During the comprehensive medication review, pharmacists determined the patient had inadvertently purchased an acetaminophen/diphenhydramine combination medication, rather than his usual acetaminophen. Use of acetaminophen/diphenhydramine for back pain without insomnia may contribute to falls. Pharmacists contacted the prescribing physician to obtain a prescription for acetaminophen. By asking the local pharmacy to dispense acetaminophen as a prescription, the risk to the patient of inadvertently purchasing an inappropriate product is reduced. #EvidenceBasedPodcast

Polypharmacy as a Clinical Pharmacist Specialist Practice Participants: Michael J Schuh Donna Bartlett, PharmD, BCGP, RPh The objective of this manuscript is to review an ambulatory care pharmacist service that evolved into a pharmacotherapy, polypharmacy service with multiple sub-models. The practice is located in a multi- specialty, tertiary care, destination medical clinic in Florida. Ambulatory care pharmacist services have evolved to demand expertise in multiple, specialized areas to address the more complex medical issues of the polypharmacy patient. Many of these patients are older than 65 years old with broad medical care needs. Growth and diversification of ambulatory care pharmacist services to evaluate the multitude of sophisticated medications, the continued direct-to-consumer commercialization and growth of dietary supplements, implementation of pharmacogenomic testing, in addition to new advances in clinical and laboratory technologies makes polypharmacy a viable pharmacist clinical specialty. Marijuana has had a turbulent history in the United States. Starting in the mid-1990s, however, there was a push to introduce the medical benefits of cannabis to the American people once again—”once again,” because before the 20th century, marijuana was almost entirely legal. Beginning in the 1910s, states began to ban the sale of marijuana, eventually leading to a bill called the Marijuana Tax Act in 1937, which decreased the amount of hemp (a version of a cannabis plant) allowed to be produced in the U.S. Later on in the century, the stigma around marijuana grew and Richard Nixon signed into law the Controlled Substances Act, which included marijuana on a list of federally banned drugs alongside heroin, cocaine, and other narcotics. By the time the 1990s came along, though, states began adopting the medical benefits of marijuana, starting with California. About 15 years after that, marijuana itself—not just the medicinal qualities—became legal for recreational use in Washington and Colorado. As of 2017, 29 of 50 states have legalized some form of medical marijuana. A 2013 study revealed that four of five doctors approve of medical marijuana and that more than 90 percent of medical marijuana patients (of around 7,500 surveyed) say that medical marijuana has helped treat their conditions. As these baby boomers age, they will develop more comorbidities and health issues, and their impact on the system and their consumption of medications and health care services will skyrocket, so, this is probably just the tip of the iceberg. Consider this: Today there are over 46.2 million adults aged 65 and older in the United States; by 2060, that number is expected to double to 98 million. Starting in 2011 and continuing until 2030, approximately 10,000 baby boomers will celebrate their 65th birthday each day. Seniors represent 14.5% of the United States population, about 1 in 7 Americans. Nearly 92% of older adults have at least one chronic condition, and 77% have at least two. Some type of disability (e.g. difficulty in hearing, vision, cognition, ambulation, self-care, or independent living) was reported by 36% of adults aged 65 and over in 2012. The coalition supports community efforts which, target medication education/management to low-income, immigrant communities. He is a graduate of the University of Connecticut (BS) and Ohio State (PharmD) Universities and completed a post-graduate residency in geriatric pharmacy at the Duke Center for the Study of Aging and Durham VA Medical Center. Dr. Jeffery is also a Clinical Professor at the University of Connecticut School of Pharmacy. His area of expertise is senior care pharmacy. Prior to joining Integrated Care Partners, Dr. Jeffery served as a consultant pharmacist for VA Connecticut's Geriatrics Consult service from 1998 – 2015. During this time Dr. Jeffery also established a PGY2 Geriatrics Pharmacy Residency Program and served as Director from 2002 – 2015. Dr. Jeffery has extensive association leadership experience including, serving as President and Chairman of the Board of Directors for the American Society of Consultant Pharmacists from 2012 – 2014 and Chairman of the American Association of Colleges of Pharmacy Geriatrics SIG in 2007 – 2008. He currently chairs the Polypharmacy Special Interest Group for the American Geriatrics Society and is a member of the Medication Therapy Management Technical Expert Panel (TEP) for the Center for Medicare and Medicaid Services. He co-founded the Coalition for Senior Medication Safety in New Haven, CT to raise awareness among seniors about medication safety. The coalition supports community efforts which, target medication education/management to low-income, immigrant communities. Starting in Asheville, North Carolina, a Value Based Insurance Design, Medication Therapy Management (MTM) has had a long track record of achieving positive health outcomes for health plan enrollees with chronic conditions as evidenced by the twenty studies which found pharmacists improved economic and clinical outcomes through direct patient care. 1 Because of this impressive history, Value Based Care (MTM) programs are included in the Medicare Part D Prescription Drug benefit. Over the past several years, Pharma-Care, Inc. has successfully implemented Value Based Care (MTM) services for many independent pharmacies and several municipalities, which, to date have yielded a 7.30:1 return on investment. They also contracted with the New Jersey Counties Division of Aging & Community Services, a division of the Department of Human Services (DHS), to provide Value Based Care (MTM) services to seniors living in their respective communities.
