Why You Should Listen:  In this episode, you will learn about the important role of hypercoagulation in chronic illness.  About My Guest: My guest for this episode is Ruth Kriz.  Utilizing her functional medicine background as well as experience in microbiology and teaching pharmacology, Ruth Kriz, MSN, APRN has spent the majority of her professional career as a Nurse Practitioner working with Chronic UTI and Interstitial Cystitis patients; as well as those dealing with hypercoagulation.  Her practice expanded to patients from almost all the states in the US as well as from 35 countries who came to her seeking answers beyond symptom management.  Through molecular testing, an understanding of the genetics common to these patients, and an understanding of how genetics contributes to chronic infection, hypercoagulation, and biofilms, she has been able to very successfully treat this population.  These factors have broad implications for other chronic infections (sinus, prostate, ear infections, wounds, etc.) as well as fibromyalgia, cardiovascular disease, and other conditions in which hypercoagulation and biofilms are an important contributor.  She has closed her medical practice, but she reinvented as a consultant to help practitioners learn how to utilize her approach for curing these patients and currently is working with MicroGenDX. Episode 72: Chronic Conditions, Pain & Positivity with Julie Taylor I am so honored to introduce Julie Taylor. Julie is an Endo and IC Warrior who's passionate about mindset as medicine. Endo is an abbreviation for endometriosis and IC stands for Interstitial Cystitis (a painful bladder syndrome). While healing with these chronic conditions, she learned the power of crystals and energy to heal and how using a positive mindset as a tool to help manage her moods and emotions helped her body build resilience. Julie's formal degrees are in writing and early childhood ed, though the innate cheerleader at home in her heart drives her to write words, she hopes will spark a mood or mindset that might brighten things for others. Julie is the author of a children's book, Blue Hissy Highness and the Shiny Stones (you can buy that here). The book focuses on how a positive mindset can reframe and heal. It also reminds children that we can move toward mutual acceptance and forgiveness, but also the very adult reminder that acceptance isn't a one-and-done ideal—a lesson we need to revisit as we age. Julie also tends a website and an online space called Stone Spelling & Witchery, which is also the name of her private Facebook group Stone Spelling & Witchery to connect and share the metaphysical and mystical, celebrate earth and moon cycles, share spells, post favorite crystals, chat about witchiness, and ask questions. Julie curates a beautiful community that serves as a magical hub for joy and positivity. In this episode, Julie and I talk about living with chronic conditions and chronic pain, how we manage our spirit during times of flares. We talk about positivity, staying positive, toxic positivity or not toxic positivity, the benefits and downfalls of emotional bypassing, and what Julie has terms LoMo—or how to manage Low Moods and shift them to help with chronic conditions. Honestly, this is such an awesome, refreshing conversation. As someone who has to manage many chronic autoimmune conditions, I find simply speaking my truth about living with pain and fatigue can be important.

It is especially frustrating when you go to the Doctor and get a urine dipstick test and it comes out negative. Many people with interstitial cystitis think they have a bladder problem and place a lot of the blame on the food they eat, but today Nicole Cozean debunks these common myths. As a pelvic floor therapist with almost 20 years of experience in the field, she takes an orthopedic approach to treating the pelvic floor. The most effective treatment for pelvic floor dysfunction is physical therapy, which has been proven time and time again. We also dive into the misconception that the IC diet is the way to recovery, when in reality most people with interstitial cystitis aren't diet sensitive or only have a few diet triggers. Is Elmiron safe and effective for interstitial cystitis?

With recent evidence Elmiron can cause long-term eye damage, many people with IC are reconsidering their use of the drug.  In this ‘sode we dive into the evidence for Elmiron's efficacy and safety to help you make an informed decision.Elmiron remains the only FDA-approved oral medication specifically for IC and many urologists prescribe it automatically because of this.However, it's unclear how much of a positive effect Elmiron has.  There have been five randomized, controlled trials with Elmiron.Two showed a slight benefit (a 25% improvement) for a small percentage of patients (between 15-16% compared to placebo)Three showed no benefit at all compared to a placebo and the largest trial wsa stopped early because the drug wasn't shown to be effective.From the data, we have to conclude either Elmiron has no benefit at all, or at best has a slight benefit from about 15% of people.  And we are left to weigh whether that upside is worth the risk.Researchers have discovered Elmiron use is linked to eye damage (though it doesn't happen for everyone on the drug).  This link gets stronger the longer you've taken the drug and the more of it you've taken.  The FDA has put a warning label on the drug and the company is being sued over it.  If you are on Elmiron, make sure to be getting your eyes examined by a retinal specialist.Other side effects of Elmiron have been called ‘mild', but for many patients are anything but.  Significant hair loss is common, as is GI upset and other issues.  With questionable efficacy in question and significant safety concerns, it would be hard to recommend anyone starting Elmiron with what we know today.  If you have been on the drug and find it helpful it's often a more difficult decision and one to make in consultation with your physician.  As with any change in medication, always make sure you discuss thoroughly with your physician.Remote ConsultationIf you're struggling to get answers from your local practitioners or aren't sure what's next, PelvicSanity offers remote consultations to give you a concrete action plan going forward.  Our expert team will evaluate your entire case and give you our honest, clear recommendations going forward.  Check out our Remote Consultation program (www.pelvicsanity.com/remote) for all the details!About UsDr. Nicole and Jesse Cozean are the founders of PelvicSanity Physical Therapy (www.pelvicsanity.com) in Southern California. One of the most common questions we get asked is whether your symptoms are truly IC or are they pelvic floor dysfunction.  It's important to know that ‘IC' is a description of symptoms.  It just means you're experiencing pain you perceive to be coming from the bladder and urinary urgency/frequency.  But it tells us nothing about the ‘why' of your symptoms or what treatments to try.It's also important to realize that IC is not a bladder condition.  It's a pelvic pain condition.  More than 90% of people diagnosed with IC have no discernable problem with their bladder.  Even those who do have Hunner's lesions are not ‘cured' when those are removed.  Among those diagnosed with IC, nearly 90% have confirmed pelvic floor dysfunction.  And at PelvicSanity, we've literally never seen someone with IC symptoms who didn't have a pelvic floor component.It's often down to your physician whether you get an IC diagnosis.  Some urologists don't like saying ‘IC' to patients because it leads them down the bladder-centric road of Elmiron, instillations and focusing on the ‘IC diet'.  Others are more liberal with the diagnosis and use it for any patient who has the symptoms that qualify.Ultimately, if you're diagnosed with IC many - if not all - of your symptoms are likely coming from the pelvic floor.  So there's a huge overlap between these terms.  It makes sense to focus less on the label and more on finding the practitioners who can help!IC Roadmap CourseIf you've been diagnosed with IC (or suspect you may have it), we have a full-length online course with all of the information you need to understand the diagnosis and formulate your plan for recovery.  You can find The IC Roadmap course here (www.pelvicsanity.com/icroadmap)!And if you haven't already, make sure you join the Finding Pelvic Sanity online community!About UsDr. Nicole and Jesse Cozean are the founders of PelvicSanity Physical Therapy (www.pelvicsanity.com) in Southern California. What is interstitial cystitis (IC) and how is it diagnosed?

Interstitial cystitis (IC) is a scary and incredibly misunderstood diagnosis.  You may have also heard terms like ‘painful bladder syndrome' or ‘bladder pain syndrome' as well.  We want to shed some light on what IC is, how it's diagnosed and what you can do about it!Definition of IC:  Interstitial cystitis is defined by the presence of two symptoms in the absence of any other explanation (like a UTI or bladder cancer).  It's:Pain or pressure we perceive to be related to the bladder, and Urinary urgency or frequencySymptoms of IC:  In addition to the hallmark symptoms of bladder pain and urinary urgency/frequency, people often have other related issues.  These can include painful intercourse, low back pain, having to go to the bathroom at night (nocturia), pain with sitting or exercise, constipation and other pelvic health symptoms. Do I really have interstitial cystitis?  IC is a diagnosis of exclusion, which means it's only diagnosed by ruling out other things (like bladder cancer or a UTI).  There's no test that can confirm you have IC (including a cystoscopy).  If you have the symptoms above - pelvic pain perceived to be related to the bladder and urinary urgency/frequency - you can be diagnosed with IC just by the symptoms.Importantly, the IC diagnosis doesn't tell you anything about WHY you have the symptoms or WHAT the best treatment option will be for you!Physical Therapy for Interstitial Cystitis.  The muscles and nerves of the pelvic floor can be responsible for every symptom of IC.  They can cause the bladder pain and urinary symptoms.  They are also the cause of seemingly unrelated symptoms like painful intercourse, low back pain, hip pain or constipation.  It's the reason symptoms are often worse when sitting or after strenuous exercise, and why they can get better after gentle stretching or a warm bath.Physical therapy is the highest recommended treatment (Grade ‘A') by the American Urological Association.  It's literally the first recommendation you should get from a urologist when IC is suspected.  We hope this gives you a good overview of the condition, and we want to leave you with the first lines of The IC Solution (link): “Thousands of people with IC live healthy, happy and pain-free lives - and so can you!”About UsDr. Nicole and Jesse Cozean are the founders of PelvicSanity Physical Therapy (www.pelvicsanity.com) in Southern California. In this episode, we discuss how important our pelvic floor is to our overall health with guest Ingrid Harm-Ernandes. Many women experience symptoms in the pelvic region during menopause. These are known as genitourinary syndrome of menopause (GSM) and are often not discussed with women as they transition through menopause.Ms. Harm-Ernandes is a physical therapist, board certified in Women's Health (WCS) and Pelvic Floor Biofeedback who has been specializing in women's health for 27 years. During her 20-year tenure at Duke University Health System, she treated a wide variety of pelvic patients in the Urogynecology and Physical Therapy (PT) clinics. In addition to her role as the Co-Director and a mentor for new Pelvic PTs in the Duke Women's Health Physical Therapy Residency Program, she was also responsible for the Women's Health team program development and participated in Pelvic Floor Disorder Network (PFDN) research projects.One of the frustrating things Ingrid encountered during her practice was how it would take patients experiencing symptoms decades before they would see her and get the help they needed. As a pelvic health PT specialist, she has addressed a myriad of pelvic health conditions that dramatically impact women's lives. She realized that there is a lack of education in pelvic health on both the practitioner's and patient's side. In 2021 she published her book, The Musculoskeletal Mystery - How To Solve Your Pelvic Floor Symptoms, which helps to better educate patients and practitioners about women's bodies, pelvic health conditions they may suffer, and what pelvic PT is all about. Her book contains guidance and support for those suffering from the physically, emotionally stressful, and often undiagnosed, pelvic floor disorders, pelvic pain, irritable bowel syndrome, endometriosis, Interstitial Cystitis, prostatitis, pain or discomfort during sex, and incontinence.Ingrid is a member of the National Menopause Foundation's Medical Advisory Committee (MAC). On this week's episode of Food Freedom and Fertility we are going to discuss Interstitial Cystitis and Fertility with Callie Krajcir, RDN, Nutrition Coach. Callie has lived with IC since she was very young.  Elementary through college her symptoms would cause her pain and put her on a rollercoaster of emotions as she continued to see a variety of doctors who did not know what was wrong with her. During college she tried a variety of treatment methods and learned to advocate for herself because doctors would eventually give up. She felt isolated and learned how to alleviate a lot of her symptoms through her diet. She entered the field of nutrition in hopes of being able to help others suffering from the same condition. Today, Callie shares with us her story and her symptoms, which started with burning while urinating. She also helps provide insight to what a flare up could look and feel like and how it affects her personal life and how to be a supportive partner/friend to someone who maybe suffering with IC. She also shares how her journey about how nutrition helps keep her symptoms minimal and exactly what foods and other triggers that can be bothersome for IC. IC is somewhat of a taboo topic because some people feel awkward with the discussion but 4-8 million women and 1-3 million men have IC in the US. There are subtypes of IC and Callie helps break down the differences.  The onset of IC is more common around the age of 30-40. Callie shares a quick list of foods that may cause a flare up and also supplements that can support IC patients, along with suggestions of what type of treatment team would be helpful. She also shares strategies that can be put into place to help with sex.
