Given the uncertainty surrounding the political landscape after the May elections, the Public Servants Association (PSA) has advised President Cyril Ramaphosa to delay the signing of the National Health Insurance (NHI) Bill. The NHI Bill was passed by the National Council of Provinces in late 2023, and sent to Ramaphosa to be signed into law. Government has stood firm in its support for the Bill, despite warnings that it undermines universal healthcare instead of advancing its progress. The Bill has been sharply criticised by doctors, health facility providers and health insurance companies. The PSA said it was extremely concerned about the implications of proposed tax hikes to fund the NHI scheme, pointing out that South Africans are not able to bear such a financial burden, particularly during the current economic climate. The PSA said government's plan to increase value-added tax and income tax to offset the NHI deficit was alarming, and it was urging government to instead address fiscal inefficiencies, including fruitless expenditure and corruption. The PSA said the funding model presented by the Department of Health lacked sustainability, saying it disproportionately burdened the poor who were already struggling to make ends meet with a high cost of living, constant increases in petrol prices and high interest rates. "In addition, salary increases have consistently lagged behind inflation rates, exacerbating the financial strain on households," PSA added. The association believes that the proposed tax hikes are likely to be insufficient to cover the costs of the NHI, and that it will also place additional strain on consumers, further diminishing their purchasing power and exacerbating financial hardships. Previous estimates suggested that government would need nearly R200-billion a year to sustain the NHI. With persistent economic challenges, the PSA again stressed that citizens would undoubtedly bear the brunt of the financial burden and that this would perpetuate financial constraints on households and deepen socio-economic inequalities. The association advised government to prioritise job creation to bolster revenue streams and alleviate the pressure on taxpayers. The PSA said any funds acquired through corrupt means must be recovered and redirected towards supporting the NHI. It also tasked government with streamlining Cabinet and reducing unnecessary expenditure.

In his speech, on Tuesday, in the debate on President Cyril Ramaphosa's State of the Nation Address (SoNA), Inkatha Freedom Party (IFP) leader Velenkosini Hlabisa painted a harsh picture of the African National Congress (ANC) and the challenges facing the country under its leadership. He noted former IFP leader Mangosuthu Buthelezi's last message to the ANC - "If the promise of South Africa is truly still alive, it is thanks to the resilience of our people. But how far is this Government willing to test our resilience? They are playing a dangerous game" - and said the ANC was moving in the wrong direction. Hlabisa pointed out that the ANC did not come into democracy and freedom by itself and lamented the passing of the Government of National Unity, which he said gave South Africans hope after the end of Apartheid. He stated that between 1994 and 1999, under the leadership of a Government of National unity, South Africa had flourished. He pointed out the progress made by that government through the passing of landmark legislation, growing skills and investment, and finding a balance between human rights and national security. Hlabisa quoted the International Monetary Fund's perception of South Africa in the first ten years of democracy, as a country that had made "impressive gains" to stabilise the economy, set up higher economic growth and strengthen finances and that since the ANC's sole governance, the perception of a well-performing economy had waned. He reminded Ramaphosa that the ANC did not govern alone between 1994 and 1999 and so, he said, that period could not be a measure of the ANC's success alone. He said it was only when the ANC gained unfettered power that the quality of good governance began to decline. "So no, Honourable President, do not keep telling us what the ANC has done since 1994. Again, the ANC did not liberate South Africa nor build democracy alone. But the ANC did, when given the chance, destroy the economy single-handedly. The ruling party did create the conditions for crime to flourish and for corruption to run rampant through the public service. You did cripple the education system and the healthcare system, destroying our hope of building our future," Hlabisa said. He blamed Ramaphosa for continued economic decline under the ANC government and an increase in crime. Hlabisa said while the IFP supported initiatives such as universal healthcare, the National Health Insurance (NHI) Bill that the ANC government wanted to pass into law would be a "recipe for disaster". "The [Health] Minister has already confirmed that government does not have the funds to hire doctors. The NHI Bill has been overwhelmingly rejected by the healthcare sector, business and the opposition. Like Outcomes-based Education, which handicapped our education system and had to be abandoned, the NHI will cripple our health sector," he warned. SA IS IN CRISIS Hlabisa echoed Democratic Alliance leader John Steenhuisen's call for change through voting and said, "It truly is now or never." He stated that the country was on autopilot and in crisis, and he accused Ramaphosa of putting the ANC ahead of the country. He dismissed another five years of ANC rule and said the IFP was ready to govern. "We did it in 1994. And the IFP is ready to do it in 2024. We will do it with our votes," he said. Hlabisa, like Steenhusien, hit back against Ramaphosa's story of Tintswalo, a child of democracy, born in 1994, and meant to represent someone who had reaped the benefits of freedom, under the ANC government. "Tintswalo's story is not the story of today's youth. Unlike Tintswalo, 41% of our youth are unemployed. Thousands of NSFAS beneficiaries are sitting at home with their degrees and diplomas, without jobs. Their dreams are fading away. Millions of children of democracy still live in shacks and informal settlements, with no clean water, toilets or refuse removal. Their only connection to Tintswalo, is that she, too, sits vulnerable in the dark, o...

Political parties and non-governmental organisations (NGOs) have criticised President Cyril Ramaphosa's 2024 State of the Nation Address (SoNA), saying it is fraught with big promises that show no sign of learning from past mistakes or changing course. Ramaphosa delivered his eighth SoNA on Thursday evening at the Cape Town City Hall. Democratic Alliance (DA) leader John Steenhuisen said Ramaphosa had not kept any of his promises made in his previous SoNAs and that he had not tabled any pragmatic, workable legislation to realise his 'New Dawn'. Steenhuisen stated that the South African economy had all but flatlined, with no new jobs, worsened corruption and crime, and millions of starving children. He noted that 30 years of African National Congress (ANC) national government had caused the country's economy to stagnate and had led to significant fiscal restraint - "an environment in which the ANC is now pursuing desperate and populist measures such as tapping into the Gold and Foreign Exchange Contingency Reserve Account of the Reserve Bank, essentially killing any economic buffer for the country against unforeseen external market shocks," he stated. Steenhuisen said a looming tax hike under the ANC would be the final nail in the coffin for millions of South African households struggling to put food on the table. He said South Africa needed a complete reprioritisation of its national Budget that cut away Cabinet perks and a bloated public wage bill, and that redirected money towards paying better social grants that met the food poverty line, fixed the electricity crisis by embracing privatisation, invested in education, and embraced policies that liberated the economy to fast-track growth and job creation. NATIONAL HEALTH INSURANCE In his speech, Ramaphosa noted that government was planning to incrementally implement the controversial National Health Insurance (NHI), the Bill for which awaits his signature. The DA said that by embracing the NHI Bill, Ramaphosa was taking a wrecking ball to the country's public health system, driving skilled doctors and medical personnel from the country, and killing South Africa's status as a world leader in healthcare innovation. He said the NHI could be pushed through Parliament when government could afford to employ and place the country's existing graduate doctors. "Furthermore, we cannot embrace NHI when the ANC cannot even feed our children. According to the Eastern Cape Department of Health, 1 722 children under the age of five in the province were newly diagnosed with severe acute malnutrition between August 2022 and September 2023. Of these, 114 have died," said Steenhuisen. Civil rights interest group AfriForum highlighted that Ramaphosa's "search for a pen" to sign the impugned NHI Bill was a shameful attempt to use the false promise of better health care to win a few votes. Solidarity said it considered the promise of "free healthcare" to be another empty promise but one with very serious consequences. "It is clear that the government, regardless of their hopeless track record, is appropriating even more power for itself. This is taking place while South Africans actually need the federal devolution of power to communities," it said. Ahead of President Cyril Ramaphosa's State of the Nation Address (SoNA), this evening, the African National Congress (ANC) assured that it remains steadfast in its resolve to overcome challenges facing the country and to continue steering South Africa toward prosperity. Ramaphosa will deliver his eighth SoNA with many expectations from citizens, political parties and business. The ANC said Ramaphosa would be delivering the address amidst progress and challenges. "This year's State of the Nation Address is expected to reflect on the gains we have made as a nation as well as affirm our determination to defend our democracy from opportunistic elements that are set on undermining the unstoppable programme of transformation. The address must be a beacon of hope as it accounts on commitments made whilst pointing us all in the direction that must now be taken to transform South Africa in the interest of a better life for all," said the ANC. NATIONAL HEALTH INSURANCE BILL The ANC also asserted its commitment to bring the controversial National Health Insurance (NHI) to fruition. It said it "strongly" supports the NHI Bill as a vital step towards achieving universal healthcare coverage. The Bill was processed by the National Assembly and the National Council of Provinces last year and sent to the President to be signed into law. The NHI Bill was met with criticism, however, with business groups pushing Ramaphosa to delay signing it into law, saying that it did not pass constitutional muster and would likely lead to many legal and social troubles down the line. In an open statement to Ramaphosa, Business Unity South Africa and Businesses for South Africa petitioned the President to first test the constitutionality of the NHI Bill before signing it into law. However, the ANC believes that by pooling resources and addressing disparities, the NHl will improve health outcomes nationwide. Ramaphosa's spokesperson Vincent Magwenya noted that Ramaphosa was still considering the NHI Bill and that the President's legal advisers were looking at the proposed legislation. SOUTH AFRICAN SUCCESS Meanwhile, the ANC said that it also celebrated the country's achievements on the global stage, from "asserting moral leadership" to showcasing the talents of South Africans in sports and in music. "Despite global shocks like the Covid-19 pandemic and geopolitical unrest, South Africa's resilience shines through, propelling us on an upward trajectory despite economic setbacks. This year's State of the Nation marks the culmination of an extraordinary term of office characterised by several severe global shocks, as well as the rise of South Africa's prominence as an international moral authority on the global stage," the ANC said. The party noted that the sixth administration faced the daunting task of reinvigorating and fortifying the State, which It said had been weakened by corruption and counter-revolutionary elements. Through concerted efforts, the ANC said significant progress has been made in combating corruption and rebuilding State institutions. The party said that the ANC-led government had dramatically improved access to education from early childhood right through to post-school education. Nearly every young person aged 15 to 24 years of age is literate, and adult literacy now stands at 85%, up from only 64% in 1996. "Today we have a growing black and skilled professional sector. Many interventions have been introduced, including school fee exemption, the school nutrition programme and scholar transport which ensures that poverty does not stand in the way of children's access to quality education. It is this administration that has seen the highest percentage of people writing matric and passing matric, a testament to the ANC's commitment to education for all," the party pointed out.

Democratic Alliance (DA) leader John Steenhuisen promised on Tuesday that if President Cyril Ramaphosa rams through the National Health Insurance (NHI) Bill, the DA will immediately take him to court to protect the country's health system against what the DA calls the African National Congress's (ANC's) gutting of the country's health system. Steenhuisen was outlining the DA's blueprint to "rescue South Africa" after the 2024 general elections, when he said a new government would immediately prepare to introduce a new Budget to reflect its new priorities. He said the DA would bury the ANC's "job-killing policies", including land expropriation without compensation, the nationalisation of mines, farms, industry and the Reserve Bank, as well as the "madness" of the NHI. Steenhuisen said the DA fully supported the call by business leaders for Ramaphosa not to sign what is widely being called an "unconstitutional" NHI Bill, and to instead refer it back to Parliament. He applauded Business Unity South Africa and Business for SA for taking what he described as a principled stand against the NHI, after they also urged the President to refer the Bill back to Parliament. The NHI Bill was passed by the National Council of Provinces in late 2023, and sent to Ramaphosa to be signed into law. Government has stood firm in its support for the Bill, brushing off warnings that it undermines universal healthcare instead of advancing its progress. However, the Bill has been sharply criticised by doctors, health facility providers, and health insurance companies. Steenhuisen noted that the process that led to the Bill ignored public input and said the Bill itself was riddled with unconstitutional clauses. On December 14, the DA wrote to Ramaphosa imploring him to refer the NHI Bill back to Parliament, making the point that given the flaws in the Bill, it was, in fact, his constitutional duty to refer it back. Last month, the DA received a response from Ramaphosa's Parliamentary Counsellor Gerhard Koornhof. Steenhuisen noted that without even so much as addressing the substance of the party's request, Koornhof simply said: "You are kindly reminded that the DA participated in the Parliamentary process related to this Bill. The Bill was thereafter approved by both Houses, in a Parliamentary process in which the DA participated." Steenhuisen criticised Ramaphosa for seeing fit to send such a "politically-motivated response", which in no way dealt with the issue of the unconstitutionality of the Bill. He said this demonstrated that the ANC was putting the country's health sector at risk purely for political reasons. "Ramaphosa's apparent determination to ram this Bill through apparently to stick it to the DA, would be a violation of his constitutional duty to this country. We, therefore, also welcome the announcement by business leaders that they are considering legal action against Ramaphosa if he persists along this route," he said. ECONOMIC GROWTH AND JOB CREATION Meanwhile, Steenhuisen highlighted that if it came into power, the DA's "jobs-friendly" Budget would redirect priorities from destructive policies, towards job creation. He said for the first time in three decades, entrepreneurs, businesspeople, investors and hard-working South Africans would see a Budget that respected them. "For too long, the ANC has viewed economic growth with suspicion, and the people who create jobs as the enemy. With the DA in government, investor sentiment will soar and our new Budget will kickstart economic growth and job creation," he added. The immediate stimulus of a jobs-friendly new Budget will be followed by the introduction of the DA's Responsible Spending Bill and the Social Impact Bill. The former will introduce a debt rule that limits the amount of debt any government can add to the fiscus. Close to over 25,000 healthcare workers, allied health specialists, and GPs are appealing to President Cyril Ramaphosa to refer the National Health Insurance (NH) Bill back to Parliament. South African Health Professionals Collaboration (SAHPC) spokesperson Dr Caroline Corbett says they would like it to go back for discussion and active engagement with all the role players. In addition, the Constitutional Court should be approached and it should be investigated awhether or not the Bill in its current form infringes on the constitutional rights of communities and citizens nationally. “…we're absolutely not going to stop and sit and watch this quietly be signed off,” Dr Corbett tells BizNews - after the National Council of Provinces (NCOP) passed the Bill this week. A major concern is that the Bill “doesn't actually guarantee quality healthcare”. The SAHPC also has “massive concerns” around the governance of the fund in a country where corruption “is rife still, unchecked still, and unprosecuted in most instances”. Business committed to building SA, Ramaphosa says after Ntshavheni's accusation More than a week after Minister in the Presidency Khumbudzo Ntshavheni accused the private sector of engineering the state's collapse, President Cyril Ramaphosa said business' current partnership with government illustrates how committed they are to the country. Ramaphosa and business issued a joint statement on Wednesday after Cabinet met with business leaders as part of their initiative to deal with the country's energy, transport, and logistics, as well as crime and corruption problems. "Since we began, we have made significant progress in establishing structures and ways of working, mobilising resources and driving implementation of key actions. We see this partnership as evidence of the business community's commitment to building our country and overcoming the challenges that we face," Ramaphosa said in a statement. At a post-Cabinet briefing last week, Ntshavheni was asked about a long-standing probe into rand manipulation that involves 28 banks. She said that "the performance of the economy has been manipulated by [the] private sector, [which] has no interest in the development of this country", adding that the private sector continued to "engineer and do machinations to ensure the government collapses". Business Unity SA said it hoped Ntshavheni would reconsider her comments and it would "raise it with government through the partnership".BUSA president and Discovery CEO Adrian Gore said in Wednesday's joint statement that there had been some movement on collaboration structures with government. Some of the highlighted successes include over 1 000MW of power generation capacity, establishing the Rail Infrastructure Manager for Transnet - which opens the doors to third-party train operators, and establishing a digital and financial Forensic Analysis Centre. "Despite these initial successes… the pace of delivery across the initiative seems to be plateauing, mainly as a result of delays in regulatory and other approvals, as well as slippage on the implementation of strategic plans, and the alignment of these to workable funding solutions," Gore said. Healthcare professionals also voiced their concerns over the NCOP's refusal to amend the bill, which has moved almost entirely unchanged through the parliamentary process since 2019. Business organisations are particularly concerned about Section 33 of the Bill, which deals with the phasing-in of the NHI and the phasing out of private medical aid insurance. It states that "once the NHI is fully implemented as determined by the Minister of Health, medical schemes may only offer complementary cover to services not reimbursable by the fund". Section 33 is certain to be challenged on constitutional grounds, due to the limitation it would impose on the right to health. Trade union Solidarity has already indicated that it will challenge this. The Bill aims to establish universal healthcare for all South Africans by abolishing the "two-tier" system, in which the bulk of healthcare spending is made in the private sector for the benefit of a few, while the mass of the population is relegated to the "under-funded" public sector. The Bill will establish a single NHI Fund, managed by the state, to buy healthcare services from the public and private sectors. Services will be free to the public at the point of delivery. Once a service becomes available under the NHI, medical schemes will no longer be allowed to cover it, implying the ultimate phasing out of private medical insurance. Proceedings in the NCOP started at 14:00, but the vote on the Bill is expected later in the afternoon.

The Public Servants Association (PSA) on Tuesday expressed concern that government is using the introduction of the National Health Insurance (NHI) to campaign for the 2024 elections. The PSA is also calling on President Cyril Ramaphosa to reconsider the NHI and instead allow for further discussions to clarify unanswered questions and concerns raised by relevant stakeholders and warned that if that is not done, the NHI will be a “dead elephant” like the e-tolls. It said while it supports the rehabilitation of the public health sector, the PSA's view is that members, like other citizens, must reserve the right to choose whether to use public or private healthcare. The PSA warned that the current single funding model for the NHI will plunge the citizenry into a deeper financial crisis as they will be burdened to fund this initiative. “The idea that workers might be expected to pay for private medical insurance and the NHI simultaneously, induces anxiety amongst public sector employees. The chief priority of health reform in South Africa should be the rehabilitation of the public health sector. This can be best achieved by a competent, non-partisan public health service, free of corruption and political interference,” said the union. The union is also concerned about the future of medical schemes, especially as it relates to employees' access to medical aid schemes. It further warned that the NHI Bill may reverse the gains workers have made in respect of access to quality healthcare.

Is the NHI a blessing or a curse for South Africa's healthcare system? That is the big question that we aim to unpack in today's discussion. The National Assembly this week  passed the controversial National Health Insurance (NHI) Bill with 205 MPs in support of it. The bill will now be sent to the NCOP for concurrence then signed into law by the President Cyril Ramaphosa. The Bill seeks to give all South Africans access to quality health care and makes provision for one pool of health care funding for private and public health care providers. Deputy Director General for the National Health Insurance (NHI) at the Health Department, Dr Nicholas Crisp says the primary source of funding for the NHI Bill will be the current tax system. A few organisations have vowed to legally challenge the implementation of the National Health Insurance Bill. For this discussion we are joined by Dr Simon  Strachan,  Paeditrician in private practice and the CEO of the South African Private Practitioners Forum  AND Dr Zamasomi Meyiwa Luvuno from the Centre for Rural Health at the University of Kwa Zulu Natal....

In a wide-ranging conversation with ‘Sunday Times Politics Weekly', EFF leader Julius Malema shared his views on the upcoming elections, and possible coalition permutations. Malema says he won't join any coalition in which Cyril Ramaphosa or John Steenhuisen are potential presidents. However, he did speak of his willingness to work with deputy president, Paul Mashatile, whom he acknowledged he had a soft spot for. Malema also addressed the willingness of the party to accept former ANC secretary-general Ace Magashule who, like himself, had been expelled from the party. Malema also reflected among other things on: the party's overall growth ahead of their 10 anniversary celebrations in July, his stance on the impending visit by Russian President Vladimir Putin, the recent passing of the NHI Bill, corruption allegations levelled against him, his past actions and their impact on how the public see him, and the party's views of the commemoration of Youth Day in the present day.
