S1 E14: Clinical Data Quality in 2023: The Good, The Bad and a Roadmap to Improve Host Beth Friedman speaks with Keith Olenik, Chief Health Information Officer at AHIMA. Keith is a senior executive health care professional with 25+ years of industry relationships, influence, and thought leadership in the areas of health information and health IT. With deep industry experience in EHRs, clinical documentation, interoperability, privacy and GDPR compliance, Keith currently leads AHIMA's membership, certification, academic affairs, workforce development and customer relations teams. While data integrity in healthcare isn't a new problem, clinicians now suffer from a wider variety of clinical data issues. This critical conversation between two industry experts explores AHIMA's six tenants of data quality, identifies nascent challenges including third-party data and person-centered care, and provides valuable privacy reminders for 2023's rapidly exploding volumes of health information.

Jami Woebkenberg is proof. Jami is the Senior Director of HIM operations at Banner Health. Or at least, that's her title now. She has had a few twists and turns over 19 years at the organization, which recently acquired a couple hospitals and clinics and added them to the system's EMR last September. Banner is HUGE. Headquartered in Phoenix, AZ, today it operates in six different states and employs more than 52,000 team members across a dizzying array of services including insurance, ambulatory health centers, urgent care, acute care including three academic hospitals, behavioral health, children's post-acute services, pharmacy, labs, telehealth, and more. It also employs 3,300 staff in revenue cycle alone, of which some 400 ultimately report up through Jami. To put this all in perspective, when she started 19 years ago Banner had one hospital implementing one EMR. “Banner is a constant state of growth,” she says, which means a lot of change and a daily dose of the unexpected. For example, Banner recently re-orged, splitting up coding, CDI, and HIM operations into three separate verticals, which changed her job completely. Jami is now knee-deep in records management, release of information, records storage, and EMR management including data integrity audits, information blocking, SDOH, privacy and security. She's survived it all with a great attitude and a mindset shift, which we get into on this episode of Off the Record. We cover Jami's strategies for change management at a basic blocking/tackling level, including establishing a change management team and communicating changes to impacted stakeholders including physicians. We also cover her helpful personal routines (morning walks), tips and tricks like calendar blocking and meeting free times, managing life-work (not work-life) balance, and strategies for introverts in leadership positions (introverts unite!). This episode is the second installment about Clinical Documentation and Coding. Today, we have a conversation about Clinical Documentation Improvement with Amber Malone-Wright, Director of Clinical Documentation Integrity at CHESS Health Solutions. I want to pick your brain about clinical documentation improvement also known as CDI. So tell me amber what is CDI all about?Well Thomas, I think the main message about CDI is around quality initiatives. Most people who ask providers why good clinical documentation is necessary, many of them are going to say that it's important for the communication to other providers about the continuity of care. Physicians generally understand the need to make documentation legible, timely, complete, and clear and you know with electronic medical records a lot of that is resolved. They also understand that documentation is a legal health record. They understand the common phrase - if you didn't document it, it didn't get done. CDI programs have increased significantly over the past ten years and are predominantly used in the inpatient hospital setting. But now this is expanding into the ambulatory and provider office setting due you value-based care and contract changes. The key is to really just engage providers to correlate how clinical documentation provides an opportunity to demonstrate the quality of care that was provided during an office visit. The American Health Information Management Association or AHIMA really says it best. They say the message to physicians should be: simple good clinical documentation will improve communication, increase recognition of comorbid conditions that are responsive to treatment, and validate the care that was provided, and show compliance with quality and safety guidelines. Why should a provider change their documentation?So physicians are taught to ask why as part of a diagnostic training that they went through and the need to understand the reason for a change in clinical documentation in order to fully embrace the concept. So if a provider challenges a CDI recommendation, it's an opportunity to explain why CDI is necessary. Explain the concept around whether it's MSDRG for inpatient or value-based care contracts and how they're designed to increase reimbursement for care of complex patients. It's also important to explain the severity or the illness or risk or mortality score that's derived from the codable diagnosis codes. It's also important that providers understand the process of audits and denials and financial impact. Not only for hospitals but the outpatient office visits as well. Documenting all of the chronic conditions that are known for the patients that affect the care and treatment for that patient impact the medical decision making by the provider and can also impact the level of evaluation and management services.Amber, tell me how a provider can implement CDI into their workflow?Electronic medical record technology has really improved the ability for medical records to be legible and timely. Physicians generally use structured templates to input documentation or they can dictate in a standard progress note format. But sometimes, the benefits of the electronic documentation are not always great. Sometimes there are significant challenges with electronic documentation, such as copy and pasting documentation, which can increase the risk of audits including outdated problem lists and then the inability for providers to find the correct diagnosis code in a drop-down selection. It's important to remember that providers are not trained in coding, yet many providers now know the codes that are important for their billing. If the provider chooses a nonspecific diagnosis code to include in the medical record, it could potentially make it more difficult for someone to code the case with a more specific diagnosis code. To say LifePoint Health is a large healthcare system is like saying Nashville has a few good country music bands. Per its website, LifePoint's healthcare delivery network spans 29 states and includes 62 community hospital campuses, 33 rehabilitation and behavioral health hospitals, and more than 170 additional sites of care across the healthcare continuum including acute rehabilitation units, outpatient centers, and post-acute care facilities. With that size and diversity comes incredible complexity and challenges. These include multiple EHRs (Epic, Cerner, MEDHOST, Meditech) and upwards of 16 different versions of said EHRs, including 9-10 versions of Meditech alone. Fortunately Angie Comfort is there to help run the show. How does she manage it all? How did she go from coal miner's daughter in the Appalachian Mountains of Virginia (no relation to Loretta Lynn) to Assistant Vice President of HIM Operations where she oversees a staff of 300? And find time to work on her Doctorate's in Business Administration, in progress? You can't call her Dr. Comfort—yet. That's coming soon. With many fun side detours along the way, as I'm wont to do. She testified before the US Senate HELP Committee and was appointed by the US Secretary of the Department of Health & Human Services to serve on the Office of the National Coordinator (ONC) Health Information Standards Federal Advisory Committee and ONC Consumer Task Force. Dr. Kennedy has served on the board of directors for Go Care, Bayou Health, and Claiborne Memorial Medical Center. In 2016, she was recognized as one of the Top 20 Professors and Minds in Health Informatics in the United States. Dr. Kennedy has received the Vantage Health Visionary Educator Award, AHIMA Triumph Mentor Award, and the 2017 AHIMA Distinguished Member.
