Dra Samira Apostolos Pereira - Dra em Neurociência pelo departamento de Neurologia do HCFMUSP, média do corpo clinico do Hospital Sírio Libanes.Esclerose múltipla é causada por uma inflamação por células imunológicas (linfócitos) que atacam o sistema nervoso desde os nervos ópticos, cerebro e medula espinhal e dessa forma podem causar várias formas de comprometimento neurológico como déficit visual, cognitivo, disestesias, paresia nos membros, etc. Este ataque do sistema nervoso pelos linfócitos provoca formação de várias placas no sistema nervoso. Esta placa foi descrita como placa de esclerose ou seja, placas de cicatriz, como são várias placas, denominou-se doença de “esclerose múltipla”. Ressaltando que esses doentes não vão ficar “esclerosados”. A doença ocorre mais em mulheres, brancas, especialmente europeus. Os principais sintomas são perda de força nos membros, choques, dor no olho e turvação da visão, redução da velocidade do processamento da memória, transtornos psiquiátricos, urgência urinária, entre outros. São sintomas subjetivos que fazem com que o diagnóstico seja demorado. No Brasil em média leva-se 2 anos para fazer o diagnóstico, porém, há uma tendência de ser mais rápido. Por serem sintomas subjetivos o paciente passa por várias consultas em especialistas diferentes antes do diagnóstico, portanto, todos os especialistas devem ter noção dos sintomas da esclerose múltipla.O exame físico do neurologista é fundamental para o diagnóstico e perceber os sinais neurológicos que podem ser sutis. Na presença de sintomas e alterações do exame neurológico a Ressonância Magnética do neuroeixo onde são vistas alterações na substancia branca que podem estar localizadas em diversas partes do sistema nervoso. O liquor é importante para pesquisa de marcadores específicos que podem apontar e quantificar essa inflamação no sistema nervoso central. Em torno de 70% dos pacientes no Brasil apresentam a banda oligoclonal no liquor. Portanto a história, exame físico e exames complementares serão importantes para diagnóstico, estratificação e acompanhamento da doença.As placas podem ser localizadas da Ressonância sem sintomas do paciente, cuja o nome é síndrome radiológica isolada onde não há necessidade de tratamento, mas o segmento do paciente é muito importante.Em torno de 80% dos pacientes de esclerose múltipla remitente recorrente, ou seja, tem sintomas com remissão espontânea que pode ser parcial ou total. Alguns pacientes tem forma progressiva da doença sem remissão. O acompanhamento do paciente depende da clinica e da imagem e deve ser feito a cada 6 meses e comparação com os achados anteriores a fim de verificar a melhora com medicação.A primeira medida do tratamento é a medicina do estilo de vida, ou seja, ter a vida saudável com bons hábitos de vida, exercício físico e alimentação adequada. A atividade física diminui até 20% a taxa de surtos da doença. O manejo do stress também é muito importante. Importante ressaltar que depressão e transtorno bipolar é mais frequente nos pacientes com esclerose múltipla. A dieta ultraprocessada também piora os surtos da doença, assim como o tabagismo. Diabetes aumenta o risco de início da doença como a tia de surtos; dislipidemia aumenta o número de surtos. O sono inadequado pode estar associado como preditor da ocorrência de sintomas. A dieta mediterrânea pode diminuir a taxa de surtos. A obesidade é um fator de risco maior da doença. A fisioterapia para recuperação de déficits instalados é muito importante para recuperação funcional.A espiritualidade pode ter relação com diminuição do número de surtos, assim como gestão de fatores de stress. Pacientes podem ter gestação normal desde que esteja tratada corretamente. O planejamento da gestação deve ser feito com o neurologista para manuseio das medicações. Importante ressaltar que a esclerose múltipla não reduz a fertilidade.#esclerosemultipla
Devido à chuva nesta quinta-feira (19), a Paróquia Imaculado Coração de Maria cancelou a tradicional confecção dos tapetes de Corpus Christi. A decisão foi tomada por questões de segurança e preservação da tradição, que depende de tempo firme para a montagem dos tapetes nas ruas. Apesar da mudança, as celebrações religiosas ocorrerão normalmente, conforme o cronograma estabelecido, mas sem a tradicional procissão. Geoff Nau, Senior Manager of Product Management for Paragon at Altera Digital Health, highlights how digital health solutions can address the challenges of nurse burnout and retention. AI and machine learning can help automate documentation tasks and streamline workflows to allow nurses better access to patient information. Improving nurse retention requires attention to factors like time-off policies and leveraging technology to give nurses more time to focus on patient care. Geoff elaborates, "I have been a nurse since 2004. I practiced at the bedside for approximately 15 years. I did pediatric and adult ER as well as NICU. So I started my nursing career on paper and transitioned to electronic. And so in that time, a couple of factors, in the beginning, there was not enough information, and click-intensive. And looking at my travels and going to hospitals and sites today, that challenge is still there as far as access to data relevant to that patient visit, whether it's trending labs, care plans, etc. My experience with most EHRs out there, most of all, is that it's just click-intensive. A one-size-fits-all is tough to do nowadays." "There is still a considerable amount of paper out there. When you start getting into these specialized areas like behavioral health, IOP or intensive outpatient, a lot of that, especially if there's anything like a treatment plan, things that may have multiple people needing to touch it, I still see a lot of that on paper. My goal from a solutions perspective is to get as much of that electronic as possible, especially given where we're at with challenges with nursing and staffing. Today, your patient loads go from four to five to one nurse." "I'm a nurse. I also have a doctorate in organizational psychology. And this was a big study of my focus during my dissertation, where I looked at why people stay on the job as opposed to why they leave. When I first got into nursing, nurses would leave for a dollar. They would go up the street to another facility. But now, having support systems in place allows nurses to take time off, as you said. I know that here, where I live in North Carolina, a couple of institutions are taking a look at their PTO policies. They're looking at their holiday policies and trying to be more flexible. They're working with PRN staffing.
