Er zijn grote zorgen over het socialemediagebruik van kinderen. Ze worden verslaafd gemaakt aan notificaties, likes, eindeloos scrollen en extreme inhoud, zonder dat ze zelf de risico's altijd goed kunnen inschatten. Het kabinet wil daarom een Europese leeftijdsgrens voor sociale media. Voorstanders zijn blij dat de leeftijdsgrens in het regeerakkoord staat, maar de Brusselse route duurt hun te lang. Tegenstanders zien vooral de haken en ogen van een leeftijdsgrens, vertelt redacteur media Jeroen Piersma. Eind december vorig jaar besloot demissionair minister de Jonge dat kinderhartchirurgiegeconcentreerd moest worden in twee centra, de UMC's in Rotterdam en Utrecht. Dit veroorzaakte, en doet dit nog steeds, veel verontwaardiging en maatschappelijke onrust. Nu maakt de huidige minister Kuipers zich ook nog eens hard voor het meer concentreren van hoog-complexe acute zorg. Dit wordt door de NZa ook ondersteund. Waar hoog-complexe zorg wordt geconcentreerd, wordt basiszorg aan de andere kant steeds meer uit de streekziekenhuizen (naar de eerste lijn) verplaatst. Kortom concentratie in de zorg is een opvallende trend van de laatste jaren die veel stof doet opwaaien. Is het een noodzakelijk kwaad?

De term ‘passende zorg' komt oorspronkelijk uit de koker van de NZa, vertelt NZa-voorzitter Marian Kaljouw in de podcast Voorzorg aan senior-redacteur Bart Kiers.

Integrale bekostiging is een onderwerp waar bijna iedereen binnen de geboortezorg een mening over heeft. Maar er zijn ook nog vele vragen! Met jullie vragen zijn we het gesprek aangegaan met de KNOV, de NVOG, de NZA en de federatie van VSV's. Welke antwoorden geven zij op de vragen? Wat vinden zij van integrale bekostiging? En welk toekomstbeeld zien zij voor zich? Je hoort het in deze 31ste aflevering van Verloskundig Baken.

Karina Raaijmakers is directeur Toezicht en Handhaving van de Nederlandse Zorg Autoriteit (NZa).  Zorgverzekeraars en zorgkantoren leggen in contracten met zorgaanbieders vast welke zorg er geleverd wordt, tegen welke kwaliteit en welk tarief. De NZa bepaalt hoe zorgaanbieders zorg in rekening mogen brengen en wat de zorg (maximaal) mag kosten.  Daarbij controleren ze of deze partijen zich aan de regels houden.Volgens Karina moet iedereen in Nederland erop kunnen vertrouwen dat er op tijd goede en betaalbare zorg beschikbaar is als je die nodig hebt. Om die garantie te kunnen bieden moet de samenwerking in de regio tussen de aanbieders, verzekeraars en het gemeentelijk domein worden gezocht. Dat betekent dat de inrichting van de zorg de komende jaren moet veranderen. Volgens Karina is dat een kwestie van meebewegen met enerzijds het maatschappelijk- en anderzijds het organisatorische belang. Hoe doen we dat? Welke rol heeft de NZa daarin? En hoe gaan we de zorg, specifiek de GGZ, toekomstbestendig inrichten?

Het begin van dit nieuwe jaar is een goede aanleiding om vooruit te kijken naar het zorgjaar 2022. Problemen die al lange tijd spelen, zoals het personeelstekort en de almaar stijgende zorgkosten door vergrijzing en chronische ziekten, nemen we mee het nieuwe jaar in. Zal daar nu eindelijk eens een oplossing voor komen?  Onze gasten Marianne de Visser van de Wetenschappelijke Raad voor het regeringsbeleid en emeritus hoogleraar neuromusculaire ziekten .  Anna van Poucke, sectorleider van de wereldwijde zorgpraktijk bij adviesbedrijf KPMG.  De uitgaven aan de zorg als aandeel van de overheidsuitgaven zijn de laatste twintig jaar verdubbeld, terwijl de uitgaven aan onderwijs gelijk gebleven zijn. Is het terecht dat gezondheid het hoogste goed is? Vergrijzing, personeelstekorten en chronische ziekten zorgen voor hogere zorgkosten. Er moet dus wat veranderen om de stijgende kosten in de hand te houden, maar hoe zorg je dat het niet alleen bij goede voornemens blijft en tot daadwerkelijke actie komt? Van Poucke en De Visser zijn optimistisch over de perspectieven die de coalitieplannen en de nieuwe bewindslieden voor de zorg bieden (Ernst Kuipers als minister van Volksgezondheid en Conny Helder als minister voor Langdurige zorg en Sport). Personeelstekort Nu werkt ongeveer een op de zes à zeven Nederlanders in de zorg, over twintig jaar zou dat één op de vier zijn, in 2060 één op de drie en dat terwijl het personeelsbestand in de zorg flink aan het vergrijzen is. Volgens de WRR is dat probleem niet alleen een kwestie van geld en hogere salarissen; werkgevers moeten ook zorgen dat de balans tussen werk en privé optimaal is, dat zorgpersoneel zich gewaardeerd en serieus genomen voelt en ook inspraak heeft in de organisatie.  Passende Zorg. Er moeten duidelijker keuzes gemaakt worden in welke zorg al dan niet passend en noodzakelijk is en dus vergoed moet worden. Niet alle therapieën, bloedonderzoeken en röntgenfoto's die nu worden genomen zijn nodig, aldus de NZA.. Regionale zorg. KPMG adviseert om meer te investeren in regionale zorg. Per regio kan dan worden bepaald waar de behoeften liggen en waar dus de focus van het zorgaanbod moet liggen. Dat vraagt ook om betere communicatie en afstemming tussen specialisaties en zorgcentra.  Volgens de WRR moet de politiek moet burgers erop gaan voorbereiden dat  in de toekomst niet meer alle zorg kan worden aangeboden of vergoedt. Ook moet er een maatschappelijke discussie komen over "wat gezondheid waard is" en welke medicijnen er nog moeten worden vergoed. Daarbij is het belangrijk dat de zorg (en daarmee gezondheid) niet alleen een voorrecht voor de rijken wordt, maar dat het voor iedereen toegankelijk blijft. Digitalisering. Tijdens de coronapandemie zagen we het nut van digitale zorg, apps, robotica en online consulten die mensen uit het ziekenhuis hielden. Volgens KPMG kunnen dit soort toepassingen zorgen voor flinke besparingen in de toekomst. Maar, zegt de WRR, op lange termijn is het niet duidelijk hoeveel er wordt bespaard en bovendien is het 'wensdenken' dat digitale toepassingen de huidige problemen gaan oplossen.
