Welcome back! First up a paper to challenge the way we think about rhythm recognition in cardiac arrest to start with, looking at the rate of VF identified on echo but not on the defibrillator. We have a huge amount of strategies to rule out acute coronary syndrome in the UK, our next paper looks at the clinical effectiveness of these, whilst also giving us some hugely important information about the incidence of ACS in those presenting to Eds. Finally we look at a paper quantifying the effect of hypertonic saline in those patients with a TBI. Once again we'd love to hear any thoughts or feedback either on the website or via X @TheResusRoom. Landowners in Clare could soon be allowed to cut roadside hedges all year round. The local authority is set to approach the Department of the Environment to see if this county's roadside vegetation can be exempted from the Wildlife Act. Under the legislation, hedges can only be cut between September and February to avoid disturbance to nesting birds. Kilmurry Fianna Fáil Councillor Alan O'Callaghan however says it's posing a serious road safety issue.

Roadside Theater is a professional ensemble of storytellers and theater makers hailing from the mountains of central Appalachia. In its decades since its founding in 1975, the ensemble has taken on the task of people's theater, engaging social issues and abandoning the ‘fourth wall' concept in order to bring audiences into the process of cultural production. Roadside Theater reflects Appalachian culture, but it has also worked over decades to build theater in solidarity with social struggles across social constructs, including in close connection with the Student Nonviolent Coordinating Committee and their associated theater troupes, Free Southern Theater and Junebug Productions. On today's show, we're joined by editors and contributors to a new book that documents that history, called Art In A Democracy. We're joined by the book's editor, Ben Fink, who worked with Roadside Theater from 2015 until 2020, and co-founded the East Kentucky–Western Massachusetts cross-partisan dialogue project Hands Across the Hills. We're also joined by Donna Porterfield, the Managing Director of Roadside Theater from 1979 to 2019, with oversight responsibility for all of the theater's personnel and financial matters; as well as AB Spellman, a poet and essayist as well as a jazz critic and enthusiast, and a figure in the Black Arts Movement who worked for about 30 years at the National Endowment of the Arts. A conversation with Czech Cider Fan Dan Samek This episode brings us to the heart of Europe by way of Vermont. Ria sat down with Dan Samek, a cider enthusiast and home distiller from Prague, to talk about the evolving cider scene in the Czech Republic. They met at Kilowatt Park which is on the west side of the Connecticut River in White River Junction, Vermont. Dan Samek This was the first stop on the Cider Tour to Vermont and New York in May 2025. In this Cider Chat featuring Czech Cider 1. A Fruitful History The Czech Republic is rich with apples, pears, plums, and cherries and exports a lot of fruit. For centuries, fruit trees lined roads to feed traveling soldiers. This is a legacy left from the  Barak times, in the 18th century 2. Cider by Way of Calvados Dan journey to cider started while he was working at a French winery He has an affection for Calvados and thus made cider to distill. His process blends natural fermentation and dry yeast (He favors the yeast known as 72B). The apples come from old trees in his one-acre garden-orchard near Prague. 3. Small Scene, Big Potential Early cider makers in the Cech Republic began around 2007. Prague had a cider shop (InCider) and a cider bar, but today you can find cider at most bars. Cidrerie Kliment and Utopia are two standout producers—Utopia leans all natural. 4. Enthusiasts Leading the Way Many Czech homes still have fruit trees. Backyard fermenters use presses ranging from barrel-style to hydraulic. Gardening clubs and university programs help spread knowledge. 5. Beyond Apples Dan grows quince and medlars and has a Sorbus domestica tree in his orchard. Morovia which is to the north of Prague has a Sorubus Domestic museum. There's a growing appreciation for traditional and lesser-known fruits in fermentation. It's something we all encounter in emergency and prehospital care, probably more than anything else, yet it's a topic we've not given a full episode to… until now! Up to 70% of prehospital patients and 60–90% of ED attendees report pain, with half of all ED presentations having pain as the primary complaint. That's millions of patients across Europe every year and we're not always optimising our approach! In this episode, we're diving deep into acute pain management; from understanding the complex biopsychosocial definition of pain, right through to tailored pharmacological and non-pharmacological strategies, plus everything in between. We'll be looking at how we define and assess pain and the importance of validating patient experience. Then we'll work through management options: from paracetamol to ketamine, NSAIDs to regional anaesthesia, and talk through barriers like bias, opiophobia, and the persistent inequalities in analgesic delivery. We'll also shine a light on special groups;  from paediatrics to chronic pain patients and those with opioid use concerns, finishing with key takeaways on safe discharge planning. This one's about being better at recognising, respecting, and relieving pain. Because pain is an emergency, and we've got the tools to do something about it. Dr. Holly Everett's work helps us better understand roadside memorials and other public expressions of remembrance. Her 1998 master's thesis, “Crossroads: Roadside Accident Memorials in and around Austin, Texas,” examines 35 memorials, exploring how they're constructed and the cultural meanings they convey. What is the intent of a roadside memorial? Is it meant to be temporary or permanent? Are these markers legal? Who builds them, and what does that process look like? How do roadside memorials compare to spontaneous large-scale public commemorations of grief? Dr. Everett offers insight into these everyday and deeply meaningful spaces of mourning. After learning more about them, you'll never look at them the same.
