The Psychology of Self-Injury: Exploring Self-Harm & Mental Health
What goes on at the neurobiological level that makes someone more vulnerable to self-injure or self-harm? What biological risk factors are at play? What roles do the vagus nerve, cortisol levels, and hypothalamic-pituitary-adrenal (HPA) axis have? You can have the perfect planner, the right system, and the best intentions, and still not follow through. It isn't a caring problem. After enough broken promises to yourself, some quiet part of you simply stops believing the plan. That's where this conversation with Dr. Tamara Rosier begins, and it reframes self-trust as something closer to a nervous system skill than a mindset you can think your way into.Dr. Tamara Rosier has written the books and built the center and stood on the stages, and she still wakes some mornings and reminds herself, deliberately, that she is a trustworthy person. The belief underneath — the one she's carried since she was small — is that she's a person who screws things up. ADHD feeds a belief like that. It chips away at your sense of who you are, one forgotten thing at a time, until distrusting yourself stops feeling like a wound and starts feeling like good judgment.So much of that, it turns out, is happening in the body. An ADHD nervous system can spend its whole life braced — fight, flight, freeze, appease — switched on and calling it normal because it has never known the alternative. For years Tamara sat frozen on the couch, melting into the cushions, sure she was resting, when she was really stuck somewhere below the place where rest actually lives. There's a narrow band where you're calm and awake at once, and a lot of us have never spent much time there. Hearing her describe it, you may quietly start to wonder whether you ever have.The way back looks like catching yourself mid-loop — Tamara tells it through the week she lost one of her chickens, and the refrain that trailed her around the house, I failed her, I failed her, I failed her — and then learning to talk back to it, to move your body, to put on the Motown, to do the next small thing that nudges you up out of the freeze. It looks like noticing the clever ways we avoid all of that, too: the new app, the next fix, the dopamine that keeps us busy on the surface so we never have to turn toward the thing underneath.And the hope here is almost disappointingly ordinary. No system is going to fix you by Thursday. What there is, instead, is the small correction, made again and again, the way a sailor nudges the tiller rather than wrenching the whole boat around and tipping it over. There's learning to read your own weather, hour by hour. There's accepting that you may always need the timer, the Post-it, the reminder, and letting that be fine rather than shameful. I think that this is an important time to pause and relook at Polyvagal Theory before continuing with Beyond Behaviors. Polyvagal Theory: Current Status, Clinical Applications, and Future Directions (Porges, S. 2025) "Social behavior and the capacity to manage challenge are dependent on the neural regulation of physiological state." S. Porges When I dove into Stephen Porges's 2025 review of the Polyvagal Theory (PVT), I felt like I'd stepped into a crossroads where neurobiology, clinical practice, trauma science, and human experience collide. This paper isn't merely a summary of three decades of work (all of which I have read); it's a spirited defense of a paradigm that's been both celebrated (by me) and contested (by others). What follows is an honest appraisal of what the article teaches us, where it sparks real insight, and where it may fall short, especially through the lens of evidence-based medicine and developmental neurophysiology. (I also went deeper into his 2022 paper in Frontiers in Integrative Neuroscience for the biophysiology of the ANS) At its heart, the article argues that the autonomic nervous system (ANS), through a set of hierarchically organized circuits centered on the vagus nerve, is not just a background player in stress and homeostasis, but a core regulator of social engagement, physiological flexibility, and behavior. Dr. Porges situates his theory as an alternative and expansion to classical views that treat sympathetic (fight/flight) and parasympathetic (rest/digest) branches as functional opposites. Instead, he proposes a three-component hierarchy: the ventral vagal complex (VVC) supporting social engagement, a mobilization circuit mediated by the sympathetic nervous system or fight or flight state, and a dorsal vagal circuit that facilitates shutdown or immobilization under extreme threat..... Neil Nathan, MD has been practicing medicine for over 50 years and has been Board Certified in Family Practice and Pain Management and is a Founding Diplomate of the American Board of Integrative Holistic Medicine and a Founding Diplomate of ISEAI. He has written several books, including "Healing is Possible: New Hope for Chronic Fatigue, Fibromyalgia, Persistent Pain, and Other Chronic Illnesses" and "On Hope and Healing: For Those Who Have Fallen Through the Medical Cracks." He has been working to bring an awareness that mold toxicity is a major contributing factor for patients with chronic illness and lectures internationally on this subject which led to the publication of his eBook, "Mold and Mycotoxins: Current Evaluation and Treatment, 2016" (now updated to 2022), and then to his best-selling book "Toxic: Heal Your Body from Mold Toxicity, Lyme Disease, Multiple Chemical Sensitivities and Chronic Environmental Illness." 2021 saw the publication of "Energetic Diagnosis," a discussion of the value of intuition and energetic devices as an aid to both diagnosis and treatment of medical illness. In 2024, his book "The Sensitive Patient's Healing Guide" was released with a focus on how to approach the treatment of sensitive patients. And most recently, in late 2025, the second edition of his book "Toxic" was released with several years of new information and insights on healing from complex chronic conditions. Dr. Nathan has been treating chronic complex medical illnesses for 30 years now, and Lyme disease for the past 20 years. As his practice has evolved, he finds himself increasingly treating the patients who have become so sensitive and toxic that they can no longer tolerate their usual treatments, and his major current interest is in finding unique ways of helping them to recover.
