In this episode of the IJGC podcast, Editor-in-Chief, Dr. Pedro Ramirez, is joined by Dr. Giuseppe Caruso discuss post-PARP myeloid neoplasms. Dr. Caruso is a fifth-year resident in Obstetrics and Gynecology and a first-year fellow of the PhD in “Network Oncology and Precision Medicine” at Sapienza University of Rome in Italy. Over the past year, he has been attending the Department of Gynecologic Oncology at the European Institution of Oncology (Milan) under the mentorship of Professor Nicoletta Colombo and has now started his research fellowship period at Mayo Clinic (Rochester) under the supervision of Professor William Cliby. His main interest areas are gynecologic oncology, personalized oncology, and clinical research. Highlights: - Myeloid neoplasms post PARPi in patients with ovarian cancer are gradually emerging as life-threatening late toxicities and should not be underestimated. - The first two years of PARPi exposure are the critical window of onset and persistent cytopenia has been recognized as an early warning sign. - Active surveillance, differential diagnosis, and prompt hematological referral are crucial. - PARPi are recommended in the first line also to improve the risk-benefit ratio. - PARPi should be used cautiously in patients with a higher baseline risk and/or those who are less likely to have a significant benefit.

Over the past year, he has been attending the Department of Gynecologic Oncology at the European Institution of Oncology (Milan) and had the huge opportunity to follow the clinical, surgical and research activities of an international center of excellence, under the mentorship of Prof. Nicoletta Colombo and Prof. Giovanni D. Aletti. His main interest areas are gynecologic oncology, personalized oncology, clinical research, and medical writing. Prof. Nicoletta Colombo is Chair of Gynecology Program and Director of the Ovarian Cancer Centre at the European Institute of Oncology in Milan. Principal investigators of several international clinical trials and author of several publications, she was President of ESGO and Chair of the ESMO-ESGO-ESTRO Consensus Conference in endometrial cancer (2015) and the ESMO-ESGO Consensus Conference in ovarian Cancer (2018). In 2020, Prof. Colombo received the IGCS Lifetime Achievement Award in recognition of her work in gynecological oncology. Highlights: - PARPi should be preferred in the upfront setting for both efficacy and safety reasons. - Mutational status and platinum response are key factors for personalizing the maintenance treatment. - Identifying better predictors of resistance to platinum and PARPi is an unmet need. - PARPi combinations could become a strategy for overcoming PARPi resistance. - The best treatment algorithm after PARPi progression needs prospective validation.

NBA Play-in games Hawks vs Heat: MIA -5 Twitter: @ZGthagrapevine Miami covers the 5 easily. Jimmy Butler always rises to the occasion in the playoffs. Plus. Trae Young sucks against the Heat. In 17 careers games Young is averaging: 21.4 points on 40.2 percent shooting, including 32.8 percent on 3-pointers. In four games this season, Young averaged 19.8 points on 35.6 percent shooting, including 20.8 percent from deep. On March 4, for example, Miami held Young to eight points on 2-for-13 shooting Eric Spolstra has figured out how to limit Young. If they can continue to do that, which it seems like they will, Miami should win this game handily. T-Wolves vs Lakers: LAL -7.5 No Gobert, which will surprisingly hurt the Timberwolves. Gobert is simply an overpaid role player who should only play against teams that have offenses that involve big men, like Anthony Davis and the Lakers. The interesting thing is I like Minnesota's offense more when Gobert isn't in the way. He clogs up the lane, which makes it tougher for ANT to get going. When he isn't in the game they can go 5-out and spread the floor. This allows KAT to shoot and ANT to create off of his penetration. BUT, the Lakers are really good defensively, and AD looks like a top 5 player in the league right now. Lebron looks good off the injury. I don't know how people couldn't take the Lakers here to win and cover. Bulls vs Raptors: TOR -5 Give me the Bulls. I'm surprised that Toronto is -5. The Bulls are more talented offensively between Derozan and Lavine. Lavine has been on a tear recently. He's averaging just under 27 ppg. Beverley and Caruso defensively are going to give the Raptor's backcourt problems. Vucevic has been good defensively this year. If Lavine and Derozan can be solid defensively then the Bulls should win. Thunder vs Pelicans: NO -5.5 I want to take the Thunder. I have loved watching them all year long, but the Pelicans are experienced. The Pelicans have two go to guys on offense in Ingram and McCollum. Outside of SGA I don't trust anyone else to get me a bucket on the Thunder. This Pelicans team was the 1 seed earlier this year, then injuries derailed them. They've figured it out, and I fully expect them to win tomorrow night and to beat the Timberwolves for the 8 seed. They'll give the Nuggets all they can handle with or without Zion.

Professionally, she is a lawyer. She graduated Law school in Pavia (North of Italy) in 2013. Three years later, she attended Law school at the Universidad Francisco de Vitoria in Madrid. In 2016 she officially became a lawyer and started to work as a legal consultant in the winery. But she soon realized that her true vocation was to pass on her family's passion, history and tradition. So, she decided to set aside civil and penal codes and devote herself entirely to the family business. She is today part of Donne del Vino (Women of Wine) an association that brings together women from all over the world, producers, sommeliers, journalists and restaurateurs united by their passion for wine. The wine industry has always been men-oriented and only nowadays it is opening more and more to women: women's influence changed the business. Women globalized the market, redefined the trading relations and represent the fundamentals of this business. She is also involved in the Board of Directors of Assovini Sicilia, an association of Sicilian winemakers that brings together over 91 wine companies to promote the quality of Sicilian wine around the world. What links the members is love and passion for Sicily and an awareness that the Sicilian wine territories represent unique value in the Italian, European and international wine markets. Today Giovanna leads the family winery, the Caruso & Minini, together with her sister Rosanna and her husband Andrea representing the fourth generation. Emma hosts Dan Bouk, associate professor of history at Colgate University, to discuss his recent book Democracy's Data: The Hidden Stories in the U.S. Census and How to Read Them.Then, Emma's joined by Akela Lacy, politics reporter for The Intercept, to discuss her recent reporting on the Pennsylvania State Legislature's attempts to impeach Philadelphia District Attorney Larry Krasner. Emma begins by running through updates on the Fed's continued interest rate hikes, Democratic pessimism as we approach the midterms, and the upholding of Brittney Griner's sentence in Russia, before diving deeper into the second pressured-abortion allegation against Herschel Walker as well as his recent appearance alongside Lindsey Graham. Professor Dan Bouk then joins, getting right into the central importance of the US Census in the delegation of power across the various branches and levels of federalism, with its inception in the first article of the US Constitution. Bouk then walks through the state of the Census bureau over the first century of the American experiment, with this mass mobilization driven by US Marshals organizing law enforcement to get people out for the count, with the infrastructure of the bureau slowly growing through the early 1900s – becoming an institution made up of hundreds of thousands of workers. Emma and Dan then explore how this massive bloat of the bureau brought forwards greater scholarly work (even within the bureau) on the failures of the census methodology, with a segregated office of Black census workers first revealing the mass undercount of various marginalized communities – including Black and brown folks, immigrants, those in poverty and without housing, and more. Next, Professor Bouk walks through the various factors that push this miscount in the census, and how these factors developed over the 20th Century, with certain aspects (like having representatives that actually want to count you, living in a house, etc) continuing to present an issue, while new ones (being part of a racial group being put in internment camps, having an atypical family structure) arose alongside developing social issues. Emma is then joined by Akela Lacy as they dive right into the 5-year campaign by the Pennsylvania GOP against DA Larry Krasner, taking up the mantle of other state-level GOP impeachment attempts by pushing rhetoric on Krasner's failure to address violent crime while simultaneously undermining his actual ability to act as state DA. They wrap up the interview by tackling the specific relationship between DA Krasner and PA Attorney General Josh Shapiro, and the short-term benefits and long-term costs of meeting the GOP in the middle on tough-on-crime policy. Emma also touches on the LA mayoral election between Rick Caruso and Karen Bass, and why a progressive movement behind billionaire real estate tycoon Caruso would be a mistake. And in the Fun Half: Emma is joined by Matt Binder and Brandon Sutton as they chat with Ben from Virginia on relating to conservative family members, be they 9-month-old AnCaps or 90-year-old theocratic fascists, before diving into the recent coverage of the Dr. Oz v.
