C dans l'air du 18 décembre - Mutuelles, consultations : la santé hors de prix ?Nouvelle augmentation des tarifs des complémentaires santé en vue, au 1er janvier 2025. C'est ce qu'a indiqué la Mutualité Française dans son enquête annuelle qui porte sur 41 complémentaires santé. En moyenne, ils augmenteront de 6 %, soit beaucoup plus que l'inflation. Dans le détail, les contrats individuels, souscrits notamment par les retraités, augmenteront en moyenne de 5,3 % et les contrats collectifs obligatoires couvrant les salariés par l'intermédiaire de leur entreprise de 7,3 %. Une mauvaise nouvelle difficile à entendre pour nombre d'assurés. D'autant que c'est la cinquième hausse consécutive après une augmentation record de 8,1 % en 2024, de 4,7 % en 2023, de 3,4 % en 2022 et de 2, 6 % en 2021. Sur les dix années antérieures, la hausse était de 2,6 % en moyenne chaque année. Alors pourquoi une telle hausse ? "L'augmentation des cotisations des mutuelles en 2025 est inévitable" en raison de plusieurs facteurs, dont la hausse "structurelle" des dépenses de santé en France, soit + 5,2 % en 2023, affirme la Mutualité Française. "Vieillissement de la population, accès à de nouveaux traitements et technologies médicales, et meilleure reconnaissance des métiers de santé expliquent ces évolutions tendancielles à la hausse", déclare-t-elle. Après de très longues négociations ces derniers mois, l'Assurance maladie et les médecins libéraux ont validé la hausse de la consultation médicale. Le tarif de la consultation chez le médecin généraliste va augmenter le 22 décembre prochain tout comme celles des spécialistes (pédiatres, gynécologues, psychiatres…). Les complémentaires santé ont anticipé cette revalorisation mais aussi d'éventuelles hausses de charges, longuement discutées à l'automne pour le projet de loi de financement de la sécurité sociale (PLFSS). En effet, il était convenu avant le vote de la motion de censure que l'Assurance maladie abaisse son seuil de remboursement de 5 % sur les consultations médicales et 5 % sur les médicaments. Les complémentaires ont alors estimé qu'elles devraient supporter autour de 900 millions d'euros supplémentaires sur l'année et ont anticipé ces charges. Pour l'heure, l'actuel Premier ministre François Bayrou ne s'est pas exprimé sur le sujet, vivement critiqué à gauche et à l'extrême droite.En parallèle, la Direction de la recherche, des études, de l'évaluation et des statistiques (Drees) pointe des dépenses d'optique et dentaires qui ont augmenté respectivement de 6,2 % et 5,5 % en 2023. Ces dernières étant désormais majoritairement prises en charge par les complémentaires santé. Selon la Drees, le résultat en santé des mutuelles est déficitaire à 0,4 % en 2023, une première depuis 2011. En revanche, sur la totalité de leurs activités, les complémentaires santé "ont été financièrement solides : leurs montants de fonds propres ont été très supérieurs aux minima réglementaires", évalue la Drees.Face à l'accroissement des dépenses de santé qui s'annonce avec le vieillissement de la population française, les mutuelles préconisent de mieux lutter contre la sur-prescription et la redondance d'actes et examens médicaux ou encore contre la fraude sociale. Elles insistent également sur un investissement plus important dans la prévention.Mais avec la hausse de tarifs, une autre question refait surface : la gestion de ces complémentaires santé. Un rapport sénatorial remis en septembre dernier épinglait les hausses de tarifs pour 2024. Les sénateurs qui ont procédé à leur propre évaluation du niveau de hausse conforme à l'augmentation des dépenses de santé et des nouvelles dispositions à assumer ont abouti à une fourchette allant de 4,5 à 6,5 %. Un niveau d'augmentation "sans commune mesure avec celui annoncé avec les mutuelles", ont-ils écrit, précisant qu'il "conviendra de rester attentif aux bilans financiers des complémentaires santé sur les années à venir, afin de vérifier le caractère strictement proportionné des augmentations de cotisations aux hausses de prestations". Les sénateurs ont également émis la piste d'une "anticipation imparfaite" des nouvelles mesures et recommandé de laisser au moins six mois entre la publication et l'entrée en vigueur des textes susceptibles de modifier la répartition des niveaux de prise en charge entre l'Assurance maladie et les complémentaires.Autre sujet récurrent dans les critiques adressées aux complémentaires, les frais de gestion des organismes. Le rapport sénatorial souligne que ces derniers ont augmenté à un rythme deux fois supérieur à l'inflation entre 2011 et 2022 (+ 33 %) avec de fortes disparités selon les organismes. En janvier dernier, l'UFC-Que Choisir avait aussi alerté les consommateurs sur des frais de gestion trop mal maîtrisés, compris entre 10 % et 28 % selon les complémentaires.Alors pourquoi les tarifs des complémentaires santé augmentent-ils encore ? La fraude sociale représente-t-elle un réel levier pour remplir les caisses de l'Etat ? Que représente-t-elle en réalité ? Qui se cache derrière la fraude ? Une étude collaborative menée par l'Inserm, la DREES et Santé publique France dresse un bilan de la mortalité en France pour l'année 2022. Cette année-là, le pays a enregistré 673 190 décès, avec une répartition presque égale entre les femmes (50,2 %) et les hommes (49,8 %). Bien que l'impact de la Covid-19 ait considérablement diminué par rapport aux deux années précédentes, 2022 a néanmoins connu un excès de 54 000 décès toutes causes confondues par rapport aux prévisions établies en l'absence d'épidémie.L'étude met en lumière les cinq principales causes de mortalité en France :1. Les tumeurs demeurent la première cause de décès depuis 2005, représentant 25,5 % des décès en 2022, soit 171 630 cas. Les hommes sont plus touchés que les femmes, avec les cancers du poumon, du côlon-rectum, du sein et du pancréas étant les plus meurtriers.2. Les maladies cardio-neurovasculaires occupent la deuxième place, responsables de 20,8 % des décès. Ces pathologies, incluant l'infarctus du myocarde, l'AVC et l'insuffisance cardiaque, affectent principalement les personnes âgées et constituent la cause de décès la plus fréquente chez les 85 ans et plus. Entre 2021 et 2022, une légère augmentation de la mortalité due à ces maladies a été observée chez les femmes, tandis qu'elle est restée stable chez les hommes.3. Les maladies de l'appareil respiratoire (hors Covid-19) ont connu une forte progression en 2022, représentant 6,7 % des décès et devenant ainsi la troisième cause de mortalité. Cette hausse pourrait s'expliquer en partie par les épidémies de grippe saisonnière et la circulation active d'autres virus respiratoires. Ces décès concernent principalement des personnes âgées, la moitié touchant des patients de 86 ans ou plus.4. Les causes externes, comprenant les accidents, suicides et homicides, ont été responsables de 44 800 décès, soit 6,7 % du total. La mortalité due aux accidents a augmenté dans toutes les classes d'âge, notamment chez les plus de 85 ans. Les causes externes représentent la deuxième cause de mortalité chez les personnes de moins de 65 ans, après les tumeurs.5. La Covid-19, bien qu'en recul, reste la cinquième cause de décès en France en 2022. Le nombre de décès dus à cette maladie a diminué d'environ un tiers par rapport à 2021, grâce à une immunité collective élevée, une large couverture vaccinale et la moindre virulence des variants.Cette étude souligne l'importance continue des maladies chroniques et des facteurs environnementaux dans la mortalité en France, tout en mettant en évidence l'impact persistant, bien que réduit, de la pandémie de Covid-19 sur la santé publique.
