marie had two previous careers. Her first career and love was fine jewelry. Growing up, she often accompanied her mother to gem shows. Then her mom would draw out designs and have these items made by jewelers that were close friends of her family overseas. After spending approximately seven years in fine jewelry professionally and even owning a small jewelry store of her own, she changed careers into finance. Marie landed a position as a financial specialist and was promoted to a small business licensed financial specialist with a large national bank based out of North Carolina. In this role, she acquired her Series 6 and 63 investment and insurance licenses. With a focus on small business, she handled businesses ranging from $0 - $3 million in annual revenue. Being a one point of contact for business owners meant helping to manage their deposit accounts, lines of credit, commercial property lending, equipment leasing, as well as investment needs such as SEP IRAs,  and 401k plans for their employees, along with all personal banking needs too. The link between all these careers she loved the most was building relationships with clients and making them feel valued. When she found speech pathology, she knew she found her home. A career where she could build a myriad of skills and a variety of settings to choose from. After experiencing acute care, SNFs, private practice and the school system, she finally decided to follow her heart and take the leap into private practice and being a business owner once again. We get into all the things! What is PDPM and How Will it Affect You? With Marc Zimmet of Zimmet Healthcare

What does Zimmet Healthcare actually do? Zimmet Healthcare is a consulting support firm specializing in skilled nursing facility reimbursement, compliance and strategic planning. Initially started as a service vendors creating cost reports and appeals for Medicare and expanded as the industry has matured. Zimmet Healthcare currently employs about 60 people servicing about 300 facilities across the country. Do you see a significant trend in the reimbursement method for SNFs from when you started out to today? There's a complete shift in SNF reimbursement. To be frank, clinical reimbursement is a complex topic and the goal of this conversation is to get a better understanding of the big picture, the coming changes and how they will affect day to day operations of your nursing home. With that in mind, let's go back to Marc's answer. In the earlier days, reimbursement was based on the costs a facility incurred when caring for a particular resident. The more the facility spent on the resident, the more was their reimbursement within reason. This is now changing to a model that is based on price and not cost. There is a fixed rate per day for a residents' condition, diagnosis and plan of care. The facility will get reimbursed at that rate regardless of the actual costs incurred by the facility. Why is Medicare Changing the Reimbursement Methods? One of the biggest challenges with the current PPS/RUG method of reimbursement is that there is too much of an emphasis on the therapy received by the resident. For example, if there are to residents who receive the same level of therapy, the facility will receive similar reimbursement for them even if one has significant other clinical challenges (such as wound care or cognitive issues etc.). PDPM - Patient Driven Payment Model PDPM which is starting October 2019, enhances the reimbursement sensitivity of the all the different payment drivers and takes the focus away from therapy as the primary driver of payment. PDPM Vs. RUGs To understand the difference in reimbursement consider the following. We are going from a RUG system which has 66 RUG scores (of which we only use 20 or so of them), to PDPM which has almost 29,000 rate composite possibilities, which boils down to 15-20,000 rate combinations that will actually be used. In simpler terms, with more words in the language we can be that much more precise. How Does This Affect the MDS Process? The MDS job description will change a bit. There will be fewer assessments, however, it will require working more extensively with the other disciplines. Whereas in the past the reimbursement was driven primarily by therapy and ADLs, now the other disciplines will play a significant role in the reimbursement process. For example, dietary will need to capture a mechanically altered diet. The psychologists, respiratory therapists and others will play a large part in this process. Who is Pushing for These Changes? The federal government is pushing for these changes. MedPac (The Medicare Payment Advisory Commission) has been almost begging congress and CMS to implement a patient specific payment model that does not recognize the frequency and duration as the payment driver because it provides an incentive to over treat the patient. Is the Government Trying to Save Money With This? Theoretically, this is supposed to be budget neutral. It's a redistribution from facilities that focus primarily on the therapy intense patients, to facilities that take the sicker, more compromised patients on the vents and trachs. The medically complex patients reimbursement will go up considerably and the therapy intense patients will go down. Will Facilities Make More Money in PDPM? Zimmet Healthcare expects that facilities will get better at taking credit for the care they provide and in the first year Medicare will actually spend more than their budgeted amount. This will be followed by a rate reduction that will recalibrate the dollars to make it budget neutral. So perhaps in year 1, the facilities that learn and implement programs to accurately take credit for the care they provide, may make some extra money. However, once the rate is adjusted, it should remain budget neutral. Who Gains by These Changes? Ultimately, these changes are indeed in the best interests of the patient. The problem with the system is that you almost have to provide a high level of therapy in order to keep your doors open. Let's take the dialysis patient as an example. In the RUG reimbursement model, the facility would lose money. The rates are simply inadequate. Facilities would try an aggressive treatment approach where they would take the resident to therapy early in the morning knowing that the resident will not be able to tolerate the therapy on their return. With PDPM, this will not be necessary because the other clinical challenges will be considered as well in the reimbursement rate. What is Changing in the Medicaid System? Being that Medicaid is not a national system, and each state runs their own program, it's difficult to speak about the specifics of the program. However, in general terms, Medicaid started out as a cost based model, many states then went to a case mix and acuity model, to a priced based system and finally to managed care. This is not universal but is the general trend. More importantly, every state has x number of dollars to spend on their Medicaid program. The main concern is not the distribution of reimbursement for the program, rather, it's the overall total funding of the program. An astute operator will learn to maximize their Medicaid reimbursement dollars in any Medicaid system. The problem lies in the total dollar amount that the state allocates to the Medicaid program. Are the Articles That State the Amount a Nursing Home Loses Per Day Accurate? It may be true that the rates are inadequate, however, the facility was never going to be profitable just by the reimbursement payments. In order for a facility to be profitable they will need a good short term rehab census as well. This together with the Medicaid payments can make the facility viable and profitable. If the facility was 100% filled with Medicaid patients with the right acuity mix, and everyone had Medicare Part B you would not be losing $37 a day. Is Owning and Operating Nursing Homes Still as Profitable Now as it was 25 Years Ago? In the early 90's it seemed like a really simple business to run. Nowadays, it's a very complex business. Today, there is tremendous potential in the post acute care continuum which SNFs are such an important part of, is where the opportunity lies. In post acute care management, there real opportunity to meet the new demands of the new nursing home residents. A stand alone nursing facility that is well managed can absolutely be a profitable business and serve the community well. Where are Those Baby Boomers? We are always being fed information by the media that with the baby boomers are coming of age and nursing homes will have to scramble to meet the need. While in reality, nursing homes are struggling to survive due to lack of patients. As mentioned earlier, there are a variety of variables that are causing this to happen, including the push to provide for these elders within the community. However, the statistics do show a sharp increase in the Altzheimers disease in the coming years of which 75% of those over 80 will need to be admitted to a SNF. That is something that will significantly increase the utilization of SNF care and should sharply affect the overall occupancy.
