Challenges to Technology Adoption in Africa: Both Dr. John and Jaapie highlighted several barriers to adopting RPM technology: Device Compatibility and Connectivity: Limited access to smartphones and stable internet connectivity is a significant challenge. Many patients rely on basic phones and are hesitant to allocate resources for more advanced devices. Trust Issues: Patients often worry about data privacy and fraud, especially in regions like South Africa, where cell phone fraud is common. Providers must build trust by ensuring data security and clarifying the purpose and safety of the technology. Power and Infrastructure Limitations: In Nigeria, power outages and the high cost of diesel for generators disrupt patients' ability to use RPM devices consistently. Economic Considerations and Funding Models: Bruce Adams noted that RPM adoption heavily depends on who pays for the service. In many African countries, healthcare services involve a mix of out-of-pocket payments and insurer-funded models. In South Africa, as explained by Jaapie, medical aid schemes (insurers) play a critical role in covering high-risk, chronic condition patients. However, integrating RPM into existing reimbursement structures and ensuring that providers are compensated for remote care remain challenging. Localized Implementation of UK-Inspired Technology: Bruce Adams stressed the importance of adapting Clinitouch's UK-developed RPM platform to local African contexts. Abstract: Detailed characterization of interneuron subtypes in primary visual cortex (V1) has greatly contributed to understanding visual perception, yet the role of chandelier cells (ChCs) in visual processing remains poorly characterized. Using viral tracing we found that V1 ChCs predominantly receive monosynaptic input from local layer 5 pyramidal cells and higher-order cortical regions. Two-photon calcium imaging and convolutional neural network modelling revealed that ChCs are visually responsive but weakly selective for stimulus content. In mice running in a virtual tunnel, ChCs respond strongly to locomotion and halting visual flow, suggesting arousal-related activity. Visuomotor experience in the tunnel diminished visual responses of ChCs and induced structural plasticity of ChC boutons and axon initial segment length. Finally, ChCs only weakly inhibited pyramidal cells. These findings suggest that ChCs provide an arousal-related signal to layer 2/3 pyramidal cells that may modulate their activity and/or gate plasticity of their axon initial segments during behaviorally relevant events. Today we're talking with Liben Gebremikael, the first Executive Director of TAIBU Community Health Centre in Toronto. He's held the position since 2008. Liben is a social worker with over 25 years of experience in the primary healthcare sector, social services, and mental health field. TAIBU was created to meet the specific healthcare needs of the Black Community – and as it's evolved, addresses all the social determinants of health including, education, income, lifestyle, and housing. Their philosophy of care is built around a Zulu word ‘Ubuntu', which means, we don't exist as individuals, we exist as a family and as a community. All issues at TAIBU are addressed from that lens and that philosophy. For Liben, there's excitement and a sense of satisfaction at developing a culturally appropriate approach to specific illnesses that affect the Black community – illnesses like sickle cell disease and diabetes for example. The larger issue that preoccupies Liben and his colleagues at TAIBU is the necessity of systems change. Eliminating barriers to care for racialized populations means big changes are needed at the systems level. As Liben explains, it begins with the four ‘As'. Quotables “Sometimes the role of a leader is very strategic, but I believe you need to continue to put your feet on the ground and really listen to people and hear people. Once a social worker, always a social worker!” – LG “People talk about a broken system. We say the system is not broken; it's working perfectly as it was designed. The challenge is, it was not designed for certain populations.” – LG “One of the African principles of self-determination is that our clients participate in design, in decision-making, in planning of services. Critical ingredients when you're talking about culturally appropriate approaches and services.” – LG “People are really engaging with the issue of equity, which is promising, but it requires a lot of work because it's very difficult to change the system quickly.” – LG “We are moving from just doing programs – we are incorporating the concept of systems change in all that we are designing moving forward.” – LG “If we continue in the long-term to address the systemic issue, then every CHC, every healthcare facility, every youth-serving agency will be meeting the needs of those racialized and marginalized communities and the system would be working for everybody.” – LG “I usually say COVID is like an x-ray. It showed us how broken the bones were.” – LG “We came through this huge impact of COVID-19 and we're still functioning as a community. On today's episode we discuss: —Climate: Primary care residents at Brigham and Women's Hospital in Boston discuss the importance of community health centers (CHCs) in serving low socioeconomic status, immigrant, rural, and racial/ethnic minority patients during the COVID-19 pandemic. The authors call for Congressional action to ensure adequate funding to CHCs and universal coverage to individuals without employer-sponsored insurance, suggesting that the pandemic may provide an impetus for improving the United States healthcare system so that CHC's could serve patients more effectively. —Epidemiology: A multicenter cross-sectional study found that among 614 patients with confirmed COVID-19, 278 had elevated troponin levels, which was associated with higher in-hospital mortality. Those with elevated troponin levels were more likely to be older, have atrial fibrillation, heart failure, hypertension, and coronary artery disease. In the hospital, patients with elevated troponins were additionally more likely to have complications including sepsis, pulmonary embolism, delirium, heart failure, acute kidney failure, and major bleeding. This study suggests that running diagnostic tests and identifying patients with elevated troponins may help earlier identify patients requiring more intensive care. —Understanding the Pathology: A retrospective study of 191 severe COVID-19 patients looked specifically at longitudinal IgM and IgG antibody responses to the nucleocapsid, spike protein, and receptor-binding domain of SARS-CoV-2. There was no significant difference in antibody positive rates between survivors vs non-survivors nor 28-day clinically improved vs non-improved patients. Four weeks after infection, patients who ultimately died had lower spike-IgG titers than survivors, and viral clearance was strongly associated with nucleocapsid-IgG and receptor binding domain-IgG titers, suggesting the possible use of spike-IgG as a predictor of adverse outcomes and the role of IgG as an important factor against SARS-CoV-2 infection. —Transmission & Prevention: A medical news reporter discussed the herd immunity threshold for COVID-19, stating that determining this threshold is difficult with this virus due to the many uncertainties about the level at which people can act as vectors of disease spread. This author also suggests that even if herd immunity is be achieved, there is no guarantee that it would last. —R&D: Diagnosis & Treatments: Epidemiologists in Spain examined the avidity of antibodies to SARS-CoV-2 in 76 serum specimens from COVID-19 patients to determine whether antibody avidity corresponded with time since acquisition of the virus. They found that 39 samples tested positive for both IgG and IgM, 37 tested positive for IgG only, and IgG reactivity was lost in 28 samples after urea treatment. Serum that lost reactivity were drawn significantly sooner after onset of symptoms than serum that retained activity suggesting that serum antibody avidity corresponds to the time since onset of symptoms and thus could be useful in predicting the acquisition date of SARS-CoV-2 infection. --- Support this podcast: https://anchor.fm/covid19lst/support

Listen NowSince the 1960s Community Health Centers (CHCs) have been providing health care services to moreover minority populations, the poor and the uninsured.  Today there are approximately 1,200 health centers providing health care to over 20 million Americans in all fifty states.  They our the nation's true safety net.  The Affordable Care Act created the CHC Fund that provided $11 billion over five years for the expansion of health centers and services throughout the country.   During this 20 minute discussion Michelle Proser discusses the work of the National Association of Community Health Centers, how CHCs work to reduce health care disparities, what work CHCs have been doing with ACA's $11 billion in  funding and the potential effect should CHC Fund moneys not be renewed when they expire later next year. Michelle Proser is the Director of Research at the National Association of Community Health Centers where she conducts research and policy analysis on a variety of topics used to empower health centers and educate policymakers and the public.  Michelle also directs NACHCs' efforts to build health center capacity for community-directed translational research.   Previously, Michelle served as a research analyst at the Center for Health Services Research and Policy at The George Washington University.   Michelle received her MPP from George Washington and is presently a Ph.D. candidate at GWU.
