In this episode of the PFC Podcast, Dennis and Andrew Fisher discuss the critical aspects of setting up a walking blood bank in military settings. They explore the importance of pre-identifying donors, logistical planning for blood collection, and the necessity of effective communication and coordination among units. The conversation emphasizes the execution and timing of blood collection during emergencies, highlighting that understanding the principles of medical procedures is more important than specific techniques. The episode serves as a comprehensive guide for military personnel involved in medical emergencies and blood transfusion protocols. Takeaways Setting up a walking blood bank is crucial for military operations. Pre-identifying donors can significantly reduce stress during emergencies. Logistical planning is essential to ensure adequate blood supply. Effective communication among units enhances coordination in blood donation. Timing is critical; aim to have blood ready within 30 minutes of need. Training all personnel in blood collection techniques is vital. Understanding principles allows for better improvisation in emergencies. Blood type identification should be done well in advance. Practice and familiarity with procedures improve response times. A well-prepared team can save lives during critical situations. This month, JHLT: The Podcast reissues our September 2023 tribute to former Editor-in-Chief, Dr. Daniel R. Goldstein. Dr. Goldstein stepped down from his role for health reasons in July 2023; he had been diagnosed with an advanced salivary gland malignancy and felt he would be unable to continue serving JHLT and the International Society for Heart and Lung Transplantation (ISHLT) to his characteristically demanding standard. Sadly, Dr. Goldstein died on 21 May, 2024, at the age of 56, leaving behind his wife, 2 children, an extended family, and a larger universe of colleagues, collaborators, and mentees who greatly benefited from his equanimity, wisdom, and commitment to his passions. The tribute, recorded in August 2023, features Michelle Kittleson, MD, PhD, then-Interim Editor-in-Chief of JHLT; Andrew Gelman, PhD, Deputy Editor at JHLT; Andrew Fisher, FRCP, PhD, past president of ISHLT and Past Chair of the Publications Oversight Committee; Daniel Tyrrell, PhD, a former post-doc of Dr. Goldstein's; and Judy Chen, PhD, a former immunology PhD student in Dr. Goldstein's lab. Two funds were created to allow friends and colleagues to memorialize Dr. Goldstein: the Michigan Biology of Cardiovascular Aging Leadership Development Fund at the Frankel Cardiovascular Center (https://giving.umich.edu/give/393178) and the Adenoid Cystic Carcinoma Foundation (https://accrf.org). The JHLT has also re-published Dr. Goldstein's farewell message in the September 2024 issue of the Journal. The complete audiobook is available for purchase at Audible.com: https://t.ly/JX7C8 Keith Murdoch's Gallipoli Letter By Sir Keith Murdoch Narrated by Sarah Bacaller The National Library of Australia describe Keith Murdoch's Gallipoli Letter in the following way: “The Gallipoli letter is an 8000-word private report written by journalist Keith Arthur Murdoch, with the help of British war correspondent Ellis Ashmead-Bartlett, after visiting the Gallipoli peninsula in September 1915. It describes the organisation and conditions of the Gallipoli campaign. It was sent to Andrew Fisher (then-Prime Minister of Australia) and Henry Herbert Asquith (then-Prime Minister of the UK). Murdoch's twenty-eight-page letter helped to establish the idea of Gallipoli as a military disaster. Fisher sent Murdoch to Gallipoli for an honest report on the campaign. Contrary to the rules of censorship, Murdoch wrote and sent his letter without submitting it to the military censors. In the eyes of some, this was a grave betrayal of Murdoch's role at Gallipoli. Murdoch, however, strongly defended his action on the grounds that his letter was not for publication but for the information of the prime minister. Whether or not Murdoch did the right thing in sending the letter uncensored, it had a serious impact and brought about rapid results. It can be argued that Murdoch's letter led directly to the ending of the Gallipoli campaign, and the evacuation of British and Anzac troops from the peninsula.” Here, this important historical document is made available in audiobook form.

With the general election less than a year away, the Conservatives, Labour and all other political parties are drawing up their manifestos. Scrutinised by the media and voters, manifestos can shape debate, shift the polls, and play a major part in an election campaign – and shape what the winning part does in government. While manifestos are described as a contract between a party and the people, the reality in government is often quite different. Pledges may prove difficult to deliver in practice, with unforeseen crises and the day-to-day challenges of governing seeing commitments fall by the wayside. In a coalition or minority government, parties may have to compromise. So how do the parties develop and write their manifestos? What does a good manifesto actually look like? What are the questions that Rishi Sunak and Keir Starmer will be considering when signing off on their manifestos? And just how important are manifestos during – and after – a general election campaign? Goldstein. Digital Media Editor Van-Khue Ton, MD, heart failure and transplant cardiologist at Massachusetts General Hospital, hosts this episode.   First, hear from senior author William F. Parker, MD, MS, PhD, on his team's study “Association of high-priority exceptions with waitlist mortality among heart transplant candidates.” Dr. Parker is a pulmonary and critical care physician, health services researcher, and clinical medical ethicist, and he runs a R01 funded lab focusing on the allocation of scarce healthcare resources.   In the study, Dr. Parker and colleagues set out to examine the Scientific Registry of Transplant Recipients (SRTR) to compare medical urgency of heart transplant patients listed with exception vs. those listed according to standard guidelines. The study's main finding: after controlling for status as a time-varying covariate, candidates with an exception had a 45% lower hazard of waitlist mortality compared with standard criteria candidates. The Digital Media Editors want to know all the details and talk with Dr. Parker about the wait-list mortality and post-transplant survival of status 1, 2, 3, and 4 candidates, plus what next steps could be in ensuring a fair allocation system.   The episode's special tribute to Dr. Daniel R. Goldstein features Michelle Kittleson, MD, PhD, Interim Editor-in-Chief of JHLT; Andrew Gelman, PhD, Deputy Editor at JHLT; Andrew Fisher, FRCP, PhD, past president of ISHLT and Chair of the Publications Oversight Committee; Daniel Tyrrell, PhD, a former post-doc of Dr. Goldstein's; and Judy Chen, PhD, a former immunology PhD student in Dr. Goldstein's lab.   These heartfelt tributes to Dr. Goldstein are worth a listen—and we thank Dr. Goldstein for his vision and leadership at the Journal.   Follow along at www.jhltonline.org/current, or, if you're an ISHLT member, log in at ishlt.org/journal-of-heart-lung-transplantation.  Don't already get the Journal and want to read along? Host Scott Fisher opens the show with David Allen Lambert, Chief Genealogist of the New England Historic Genealogical Society and AmericanAncestors.org. Fisher begins discussing his recent 3D printing project, by which he recreated the 1856 New York City volunteer firemen's badge, number 1475, lost by his great grandfather. Andrew Fisher ran an ad in the New York Daily Herald on March 19 of that year seeking its return. Fisher explains the process and the cost. David then sends out 104th birthday greetings to the last surviving pilot of the Battle of Britain. Hear his story. Then, a new theory is out there on the true identity of Jack the Ripper! Could they have it right this time? Another previously unidentified victim of the Japanese attack on Pearl Harbor has been laid to rest. David will tell you his story. Meanwhile, in Canada, researchers are combing through remains of a cholera cemetery that has burials from the 1830s and the 1850s. And finally, an ancient Iranian language, dating from between 200 BC and 700 AD has been translated. Hear how it was done. Then, Leeanne Hay, author of “NPE- A Story Guide For Unexpected DNA Discoveries,” talks about her own shocking find and how her book has been made to assist those whose DNA test results were not what they were expecting. In this fiery conversation with my husband and friends Amelia and Matt, we discuss and debate the meaning and value of sex, marriage, and monogamous vs. polyamorous relationships.  Amelia and Matt recently celebrated their love in a non traditional marriage ceremony after dating for 5 years. They practice consensual non monogamy and value freedom, self expression and evolution as the foundation of their relationship. They share polar opposite life experiences to me and my husband Andrew, who have only ever been with each other intimately and waited until we were married to have sex. We've been together for over 20 years, started dating when we were 15 and 16 years old and our marriage vows are based in a life long commitment of growth and unconditional love, through the easy times and the hard times. We get into our differing perspectives on love and if having multiple sexual partners (either at the same time or throughout our lives) is best for society, raising children, and ourselves as individuals.  Sponsors: Branch Basics | Get 15% off all Starter Kits (except the Trial Kit) https://links.branchbasics.com/ellen15 Anima Mundi Herbals: enter the code ELLEN20 for 20% off everything at https://glnk.io/nr9jm/ellenfisher We discuss topics such as: problems within monogamy vs polyamory for relationships, children, and society  The purpose and meaning of sex - is it sacred, solely for pleasure, etc? UFOs? Balloons? Spy Cams? Here's What's Going On This week, the saga over UFOs, balloons, and spyware continues. The drama all started with a Chinese surveillance balloon, and then—one by one—governments kept finding others in the U.S. and Canada. Earlier this week, President Biden announced, “We don't know yet exactly what these three objects were. But nothing right now suggests they are related to China's spy balloon program or that they were surveillance vehicles from any other country.” So what do we know about these balloons? And why is this such a big deal all of a sudden? Casey Crownhart, climate reporter at the MIT Technology Review, joins Ira to discuss the hullabaloo surrounding these flying objects and other science news of the week. They also talk about the outbreak of Marburg virus in Equatorial Guinea, Tesla agreeing to open some charging stations to other EV drivers, the startups trying to remove methane from the air, what a pencil-shaped robot taught scientists about the “Doomsday Glacier,” and why researchers modeled a new camera after cuttlefish eyes.   How The Western U.S. Could Rebuild Its Water Infrastructure In early January, California was inundated with record-breaking rainfall. The state was battered by back-to-back storms, which caused severe flooding and power outages. But could there be a silver lining in those storm clouds? Given the historic drought conditions plaguing the western U.S., a way to collect or divert rainwater to use when the dry season hits is especially appealing. However, potential solutions are not within easy reach. Ira talks about the limitations and opportunities of storing and diverting rainwater with Dr. Andrew Fisher, hydrogeologist and professor of earth and planetary sciences at the University of California, Santa Cruz. Later, Ira is joined by Dr. Sharon Megdal, director of the University of Arizona's Resources Research Center, to talk about the drivers of the water crisis and some of the policy solutions being floated to solve the problem.   Ohio Residents Want Answers About Risks Of Train Derailment Hundreds of people packed the high school gymnasium in East Palestine, Ohio, Wednesday evening, trying to get some answers about whether they were safe in their homes after an explosion and the release of numerous toxic chemicals following the train derailment two weeks ago. What started as an open house with tables set up around the floor for the US EPA, the Ohio EPA, the state Division of Wildlife, and the county health department to answer individual questions morphed into a town hall meeting. Residents sat in bleachers and yelled their questions to the officials. Many were angry, largely because Norfolk Southern, the rail operator, did not show up to the meeting. East Palestine Mayor Trent Conaway, surrounded by the media, said the company feared for the safety of its employees because there was so much anger against them. Conaway said people keep blaming him for this horrible incident, and it's not his fault. A recent study from the Queensland Brain Institute confirms what herbalists have long said: There are properties of this mushroom that build brain cells. In mice, these properties promoted neuron growth when diluted in water, resulting in better hippocampal memory. This result is a very good sign that the properties in lion's mane mushrooms can protect against dementia and Alzheimer's, the researchers behind the study say. Using the isolated components of the mushroom could be a step forward in the treatment of these devastating brain conditions. Joining Ira to talk about this study is the study's co-author, Dr. Ramon Martinez-Marmol, research fellow at the Queensland Brain Institute, based in Brisbane, Australia.   Transcripts for each segment will be available the week after the show airs on sciencefriday.com.
