In today's episode, I speak with Dr. Mark Kargela, a seasoned clinician and educator, about chronic pain—its complexity, its impact, and how practitioners can better support patients struggling with it. The discussion focuses on shifting clinical approaches from tissue-centric models to ones that integrate contemporary neuroscience and lived experience. Key topics include: Understanding Chronic Pain: The conversation defines chronic pain not simply by duration (e.g., beyond three months), but as a biopsychosocial phenomenon that fundamentally changes the nervous system. Dr. Kargela emphasizes that chronic pain is often not a direct marker of tissue damage, but a protective output of the nervous system influenced by biology, psychology, and social factors. Pain Neuroscience Education (PNE): Both clinicians stress the importance of PNE in helping patients reframe their pain experience. Teaching people how pain works can reduce fear and catastrophizing, improve self-efficacy, and set the foundation for graded movement and recovery. Central Sensitization and Nervous System Dysregulation: The discussion highlights how sensitized neural pathways can perpetuate pain even in the absence of tissue pathology. Techniques that modulate the nervous system—such as breathing, sleep optimization, gentle movement, and mindfulness—are presented as key therapeutic tools. Limitations of Traditional Biomedical Models: Dr. Kargela critiques overly mechanical approaches that chase tissue “damage” or perfect biomechanics, advocating instead for approaches that validate patient experiences and support behavior change. Clinician Takeaways: The episode encourages health and fitness professionals to: Ask better questions that uncover contributors to a person's pain narrative. Shift from “fixing” people to coaching them toward resilience. Recognize that empathy, listening, and patient-led goals are central to helping people move forward. We talk about his book, Pain and the Survivor Brain, and why understanding pain as a form of communication can change everything for both patients and practitioners. Kieran breaks down why people stay stuck in chronic pain, how the healthcare system often fails them, and what clinicians can do to truly help. Today's episode is a follow-up on last week's pain science episode, so make sure to check that one out at the following link before diving into this week's episode.  In today's episode, we look at the pain loop and how sensory messages (afferent messages) ascend up the spinal cord to the brain and then create efferent responses (movement, pain stress) that descend back down the spinal cord to relevant regions of the body and muscles that will help the person move away from dangerous stimuli. This episode also addresses the three major types of pain (nociceptive, neuropathic and chronic) and how we think about treating these types of pain. I hope you find this episode to be helpful! Brett shares insights from his scoping review on the integration of sleep measures in chronic low back pain research, highlighting a significant gap in current practices. The conversation covers the impact of sleep on pain, the role of wearables, and practical advice for clinicians on how to incorporate sleep assessments and interventions into their practice. Brett emphasizes the importance of understanding individual sleep needs and using tools like the CBTI Coach app to help patients improve their sleep quality.CBT-i Coach App Scoping Review TIMESTAMPS:00:00:00 Episode begins00:01:43 Interview Start00:02:53 Brett's Work on Sleep00:05:04 What did they find in their systematic reveiw?00:08:25 Clinical pearls gained in pre-conference course00:10:43 What apps or tools are out there for clinicians?00:13:46 Can tracking sleep excessively become anxiety generating?00:14:57 How do we determine the right amount of sleep for the individual?00:19:47 Mindulness or meditation for sleep00:23:12 How do we get this in curriculum?00:26:20 How do we get this as every day practice?00:31:59 Sleep as a part of the big picture in painKey Takeaways:Sleep and Chronic Pain Connection: There is a strong link between poor sleep and chronic pain. Addressing sleep issues can potentially alleviate pain symptoms.Research Findings: A scoping review revealed that less than 10% of chronic low back pain trials include sleep measures, highlighting an area for improvement in research practices.Role of Wearables: While wearables can provide useful sleep data, they should be used cautiously to avoid causing anxiety. It's best to view sleep data in the context of long-term trends rather than daily fluctuations.Screening and Interventions: Simple questions about sleep quality can help identify patients who may benefit from further sleep assessment. Tools like the CBTI Coach app, developed by the VA, can assist clinicians in guiding patients through cognitive behavioral therapy for insomnia.Individualized Approach: Quality of sleep is more important than quantity. Clinicians should tailor their recommendations based on the individual needs and experiences of their patients.Practical Tips for Clinicians: Incorporate basic sleep questions into patient assessments, use validated sleep measures, and educate patients on sleep hygiene practices. Encourage patients to find relaxation strategies that work for them, whether it's mindfulness, reading, or other calming activities.Educational Integration: Teaching about sleep and its impact on health should be integrated into PT education to better prepare future clinicians.*********************************************************************

Welcome to Episode 67 of the Movement Logic podcast! In this episode, Laurel and Sarah discuss what current science, versus outdated advice and conventional wisdom, have to say about the causes and solutions for sacroiliac joint (SIJ) pain. Learn what research says about whether or not the SIJ is an inherently robust or fragile structure, whether things like lots of stretching in yoga or joint laxity during pregnancy contribute to its instability, and what therapists can and cannot reliably know about the causes of SIJ pain. You will learn: The anatomy and biomechanics of the SIJ. How Sarah differentiates between low back pain and SIJ pain with her patients. What joint incongruency is and what therapists can and cannot know about joints through palpation. What amount of force is required to dislocate the SIJ. What SIJ form and force closure are, and how they are used to explain SIJ pain. Some common explanations, assessments, and treatments for SIJ pain that lack evidence. Why muscle testing is an unreliable way to assess muscle strength or weakness.  Happy Holidays ! Enjoy this best of replay of my discussion with Colleen Jorgensen about how we interpret chronic pain science.Welcome back to the work in everyone. There are so many frustrating things about health and wellness information. As fitness professionals and coaches we watch for the latest information in science and do our best to curate and explain it to our clients and students. As consumers of that information, it can be so confusing to try to understand what the latest science actually means for our day to day lives, and for a topic as complex as chronic pain, that becomes even more challenging. On the one hand, it's great to get new science, new understanding about the body and the nervous system so we can make the best choices in our care. On the other hand, what does that mean for everything that we knew before? does it all go out the window? How do we how do we integrate that new understanding of pain care science in meaningful ways, and share it when it comes to posture, alignment and movement therapy? My friend Colleen Jorgensen reached out to me a few weeks ago with this question as a topic for the podcast. And I thought it was a really great idea. I love bringing clarity to topics like this and I always learned so much from Colleen. She's been on the podcast before. We've talked about chronic pain, the nervous system pain care where language and the importance of professional communication skills. She's an osteopath, the yoga Pilates instructor and a teacher trainer was specialties in pain care where language among many other skills and she is the one I trust to help translate some of this new information. I am thrilled to have Colleen back on the podcast to talk more about this topic. And I think in the course of the conversation, we can find new ways to evaluate other areas of health and wellness science as well. So let's start our work. In with Colleen Jorgensen. As teachers, we have the amazing opportunity to create spaces where growth, freedom, safety, and a spirit of play can thrive! Today, we embark on an in-depth exploration of pain as a complex phenomenon. Pain is a manifestation of the intricate processes within the nervous system, which processes information from various sources. Everything, from the minutest detail of ensuring our clients feel safe and cared for to the language we employ and even our grounded presence as instructors, collectively influences how the nervous system perceives stimuli, ultimately determining whether pain is triggered. Today, we are thrilled to introduce you to the first guest on the podcast, Colleen Jorgensen! Colleen is an osteopath, athletic therapist, pain care educator, speaker, and Therapeutic Pilates, Yoga, and somatic teacher. Having endured a decade of pain herself, Colleen possesses firsthand knowledge of its challenges. Some of the internal signals that we receive as a result could be interpreted as pain, so how do we know when we should push through and when we should pull back? Sam Visnic is a Certified Massage Therapist specializing in neuromuscular and clinical massage, as well as a Certified Corrective Exercise Therapist. He's also the host of Who Knows This?? Podcast. Follow Sam Visnic @releasemuscletherapy Follow Chase on Instagram @chase_chewning Key Highlights What is hypnotherapy? Hypnotherapy is a therapeutic practice that uses guided hypnosis to help a client reach a transcendent-like state of focus, concentration, diminished peripheral awareness, and heightened suggestability. Sam explains how hypnotherapy and sensory-based focus can be used in therapeutic processes to reconstruct our internal experience. He notes the similarities and differences between hypnotherapy, meditation, and mindfulness. The world is so stimulating that many of us have realized we're not as relaxed as we think we are. That's why people feel loopy and out of it after their first hypnotherapy session - it's because they're actually relaxed and they've forgotten what that feels like. Once you build that awareness, you can begin cultivating more of that in your life. Sam describes how creating deep change isn't harder than creating simple change… it just requires a different set of skills. Resilience is one of the best skills you can develop to create change, maintain momentum, and accomplish your goals. People are using hypnotherapy to change the gut microbiome, improve gut health, and even heal IBS. How? Does hypnotherapy work just as well as the FODMAP diet? Tune in to find out! Lastly, Sam highlights the benefits of hypnotherapy in treating chronic pain.
