Dr. Michael Weinstein interviews Dr. Eric Shah, director of the Michigan Medicine GI Physiology Laboratories about a study showing that despite a federal law that says bowel preparations needed for colonoscopy should be covered by insurance, many patients are still paying out of pocket. The study, from the University of Michigan Health, analyzed nearly 2.6 million insurance claims from both commercial insurance plans and Medicare Part D, and looked at how many patients paid for bowel prep medications needed for colonoscopy. The Affordable Care Act (ACA) requires that certain preventative services, including colorectal cancer screening, must be covered by insurance plans without cost-sharing for patients. Unfortunately, prescription bowel prep for screening colonoscopy is still subject to cost sharing for most patients, running contrary to the ACA's mandate. When considering that nearly half of patients are cost-shifted toward the use of non-FDA approved over-the-counter regimens, the data suggest that only 17 percent of patients pay zero for their bowel preparation for screening colonoscopy. Of the prescription claims analyzed, only 36 percent had no out-of-pocket costs. Additionally, high-volume preps were more likely to be free compared to new, low-volume or tablet preparations. However, when patients did pay, high-volume preps cost a median of $10, while low-volume preps cost $60. For Medicare Part D, only 25 percent of high-volume preps were free, compared to 10 percent for low-volume preps. Join Dr. Weinstein and Dr. Eric Shah as they discuss how these co-pays for bowel prep can impact patients and what private practice gastroenterologists can do to educate their communities and to engage in advocacy so that co-pays do not present a barrier to colorectal cancer screening. It seems like hospitals have been in the news a lot in recent years related to hospitals consolidating into larger systems. What is driving these activities? How does hospital and health system consolidation impact access to care, particularly for rural Michigan? What are the benefits of hospital mergers and acquisitions? Do they lead to increased healthcare costs? Do you anticipate we will continue to see more activity like this in the near future? All About Sudden Cardiac Arrest

With Dr. Kim Eagle, a cardiologist from the University of Michigan and the Director of the Frankel Cardiovascular Center.   Did you know that October is Sudden Cardiac Arrest Awareness Month? What is a sudden cardiac arrest and how does it differ from a heart attack? Although the two are thought to be similar, they are actually quite different. A heart attack occurs when there is an insufficient flow of blood to the heart. This is typically caused by a circulatory problem such as a blockage or an Aneurysm. But a sudden cardiac arrest results from “an electrical failure” where the heart malfunctions due to an irregular rhythm and suddenly stops beating. While a heart attack may be preceded by a history of warning symptoms, such as fatigue or shortness of breath, a sudden cardiac arrest or SCA usually occurs without warning, and in 95% of the cases, it will be fatal. Although an unexpected blow to the chest, such as the one suffered by the Buffalo Bills' Damar Hamlin, could disrupt the heart's electrical rhythm, an acute onset of emotional distress or anxiety could also trigger life-threatening arrhythmias. The signs of sudden cardiac arrest are a sudden collapse and the loss of consciousness. If a person has stopped breathing after collapsing, it is likely they're experiencing an SCA. It is critical to act immediately by calling 911 and promptly administering CPR. The likelihood of survival may increase if you have access to a portable defibrillator, more commonly known as an AED. These devices are frequently found in public places such as sports stadiums and come equipped with voice-guided instructions for use by untrained individuals. Prior to her death in October of 2023, Florine recorded an interview with Dr. Kim Eagle, a renowned cardiologist from the University of Michigan and the Director of the Frankel Cardiovascular Center. If you want to learn more about the signs and symptoms of heart disease and the simple lifestyle changes that could lessen your risk of a heart attack, please listen to Florine's interview with Dr. Eagle.   Please note: This episode was previously recorded prior to Florine's death in October of 2023.   What You'll Hear in This Episode: How does heart disease impact other health conditions such as diabetes, high blood pressure, and obesity? What type of screening occurs for cardiac health? The importance of getting an annual physical. How does the lack of resources affect those who get treated for cardiovascular disease? What are four simple things we can do to improve our heart health today? What foods should we avoid for heart disease, and which should we try to eat more of? What about coffee or alcohol? How regular exercise helps to extend our life span. How Dr. Kim gets in his 10,000+ steps a day.   Today's Takeaway: I want to thank Dr. Kim Eagle for being my guest today. Heart disease is the leading cause of death for men and women in the United States. Every 36 seconds, someone in the U.S. dies from cardiovascular disease. On an annual basis, every one in four deaths in the U.S. are caused by heart disease. Since 2014, the financial toll in the U.S. from heart-related deaths including medical services, prescription drugs, and loss of productivity due to death exceeds $219 billion per year. That's $219 billion and this year's numbers might well exceed that figure due to the complications of COVID-19! Something's got to change. If you want to ensure that you and your loved ones don't become part of these statistics, Dr. Eagle has given us four simple changes that you can implement today. If you just follow his suggestions, you can lessen your risk of developing heart disease. I'm Florine Mark, and that's “Today's Takeaway.”   Quotes: When we talk to patients about trying to understand their risk and prevent future events, the first thing is to start with understanding their numbers, and that requires them to work with a doctor.” — Dr. Eagle [4:52] “It's very important to have an annual physical.” — Dr. Eagle [6:05] “Cardiovascular disease and resources matter.” — Dr. Eagle [6:35] “When there is a disparity of socio-economic status, this can then lead to a lack of access to medical care, preventive therapies, and so forth.” — Dr. Eagle [7:51] “If you look at your plate, you want your plate to be rich in color, full of fruits and fresh vegetables.” — Dr. Eagle [12:07] “A Mediterranean diet is rich in fruits and vegetables, where the protein is a healthy protein.” — Dr. Eagle [13:11] “Any movement at all is incredibly healthy for cardiovascular health, and also mental health.” — Dr. Eagle [16:37]   Brought to You By: Gardner White Furniture   Mentioned in This Episode: Dr. Kim Eagle Frankel Cardiovascular Center, University of Michigan Health

Go deeper, Why does MHA believe the bills will negatively impact business owners and the general public? What brought it about? Was it an idea floated to deal with talent, or something else? MHA Issues Statement On Important Auto No-Fault Legislation - tell us more? That pair of bills aren't the only ones you are following in Lansing, what are some others we should be aware of? Up to speed: 82% of Michigan Voters Oppose Mandated Hospital Nurse Staffing Ratios LANSING, Mich. — The Michigan Health & Hospital Association (MHA) released data today illustrating strong public opposition to legislation proposing one-size-fits-all state-mandated hospital nurse-to-patient staffing ratios and hospital survey data on the severe impact of the legislation on access to care for Michiganders. A survey of 600 Michigan voters conducted by EPIC-MRA in August shows 82% of Michigan voters do not believe the government should mandate one-size-fits-all nurse-to-patient staffing ratios in every hospital, which is currently proposed in Senate Bills 334 – 336 and House Bills 4550 – 4552. If the bills were to become law, 83% of Michigan voters would be concerned about their ability to receive care, or the wait times, in a Michigan hospital emergency room. BRIAN PETERS“These survey results are abundantly clear; Michigan voters have no appetite to remove hospital staffing decisions from clinical nursing leaders to implement an arbitrary one-size-fits-all mandate by politicians,” said MHA CEO Brian Peters (left). “Such a decision would be harmful to patients and have dire consequences for healthcare throughout the state. Our hospitals and health systems are focused on proven solutions to address staffing shortages that address the talent pipeline and retain existing nurses.” The MHA also released the results from a survey of 109 Michigan hospitals conducted in July and August on the potential impact of the proposed legislation. Implementing staffing rations will either require hiring 12,954 registered nurses or the state risks closing up to 5,074 hospital beds to comply. These results follow a survey of 95% of the MHA membership in March 2023 which found Michigan hospitals had 8,438 immediate openings for nurses amid a nationwide nursing shortage. The loss in hospital bed capacity is roughly equivalent to Michigan closing its six largest hospitals determined by licensed hospital beds. “The significant and devasting impacts these bills can have on patient care and patient access make these the top concern for hospitals and health systems throughout our state,” said Shannon Striebich, MHA Board Chair and Ministry President and Senior Vice President of Operations at Trinity Health Michigan. “We value our nurses and are working diligently to offer recruitment and retention options that do not come at the expense of access to care for Michiganders.” Hospital staffing decisions and nurse-to-patient ratios are currently made by nursing leaders in each individual hospital based on years of clinical experience and a complex set of variables. These decisions weigh a multitude of factors which vary from each hospital and community and can include the number of patients in a hospital unit, how sick each patient is, the training and experience level of nurses and other members of the care delivery team, available technology and existing hospital data and metrics.

You helped us understand its qualities and features. Where else in Michigan would someone have to go for these, or is this the only one? 500 W. Grand Avenue on the downtown Lansing riverfront. First, give listeners who are only slightly familiar with the capital city an idea where that is, and was the riverfront selected to tie in with other placemaking efforts for the area? Will this be a seasonal Spring to Autum location or be used year around? LANSING, MICH. —Play Michigan!, the region's first universally-accessible playground, is open after a ribbon cutting Friday, September 15. Play Michigan! is designed to meet the needs of children with physical, sensory, and developmental disabilities. The playground features wheelchair-accessible play equipment, specialty swings and ramps, sensory play areas, and other inclusive features, ensuring that children of all abilities can play side by side. The 10,000-square-foot playground is part of a 66,000-square-foot park, located at 500 W. Grand Avenue on the downtown Lansing riverfront. The park also includes a picnic deck, pavilion, and dedicated barrier-free parking. In addition, a wide boardwalk and dock allow every child to experience the waterfront. Feedback from the Autism community prompted the Foundation to add safety features to help prevent children from running to the water. "As parents of children with disabilities, it can be so disheartening and such a struggle to find access to places and events that others attend without issue,” said parents Julia and Marlon Olivas. “We are so excited and grateful for such a beautiful, sensory-friendly playground that is so well thought out to consider the needs of all community members and be accommodating and inclusive to everyone regardless of ability, so kids like ours can have fun and enjoy themselves without barriers." This innovative playground-park, designed to promote inclusivity and the well-being of children of all abilities, has become a reality thanks to generous sponsors and gifts from community supporters, all matched dollar-for-dollar by the Community Foundation. Major funders include AF Group, CASE Credit Union, University of Michigan Health, Mary Free Bed, Peckham, John and Jewel Bos, Eileen and Jerry Fhaner, and Linda and Joel Zylstra. Linda Zylstra initiated the concept as part of the Community Foundation's vision for developing the downtown Lansing riverfront. "After overcoming pandemic-related delays and permitting challenges, we are delighted to unveil Play Michigan! to the public," said Laurie Strauss Baumer, president & CEO of the Community Foundation. "The original estimate of $1.6 million doubled over time to $3.2 million, but the Foundation was wholly committed to this project so that every child, regardless of ability, has the opportunity to play in an inclusive environment. While the Foundation led the project and contributed most of the cost, we couldn't have done it without our sponsors, donors, and partners – the City of Lansing, Wieland, Viridis Architects, Sinclair Recreation, and Disability Network." The Community Foundation is passionate about creating vibrant, thriving communities throughout Michigan's capital region. The Foundation serves donors to meet their unique charitable goals, supports area nonprofits with grants and coaching, and leads innovative projects that drive community growth.

The sleep economy is booming – from hi-tech sleep trackers to apps and cooling mattresses – and its worth is estimated at hundreds of billions of US dollars every year. So why do some of us still have trouble dropping off? Part of the explanation may lie in our sex, as some studies suggest that women are 40 per cent more likely to experience sleep disruption than men. And throughout a woman's life, puberty, pregnancy, menopause and caring responsibilities may all have an effect on the amount of good quality sleep available to women.
