On episode 109 of Astonishing Healthcare, host Justin Venneri explores pharmacogenomics (PGx) with a trio of experts: Caitlin Munro, PharmD (Clinical Partnerships Lead, Judi Health); Haleh Campbell, PharmD (Clinical Programs Administration Manager, Judi Health); and Houda Hachad, PharmD (Vice President of Clinical Operations, Aranscia). Together, they break down what PGx is, why now is the right moment for broader adoption, and how genetics can shape the way each of us processes and responds to medications. The conversation moves from the basics to the practical: where the evidence is strongest, which patients stand to benefit most, and what it takes to apply PGx in a way that's clinically responsible and genuinely useful for patients and providers. They also dig into findings from a study the Judi Health team recently presented at the Pharmacy Quality Alliance (PQA) annual meeting and explain why pharmacist support and clinical decision support, not just a lab report, make all the difference.Key TakeawaysPGx adds a piece to the puzzle; it's not a silver bullet. Pharmacogenomics looks at how inherited genetic differences affect the way a person processes or responds to medications. It narrows uncertainty and supports clinical judgment, but it doesn't replace it.Several forces are converging to make this the right moment. Testing costs have dropped, evidence-based guidelines from expert consortia are now widely adopted, and hundreds of FDA labels include pharmacogenomic information. The strongest use cases involve complexity and risk. Patients on multiple medications, those starting new therapies, and people who've had treatment failures or unexpected side effects often benefit most, especially across common drug classes like antidepressants, cardiovascular medications, and pain therapies.Human support drives engagement and trust. Rx Helix data showed that testing adoption was significantly higher among members who received clinician-led, pre-test telephone outreach. The future is proactive. With unified claims processing that combines pharmacy and medical data, leaders see a path toward PGx-enabled medication management, identifying patients before therapy begins to optimize treatment earlier in their journey.Related ContentHealth Benefits 101: The Importance of Clinical ProgramsAH073 - How Low Cost Alternative Programs Can & Should Work, with Jackie Lolos, PharmD, and Haleh Campbell, PharmDReplay – The Bridge to Better Healthcare: Uniting Medical and Pharmacy Services on One Platform to Achieve Value-Based CareAH060 - A New Approach to Colorectal Cancer (CRC) Screening, with GeneoscopyFor more information about this episode and its transcript, please visit Judi Health Insights.

Hospitals already have felt some of the effects of the Inflation Reduction Act on 340B savings, but with the IRA set to expand to more drugs in 2027, hospitals also are starting to project how it might affect their bottom lines next year. 340B Vice President of Pharmacy Services and Education Steven Miller joins us to explain how hospitals can be making those projections now.The IRA Will Expand to Another 15 DrugsNext year, an additional 15 drugs will be subject to Medicare price caps under Medicare Part D on top of the 10 drugs that saw caps this year. Steve says this will cut into 340B savings and overall margins even more — with some 340B discounts possibly dropping to their statutory minimums. These reductions also will translate to commercial and cash-pay dispenses, changing the overall financial outlook for hospitals.Hospitals Cannot Rely on Current 340B Savings Levels for 2027Steve says the 2027 changes are key for future budgeting. If hospitals do not adjust how they are budgeting for 340B drugs subject to Medicare price caps, they are likely to be short on their budget projections. He strongly recommends 340B teams have important conversations with finance teams now about how the IRA will affect their hospital or health system next year.Hospitals Can Be Planning NowFor the rest of 2026, Steve recommends hospitals monitor list pricing and 340B ceiling pricing regularly and to increase monitoring of purchases overall, given how drugmaker pricing behavior affects future 340B prices and savings. As the IRA continues to broaden over the next several years, including to Medicare Part B dispenses, he also recommends hospitals consider securing funding or support from other areas for any 340B-funded services that might see negative IRA impacts.Resources:Prepare Your Leadership for 340B Changes From 2027 Medicare Drug Price Caps

This podcast episode features an interview with Russell Phillips, Amie Pollack, and Dru Ricci from the Center for Primary Care at Harvard Medical School and authors of the Primary Care Investment Guide, with special guest, Todd Sorensen from the University of Minnesota College of Pharmacy. The Primary Care Investment Guide advocates for the integration of clinical pharmacists into primary care teams. The authors explain that their research used a mixed-methods approach, interviewing healthcare leaders from five states and conducting literature reviews, to identify sustainable strategies in primary care settings that enhance patient outcomes and reduce costs. Their findings highlight that clinical pharmacists provide a significant return on investment by managing chronic diseases, optimizing medications, improving medication access, and decreasing hospitalizations. The discussion emphasizes that successful integration can be facilitated by payment reform, moving away from fee-for-service models toward capitated payments and other alternative payment models that support optimal care delivery by primary care teams. Ultimately, the Guide provides a strategic framework for healthcare leaders and policymakers to justify and implement the provision of high-value services in primary care settings. Funding for the Primary Care Investment Guide was provided by The Commonwealth Fund and the California Health Care Foundation. Hospital clinical pharmacies play a key role in ensuring patients are taking medications effectively and supporting other providers who are managing their care. Mark Riggle, the assistant chief pharmacy officer at UC Davis Health, explains how 340B helps make this direct care and ancillary support possible.Clinical pharmacies are a bridge between providers and patientsRiggle says clinical pharmacy teams serve as drug experts that can help teach patients about potential side effects and how to take their medications properly. But these teams also can take the lead on supporting other providers on refills, prior authorizations, financial assistance, and more. That allows the other providers to focus less on those processes and more on providing medical care to the patients. In-house pharmacies can improve patient care and generate revenueUsing an in-house clinical pharmacy has benefits for both patients and the hospital. Riggle notes that keeping prescriptions and pharmacy support in-house can enable hospitals to keep better track of patients' health and identify if there are issues or questions with certain medications. At the same time, it generates revenue and 340B savings that support even more hospital services and improve patient health outcomes even more.Ramping up clinical pharmacy services and overcoming barriers can take timeRiggle says expanding clinical pharmacy initiatives at UC Davis Health has come with some challenges. It took years to roll out a refill program for all the providers who needed that support, and achieving success with a prior authorization program involved an evaluation of workflows to improve efficiency. But he noted that hospital pharmacists can present a value proposition to their leaders for how investing in 340B-supported clinical pharmacy services can be worth it to improve care quality and provider satisfaction.ResourcesFederal Judge Allows 340B Health, Member Hospitals To Intervene in Rebate Lawsuits

“Hey Cory, what do you do?” I am a comedian, pharmacist, author, and speaker. With the eye for detail of a pharmacist and the mind of a comedian, I am out to help others challenge the conventions and PREscriptions holding them back while adapting to the unknown and living in the moment. All while getting some laughs along the way. Rewind to 2013, two years into my pharmacy career and there I was, struggling to adapt to the unscripted world of pharmacy and healthcare. On paper I had checked all of the boxes: perfect grades, residency, board certifications. But then I realized something was missing - the ability to create meaningful connections in the moment with those I cared for, and the ability to adapt and remain flexible. 2013 was the year I went to my first improv class and two simple words changed everything for me: "Yes, and." _____________ The Pennsylvania Pharmacists Care Network (PPCN) is pleased to announce that Dr. Rob Maher has been selected to serve as its new President and Chief Operating Officer (COO).
