Most organizations put "Collaboration" on a wall. Few actually live it — and in healthcare, the cost of not living it isn't a missed deadline. It's a missed patient. In the first episode of Your Health University's brand-new Most organizations put "collaboration" on a wall. Few actually live it — and in healthcare, the cost of not living it isn't a missed deadline. It's a missed patient. In the first episode of Your Health University's brand-new Our Values Series, host Jamie Preston gathers four members of Your Health's patient experience team — Rebecca Dillard (VP of Organizational Experience), Jennifer Kessler (Division President of Product), Whitney Myers (Senior Solutions Advisor), and Carlos Hayward (Business Office Manager) — for an unfiltered conversation about what genuine collaboration looks like inside a fast-moving, mission-driven healthcare organization. No theory. No platitudes. Just the real, messy, mundane, and occasionally remarkable daily practice of people choosing to work together when it would be easier to go it alone. What you'll hear in this episode: Why real collaboration means recognizing what the person next to you brings that you simply cannot replicate — and building toward that, not around it The true story of a patient found living in an RV without his medication — and how cross-team collaboration made the difference between crisis and care Where collaboration most commonly breaks down in healthcare settings, and the small documentation and communication habits that prevent it The one question — "How can I do my job differently to make yours better?" — that builds trust across departments faster than almost anything else The daily habits these four healthcare professionals actually practice to keep collaboration alive, from weekly team check-ins to learning someone's preferred communication style before you assume Collaboration isn't a value you perform. It's a choice you make — one conversation, one phone call, one honest mistake admitted at a time. "You see this dissonance between the current you and the past you or other people, and the only gap across the bridge is action and implementation." There's a moment when you realize the life you're living and the life you actually want are no longer aligned, and no amount of thinking, planning, or hoping is going to close that gap for you. In this episode, Heather and Melissa Apa explore that tension as she shares what it looks like to navigate a child's diabetes diagnosis, the weight of ongoing crisis, and the secret decisions that change everything over time. This isn't a conversation about doing it perfectly or becoming a "better" person. It's about learning how to live, lead, and even thrive inside circumstances you didn't choose, without letting them define you. What to listen for: ✨ The power of managing a disease like diabetes without making it your identity ✨ Melissa's experience getting her child's diagnosis and navigating the initial stress ✨ The trap of thinking life will finally be manageable after the crisis you're walking through "I was like, my life is clearly just going to continue to be chaos, and I need to figure out how to still be able to get to joy and thriving and loving life because we only get one life, and this is the one I have, so I would like to enjoy it." ✨ The secret, silent decisions we make internally when we're finally ready to change ✨ How Melisa's multi-year experience of dealing with crisis made the journey more easeful ✨ Managing life with a chronic disease isn't a morality issue; it's just what's happening "You're not morally a good or bad person if you're struggling to control a chronic disease. You just don't have the right information, the right tools, or the right support. It doesn't mean that you're bad." ✨ Teaching kids how to advocate for themselves, especially with health issues ✨ Learning how to thrive with your disease rather than settling for just managing it ✨ Why action and implementation are required to experience behavioral change "The biggest thing that I see in the people who don't make changes is that they don't even see it as a possibility. It's never even occurred to them that it doesn't need to be this way. They have accepted that it will always be hard. That's not what I want you to accept. We need to accept some things, and we need to challenge some others here." ✨ How Heather helped Melissa recognize that chronic overwhelm was her comfort zone ✨ Being emotionally calm is the foundational work to build stability and systems from ✨ Why your leadership and how you manage your energy directly affect your family *** About Melissa Apa: Melissa Apa is a Clinical Pharmacist and Certified Diabetes Care and Education Specialist with over 15 years of experience—turned diabetes coach. She is a runner, and mom of two boys, including one living with type 1 diabetes. In this episode of The Disrupted Podcast, Jamie and Scott have a raw, specific, and deeply personal conversation about Care Managers: who they are, what they're actually supposed to do, and why getting this role wrong is costing patients their health and organizations millions of dollars. Scott opens with a story that hits hard: his 91-year-old father's recent hospitalization, the mistakes that nearly happened, and what it cost him — financially and emotionally — to navigate a system that wasn't built for the patient. What you'll hear in this episode: Why care managers are controllers, not schedulers — and what happens when organizations get that wrong The medication reconciliation crisis: how discharge errors are sending patients straight back to the ER How Your Health's new geographic mapping tool is transforming how care teams schedule 30 days of visits in advance The shared bonus model designed to stop care team members from fighting over visits — and start winning together What care managers should never be doing — and the analytical skill set that separates great ones from average ones If you're building care teams, leading a healthcare organization, or just trying to keep a loved one safe in a broken system, this episode will change how you think about the people standing between your patients and the hospital. www.YourHealth.Org

Joining us today is our good friend and colleague, Jim LaValle, a Clinical Pharmacist, Author, Board Certified Clinical Nutritionist, and Health Expert. Jim joins us to discuss a very exciting, NEWLY published study that shows that Kyolic Aged Garlic Extract supplements can significantly reduce periodontal pocket depth – a key indicator of gingivitis and periodontal disease progression. It's an alarming fact, but periodontal disease affects most adults in America.According to the National Institutes of Health, nearly HALF of American adults over the age of 30 suffer from some form of periodontal disease which is linked to tooth loss and systemic conditions like cardiovascular disease, type 2 diabetes, COPD, and other respiratory issues. In the latest study, which appeared in the February 2025 issue of Biomedical Reports, 300 otherwise healthy adults with moderate-to-severe periodontal disease were divided into four treatment groups—three groups who were assigned to take various doses of AGE and one control group. Over the next 18 months, researchers found that the participants supplementing with AGE experienced marked reductions in periodontal pocket depth - a key indicator of disease progression—compared to those taking the placebo. The study, which was conducted by scientists at the Hebrew University of Jerusalem, also demonstrated that a greater daily AGE intake led to better outcomes. Statistical modeling confirmed that improvements were most strongly associated with participants' baseline pocket depths, smoking status, and AGE dosage.Learn more about KyolicLearn more about Jim LaValleKyolic Aged Garlic Extract formulas are available at your local health food store and Online. For more information about Kyolic, or to find a retailer near you, visit Kyolic.com

Episode 29: “Perinatal Mood Disorders Don't Discriminate” with Jasmine @pearlsofjasmine This week, Rachael and guest Jasmine discuss a very important and often stigmatized topic, postpartum mental health and perinatal mood disorders. Dr. Jasmine is a clinical pharmacist who is incredibly vulnerable and open in talking about her own experiences with postpartum anxiety, depression, and insomnia after the birth of her daughter and how that experience led to her decision to be “one and done”. Jasmine shares her expert knowledge as a pharmacist and discusses the stigma of medications, the pros and cons of the "new" postpartum depression medication, and discusses her own decision to start medication after the birth of her daughter. Find out some common misconceptions or stereotypes about perinatal mental health or treatments, how being an Asian-American woman influenced her during her postpartum experience, and how culture and representation is so incredibly important for women on this journey to not feel so alone. Jasmine is a clinical pharmacist and mom of one. She became very passionate about maternal mental health after she suffered severe postpartum depression, insomnia and anxiety shortly after having her child. As a healthcare provider, she never thought she would be affected by PPD because she assumed she'd recognize the signs and know how to ward it off. Writing about her journey on her blog and then sharing some of these moments on Instagram have been a way for her to heal and try to normalize mental health for other moms. Just a few months ago, she completed her perinatal mental health certification (PMH-C) to continue to educate herself further on perinatal mood and anxiety disorders.
