In this episode we're going to be running through adrenal presentations; both Adrenal insufficiency and Adrenal Crisis. There are some parts of these that aren't completely understood and a lack of a universal definition of Adrenal Crisis, but both insufficiency and a crisis are similar problems at different points on a spectrum and solid understanding of the endocrinology and physiology can really help to improve care in this area. There is huge potential for improving current morbidity and mortality. We'll run through both primary and central adrenal insufficiency, describe how this leads to different effects on mineralocorticoids and glucocorticoids and the signs and symptoms that will occurs as a result. Many of the patients presenting to the department will be unknown to have adrenal insufficiency and we'll run through those who are at higher risk, including a huge group due to ongoing medication, who may be those on steroid doses much lower than you would previously have considered as significant. NICE published their most recent guidance on Adrenal Insufficiency in August this year and we'll be referring to a lot of this as we run through the episode. We'll finish up looking at the critical presentation of Adrenal Crisis and the emergency and ongoing management, along with how we support patients with insufficiency to prevent a crisis occurring. This is the MOST frustrating challenge I have with living with ADRENAL INSUFFICIENCY. It is the F WORD of Addison's Disease. Dealing with a STATIC dose of medication in a world that is forever changing and every moment it takes a bit more cortisol leads to a crippling existence of low cortisol symptoms. Living in an illness lacking the tools to measure my critical LIFE SUSTAINING CORTISOL levels I am left to fend for myself based on physical symptoms. At one point Dawn from a small town in Alberta couldn't walk to the end of her driveway without being winded. She has now conquered her diagnosis in 2020 with secondary adrenal insufficiency and now walks thousands of kilometres a year. Dawn's secondary adrenal insufficiency was caused by the removal of a pituitary tumour that was producing ACTH hormone. But her journey started years before SAI with a life full of illness, steroids, autoimmune disorders, asthma and migraines. She has learned to set boundaries, manage her medication, improve her overall wellness and live a life with SAI. I share with you more of what happened to my almost 10-year-old Doberman Meko on Good Friday who suddenly passed away. In a high-stress situation, I was left to assess my own health status to make sure I did not go into an adrenal crisis. Let the loss of my boy Meko remind us all of the importance of being prepared for our own emergencies and being prepared and setting boundaries when we are dealing with an emergency situation. In this episode we discuss adrenal insufficiency You can have either primary, secondary, or tertiary adrenal insufficiency. Primary adrenal insufficiency is also called Addison's disease. When you have this type, your adrenal glands are damaged and can't make the cortisol you need. They also might not make enough aldosterone. Secondary adrenal insufficiency is more common than Addison's disease. The condition happens because of a problem with your pituitary gland, a pea-sized bulge at the base of your brain. It makes a hormone called adrenocorticotropin (ACTH). This is the chemical that signals your adrenal glands to make cortisol when your body needs it. If your adrenal glands don't get that message, they may eventually shrink. This is the type our guest on this week's episode is dealing with. Tertiary is due to hypothalamic disease and a decrease in the release of corticotropin releasing hormone (CRH). Causes can include brain tumors and sudden withdrawal from long-term exogenous steroid use (which is the most common cause overall) The most common cause of Addison's disease today is an autoimmune problem, when your immune system malfunctions and attacks and damages your own body, in this case, your adrenal glands.

A very special episode as Catharine joins us to share her journey with adrenal insufficiency. Her journey is from the perspective of a parent whose 3-year-old son was diagnosed with adrenal insufficiency in March of 2023. Catharine's MOMMY INSTINCT and MOMMY SUPER POWERS dominated diagnosis and have fast-tracked her son's prognosis. For over 4 years Ellen struggled with failing health which led her to wonder if this is the end. Multiple diagnoses and treatments during the pandemic left Ellen often alone and fighting for her health. Symptoms that were physically crippling and reoccurring bouts of abdominal pain, extreme muscle weakness and eventually tan skin and extremely low blood pressure. With two small children she was unable to take care of and a caring devoted husband Ellen created her BUCKET LIST of things she wanted to experience fearing her end was near. She was often treated with prednisone as the doctors desperately tried to weed through her symptoms and make her well. A blood test of low cortisol from years early was an important piece of the puzzle when Ellen mentioned it to her doctor. Jill experienced a PROUD Mommy moment and another confirmation about the 'reason' she was diagnosed with Adrenal Insufficiency. Since diagnosis in March of 2011, Jill has worked HARD at having a CARING, COMPASSIONATE medical team that understands the seriousness of ADRENAL INSUFFICIENCY and recognizes that Jill's quality of life directly reflects how well the illness is being managed. A daughter who is being 'watched' for ADDISON'S DISEASE is off to post-secondary education away from home. She is dealing with being 'sick' away from home and made her MOMMA proud. The first diagnosis was STREP THROAT but through her persistence, she was diagnosed with MONO. It was blood work, follow-ups, and dealing with a medical community when she mentioned "I am being followed for ADDISON'S DISEASE" which clearly communicated their lack of knowledge and concern. Her MOMMA has set high standards for her care and she did an amazing job managing her health away from home. Jill sees the quality it has already added to her daughter's life and IF she is diagnosed with ADRENAL INSUFFICIENCY Jill knows she will be a ROCK STAR JUST LIKE HER MOMMA.
