"The question about the biochemistry is really interesting, because what your question now is, is 'what is it, and where is it?' And the problem that we have is that the liver enzymes aren't gonna tell us, because it's really hard to separate hepatic from post-hepatic on bloods. ALP is very insensitive in the cat and will go up with both hepatic and post-hepatic. ALT will go up with hepatic and post-hepatic. Bilirubin will be up. So what?!" We recorded this case-based interactive session about how to work up the jaundiced cat with one of the world's leading Small Animal Medicine specialists and teachers, ⁠Prof Jill Maddison⁠ at session our live event in 2022. Prof Jill Jill Maddison, BVSc, DipVetClinStud, PhD, SFHEA, MRCVS, is a professor of general practice, the director of professional development, and the BVetMed and CertAVP course director at Royal Veterinary College. She is also a coordinator for London Vet Show and is a consultant at a local veterinary practice and at Beaumont Sainsbury Animal Hospital in London. Dr Maddison is the senior editor of the second edition of Small Animal Clinical Pharmacology and the senior editor of Clinical Reasoning in Small Animal Practice. She has lectured worldwide on clinical problem-solving, small animal internal medicine, and clinical pharmacology.  In this session she helps us reason through a case of jaundice in a cat to create a deeper understanding, rather than a simple list of facts. She also shifts some long-standing beliefs around diagnostic testing in liver disease in both dogs and cats. "The problem that we have is that the liver enzymes aren't going to tell us." 19:51 Why bile acids are useless in the diagnosis of liver disease. "The bile acids don't tell you anything more than you already know. Ep.16 Show Notes   In this Skin Flints episode, Sue, John and Paul welcomed Dr Arielle Griffiths to the platform to discuss a very topical subject - sustainable pet food.   (00:00) Introduction   Chapter 1: “Understanding the Urgency: Why Sustainable Food is Necessary for Our Pets”   (03:44)  John welcomes Arielle to the pod, who talks about her work in the industry and in setting up the Sustainable Pet Food Association. As a GP vet she became involved in nutrition and did extensive research before also becoming environmentally and sustainably focused as a result of seeing a change in the world.   (07:31) Sue asks Arielle to clarify what is meant by obesity-based diets and Arielle says this is where owners are (through love) overfeeding their pets and potentially causing arthritis, heart disease and a number of conditions relating to the excess weight. This tipped her to realise the use of plants as a base in food can really help, which was a big factor in her  becoming vegan herself.   (09:08) Sue clarifies we are talking about people feeding too much or an imbalanced diet and the carbon footprint of that diet – and that we are discussing dogs here and not cats. Arielle says that the need for palatability in foods has resulted in an excess of protein in the diet and more meat being used than needed, affecting the sustainability. She shared that wet, meat-based diets have the largest carbon footprint, including raw lean diets – with one study in Brazil demonstrating a dog on this diet matched that of a human in that country.   Chapter 2: "Exploring the Landscape: What Constitutes Sustainable Food?"   (11:49) John asks why vegetarian or vegan food is a more sustainable option and Arielle says it is proven that animal agriculture for the use in pet food accounts for 2.5 - 3% of the entire carbon footprint of the world. This comes from deforestation to provide this food and the by-products of the food as a result of the market.   (13:40) Sue clarifies this as methane production from the animals used increasing the carbon emissions along with the deforestations. Livestock accounts for over 70% of global farming land use but only produces 18% of the world's calories and 37% of total protein - with dog and cat food being equivalent to an entire country's worth of production. But Arielle says the health benefits are what turned her more to vegetable based foods.    (15:29) Sue comments on the information on the human side for the health benefits, with more GPs suggesting it – she asks if there is evidence to support this on the pet side. Arielle says there is and comments on how in the 27,000 years of domestic evolution dogs have developed to require 52% of their diet to be carbohydrate due to the change in their genes over that time compared to the wolf they descended from, which only needs 1.2% carbohydrate. She also says dogs 3,000 years ago were primarily plant based.   (17:09) John asks if the theory of raw feeding being more natural for dogs is therefore unmerited and Arielle agrees, explaining that dogs obviously love eating food like this which is the success of the industry – but in terms of the environment there is significant evidence that resistant bacteria has been shown to be happening as a result of raw feeding, as well as it not being healthy for the dog. And she reiterates - a dog is not a wolf!   (19:18) John goes on to clarify Arielle is advocating a formulated dog food which is vegetable based and asks if it could be insect based. Arielle says it could and there are a number of companies for this, but she focuses on vegetable based and insects are just using another way of recycling protein and therefore whilst they are more substantiable – they are not as much so as the vegetable equivalents. She mentions how she was one of three vets speaking on the subject at London Vet Show along with Professor Andrew Knight and Dr Mike Davies - talking about animal nutrition and the evidence for vegetable based diets, which previously had looked to have a vegan diet, but a new independent study from Australia and Mexico reviewing all the papers indicated the evidence is sound. She argues now we know they are healthier for our pets and our planet there is an urgency.   Chapter 3: "Making Informed Choices: Considerations for a Sustainable Diet"   (24:40) Sue asks what we do about different life stages and different conditions and Arielle says there is a puppy plant based food and a senior plant based food – and in fact any plant based food is good for senior dog.   (26:12) Sue asks about particular conditions as well (e.g.) skin conditions and Arielle says she's getting 2 or 3 people a day asking her to transition their dogs from meat diets because of an intolerance to it, and much has been shown to evidence the gut microbiome health being linked to that of the sin and therefore the skin health.   (28:19) Sue asks if you can transition to a plant based diet overnight. Arielle advises that the cases where they are really uncontrolled and unhappy on the food can transition overnight with a care to not over feed. But for the majority of dogs - as with any dietary change - a slow transition over 2-3 weeks is better; 4-5 weeks for raw fed dogs, in order to allow the gut to adapt to the change.   (29:29) Sue asks about the evidence suggesting grain free diets can lead to heart disease and Arielle says this came about in 2018 with increased instances of dilated cardio myopathy in breeds you wouldn't normally see. She explains this is a result of substitute ingredients and is resolved with the addition of taurine to diets – which is an important reason to use a formulated diet. The number of cases has now dropped as a result of these changes.   (32:45) John wraps the conversation and asks Arielle if there are any resources to highlight and Arielle again recommends the Sustainable Pet Food Association as a great resource to find out more, and find the right food.   (36:18) Outro – Sue raps up as an ever-wise voice of reason.   (40:57) John asks Sue and Paul - Beef or beans?!

Sean Wensley was BVA president when the first strategy was launched in 2016 – and ahead of his lecture at congress, he joined a special episode of the Vet Times Podcast to discuss its success and consider how it may develop further over the next five years. ___________________ Sean Wensley BVSc, MSc, FRCVS, is senior veterinary surgeon (communication and education) at PDSA. He chairs the Animal Welfare Working Group of the Federation of Veterinarians of Europe (FVE), was president of the BVA from 2015-16 and is a Fellow of the RCVS. He is also an honorary lecturer in animal welfare at the University of Nottingham, a member of the Companion Animal Welfare Council, and a committee member of the Animal Welfare Science, Ethics and Law Veterinary Association. Sean has assisted with animal welfare and conservation projects in East Africa, China, India, continental and Eastern Europe, and the Caribbean. He holds a master's degree in applied animal behaviour and animal welfare. In, 2017 Sean received the inaugural World Veterinary Association Global Animal Welfare Award for Europe.

The BVA's 2021-22 president is Justine Shotton, who has pledged to make sustainability a key priority for the veterinary profession over the next 12 months. Dr Shotton has taken a sabbatical from her role as veterinary services manager at Marwell Zoo in Hampshire to focus on the presidency, and took time out of relocating to London to join the Vet Times Podcast to discuss her objectives for the year ahead. She also previews some of the sessions taking place at BVA Congress – being hosted by the London Vet Show in November – that will explore the role the profession can play in tackling climate change. ________________________ Justine Shotton BVSc, MSc, MRCVS, is a 2008 University of Bristol veterinary graduate with a first class intercalated degree in veterinary pathology from the RVC She has always had a passion for conservation – after graduating she volunteered at wildlife rehabilitation centres in Guatemala and a small animal clinic in Honduras before spending five years working as a mixed practice vet in Oxfordshire. After completing a master's in wild animal health from the RVC and Zoological Society of London (ZSL), she joined the ZSL as a postgraduate research assistant and wildlife vet for its Species Recovery Programme. While there, she researched and advised on disease risks and management strategies for reintroductions and translocations of native and European wildlife species following local extinctions or population reductions. Since 2015, Dr Shotton has worked at Marwell Zoo in Hampshire. As veterinary services manager, she leads a team of veterinary professionals and animal scientists to optimise the welfare of zoo and wild animals, and is involved in several wildlife reintroduction projects, including the international programme to reintroduce the scimitar oryx back into the wild in Chad in 2016. Alongside her veterinary and conservation work, Dr Shotton works as a small animal and emergency veterinary locum in Hampshire. When not in practice, Dr Shotton lectures on exotics and zoo animal medicine at the University of Surrey School of Veterinary Medicine and helms the veterinary zoological elective for the university's final-year veterinary students. During her year as BVA junior vice-president (2020-21) she has played an integral role in development of the imminent BVA policy on responsible use of parasiticides, supported the launch of the Greener Practice Checklist and represented the BVA on the UK One Health Co-ordination Group and the Federation of Veterinarians of Europe.

I absolutely loved these two! This was a delight of a conversation with a lot of laughs and I'm so pleased that we made it happen - I should probably add that Liz was 39 weeks pregnant at the time of recording... Liz Bode is a veterinary cardiology specialist - our second one featured in the COVID specials, who'd have thought it - who works at ChesterGates Veterinary Specialists in Cheshire. Scott Kilpatrick is a specialist in small animal internal medicine and was recently appointed head of medicine at Wear Referrals in the north east. In tandem with with their other halves, Andy and Sean, Liz and Scott are also both co-directors of the Veterinary Thought Exchange, a new CPD company that they recently founded. Steph is a general practitioner with 35 years of experience under her belt, and both are past-presidents of SPVS, which is how they met. Bex works as a small animal vet in West London and is also an artist, spending much of the free time she has painting and and selling her artwork. Taimur left clinical practice to found his company IVALA, a tech company that creates interactive 3D learning content for the veterinary community, that he sold to the Veterinary Information Network and continues to develop. You may be familiar with today's guests on Borborygmi. Pete Wedderburn and Emma Milne are small animal vets with decades of experience between them but both of them have moved away from full time general practice and into the media sphere. Pete is a regular columnist for The Telegraph who often appears on television and radio, and Emma is a published author and commentator, who you are likely to have seen grace our screens on all 11 series of BBC1's hugely successful show ‘Vets in Practice'. Today's guests on Borborygmi are Nat Scroggie and Cat Auden, and this recording was actually the first time they'd met in person, which is a great testament to the power of social media and the virtual friends you can make. Nat had a miscarriage in 2018 and subsequently set up Vet MINDS (miscarriage, infertility, neonatal death and stillbirth), a closed Facebook group to support others within the profession who have been through similar situations. Cat has previously had a miscarriage but now has 3 children. I'd really like to particularly nod to these two for this conversation - it was so open and warm, and they were both incredibly honest, so thank you. We are listening to a conversation today between Shereene Williams and Amy Barstow.  Both Shereene and Amy work for the international animal welfare charity The Brooke, though they came to it from quite different avenues. Shereene qualified as a vet from Nottingham in 2011 and after a period of time in equine and general practice, went travelling and came back to join Brooke four years ago as an Animal Health Veterinary Advisor. Amy qualified in 2013 from the RVC and spent time volunteering before starting an equine internship at the Animal Health Trust and completing a PhD at the RVC. Amy spent some time teaching before joining Brooke in mid 2019 as a Global Animal Health Advisor. Here, they talk about the paths that led them to working there and what moving in a slightly different direction meant for their job satisfaction. Dave is based in Sheffield, and works in emergency and critical care. Laura Higham graduated from Edinburgh Uni in 2008 and having been in practice in both the UK and New Zealand, now works for FAI, the Farm Animal Initiative, advising on the sustainability of food and livestock supply chains. She has a particular interest in international animal health, the future of food production and environmental issues, which led her to set up Vet Sustain, in the process meeting Anna Judson, the Junior Vice President of SPVS. Anna was a partner and practice owner for many years, and after selling her practice recently is now further pursuing her interests in wildlife, conservation and climate change. They're chatting about sustainability in the veterinary profession and how we can all get involved. Polly and I went to vet school together and we both now fit into the ‘diversify' category - Polly has worked in practice, done a masters in epidemiology and public health, a clinical research residency, worked overseas for the equine charity The Brooke, and is now doing a pHd on foot and mouth disease in Kenya. I've also worked in equine practice both in the UK and abroad, and currently work for the British Horseracing Authority on the regulatory side of the sport, as well as doing some research. They became firm friends since working together in Liverpool and here, they talk about how big shifts in their personal lives led them to explore other opportunities and diversify. They reflect on the changes that they've made and discuss how moving away from full time clinical practice has been good for them both, helping them to find the right work life balance and becoming much happier in the process. As a new-ish faculty, the veterinary department at The University of Surrey is a little different to the more traditional older vet schools in the way their final year works, as students travel to partner practices all round the country for their rotations, rather than being in a teaching hospital on the university campus. Today's guests on Borborygmi are Cat Auden and Lawrence Brown, two friends who met at Edinburgh University, from where they graduated as veterinary surgeons in 2010. Cat previously worked in small animal practice and is now Head of Collaborations for VetLED, a company that looks at enhancing team performance and improving patient safety in veterinary practices. Lawrence initially worked in mixed practice, but is best now described as an entrepreneur. He has multiple projects on the go, including consultancy for the Department for International Trade on animal sciences and aquaculture.
