Nuacht Mhall. Príomhscéalta na seachtaine, léite go mall.*Inniu an séú lá de mhí Mheán Fómhair. Is mise Barra Mac Giolla Aoláin.Tharla drochthimpiste ar an iarnród cáblach i Liospóin Dé Céadaoin. Maraíodh seisear déag agus gortaíodh duine is fiche eile ar an Elevador da Glória i lár na cathrach, a iompraíonn turasóirí agus muintir na háite idir cearnóg lárnach, Praça dos Restauradores, agus Bairro Alto, ceantar eile a bhfuil siamsaíocht oíche ann. Bhí daoine ó thíortha éagsúla i measc na n-othar gortaithe, lena n-áirítear an Ghearmáin, an Spáinn, an Fhrainc, an Iodáil, an Eilvéis, Ceanada, an Chóiré Theas, Maracó agus Rinn Verde. Deir an Roinn Gnóthaí Eachtracha nach bhfuil aon saoránach Éireannach i measc na dtaismeach. Tharla an timpiste mar gur bhris an cábla agus gur sciorr an carr den rian. D'fhógair an rialtas sa Phortaingéil Lá Náisiúnta Dobróin agus tá fiosrúchán ar siúl.Tá Conor McGregor tar éis físeán a phostáil lasmuigh d'Fhoirgnimh an Rialtais, ag impí ar dhaoine dul i dteagmháil lena gcomhairleoirí contae chun tacú leis mar iarrthóir uachtaránachta. Chun a bheith ar an mbosca ballóide, caithfidh sé ceithre údarás áitiúla nó 20 ball den Oireachtas a fháil lena thaobh. Osclóidh fuinneog na n-ainmniúchán amárach agus caithfear iad a chur isteach roimh an cheathrú lá is fiche de mhí Mheán Fómhair. I bhfíseán dhá nóiméad go leith, luaigh sé Forógra na Saoirse agus rinne sé comhartha na croise ag an deireadh. I mí na Samhna anuraidh, d'aimsigh giúiré i dtriail shibhialta go ndearna McGregor ionsaí gnéis ar Nikita Hand in 2018, agus tugadh damáistí beagnach €250,000 di. Chaill sé a achomharc sa chás i mí Iúil i mbliana. Tá sé ráite ag Donald Trump, Uachtarán na Stát Aontaithe, go mb'fhéidir go gcuirfeadh sé comhaontuithe trádála ar ceal má deir an Chúirt Uachtarach go raibh na taraifí a thug sé isteach neamhdhleathach. Rialaigh cúirt achomhairc le déanaí nach raibh údarás aige na taraifí a ghearradh gan cead na Comhdhála. Deir Trump go bhfuil muinín aige go rialóidh an Chúirt Uachtarach ina fhabhar, ach d'admhaigh sé go mbeadh caillteanas mór ann dá gcaillfeadh sé. Mhaígh sé freisin go raibh an tAontas Eorpach ag íoc “beagnach trilliún dollar” leis na Stáit Aontaithe, cé gur i ndáiríre is iad lucht iompórtála Meiriceánach a íocann na taraifí sin. Rinne an Seanadóir Mitch McConnell comparáid idir polasaithe Trump maidir leis an gheilleagar agus gnóthaí eachtracha agus polasaithe na naoi déag tríochaidí, agus é ag rá gurb é seo "an tréimhse is contúirtí ó roimh an Dara Cogadh Domhanda".*Léirithe ag Conradh na Gaeilge i Londain. Is our obsession with celebrities, sports, consumer culture and social media quietly dismantling democracy? Psychotherapist and award-winning author Kurt Warner thinks so, and he's sounding the alarm in his new book, False Idols: How Diversion Is Destroying Democracy. Please refer to the press release below for additional information, and let me know if you would like to receive a copy of False Idols for interview and/or review purposes. Watch a recent interview regarding one of Kurt's previous books here: https://www.youtube.com/watch?v=zFDCwwnAZ0c. FOR IMMEDIATE RELEASE Are ‘Bread and Circuses' Dismantling Democracy? New Book Warns of the Hidden Cost of Modern Distractions NEW YORK, August 19, 2025 — In ancient Rome, poet and satirist Juvenal used the phrase “bread and circuses” to refer to superficial appeasements used by the powerful to placate the public and distract them from political corruption. In False Idols: How Diversion Is Destroying Democracy, licensed clinical social worker, psychotherapist and award-winning author Kurt Warner revives and reinterprets this age-old warning, examining how contemporary America has fallen into the same trap — only now, the distractions are more abundant, more seductive and more dangerous than ever. “This is a call to arms for people to reprioritize their values for the betterment of society and for the betterment of themselves,” Warner explained. “It suggests that we are asked and led to desire and chase what we don't want rather than focus on what we do want by systemic forces in society.” Drawing on both psychological insight and social critique, False Idols dissects the many ways society is conditioned to idolize the wrong things: celebrities, musicians, athletes, influencers, political personalities, brands and even virtual realities. Warner argues that this cultural fixation is no accident — it's a systemic diversion, engineered to pacify the masses and preserve the status quo, all while democratic ideals quietly wither. “As long as everybody is chasing and distracted by these ‘bread and circuses,' they are willfully negligent to the goings-on in their society, government and country,” Warner noted. A compelling mix of social commentary, political philosophy and psychological insight, False Idols is a wake-up call for anyone feeling disillusioned by the noise of modern life and the hollow allure of American celebrity and consumer culture. “As a licensed clinical social worker, it is my passion to pursue macro and micro level change to bring about betterment in both systems and individuals — this is the goal of the book,” Warner added. Heart failure remains one of the most urgent challenges in health care, affecting millions of individuals and imposing a significant burden on families and the broader health system. Although guideline-directed medical therapy (GDMT) has been shown to improve clinical outcomes and reduce hospitalizations, adoption of these therapies remains suboptimal nationwide. Addressing this treatment gap is essential not only for enhancing survival and quality of life but also for mitigating the rising economic impact of heart failure as its prevalence continues to increase. Gluckman also led the session “Addressing Underuse of Guideline-Directed Medical Therapy in Heart Failure” at the July 9 Institute for Value-Based Medicine® (IVBM) event in Garden Grove, CA, where he addressed the need for integrated approaches to drive sustainable, system-level change in cardiovascular care. On this episode of Managed Care Cast, Ty Gluckman, MD, MHA, FACC, FAHA, FASPC—a practicing cardiologist at Providence Heart Institute in Portland, Oregon, and medical director of Providence's Center for Cardiovascular Analytics, Research, and Data Science—discusses strategies to improve GDMT implementation. The conversation explores opportunities for payers and providers, the potential of remote patient monitoring and digital health tools, and the role of value-based care models in supporting optimal therapy. Emphasis is placed on the importance of aligning clinical guidelines with managed care policies to drive meaningful improvements in patient outcomes.

Episode 80: Balancing Relief and Risk: Pain Management and Opioid Prescribing in Children and Adolescents Evaluation and Credit: https://www.surveymonkey.com/r/medchat80 Target Audience This activity is targeted toward primary care physicians and advanced providers. Statement of Need This podcast will address effective pain management in adolescents and teens and the utilization of opioids and risk reduction. Pediatricians may not have up-to-date knowledge and skills to effectively balance pain management with opioid safety in children and adolescents. Current practice often reflects underuse of multimodal pain strategies, inconsistent application of opioid prescribing guidelines, and limited screening for substance use disorders (SUDs) in youth. This educational activity addresses the gap between current and optimal practice by enhancing pediatricians' competence in evidence-based opioid prescribing and their performance in implementing risk mitigation strategies in clinical settings. Objectives Describe evidence-based guidelines for prescribing opioids in a manner that optimizes both pain treatment and safety for children and adolescents (“youth”). Discuss evidence-based strategies for the prevention, screening, and treatment for substance use disorders in youth.
