Today's episode is a deep dive into tactile defensiveness and sensory distress, especially around clothing. My guest is Kathryn Hamlin-Pacheco, an occupational therapist who helps kids and families understand their brains and bodies through everyday neuroscience. Kathryn will break down what's actually happening in the brain and nervous system when children experience tactile defensiveness, and why clothing can feel so overwhelming for some kids. We talk about the role of co-regulation, how parents can help create positive associations with getting dressed, and practical strategies for supporting children in navigating their sensory experiences with more safety and less stress. This episode is a grounding, compassionate look at sensory processing—and a reminder that when we understand what's underneath the behavior, everything shifts. About Kathryn Hamlin-Pacheco Kathryn (Katie) Hamlin-Pacheco, M.S., OTR/L, ASDCS, is an occupational therapist, former teacher, author, and founder of the Brain Executive Program. Kathryn is an Autism Spectrum Disorder Clinical Specialist (ASDCS) and holds certifications in Neuroscience for Mental Health Professionals and in Brain Structure and Function: Application to Sensory Integration and Processing. She graduated from Virginia Commonwealth University with a Master's degree in Occupational Therapy, where she also worked with the Virginia Leadership Education in Neurodevelopmental Disabilities program to pursue her desire to be an advocate and leader in pediatric healthcare. She has shared her work at AOTA's Inspire Conference (the world's largest gathering of occupational therapy practitioners!), Sensory Integration Education's international conference, and at William & Mary's Center for Gifted Education. In addition, Katie has written for OT Practice Magazine, Autism Parenting Magazine, Washington Family Magazine, and Stars & Stripes Magazine. We are now seeking feedback on a proposed draft of the guidelines. The guidelines are meant to inform providers from a variety of backgrounds including gynecology, urology, oncology, family medicine, sexual health, pelvic floor physio and midwifery. We are currently seeking feedback on these guidelines from patients who have received pelvic health care in the past 5 years, who also have a history of experiencing medical or sexual trauma. The survey should take about 15-20 minutes and is best completed on a computer. The survey is approved by the Conjoint Health Research Ethics Board. On this episode, we are joined by Jessie Perchaluk, an occupational therapist and Certified Hand Therapist who has taken a special interest in learning more about and implementing remote therapeutic monitoring (RTM) into her practice. Jessie shares with us the various ways that we can utilize RTM as an adjunct to the care we currently provide and how we can stay connected to our patients.Jessie Perchaluk, OTD, MS, OTR/L, CHT, CLT is an occupational therapist at NovaCare Rehabilitation (a division of Select Medical), and a core adjunct faculty member at Thomas Jefferson University in their hybrid MSOT program, as well as adjunct instructor at the Medical University of South Carolina's hybrid OTD program. She is a board-certified hand therapist and lymphedema therapist specializing in upper extremity and oncological rehabilitation, balancing full-time clinical practice with graduate-level teaching in functional anatomy, clinical skills, and upper extremity rehabilitation.With over eight years of experience, Jessie is passionate about delivering personalized, evidence and data informed care in order to restore function and improve quality of life. She is equally dedicated to advancing the profession through education, mentorship, and research. Jessie leads student fieldwork and capstone initiatives across Pennsylvania and Delaware for Select Medical, to which she has been nationally and locally recognized for her work.Jessie also serves on AOTA's Commission on Education as the fieldwork educator representative, contributing to national education policy and best practice initiatives. She combines her clinical knowledge and passion for furthering evidence and data informed care by continuing to be actively involved in ongoing research projects. Jessie is further advancing her knowledge in healthcare at Brown University in their Masters of Healthcare Leadership.When she is not working, Jessie enjoys spending time with her husband and three daughters. But in practice, the unthinkable nearly came to pass last week when a high school student in Baltimore County, MD was hanging outside his school and was soon confronted by armed police ordering him to the ground. The boy had finished a bag of Doritos, crumpled it up and placed it in his pocket, which the school's AI security system deemed to be a weapon. In this special episode, you will be learning from two leading powerhouses in Occupational Therapy leadership, Samia Rafeedie, Samia H. Rafeedie, OTD, OTR/L, BCPR, CBIS, FAOTA (OTAC) on a state level and Arameh Anvarizadeh, OTD, OTR/L, FAOTA (AOTA) on the national level. We will learn about their experiences at the AOTA Hill Day event in Washington D.C., where occupational therapy practitioners meet with legislators to advocate for the profession. Both leaders shared their motivations for participating, with Samia highlighting her roles as an OTAC president and professor at USC, while Arameh emphasized her experience as AOTA president and the importance of collective advocacy in areas like mental health, school-based services, telehealth, and Medicare. We are so fortunate to witness our state and national associations in collaboration for OT Representation on Capitol Hill. You will know exactly what I mean when you listen to this energizing episode. Samia H. Rafeedie, OTD, OTR/L, BCPR, CBIS, FAOTADr. Samia Rafeedie is the Director of the Professional Program in Occupational Therapy at the University of Southern California (USC), and professor of clinical occupational therapy. She has been on the faculty since 2008 and has primarily taught adult physical rehabilitation, kinesiology, and topics on professional development and association engagement. Samia has been an occupational therapist for 24 years, with additional practice interests in productive aging and geriatrics, pedagogy and higher education, mentorship, and volunteerism. She is the President of the Occupational Therapy Association of California, and is invested in strengthening the association by recruiting and maintaining members. Some of her favorite occupations include spending time with my family, taking trips together and going out to eat for Japanese food (our favorite!). She also LOVES volunteering for our state association. It fills her cup!Arameh Anvarizadeh, OTD, OTR/L, FAOTADr. Arameh Anvarizadeh is a Professor of Clinical Occupational Therapy and former Director of Admissions at the USC Mrs. T.H. Chan Division of Occupational Science and Occupational Therapy. As the architect of USC Chan's holistic admissions process, She led transformative efforts that resulted in the most diverse cohorts in the Division's history—all while preserving rigorous and effective admissions standards. Her leadership has also been pivotal in strengthening recruitment and retention strategies, creating pathway programs, and promoting inclusive pedagogy. A Founding Member and former Chair of the Coalition of Occupational Therapy Advocates for Diversity (COTAD), Dr. Anvarizadeh has been a driving force in advancing justice, equity, diversity, inclusion, anti-racism, and anti-oppression within the occupational therapy profession. She is the visionary behind many of COTAD's cornerstone initiatives, including COTAD Chapters, the COTAD Toolkit, the Ignite Series, and the Fieldwork Task Force. Dr. Anvarizadeh made history as the youngest and the first Black and Iranian woman to serve as Vice President of the American Occupational Therapy Association (AOTA) and to be inducted into the prestigious Roster of Fellows (FAOTA). She made history again with her landslide election as AOTA President, a role she officially assumed in July 2025. In addition to her current role as AOTA President, Dr. Anvarizadeh has held numerous national leadership positions, including Chair of the Credential Review and Accountability Committee (CRAC), member of the Representative Assembly Leadership Committee (RALC), member of the Governance Task Force, and liaison between the Governance and DEI Task Forces. She also served on the AOTA Special Task Force on Entry-Level Education and was a key contributor to the national dialogue on entry-level degree requirements. Dr. Anvarizadeh is a proud alumna of the 2020 Executive Leadership Program for Multicultural Women. In her home state of California, she has been recognized with the Janice Matsutsuyu Outstanding Service Award and the Vision Award from the Occupational Therapy Association of California (OTAC). She previously served as OTAC's Communications Chair for seven years and contributed to various committees, including Advocacy and Government Affairs and Conference Planning. An accomplished author and dynamic speaker, Dr. Anvarizadeh has presented at numerous state, national, and international conferences, delivering keynotes, lectures, and commencement addresses. She has authored multiple peer-reviewed articles and book chapters and is a contributing editor to the recently published book Occupational Identity. Most recently, Dr. Anvarizadeh is embracing her most meaningful role yet: motherhood. She is intentional about honoring this life shift, leaning into her community, and nurturing balance. Deeply passionate about building sustainable, healthy, and inclusive communities, she continues to champion equity, empowerment, and advocacy.
