I was lucky enough to get in amongst it right there at the roadside all week and, to make it better, I was joined by my old mate - the Doughboy himself - Ian Boswell.  For this episode, I wanted to take you guys inside the Tour Down Under; not just the racing, but the atmosphere around the race and the vibe that gets cooked up in Adelaide every year. The racers lining up for the first time, getting use to their new teams and equipment, the fans at the side of the road, and all the sweet little events that go off each morning out of Adelaide.  Ian and I kicked off our week with a big bang, getting stuck into the cut and thrust of the short, sharp prologue from inside the team car! I hopped in the NSN passenger seat to follow Ethan Vernon - who actually went and finished 2nd on the stage, missing the win by less than a second…maybe his DS was a bit distracted? In the paddocks, we got a chance to catch up with some of the riders who've come Down Under to start their 2026 racing season. Guys like the new Natty champ Pat Eddy, British up and comer Matty Brennan, the 16 wheeler himself Harry Sweeny, and heaps more. We spoke about the race, as well as some general questions about their seasons and - of course - a cheeky little mini CommuniQuiz to keep them on their toes! Of course, this race is defined by the fans. People have travelled from far and wide to be here - one girl I spoke to even made the journey all the way from Texas in the USA! Paediatric seizures are common, time-critical events and they're something most of us will deal with, whether that's pre-hospital, in the emergency department, or on the ward. They make up around 1–2% of ED attendances, and about 1 in 20 children will have a seizure at some point. Most seizures self-terminate, but the longer they go on the harder they are to stop, and the higher the risk of harm. In paediatric seizures, time really matters. In this episode we take a step-by-step look at how to assess and manage a child who's seizing. We start with the fundamentals; how seizures are defined and classified, what status epilepticus actually means in practice, and why recognising it early makes such a difference. We then dig into the physiology behind seizures, exploring why early benzodiazepines work well and why delayed treatment often doesn't. Understanding what's happening at a receptor level helps make sense of when to escalate treatment and why different drugs work at different stages of a prolonged seizure. Pharmacology is a big part of this episode. We talk through first- and second-line anti-seizure medications, routes of administration, and how effective they really are. We cover the EcLiPSE and ConSEPT trials comparing levetiracetam and phenytoin, and look at newer evidence from the Ket-Mid study and what that might mean for managing refractory status and thinking about RSI. We also work through the approach to cases, pre-hospital management and in-hospital care aligned with UK and European recommendations. There's a clear focus on febrile seizures too, separating simple from complex presentations and helping you decide who needs investigating, admitting, or reassuring and discharging. As ever, the aim is to turn guidelines and evidence into something usable on the shop floor. Paediatric seizures are stressful, but with a structured approach, early treatment, and good airway management, they're absolutely manageable and we can make a real difference on outcomes. Rob and his friends stayed long enough to see three towering figures step out from the tree line, the largest close to eight feet tall. His experiences didn't end in the wilderness. The crackdown on drug drivers is due to start today. Starting in Wellington first, saliva testing will be used to test the drivers for 4 drugs; cannabis, meth, MDMA and cocaine. The 2025 resuscitation guidelines have landed after further collaboration between ILCOR, the ERC and the Resuscitation Council UK and in this episode we break down exactly what's new, what's stayed the same, and how it all fits into day-to-day practice. Across the board the 2025 updates represent evolution, a steady refinement of evidence rather than wholesale change. Adult ALS remains rooted in early recognition, high-quality compressions and rapid defibrillation, but you'll notice sharper guidance around ventilation, pad positioning, and the sequence of vascular access and drugs. There's a new section on physiology-guided CPR and the emerging science behind arterial-line-driven resuscitation as we covered in the SPEAR epsiode. We also take a look at the special circumstances algorithms from hypothermia to traumatic and obstetric arrest and discuss how an emphasis on reversible causes, data-driven debriefing and system performance might reshape post-event learning.  Paediatric and newborn life support see subtle but important refinements too, including pad placement, shock energy escalation, simplification of adrenaline timings and a new Out-of-Hospital Newborn Life Support algorithm aimed squarely at the pre-hospital world.
