The implementation of roadside drug testing is being hailed as a huge step in the right direction. From December, Police across Wellington can do random roadside saliva tests – screening for cannabis, methamphetamine, MDMA, and cocaine. They'll spread to cover the whole country by mid-next year. Drivers testing positive will need to do a second test that checks for 25 substances. Drug Detection Agency Chief Executive Glenn Dobson told Mike Hosking data shows 30% of all road deaths now involve an impairing drug. This week's edition of Overdrive Radio drops into our awards ceremony October 23, 2025, with four Small Fleet Champs -- the owners of Clifford Hay Inc., Thomasville Funiture Xpress, Turnage & Sons, and Oberman Logistics all on hand for the event in Nashville, Tennessee. Before the dinnertime program got started that fine Thursday evening, Overdrive editor Todd Dills had the chance to sit down with all of the owners, with results in this wide-raning roundtable talk around what TFX co-owner Scott Denmark pointed out was more of a rectangular table in fact. Be that as it may, the pair of Scotts (Scott Cruthis is Denmark's co-owner) is joined here by Clifford Hay and Wes Oberman, likewise Robbie Turnage, all swapping stories and biz advice in response to two principal questions: 1. What's been your biggest business challenge in recent years, and how are you working to overcome it? 2. What's the best piece of advice you might give an aspiring small fleet owner? Topics range across matters of trucking insurance hikes, investment to handle tire maintenance in-house for sizable savings and no small number of breakdown headaches and towing horror stories met head-on. Both Cruthis and Turnage own their own fifth-wheel tow hooks, giving the fleets capability to rescue a rig sidelined without getting dinged with a huge tow bill (and saving on maintenance by doing necessary work in-house,too). Turnage told the story of a near $15K tow bill for a grand total of four miles of towing for a job the tow company claimed required a rotator and a hefty "EPA clean-up fee." Turned out the tow operator didn't even own a rotator and certainly didn't use one for this particular job. Turnage found it out when he showed up in Pennsylvania with an appointment to pick his truck up, and the tow operator put him off and put him off for hours before finally relinquishing the equipment. How'd he get out of that one? Hear more about it in the podcast, along with a variety of other war stories from each of the individual owners. All are certainly doing a lot right, and with similarities amongst each other in many respects, though their operations couldn't be more different. Turnage and Sons operates all company-owned equipment, hauling milk in big tankers. Thomasville Furniture Xpress run less-than truckload -- yes, furniture, with a mix company and owner-operator equipment. Oberman Logistics is all owner-operator, mostly platform freight run through brokerage partners, and Clifford Hay up in New York has six owned trucks and probably couldn't be more diverse in terms of trailers owned and utilized for a wide variety of freight. Along the way, hear Overdrive's Dills introduce each fleet from the stage, and plenty advice from the champs about preparing to make any big move from one truck to many. There's also an anecdote about a 579 that gets misstated as a vintage, just 1-million-mile Pete 359 -- with plenty of surprise, laughter and obvious camaraderie amongst the owners assembled, that's certain. How, when and why to make the call… The pre-alert is one of the most powerful and sometimes most painful parts of emergency care. It can feel like the Spanish Inquisition, trigger tension between pre-hospital and ED teams, or drop another challenge into an already overflowing department. But done well, a pre-alert isn't an irritation; it's an opportunity to line up critical care for the next patient and genuinely improve outcomes. In this episode, Simon, Rob and James break down The UK NHS Ambulance Services and Emergency Department Pre-Alert Guideline, jointly released in July 2025 by RCEM and the Association of Ambulance Chief Executives. It's the first national attempt to give clear, shared expectations on who to pre-alert, what to say, and how to receive those calls, it's full of practical recommendations for both sides of the phone. We kick things off with a review of the evidence base, including brand-new studies showing just how varied pre-alert practice is across the UK. From inconsistent criteria and mixed training to the problem of “pre-alert fatigue”, the data make a strong case for standardisation. We then walk through the new guideline's key principles: pre alerting for pre-specified physiological parameters or specific conditions. We finish off with top tips for making and taking better pre-alerts - selling a story, leading with the headline, and understanding what the other side actually needs. a focus on its acute presentations and the care we can deliver to improve outcomes for our patients. Sickle cell disease (SCD) is a lifelong inherited blood disorder that affects over 15,000 people in the UK, and millions worldwide. It's caused by the production of abnormal haemoglobin molecules, which distort red blood cells into a crescent, or “sickle,” shape. These rigid cells can block small blood vessels, leading to painful vaso-occlusive crises and organ damage. While the condition has long been most prevalent in parts of Africa, the Middle East, the Mediterranean and India, today it's a global health issue, and one we encounter regularly in UK emergency care. Tragically, failings in care have too often led to avoidable harm. The 2021 parliamentary report “No One's Listening” laid bare some of these cases, highlighting missed opportunities, poor awareness, and systemic issues that cost lives, such as the death of Evan Nathan Smith. So why are we revisiting this now? In 2024, RCEM published new Best Practice Guidelines on managing sickle cell disease in the ED. These provide clear, evidence-based standards for recognition, triage, analgesia, infection control, and safe discharge. In this episode, we take you through the key elements; Pathophysiology – how a genetic mutation drives sickling, vaso-occlusion and inflammation. Clinical presentations – from painful crises and acute chest syndrome, to stroke, anaemia, infection, priapism and pregnancy-related complications. Recognition and triage – why timely pain control within 30 minutes is a must, and how to spot red flags. Investigations and treatment – including the role of reticulocytes, the importance of knowing a patient's baseline haemoglobin, and principles of analgesia, transfusion, oxygen, and supportive care. Discharge and ongoing care – ensuring safe, joined-up planning, and involving haematology and specialist pathways wherever possible. The take-home message? Every sickle cell crisis is a medical emergency. We need to listen to patients, escalate early, involve haematology, and deliver care that meets the standards they deserve. Work zone deaths are on the rise, affecting construction crews and everyday drivers. MoDOT's Jon Nelson shares critical safety practices, including training, risk assessment, communication systems, and accountability measures that can prevent work zone incidents and save lives.
Since 1994, the Washington Department of Transportation has operated a roadside memorial program that allows for the creation and installation of signs along state highways in honor of victims of fatal collisions. Washington was the first state in the nation to start a roadside memorial program, and there are now more than 1,000 signs posted along state highways bearing a message such as “Reckless Driving Costs Lives” or “Please Don’t Drink and Drive” above the name of a crash victim. Nearly 70% of the roughly 500 signs installed in the first 15 years of the program are still standing. The Columbian recently reported on WSDOT’s current efforts to collect public feedback about the future of its roadside memorial program and possible changes to it, such as limiting how long the signs can be up for and how often they can be renewed by family members. Kelly Moyer, a staff reporter at The Columbian, joins us for more details, including concerns that family members who had paid for memorial signs shared with her. We also hear from Sarah Risser, the treasurer and board member of the Portland chapter of the national nonprofit Families for Safe Streets. In 2019, while Risser was driving with her 18 year-old son in Wisconsin, a motorist crashed into the vehicle, killing her son and leaving her injured. Last year, Risser placed a sign honoring victims at every fatal crash site in Portland, and she has also created bike memorials at the request of families of cyclists killed on Portland roads.
