In this episode of the NCS Podcast Currents series, host Lauren Koffman, DO, MS, FCCM, speaks with Tarek El Halabi, MD, who leads acute neurology services at the American University of Beirut Medical Center, about his Currents article, "Neurocritical Care in Lebanon: Progress Within Constraint." El Halabi didn't build a neuro ICU from the ground up. He joined an already established mixed medical-surgical unit and worked to change it from within, winning over colleagues and standardizing care for the emergencies that arise most often, including stroke, status epilepticus and bleeds. He and Koffman talk through what that took, from identifying the right people to lead each effort to investing heavily in nursing and resident training. While technical skills can be learned relatively quickly, building a lasting culture takes much longer. They also dig into what "resource-limited" really means in a country with highly trained physicians and advanced medicine. In Lebanon, El Halabi explains, the constraint isn't expertise, but financial access and consistency, since a patient's care can depend heavily on their insurance and where they happen to present. He describes how his team redesigned stroke workflows to meet AHA/ASA benchmarks despite these limits, as well as where the program goes from here, including adding more faculty, developing a neuro ICU fellowship, strengthening research ties with NCS's MENA region colleagues, and eventually expanding telemedicine outreach beyond Beirut. Show Notes Should an acute lower-extremity DVT automatically stop mobility when anticoagulation is subtherapeutic? In this episode, Dr. Leo Arguelles and Dr. Nicole Neveau speak with Aubrey Little, PT, DPT, CCS, and Becky Johnson, PT, DPT, PhD, about the clinical question that grew from a quality-improvement project into a case-control study published in the Journal of Acute Care Physical Therapy. Little and Johnson explain why a single laboratory value or protocol checkpoint cannot replace a comprehensive assessment. They discuss weighing the risks of immobility against potential adverse events while considering clot location, medical acuity, hemodynamic status, medications, laboratory trends, discharge pressures and the patient's overall trajectory. The conversation also explores the documentation needed to make clinical reasoning visible and the value of bidirectional conversations with the interdisciplinary team. The study included 30 patients and 60 physical therapy sessions. Its findings do not establish that mobilization at subtherapeutic anticoagulation is universally safe. The researchers emphasize the limitations of a small retrospective sample and their inability to determine whether pulmonary embolism occurred. Instead, the episode offers a thoughtful look at risk stratification, evidence-informed judgment and how clinicians can question automatic holds without abandoning patient safety. In this episode, host Erin Gallardo, PT, DPT, NCS, talks with returning guest Rose Tilton, OTR/L, OTD, an occupational therapist at Marquette University's Neuro Recovery Clinic, and new guest Dr. Yi-Kai Lo, PhD, CEO and co-founder of ANEUVO, about transcutaneous spinal stimulation (TSS) for people with spinal cord injury. Rose shares how she discovered TSS through her own research, became a clinical trial site investigator at Marquette, and now uses ANEUVO's ExaStim® device — a multi-channel electrode array system that can be individually programmed and timed to match specific functional movements — alongside other TSS devices on the market, including ONWARD Medical's ARC-EX. Dr. Lo walks through the engineering behind ExaStim, how the device is positioned and calibrated for each patient, and the proposed mechanism of amplifying descending brain signals to the spinal cord. Together they cover who's actually a good candidate (spoiler: it's hard to predict), what a typical treatment session looks like from aerobic priming through hand-strengthening work, and real patient outcomes ranging from improved grip and transfers to unexpected anecdotal gains, like bowel and bladder function for some. What does it take to move acute care physical therapy research from the journal page to the bedside? In this episode, Dr. Leo Arguelles and Dr. Daniel Young sit down with Dr. Barbara “Barb” Smith, associate professor of physical therapy at the University of Florida, clinician-scientist with more than 25 years of ICU experience, and recipient of the 2026 APTA Acute Care Lecture Award. Barb reflects on how acute care research has evolved from a time when PTs often contributed to studies without leading them to a future in which clinicians, educators and scientists shape the questions that matter most. Constraint-induced movement therapy has decades of research behind it, yet its exclusion criteria — especially around cognitive impairment — rule out a huge share of the stroke survivors clinicians actually see. Briana walks through how she built the protocol around safety and supervision in inpatient rehab: getting a physician order for the constraint mitt, educating and enlisting his wife, keeping a consistent daily schedule, breaking the three-to-six hours into non-consecutive blocks, and removing the mitt before late-day sundowning. She shares the outcomes that surprised even her skeptics — meeting the minimal detectable change on the Box and Block Test and a standardized motor scale within roughly thirteen days, plus real gains in grooming and feeding independence — alongside the honest caveats, including that his cognition never improved and that real-world delivery never looks as clean as the published article. It's a practical, encouraging conversation about reading the research, respecting its limits, and giving more patients a shot at an intervention they're usually told they can't have.
