L’Inde continue à battre des records de contaminations à la Covid-19. Le pays a dépassé les 400 000 contaminations quotidiennes ce weekend, pour plus de 3 600 décès. Presque tous ces malades ont besoin d’assistance respiratoire. L’oxygène est ainsi devenu le bien le plus précieux et le plus rare. Et tous les jours, une lutte vitale se tient donc devant les usines qui en distribuent. De notre correspondant à New Delhi, Il est 16h et Baskshi Prasad a l’air hagard dans la chaleur de New Delhi. Cela fait 28h qu’il attend de pouvoir remplir une bouteille d’oxygène. Il a enfin atteint le devant de la file, mais rien n’est gagné. « Ils ont dit que ça arriverait à 9h, puis 11h, maintenant ils disent 17h… C’est comme cela depuis hier. Je survis en mangeant des biscuits. Mais je suis épuisé » soupire-t-il. Dehors, une centaine de personnes attendent, avec angoisse, leurs bouteilles d’oxygène alignées sur le pavé. Tous ont un proche, malade du Covid-19, qui a besoin de cet oxygène. Mais la pénurie n’épargne personne. Javed Khan rebrousse chemin, après 35h d’attente. « Ma mère est très malade, sa concentration d’oxygène est tombée à 36, elle n’a pas d’oxygène depuis hier. Nous n’avons rien obtenu ici, mais un ami vient de m’appeler : sa mère vient de décéder. Nous allons donc récupérer sa bouteille d’oxygène », explique-t-il. Cette usine de distribution d’oxygène, appelée Vaibhav, reçoit 18 tonnes d’air liquide par jour en ce moment, mais l’essentiel est donné aux grands hôpitaux, comme l’explique son directeur, Krishan Kumar. « Deux de mes camions citernes sont dans l’usine de production, en attendant d’être remplis. Deux autres sont sur le terrain, en train de livrer les hôpitaux. Les priorités de livraison sont données par le gouvernement. Et ce n’est pas facile, car la demande en oxygène ne cesse d’augmenter. Personne n’a  jamais vu une situation pareille. » Les plus petits hôpitaux, eux, ne bénéficient pas de ces livraisons et doivent se débrouiller seuls. Sushil Roy est le chauffeur de la clinique Eden, qui compte une vingtaine de patients Covid dans le sud de la capitale. Il attend donc avec les autres à l’extérieur, devant les grilles installées par la police. « J’ai 10 grandes bouteilles dans la camionnette et je cours jour et nuit pour les remplir. Ici, je ne reçois rien, donc je vais partir dans une autre usine, à 40 km d’ici. 90% de nos patients ont besoin d’oxygène. Ces bouteilles, si je les remplis toutes, cela correspond à leur consommation d’une nuit », affirme le chauffeur. Tous les jours, de nombreuses cliniques envoient des appels de détresse aux autorités, car leurs réserves d’oxygène sont sur le point de s’épuiser. Cela ne permet pas toujours d’éviter le pire : des dizaines de patients hospitalisés sont déjà morts à New Delhi par manque d’oxygène.

Javed Khan is chief executive of Barnardo’s, one of the UK’s largest charities which supports over 350,000 vulnerable children, families and carers each year. With roots going back to 1867, Barnardo’s today has 700 shops, 8,000 staff and 14,000 volunteers looking after children who have suffered from abuse or violence. The impact of Covid-19 on retail and fundraising means it must cut back some services. Khan trained as a maths teacher, entering local government at Birmingham City Council and then becoming director of education at Harrow Council. After a civil service secondment, he was appointed chief executive of Victim Support in 2010 and has led Barnardo’s since 2014. Simon Levine is global co-CEO of the law firm DLA Piper, which has lawyers in more than 40 countries and revenues last year in excess of £2bn. DLA’s clients include nearly half of the FTSE 350, governments and public sector bodies. Levine’s expertise lies in intellectual property, media and sports law. He has advised pop stars and film studio heads in a 30-year career. He joined DLA in 2005 from Denton Wilde Sapte and was elected to lead by partners in 2015. Javed Khan is SVP and GM at Cisco Collaboration, a multi-billion dollar division inside of Cisco, a worldwide leader in IT, networking, and cybersecurity solutions. Prior to his current role, Javed ran Cisco’s Cloud Calling business as the VP and General Manager and before that he led the Webex Meetings business unit.  Just like many organizations around the world, when Covid first started becoming a global issue Cisco had to take action in order to keep business going while also enabling employees to work from home. Because of the products they build, they had some advantages over other companies as they already had communication and collaboration tools everyone was familiar with.  But one challenge they had to overcome was an overnight need for their products from existing and new customers. The demand for their products was suddenly three times bigger in a very short period of time. So as a company they had to hurry to scale very quickly in a time when they also needed to move their workforce out of the offices. It required the team to come together, work a lot of long hours, and support their customers.  How to create and foster casual interaction with a remote team  Javed and other leaders at Cisco also understood that they needed to support employees in a new way. This new way of working made collaboration and casual interactions with coworkers very different, but leaders at Cisco knew they had to find a way to keep both collaboration and social interaction going, because they are so important. Especially in the times we are facing now when people are anxious, worried, scared, etc… At Cisco, they have done a few things to keep employees informed, involved, and engaged. First of all the leadership team leads frequent check in calls, this is different from their quarterly all hands meetings where they talk business and give updates. These check in calls are where people can casually hangout, sometimes they talk about social topics relevant at the time, sometimes they have entertainment, and sometimes they bring in external speakers.  They also have small groups that will get together to talk, without an agenda, about anything they want. These are usually on Friday evenings and employees are allowed to include their family in the calls if they want, but they are just casual conversations to allow small groups of employees to catch up and have fun.  The future of the office With our current events there has been a lot of debate around whether or not employees will go back to working in offices once this is all over. Will everything go back to the way it was, or will office buildings disappear?  Javed believes we will see a hybrid setup post Covid. There will be an increasing acceptance of people working from home and we will have more technological advances that make it possible. But there will still be roles and situations that require working from an office. Some people will be able to work from home 100% of the time, some will have to be in the office 100% of the time, but a lot of people will probably do some kind of hybrid of working from home and occasionally going into the office.  Also, while we have technology to connect with each other and it will only get better, there is no substitute for in-person, face to face interaction.  Digital transformation during Covid One thing Covid has done for businesses is it has made leaders realize the need for digital transformation. And it has forced companies to go through this transformation very quickly. Where leaders in the past have asked will this technology make us more productive, will it make our lives easier, will it improve our bottom line, etc..Now when thinking about digital transformation they are thinking about it in terms of, will this help me to stay in business.  As Javed shares, “People immediately went into a survive mode, where suddenly these technologies got deployed and tested overnight, and there were some learning pains. But I feel like companies fell into two categories. There were companies who had already started on this journey of enabling remote work. And they had a baseline of the stack already enabled. for them. It was a matter of, Oh, I have 10,000 employees. You know, I have 100 employees who had worked effectively using this, how do we scale it out to the entire corporation, but I already have this baseline technology in place. And there were others who had not started on this journey. And in those cases it took a little bit more work, you know, retraining your employees. But once you got through that initial phase--raise technology and develop product and some of the devices we have, once you go through that initial learning, we were able to get most of the companies up and running pretty quickly. But nothing forced that digitization more than the last few months. We've been trying to get people to use video for a long, long time and I think now, video you know, if your video is not on you get reminded--I can't see you on video.” How to evaluate how employees are doing without an annual review One thing that Cisco is known for is their unique culture and getting rid of their annual performance reviews. So how do leaders know if employees are engaged and being productive if they don’t have an annual review?  As Javed shares, it is about having ongoing, honest conversations with employees. It is important to check in regularly and let employees know about the good things they are doing, and the things they need to improve on throughout the year, not just on one specific date.  Keeping conversations to once a year makes issues a surprise to employees, and it is hard to remember something that you did 5-6 months ago. Meeting one time a year is not effective and it can damage the manager/employee relationship.  Cisco also understands that keeping company culture consistent is key. Whatever culture you are trying to create, make sure you stick with it through good times and bad. If employees see a change in culture during bad times like we are going through now, they will see right through it and know they can’t trust their leaders.  Advice for people early in their careers Javed is the leader of a multi-billion dollar division inside Cisco, but he has learned a lot on his way to the top and he has a lot of experience and advice to share with others. When asked his advice for people early on in their career who want to advance, Javed says, be curious and be willing to learn.  He says, “I did not set out to be a manager as an example, I started my career as an engineer and thought I was just going to be coding for most of my life. But as I got in, you know, got into trying out, leading a small team, or learning to lead. I think just being curious and trying those things out has helped me out a lot. The other thing is just learning from other people who do that better. Right. So a big part of me transitioning from an engineer to becoming a leader was watching other leaders motivate and speak and inspire. And while I might have thought that that might be something that I wouldn't be doing, I think watching them learning from them, then being curious about the techniques and how leaders lead, I think has been a big part of my success. So be thirsty.” Advice for leaders today Javed also shared his advice for leaders who want to adapt, stay relevant, and better themselves. And he says his biggest piece of advice is to stay connected with your employees personally. Especially in the difficult times we are in, it is important to know how they are doing, what are they excited about, what are they scared about, how do they feel about work, what do they care about...etc… “Because the rest of it, I think we've got a system and processes in place. You're able to measure, you're able to be effective, But in today's world, I think spending more time with your  employees making sure you're understanding what else is going on, and what else they need beyond just the tools that work, I think is a big part of leadership today. And the biggest learning I've had in the last few months, you'd be surprised at how much else is going on in typical person's lives and how it might be impacting their ability to be productive.”

This JCO Podcast provides observations and commentary on the JCO article “Maintaining Outstanding Outcomes Using Response and Biology-Based Therapy for Intermediate-Risk Neuroblastoma: A Report From the Children's Oncology Group Study ANBL0531” by Twist et al. My name is Javed Khan, and I am Deputy Chief and Senior Investigator at the Genetics Branch and an Attending for the Pediatric Oncology Branch of the National Cancer Institutes of the NIH in Bethesda, Maryland. My oncologic specialty is Pediatric Hematology Oncology. Childhood cancer is a life-threatening disease where survival rates have increased exponentially to over 75% over the past three decades. However, this has come at a considerable cost with significant incidence of late effects including hearing loss, scoliosis, hypothyroidism, growth and development delay, infertility, psychological, emotional, cognitive and neurological sequelae, and secondary malignancies. Most of these late effects have been attributed therapy including chemotherapy, surgery and radiation. In the manuscript that accompanies this podcast, Twist et al report on the results of the Children’s Oncology Group study ANBL0531, whose primary aim was to reduce therapy for subsets of children aged =95% for the entire cohort. Neuroblastoma is an extracranial pediatric malignancy of neural crest origin that has often been described as an enigmatic cancer due to its significant clinical, molecular and biological heterogeneity. For example, some patients with widely metastatic disease are cured with minimal or no therapy whereas others have relentless progression to death despite intensive therapies.  There has been considerable progress in dissecting out the causes of this heterogeneity and developing prognostic biomarkers to stratify the risk based on several clinical, histological and molecular markers. At the time this clinical study was performed, and the manuscript written, the authors stratified patients using several clinical and biologic parameters including age, the International Neuroblastoma Staging System, MYCN amplification status, the International Neuroblastoma Pathology Classification of histopathology, and tumor DNA index. An additional marker utilized in this study was loss of heterozygosity determination of the tumors at chromosome bands 1p36 and 11q23. This stratification strategy has led to the identification of low and high-risk groups where therapy is more clearly defined and patients with low risk have excellent outcome for most patients with surgery alone and watchful waiting. For high-risk disease, significant strides have been made using a combination of surgery, aggressive chemotherapy, radiation followed by Chimeric Antibody 14.18 anti GD2 therapy, GM-CSF, IL-2 and Isotretinoin, however the five-year survival remains guarded at 40-50%. In the intermediate-risk group, the subject of this study, although the 5-year survival rates of 90-95% have been achieved, this group is significantly heterozygous raising the possibility of reducing therapy for subsets of patients within this group. This was the motivation for this study. Thus, the goal of this clinical protocol, ANBL0531, was to refine the minimal therapy needed to achieve excellent outcomes for patients with intermediate-risk neuroblastoma, with the aim to maintain an overall 3-year OS rate of > 95% for the entire cohort. Patients were stratified into Groups 2, 3, or 4, who were assigned to receive a minimum of 2, 4, or 8 cycles of chemotherapy, respectively. Chemotherapy was identical to that used on a previous study A3961. Reduction of therapy was planned to be achieved for patients with biologically favorable tumors (Groups 2 and 3) by using the achievement of Partial Response (PR) to chemotherapy +/- surgery to be the endpoint of therapy, rather than the achievement of Very Good Partial Response (VGPR) as was previously used on A3961. If the treatment endpoint was not achieved after completion of the scheduled chemotherapy courses, additional cycles of therapy could be administered, and patients re-evaluated following every 2 cycles to determine if the treatment endpoint has been achieved. VGPR, a more stringent response criteria, was used for Group 4 patients which includes those with age< 365 days, stage 4 disease, MYCN-not amplified, and either Unfavorable Histology (UH) or DNA Index or Ploidy (DI)=1. Patients within this subgroup received 8 cycles of chemotherapy on A3961, which was a reduction in therapy for these patients compared to prior studies. Group 4 patients also includes 2 subgroups who were previously stratified as high risk including those who had stage 3 disease, age 365- < 547days, stage 3, MYCN-NA, UH, any ploidy and also those with stage 4 disease, age 365 - < 547 days, MYCN-NA, FH, DI>1). These patients also received Isotretinoin (13-cis-retinoic acid, Accutane) for maintenance therapy, as they would have previously received this treatment on high-risk protocols. Thus, these patients would receive a significant reduction in therapy compared to prior studies.  These patients were closely monitored by interim stopping rules. Rescue therapy for this study for patients with an inadequate response to initial therapy and for patients with progressive, non-metastatic disease utilized standard cyclophosphamide and topotecan. Reduction of therapy was also planned and achieved on their study by reducing potential surgical morbidity for patients with stage 4S disease, who would no longer be required to undergo resection of their primary tumor. In the manuscript, the authors report that 404 evaluable patients were enrolled between 2007 and 2011, They found that compared to legacy COG studies, subsets of patients with locoregional disease, infants with stage 4 or 4S tumors, and toddlers with stage 3 or 4 disease had a reduction in treatment. The 3-year event-free survival (EFS) and OS were 83.2±1.9% and 94.9±1.1%, respectively. Infants with stage 4 favorable biology tumors (n=61) had superior 3-year EFS compared to patients with >= unfavorable biological feature (n=47; 86.9±4.4% versus 66.8±7.0%; p=0.02), with a trend towards OS advantage (95.0±2.8% and; 86.9±5.1% respectively p=0.08. For patients with localized disease OS was 100%. They conclude that comparable/excellent survival was achieved with this biology- and response-based algorithm, with reduction of therapy for subsets of intermediate-risk neuroblastoma patients. However, more effective treatment strategies are still needed for infants with unfavorable biology stage 4 disease. For the 32 infants with hyperdiploid, favorable histology (FH) stage 4 tumors with LOH at 1p36 or 11q23, or missing LOH data, intensification of treatment on ANBL0531 compared to A396110 did not reduce the risk of relapse previously reported for infants with tumors that harbor these genetic anomalies. Three-year EFS for this cohort was 68.6% (95% CI: 52.2-85.1%) versus 86.9% (95% CI: 78.3-95.4%) for stage 4 infants with favorable biology (p=0.04). Overall survival was not significantly different, indicating that many of the infants with 1p36/11q23 aberrations were successfully salvaged. Of the 20 patients who received CPM/TOPO due to an inadequate response to initial therapy, 9 achieved ≥VGPR. However, 6 of the 20 patients developed PD or relapsed, and 1 died, indicating that more effective treatment is needed for patients who do not meet the defined treatment endpoint after 8 cycles of chemotherapy. Take home messages: This is an important study with clinical implications for intermediate-risk neuroblastoma where the authors show that it is possible to reduce therapy based on a clinical Partial Response (PR) endpoint compared to Very Good Partial Response (VGPR). This most likely reflects the biology of the disease such that tumors that are responding to therapy will continue to involute. However, more effective therapy is needed for those patients who develop progressive disease (PD) or relapse or do not meet the defined treatment endpoint after 8 cycles of chemotherapy as these patients continue to have poor outcome. This will await the result of current biological and genomic studies and the development of novel therapies targeting ALK or RAS, or epigenetic targeting of MYC or MYCN activated tumors, or immunotherapeutic approaches such as adoptive cell therapies consisting of chimeric antigen receptor (CAR T cell) or, NK cells, or the use of immunocytokines such as hu14.18-IL2 or antibody drug conjugates. All these holds promise for future sub-stratification and new therapies for children with intermediate and high-risk neuroblastoma. This concludes this JCO Podcast. Thank you for listening.

In this episode, I feature examples of the AI technological take-over and of the impact of social media through technology on our lives. A technocracy involves unelected figures running countries and society, and this is the world of the Hunger Games society, which is relevant to stories I feature this week. Theresa May claims traffickers are using travel agent style adverts on Facebook to encourage migration into the UK. I have said before that migration is a co-ordinated agenda, and this is another example. Social media is causing mental health problems for kids, according to Javed Khan, Barnardo's chief. Three kids in every classroom are suffering with mental health problems and I place this into context with other pressure on kids. Amazon is aiming to open 3,000 cashierless stories by 2021. I talk about the wider context of this story. Amazon also plans to make their Amazon Echo Alexa assistant a bigger part of people's lives. I talk about what this is really about, and it's not convenience. Police 'Minority Report' style technology has faced calls to be regulated, as it may be unreliable. I talk about the bigger picture of this story and where law is heading in the not too distant future...
