What if the biggest growth opportunity in your clinic isn't new technology, better marketing, or more patients—but the person standing right beside you?In this episode of The 20/20 Podcast, Dr. Harbir Sian sits down with Dr. Romie Dhaliwal and optician-entrepreneur Arshdeep Manghera to unpack a topic that quietly shapes every eyecare practice: the relationship between optometrists and opticians.Too often, that relationship sits somewhere between neutral and strained—defined by silos, hierarchy, and missed opportunities. But Romie and Arsh have built something different. From a pivotal moment during pregnancy to a bold decision to co-own a practice, Romie shares how she stepped away from comfort and into uncertainty. Arsh brings a story rooted in persistence—building a relationship with a clinic owner over seven years before finally acquiring the opportunity to take over. Together, they transformed a 30-year-old optical into a modern, thriving practice that blends clinical care, retail excellence, and community connection. But what makes their story truly compelling isn't just the business success—it's how they've achieved it.By breaking down the traditional “front vs back” divide, they've created a seamless patient experience where clinical knowledge and optical expertise work hand-in-hand. Their collaboration allows for better patient education, stronger trust, and more effective treatment adoption—especially in areas like dry eye.Beyond the clinic walls, they've reimagined what an optometry practice can be. From hosting comedy shows and wellness events to partnering with local businesses and community organizations, they've turned their space into a hub for connection. It's not just about attracting patients—it's about becoming part of the fabric of the neighborhood. This conversation also explores the realities of ownership—structuring partnerships, navigating legal complexities, and building systems that support growth. And as they look ahead, Romie and Arsh are investing in branding, social media, and influencer collaborations to take their practice to the next level. At its core, this episode is a reminder that growth doesn't always come from doing more—it often comes from working better together.Key TakeawaysCollaboration between ODs and opticians can dramatically elevate both patient care and business growth Breaking down traditional roles leads to better communication, trust, and outcomes Community engagement is a powerful (and underused) marketing strategy Old-school networking still creates massive opportunities Growth starts when you step outside your comfort zone Memorable Quotes“If we're not collaborating with each other, how can we collaborate with anyone else?” “Working together has taken our patient care from neutral to above and beyond.” “You either win or you learn—the worst they can say is no.” “When you get too comfortable, you don't grow.” Connect with the Guestseyesonacademy@gmail.com @academyoptical @drromiedhaliwal @thetailoredopticianLove the show? Emma Jane Kirby has worked as a Foreign Correspondent for the BBC, written documentaries, audio drama, a novel, and so much more. The main reason for their interview is the recent audio series A Most Audacious Heist, that looks in to real life theft of the Sisi Star, which looks into the lives of Empress Sisi herself, and the likable rogue Daniel Blanchard, who sole the star in a way that gives Ethan Hunt from the Mission Impossible films a run for their money. Or did he? The conversation looks at how EJ's history as a foreign correspondent for the BBC has inspired her writing these great stories based on true stories. They also discuss the audio series The Butterfly King, which is about a former European king, who died after meeting Adolf Hitler, and after denying Hitler's request to hand over Jews that were being protected in their country. They also discuss EJ's novel, The Optician of Lampedusa, based on the true story of an optician who whilst vacationing on the Meditteranean Sea, saved a few hundred people trying to escape oppression in their own country, who were floating after their boat sank. Also mentioned during the show is the superb work that production company Blanchard House, under the creative directorship of previous show guest Rosie Pye. In this episode, keynote speaker and culture expert Greg Hawks shares his philosophy of Ownership Culture and how practices and businesses identify where their people are and engage them deeper. Key Takeaways:Culture starts with the values the business has and are often the really good things of how the practice runs. Start by defining and spotlighting them regularly.Culture in a small business is easier than a large business. Small changes make big impacts in short time.Ownership Culture focuses on employee engagement. Owners are engaged. Renters are disengaged. Vandals are actively disengaged and are divisive, toxic, and actively sabotaging the business's success. Owners show up daily with "head and heart", doing their job with drive and passion. Renters do their job well, offering their skills but are not engaged further. The reasons vandals stay within a business:They are significant revenue generators and they know itThey've been there a long time and it's awkward now to do something about itNepotism - they're family membersThey are also the ownerRenters often become renters because they don't have the ability or opportunity to offer their full thoughts, ideas, and view of the world into how the business improves and grows. Creating space for this helps move renters towards owners.More Resources:Episode 50: Culture for Cold Starts with Davis Capaccioli, ODFor more on Ownership Culture - Email Dr. ManningWhat Greg is Reading:The 12 Week Year by Brian Moran

The Top 5 Pitfalls Hurting Your Optical with Kayla Groves

In this episode, I sit down with PECAA's Optical Business Advisor, Kayla Groves, to discuss the biggest pitfalls that practices fall into with their optical. Key TakeawaysPitfall #1: Overstocking of frames. Turn rate should be 3x on average in a practice. Frames that aren't turning at this rate shouldn't be stocked.Pitfall #2: Capture rate isn't calculated or calculated poorly. Not having this data affects ability to make key decisions well within the optical.Pitfall #3: Not maximizing reimbursement from insurance by taking wholesale and Frames Data and taking into consideration the type of optical they have: heavy managed vision care, budget, or luxury. Pitfall #4: Not actively managing the frame board and inventory, taking into consideration the practice goals, the practice business model, demographics, price assortment, and turn rate.Pitfall #5: Not training all team members on the essentials of the optical.Optical is still a significant opportunity for growth in private practice. The patients purchasing online and from e-commerce players are not ones who would purchase anyways from a practice. A large proportion of patients still want an in office experience. What Kayla is reading:The Missing by Kiersten ModglinWant to learn more about PECAA's Optical Management resources? For thousands of years, humans have communicated history and new ideas through stories and storytelling. This powerful tool has a dramatic impact on your practice's ability to attract new patients and capture more optical Sales. In this episode, PECAA Member Business Advisors Shelby Wade and Kayla Groves share strategies for maximizing your success. Key Takeaways:Without wrapping them up in a story, humans only remember 5% of facts that you tell them.Patient consumers make purchasing decisions first on emotions and then support that decision with logic and rationalization. Storytelling opens up that emotional connection that primes the individual to purchase from you. In the optical conversation, tell stories that connect with the patient. It's ok to tell your story that helps then connect to your passion for optical and eye care. The story should be easy for the patient to connect to their own life and their own story. Don't just show the frame and style the patient but share the story of the brand and what matters mostSpend time with the frame reps that come into your practice in order to understand not only the quality of product but know the brand story. What Kayla is reading:

On this episode, I sit down with one of the foremost experts on sales in eyecare, Robert Bell, founder of EyeCoach. In this episode, we discuss how we really should look at sales and strategies for improving the sales experience within your practice. Key Takeaways:True sales is not persuading or convincing someone else to buy your product or service. Sales really should be seen as the process of helping someone get what they want and/or need."No" is not a bad word to hear in the sales process. The faster you can get to the "no", the easier and faster it will be to get to the solution that creates a win-win for the patient and practice. Don't let the word "no" be taken personally.Stop using the word "recommend." Start using "I'm prescribing."Only educate patients on the features and benefits after you've received the patient's permission and only share the features and benefits that directly connect to the patient's problems and needs. What Robert is reading:Predictably Irrational by Dan Ariely On The Road by Jack Kerouac
