Kommentarsfältens hatare, som befolkar sina liv med hårda ord, vet nog vad jag pratar om. I stunden fylls ett tomrum, det värmer lite, av en föreställd gemenskap ur föreställd upprättelse. Men snart förlorar hatet sin kraft. Allt måste börja om på nytt, tills ensamheten upphör. Det brukar sägas att historien inte upprepar sig men rimmar. Fjodor Dostojevskij är den författare som nog bäst har fångat människans likartade klangljud genom tiderna. I en essä från 1877, i den egenutgivna tidskriften Författarens dagbok, diskuterar han ingående ämnet anonyma hatiska brev. Han hade själv fått flera och gjorde då det som en Dostojevskijtyp alltid gör, tog sig en närmare titt på de sociala och psykologiska faktorerna bakom dem. Slutsatsen ekar i vår tid. Samhällen som snabbt förändras och där många lämnats med en känsla av att ha åsidosatts, rent av övergivits, kan förklara den frustration som gör att någon låter sin penna blöda elaka brev. Sin vana trogen såg han förlåtande på individen. Och till det kan ju läggas att det kunde ha varit betydligt värre än brev. För att anonymt ljuga, baktala, sprida rykten och hata är, skriver han, en mildare form av den instinkt som också driver mordbrännaren att tända på. Utloppet måste ta vägen någonstans och visst är det väl bättre med anonyma ord än byggnader som sätts i brand.Det kan vara svårt att höra historiens många rim om ingen läser dem högt för oss. I Emily Cockaynes bok ”Penning Poison: A history of anonymous letters” hörs de tydligt. Hon ger flera intressanta perspektiv på illasinnat brevskrivande från 1760 till 1939. Men linjerna i boken dras också in i vår tid då det anonyma hatet till och med gjorts till en industri med trollfabriker som systematiskt utsätter utvalda personer för hat. Frimärkets anonymitet, liksom långt senare internet, ledde enligt henne till en explosion av hat utan spårbara avsändare. Postväsendets infrastruktur födde monstret som under 1800-talet växte sig allt större och starkare och skrämmande. Många bröllopsfoton har i vredesmod slitits mitt itu efter otrohetsrykten delgivits anonymt. Upplysningar ur det förflutna som raserat liv. Anonymt förmedlad information om förbjudna drifter och åsikter har ofta fått samma effekt. Ibland var det sant, allt det där som har hemligskrivits genom åren, ibland falskt, men oavsett är det alltid maktutövning. Och det är enkelt att föreställa sig mottagarens skräck, från en nyfiken undran om vad det kan vara för brev som kom med posten nyss, till det som rad för rad av anklagelser och hot om avslöjanden övergår i rädsla. Den makt som meddelandena påtvingar offren har genom historien antagit många former – lappar, brev och nu också epost, exempelvis. Avsändarnas syften har förstås också skiftat, men några saker är alltid gemensamma. Anonymiteten ger en ojämlik, asymmetrisk, relation. Avsändaren säger sig veta något om någon som i sin tur försvarslös famlar i mörkret. Det är ett grymt maktspel där avsändaren tvingar ner mottagaren i en underordnad position, ofta för att få utlopp för upplevd förbiseddhet och psykologisk frustration.Men så finns även de andra mer kallt kalkylerande utövarna av anonymitetens makt, fabrikörerna om man så vill. Fjodor Dostojevskij vägleder oss även på den punkten ända in i vår tid. Han intresserade sig, som alla hans läsare vet, för människans olika sidor och visar genom hennes skuggsida på själens samt integritetens värde. Därför finns det gott om uslingar i hans verk. Pjotr Stepánovitj Verchovjénskij är en av de värsta. I Onda andar från 1872 tar han som ledare för en nihilistisk revolutionär grupp sig friheten att ur det fördolda härska över andra. Det demoniskt själlösa i honom framträder tydligt. Smicker varvat med hot är han särskilt bra på, allt för att sälja in nödvändigheten av illgärningarna som gruppen för den goda sakens skull gör sig skyldig till. Genom exempelvis anonyma brev sår han split, väcker tvivel och hotar med skamliga avslöjanden. Handlingsplanen syftar till att förändra samhället men leder till mordbrand, självmord och politiska mord i den lilla staden, allt dirigerat av rädslan som onda andar vet att frammana med hot om att privat ska bli offentligt. En fiktion bara, visst, men kvalitetslitteratur speglar ofta livet på ett mer greppbart sätt än om vi enbart lever det. Pjotr Stepánovitjs födelse ligger visserligen mer än 150 år tillbaka i litteraturens historia. Ändå passar han utmärkt nu också, hade snabbt funnit sig till rätta här hos oss. Man kan se honom framför sig då han mejlar uppdrags- och arbetsgivare, om upplysningar …ska ni verkligen låta honom göra sin röst hörd…ni vet väl att…sagt i all välmening förstås…för tiden kräver ju det…så inte er fina organisation också ska… Ja, han hade stortrivts här, nu när i stort sett vilket ändamål som helst tycks kunna helga vilka medel som helst. Helt utan skam, i övertygat lugn, hade han vandrat rakt över sina cancellerade lik. Det är så att säga hans grej. Som sagt, fiktion, visst. Jag önskar bara han vore osann också. Men han är ett förebådande, den långa skugga som drog genom de anonyma anklagelsernas Sovjetunion, Nazityskland och Östtyskland, platserna där integriteten dog, för att senare dra vidare in i vårt sociala medielandskap där uthängningar och utfrysning tycks bli allt vanligare. Vi är alltför många som har känt honom dra förbi, men trots det är svaret inte att begränsa möjligheten till anonymitet, tvinga alla troll ut i ljuset. Då sträcker sig bara fler efter tändstickor. Dessutom skulle det tysta även rättmätig kritik som bara kan sägas utan avsändare. Svaret är ett samhälle som vägrar vara anonymitetens tysta förlängda arm, ett samhälle som med sakligt förnuft i varje stund förhåller sig källkritiskt och aldrig glömmer det rättviseideal som säger att ingen någonsin ska dömas ohörd. OutlineChapter 14- Hypovolemic States- Etiology - True volume depletion occurs when fluid is lost from from the extracellular fluid at a rate exceeding intake - Can come the GI tract - Lungs - Urine - Sequestration in the body in a “third space” that is not in equilibrium with the extracellular fluid. - When losses occur two responses ameliorate them - Our intake of Na and fluid is way above basal needs - This is not the case with anorexia or vomiting - The kidney responds by minimizing further urinary losses - This adaptive response is why diuretics do not cause progressive volume depletion - Initial volume loss stimulates RAAS, and possibly other compensatory mechanisms, resulting increased proximal and collecting tubule Na reabsorption. - This balances the diuretic effect resulting in a new steady state in 1-2weeks - New steady state means Na in = Na out - GI Losses - Stomach, pancreas, GB, and intestines secretes 3-6 liters a day. - Almost all is reabsorbed with only loss of 100-200 ml in stool a day - Volume depletion can result from surgical drainage or failure of reabsorption - Acid base disturbances with GI losses - Stomach losses cause metabolic alkalosis - Intestinal, pancreatic and biliary secretions are alkalotic so losing them causes metabolic acidosis - Fistulas, laxative abuse, diarrhea, ostomies, tube drainage - High content of potassium so associated with hypokalemia - [This is a mistake for stomach losses] - Bleeding from the GI tract can also cause volume depletion - No electrolyte disorders from this unless lactic acidosis - Renal losses - 130-180 liters filtered every day - 98-99% reabsorbed - Urine output of 1-2 liters - A small 1-2% decrease in reabsorption can lead to 2-4 liter increase in Na and Water excretion - 4 liters of urine output is the goal of therapeutic diuresis which means a reduction of fluid reabsorption of only 2% - Diuretics - Osmotic diuretics - Severe hyperglycemia can contribute to a fluid deficit of 8-10 Iiters - CKD with GFR < 25 are poor Na conservers - Obligate sodium losses of 10 to 40 mEq/day - Normal people can reduce obligate Na losses down to 5 mEq/day - Usually not a problem because most people eat way more than 10-40 mEq of Na a day. - Salt wasting nephropathies - Water losses of 2 liters a day - 100 mEq of Na a day - Tubular and interstitial diseases - Medullary cystic kidney - Mechanism - Increased urea can be an osmotic diuretic - Damage to tubular epithelium can make it aldo resistant - Inability to shut off natriuretic hormone (ANP?) - The decreased nephro number means they need to be able to decrease sodium reabsorption per nephron. This may not be able to be shut down acutely. - Experiment, salt wasters can stay in balance if sodium intake is slowly decreased. (Think weeks) - Talks about post obstruction diuresis - Says it is usually appropriate rather than inappropriate physiology. - Usually catch up solute and water clearance after releasing obstruction - Recommends 50-75/hr of half normal saline - Talks briefly about DI - Skin and respiratory losses - 700-1000 ml of water lost daily by evaporation, insensible losses (not sweat) - Can rise to 1-2 liters per hour in dry hot climate - 30-50 mEq/L Na - Thirst is primary compensation for this - Sweat sodium losses can result in hypovolemia - Burns and exudative skin losses changes the nature of fluid losses resulting in fluid losses more similar to plasma with a variable amount of protein - Bronchorrhea - Sequestration into a third space - Volume Deficiency produced by the loss of interstitial and intravascular fluid into a third space that is not in equilibrium with the extracellular fluid. - Hip fracture 1500-2000 into tissues adjacent to fxr - Intestinal obstruction, severe pancreatitis, crush injury, bleeding, peritonitis, obstruction of a major venous system - Difference between 3rd space and cirrhosis ascities - Rate of accumulation, if the rate is slow enough there is time for renal sodium and water compensation to maintain balance. - So cirrhotics get edema from salt retension and do not act as hypovolemia - Hemodynamic response to volume depletion - Initial volume deficit reduced venous return to heart - Detected by cardiopulmonary receptors in atria and pulmonary veins leading to sympathetic vasoconstriction in skin and skeletal muscle. - More marked depletion will result in decreased cardiac output and decrease in BP - This drop in BP is now detected by carotid and aortic arch baroreceptors resulting in splanchnic and renal circulation vasoconstriction - This maintains cardiac and cerebral circulation - Returns BP toward normal - Increase in BP due to increased venous return - Increased cardiac contractility and heart rate - Increased vascular resistance - Sympathetic tone - Renin leading to Ang2 - These can compensate for 500 ml of blood loss (10%) - Unless there is autonomic dysfunction - With 16-25% loss this will not compensate for BP when patient upright - Postural dizziness - Symptoms - Three sets of symptoms can occur in hypovolemic patients - Those related to the manner in which the fluid loss occurs - Vomiting - Diarrhea - Polyuria - Those due to volume depletion - Those due to the electrode and acid base disorders that can accompany volume depletion - The symptoms of volume depletion are primarily related to the decrease in tissue perfusion - Early symptoms - Lassitude - Fatiguability - Thirst - Muscle cramps - Postural dizziness - As it gets more severe - Abdominal pain - Chest pain - Lethargy - Confusion - Symptomatic hypovolemia is most common with isosmotic Na and water depletion - In contrast pure water loss, causes hypernatremia, which results in movement of water from the intracellular compartment to the extracellular compartment, so that 2/3s of volume loss comes from the intracellular compartment, which minimizes the decrease in perfusion - Electrolyte disorders and symptoms - Muscle weakness from hypokalemia - Polyuria/poly dips is from hyperglycemia and hypokalemia - Lethargy, confusion, Seizures, coma from hyponatremia, hypernatremia, hyperglycemia - Extreme salt craving is unique to adrenal insufficiency - Eating salt off hands ref 18 - Evaluation of the hypovolemic patient - Know that if the losses are insensible then the sodium should rise - Volume depletion refers to extracellular volume depletion of any cause, while dehydration refers to the presence of hypernatremia due to pure water loss. What is the legacy of the Enlightenment in political struggles today and how are socialists and Marxists to relate to the Enlightenment? Must we rely on first principles and an a priori theory of knowledge in our understanding of capitalism and exploitation? Or must we proceed on the basis of an appeal to empiricism and experience primarily in our understanding of social struggles? What is the role of philosophy in our political practice? How do we know that the political causes we champion are just or right? We welcome Marxist thinkers Landon Frim and Max Tomba for a debate on Marxism and the Radical Enlightenment to help us get at the heart of these questions, and much more! An Alternative Legacy of Modernity, with Oxford University Press, published in 2019, which was the co-winner of the 2021 David and Elaine Spitz Prize for the best book in liberal and/or democratic theory published in 2019.  Landon Frim is Associate Professor in Philosophy at Florida Gulf Coast University and he is a specialist in Spinoza, enlightenment rationalism and he has written in popular outlets including Jacobin Magazine, The New Republic, Salvage Magazine, and Inside Higher Ed. With Harrison Fluss, Landon wrote Prometheus and Gaia: Technology, Ecology and Anti-Humanism which is an examination of the ideological positions of Futurism and Eco-Pessimism. "When I got started, there wasn't much diversity at all." The young DJ discusses Australia's QTBIPOC community, debuting at Dekmantel and more live from Pitch Festival. Australian talent Charlotte Frimpong, the DJ better known as C.FRIM, has become one of the country's most in-demand DJs over the last year. The Ghanian-Filipina artist kickstarted her career with a Boiler Room set at Sugar Mountain in December 2022, and since then, she's toured around Europe (and debuted at Dekmantel), becoming known for digging deep into her musical ancestry and weaving amapiano and Afro-electronic sounds creatively through her sets. In this interview recorded live at Pitch Music and Arts Festival, Frimpong talks to journalist Tanya Akinola about "air DJing" in her bedroom before being able to afford gear, and how her career has skyrocketed at such a rapid rate. Even though she hasn't been a name on the stage for a long time, she's managed to carve out a unique space for the QTBIPOC community with the party she co-runs, Dutty, which she says is for people who can relate to the afro-diaspora or who just want to open their minds to it.
