As far as we've come in the 50 years since Balfour Mount and Sue Britton opened the first palliative care at the Royal Victoria Hospital in Quebec, have we lost something along the way? In today's podcast we welcome some of the early pioneers in palliative care to talk about the roots of palliative care.  Sue Britton was the first nurse hired on that palliative care unit. Michael Kearney on a transformational meeting in Cicely Saunders's office, with Balfour Mount at her side and a glass of sherry.  Justin Sanders wants to be sure the newer generations of palliative care clinicians understand the early principles and problems that animated the founders of hospice and palliative care, including: Origins of the word “palliative” - it's not what I thought! Yes, it means “to cloak,” but there's more… Whole-person-care Total pain Healing as a process distinct from the deterioration of the body Sympomatologists The patient and family as the unit of care  Our guests referenced many articles on this podcast, linked above and below.  If you read just one, read Palliative Medicine - Just Another Specialty? by Kearney. I promise it's short. 2 pages.  Here's a taste:  …While there is an abusive and useless dimension to illness, pain and suffering which needs to be removed if at all possible, there is also potential in such experience…If we in palliative medicine fail to accept this view, a view which allows that there may also be a potential in the suffering of the dying process, if we sell out completely to the literalism of the medical model with its view that such suffering is only a problem, we will be in danger of following a pattern which could significantly limit our scope for development and lead to our becoming 'symptomatologists', within just another specialty.  And love that Jim Croce choice.  What's in a name? I've got a name.  Enjoy!  -Alex Smith   Links Link to the McGill National Grand Rounds Series on Palliative Care, Michael Kearney as initial presenter, and registration for future events. By Adam Turteltaub Document retention is one of those persistent issues that comes with a great deal of complexity. As Michael Kearney (LinkedIn), Head Solution Architect, Redgrave Data explains in this podcast, organizations have to deal with a dizzying array of rules. HIPAA has one set of requirements, state laws for medical records another, financial documents have a third, employment records a fourth and on and on it goes. In addition, there are business needs for retaining and disposing of records. So, what's a compliance team to do? He recommends working with the business unit and other affected teams to write policies that meet the needs of all involved and work out any conflicts internally or among the regulations. Work, too, with employees who may want to hold on to documents longer than policy dictates. You may find that what they want to keep is the data, not the document itself. And, if there is a litigation hold, be prepared to work quickly with legal, IT and others to ensure that the relevant documents are preserved while your ongoing document retention processes continue.

Michael Kearney started off this podcast defending the political durability of the IRA…and he had us convinced. If that doesn't have you hooked, I don't know what will! Building for deeptech founders is HARD…like folding a fitted sheet hard. And that's why we need someone like Mike to do it. Mike himself did his undergrad and Ph.D. at MIT, so he's knows the MIT innovation ecosystem like the back of his hand (The Engine is an MIT spin-out). In between, he spent his time drawing power from the Sun…well not literally…he worked on grid-scale renewable energy at Ambri in corporate development. Now, he's focused solely on supporting “Tough Tech” founders (side note, that's definitely a favorite term of this podcast now). Mike clearly loves doing really easy things…right?---

Scientific peer review is widely used as a way to distribute scarce resources in academic science, whether those are scarce research dollars or scarce journal pages. At the same time, peer review has several potential short-comings. One alternative is to empower individuals to make decisions about how to allocate scientific resources. Indeed, we do this with journal editors and grant makers, though generally in consultation with peer review. Under what conditions might we expect individuals empowered to exercise independent judgement to outperform peer review? This podcast is an audio read through of the (initial version of the) article "Can taste beat peer review?", originally published on New Things Under the Sun.Articles mentionedWagner, Caroline S., and Jeffrey Alexander. 2013. Jane Elith and Michael Kearney, University of Melbourne, Australia and Steven Phillips, AT&T, USA talk with Graziella Iossa about their work: 'The art of modelling range-shifting species'. Jane explains that this is a method to predict species distributions, whose range are shifting, like invasive species or species responding to climate change. Mike Kearney then specifies why they used cane toads as a case study for their work. By taking characteristics of the animal and putting this information together they could ask from a physiological point of view, where cane toads could not live. They also asked how to bring together this mechanistic approach with more traditional approaches. This work advances methodology by combining information from physiological models to data in the correlation-based ones; by looking at details of how you can do the modelling; and by looking at tools for understanding models and data, something that a lot of people will find interesting. Then Steven explains about MaxEnt, a programme that models species distributions based on a machine-learning approach, developed with other colleagues and freely available on the web. For example a common use of the programme is predicting how climate change will affect species distributions. Finally, Mike reports that this method should be useful to anybody trying to predict species with unequal distributions. Jane also precises that students, managers, researchers could be potentially interested, especially given that MaxEnt is freely available.
