National Notebook Day is celebrated on the third Thursday of May, and this year it falls on May 15th. It's a holiday that encourages people to record their thoughts, events and information in a notebook. It's especially important to have a notebook or journal if you're the type of person whose mind goes to so many places at once. One of the many reasons why keeping track of varieties is beneficial is that it helps you focus the mind. People will often revisit their journal entries when they need inspiration or ideas. And I just thought, focus the mind, we, I feel like it's a Kevin Carton thing, but also, because I can't choose one, I have ADHD, and I like a lot of things. And so also, Today is National Senior Fraud Awareness Day. So go check that out. Essentially, it means what it means. Make sure you're seeing your people in your life. And then also, it's Tuberculosis Day, TB Day. There's actually a couple different holidays to have to do with this. Brad Crowell 8:27 Right, so Fit Prom Karen is asking on YouTube, hey, what's the difference between the black nylon and leather straps? How do they hold up or move for circular movement? And she was talking about on the Reformer. Lesley Logan 8:40 Yeah, so, okay, there's so many different types of Reformers out there, and there are Reformers that use, like, ropes and pulleys, and they usually have loops, and those loops can be cloth, and then, because of COVID, they've turned into this, like, vinyl type of a thing, which is not super comfortable. It's a little thick, but it's easy to wipe down. And then there's also the leather straps that also on wheels, and they also have handles, and then they have extension straps to make the loops. So there's a huge difference. And as far as the circular movement, I actually think that if you work with the leather straps and handles. Brad Crowell 9:11 Sorry, where are these straps? I'm trying to envision this because there's, like, multiple straps on a Reformer. We're not talking about the long straps, are we?Lesley Logan 9:18 We're talking about the long straps. Brad Crowell 9:19 The long straps, okay, so we're not talking about the things that hang over the tail.Lesley Logan 9:22 You know, when we're on a tour, yeah, not the one, well, those are extension straps, but yes. Brad Crowell 9:22 Extension straps, okay, so that's the difference.Lesley Logan 9:22 You know when we're on tour and some of them I have to push the risers down because they use ropes. Okay, so the particular device that it's on will turn around so when they make circles, the ropes stay in this little groove thing, and it circles like it turns with it. And the leather straps don't have that. They're just on a wheel, but you can still circle. You can still circle. What I would say is actually a lot, a lot of people that I work with who use the ropes. I highly encourage you to order the neoprene handles. They usually are on your Cadillac, or you can order them on their own and clip it on to your straps, because you are going to get even more out of those circles every single exercise if you have a handle in your hand, because the way your hand reacts to the handle will tell me if your arms are connected to your back. So I can look at a room of people. Brad Crowell 9:49 Which does not happen if it's just a loop. Lesley Logan 10:18 Well, it can, but you have to, you have to be aware, and it's really hard because you don't have anything visually to show you that your hand is turned at an angle. So if you're watching on YouTube, it's really easy for your hand to be at a 45 degree angle, versus perfectly holding a bowl of soup or perfectly ready to shake someone's hand. Brad Crowell 10:38 Is that what we're doing, we're holding soup? Lesley Logan 10:39 Usually I tell people to hold a margarita. So at any rate, the pinky side of your hand needs to be attached to the handle at all times. And when you're using those loops, it's kind of touching it. So you think it's working, but then your hand is actually not the right angle to help you to connect your muscles from your back. And a lot of people end up using the top of their shoulders or the front of their shoulders, or their biceps and their elbows, and they're not using their back. And then the other thing that those loops can encourage without you noticing it is really easy for your wrist to bend, and so then you're holding this loop with your bent, wrist is bent, and now you're not getting any of the benefits. So loops are great for the feet. There's nothing wrong with it, and I actually am not knocking anything like, we have affiliate links to Balanced Body and to Contrology, so if you prefer the ropes, you can have them. I just highly encourage you to get handles and use handles in your hands and loops on your feet. Lesley Logan 12:46 You know, I love when people make sure alliteration happened. It just all flows together. So many things to love about what he said. But I really enjoyed that he talked about he believes that our soul's purpose is really what our heart calls for the most to live the most alive life that we possibly can. And he, also about focusing on becoming more alive, more aware, more happy, more loving. He also said that if you're struggling to identify your purpose, it's because you have to get curious. And it made me just think, you know, when you're a kid, you're like, asking all these questions. You're like, so curious. Why is the sky blue? Why is this? Why is it? And because parents are fucking exhausted and overworked and underpaid, and they are stressed out to the ninth, especially the parents today, because why wouldn't you be, it's really easy to kind of like, crush that curiosity and. Brad Crowell 13:31 I mean even, not, also inadvertently, it could just be like, here's an iPad I can't deal with you right now.Lesley Logan 13:36 It's all, I actually would say it's all inadvertently. But and then at school, the curiosity is not what actually is you're graded on. It's like, on how well you memorized. And so I think that, like, you start as a kid as very curious, and then you go through life and you're just forced to, like, memorize what's right and what's wrong. And so you don't actually cultivate a curious brain. I actually was really grateful, because Pilates made me get really curious. Because, well, not not just Pilates in general, I will say very specifically, Pilates with Vintage, with Jay, with Sandy and Karen. Like, there was not a one answer to a question. It was like, well, I don't know, how's it feel? And they would want you to get curious, like they. Brad Crowell 13:43 You have to dig in and connect with yourself to actually draw a conclusion. Lesley Logan 14:15 Was really hard for me. I would just go, just tell me what it is, right. I just want to get it right. So I think that's why I can really attract this perfectionist, but also why I love what we do. But I just thought it was really great, like, be more curious, you know. And then he also said, we're given dreams, and it's up to us to accept and become aware of them like we're given them. And that made me just think of Big Magic and how, like, ideas want to be born and things want to happen. And if we're not there to, like, pick what's being left for us, they're gonna go. But if we can be aware of them, we get to actually enjoy them, and we get to have this gift that we've been given. What did you love? Brad Crowell 14:49 He talks about the four levels of consciousness. This is effectively applied to how each of us lives, our lives, how we experience our lives. So there's four levels of consciousness, and I'm gonna briefly run through with them all. So level one is victimhood, two is responsibility, three is called flow, and then four is oneness. Okay? Now, what does that actually mean? Level one, victimhood, life is happening to me, at me, you know, against my wishes. Why do these things happen to me? I'm blaming everybody except for myself, right? It's considered the lowest state of consciousness, and it's actually it diminishes your life. So sometimes what we default to, and the thing is, it's easy to stay in the victimhood. Lesley Logan 15:33 I think we all get annoyed by those people. Like, once you're not that person, you're like, I gotta get away. Brad Crowell 15:37 Yeah, so level two responsibility, life is happening by me. You're taking responsibility for your actions, for your thoughts, for your responses. You're recognizing your own ability to create desired outcomes. And this is like, probably, I think, most people live life more often than not, in levels two and three, kind of transitioning between two and three, and then four. We tap into four occasionally, but then we shift around like that was a good question that you had for him. But life is happening by me is level two, responsibility on making decisions, on purpose. Level three. Life is happening through me. This is the flow state, a sense of connection to a larger life force, letting go of excessive control, experiencing a natural flow of things, where things work out more easily. You know, I think we've all experienced the flow state, right? We're in the zone, as like to call it. I know when I was working in the office, my entire goal, like when I worked for somebody else, my entire goal was once a day, to forget what time it was, because I was so in the zone, right? Put my headphones on, drown out the rest of the world. Get focused and allow myself to be in that state. And honestly, when that happens, I would be happy about it. At the end of it I'd be like, yes, I got so much work done today. When I didn't get into the flow state, I will be frustrated about my day. And be like, oh, I feel like I didn't do anything today, right? So life is happening through me, and then finally, oneness, life is happening as me. This is definitely more of like a mystical experience, no separation between yourself and the entirety of the universe. And you know, you can experience this through meditation, connection with others, with the nation, but also, actually, he also suggested that oneness, or life is happening as me, the most common experience of this is an orgasm where you're experiencing it with somebody else. I thought it was interesting to think of that that way. And it's not something that you're in that state for the longest periods of time, but you can tap into that occasionally here and there. I just found these really interesting. Lesley Logan 17:44 These are the four things that he dug into more deeply in his talk, right? Brad Crowell 17:48 Yeah, he, we also went through these levels of consciousness and, in fact, also his tool at the end of the episode, where he's like, hey, this is my gift to you. I subscribed to the pod because I was like, I think I want to keep listening to this person. I really like what they're saying. And I think also I got a lot of those stages, and it's like, oh, that last one, I kind of just thought that's what the shamans do in the white, you know, on the top of the hill, and they leave the world behind. But I liked he's, like, no this is like, you could just tap into it. You could just be, like, in this moment, we could be part of the world again. So I think that's really cool. What bold, executable, intrinsic or targeted action items can we take away from your conversation with Kevin Carton? He talked about the power of a clear vision. He defines a vision as a written out version of the life you love to live, a written out version of the life you love to live. And I thought was really cool, because we have this idea for ourselves, but how often do we actually put it down on paper so we can see it, read it, hold it. We won't forget it, because we wrote it down. He said be detailed, be clear, be specific. Use emotion and just be big. And he said the main thing that people and he and I had a really interesting conversation about this, one of the things that I used to do, and I still catch myself doing occasionally, is saying to myself, I want this thing for me, whatever this thing might be. And then I would say, it would be really nice if this thing happened for me or to me, ironically, it's like victimhood level one. It would be really nice if this thing happened for me, right? That to me is a cop out. Cop out in two ways. One, I might not actually be actively pursuing this thing that I want to happen for me. And two, if the thing doesn't happen, then it's okay. It didn't happen to me, for me, right? This is really interesting, because I used to intentionally not get my hopes up, right? Because if it never happened for me, then I'd be really disappointed, or it'd be failing, or I'd be let down. And I just found, like what he said, when you're writing out your vision, the mistake that most people make is taking that approach. It'd be great for me if this happened right, and then, and then, what's the next generation of that is we say, oh no, no, not if it happens, when it happens right? And he's saying, not even that but there's one step further, I am that thing. I have that thing already. I am an entrepreneur. I am a super mom. I am a race car driver. I am a whatever it is, the thing that you want, you know, in your life, instead of when that happens for me, you act as if you are it now already. That is the definition of be it till you see it. Lesley Logan 20:58 Oh, yeah. My, my client, I was like, hey, baby, I can only see you at eight o'clock or whatever I gave her. And I really only did it for her, because I would never do it for anyone. And she said yes. And I was like, oh, okay. And I said well, I'm really surprised you accepted this. She's like, well, I am a healthy person, and a healthy person would work out at this hour. And I was like, oh. Brad Crowell 21:17 Great for her. Lesley Logan 21:18 I'm practicing this I am thing and helping me filter my decisions. And I think it's, you know, it takes some time, be kind to yourself, like I just saw her again today, and she's like, still working out the I ams and I haves, but you can do it.Brad Crowell 21:32 Good. Good for her. That's awesome. I thought that was super powerful. What about you? Lesley Logan 21:38 I love that you know about gratitude. Gratitude is a powerful practice for being it till you see it. You know what, gratitude, I think it gets tossed around a lot. It's like, do this, but actually, do you really do it? Do you have a gratitude practice, really? And I'm gonna. Brad Crowell 21:42 What is it? What does the gratitude practice look like? Lesley Logan 21:49 Well, I will give you a perfect example from someone else. So the book is called Buy Yourself the Fucking Lilies, or tulips, or something like that. And I think her name is Tara. I would love her on the pod. I would obviously need to learn her name, but it's from five years ago, so it's really, really great. And so I was like, I'm gonna try this thing, and I open up my reMarkable tablet, and I have my schedule of the day on my reMarkable tablet, and if you just scroll, like, swipe, it's a journal place, and it's not enough to do like, journal entry, really. So I just list 10 gratitudes. And literally, just go, one, I'm grateful that I have the time to do this. Two, I'm grateful. And you just keep going, and what you find is like, by the end, when you get to 10, like to find 10 things to be grateful for in a day, especially when it feels like it fucking sucks, changes mine all the way around. The published credentials are not for the Oracle Cloud. No Oracle Cloud customers experienced a breach or lost any data."To substantiate their claims, the hacker shared an Internet Archive URL indicating they uploaded a text file containing their ProtonMail email address to the login.us2.oraclecloud.com server. The threat actor also suggested that SSO passwords, while encrypted, could be decrypted using available files.The hacker's demands are multifaceted: they are selling the allegedly stolen data for an undisclosed price or seeking zero-day exploits. Further investigation revealed multiple layers of web shells that allowed the hackers to move laterally within the network while remaining undetected.Sygnia's analysis suggests the campaign's ultimate objective was long-term espionage, enabling continuous information collection and potential future strategic operations. The hackers' ability to maintain access for four years, despite repeated elimination attempts, underscores the sophisticated nature of state-sponsored cyber intrusions.Cloudflare Launches Aggressive Security Measure: Shutting Down HTTP Ports for API Accesshttps://blog.cloudflare.com/https-only-for-cloudflare-apis-shutting-the-door-on-cleartext-traffic/Company Takes Bold Step to Prevent Potential Data ExposuresCloudflare has announced a comprehensive security initiative to completely eliminate unencrypted HTTP traffic for its API endpoints, marking a significant advancement in protecting sensitive digital communications. The move comes as part of the company's ongoing commitment to enhancing internet security by closing cleartext communication channels that could potentially expose critical information.Starting immediately, any attempts to connect to api.cloudflare.com using unencrypted HTTP will be entirely rejected, rather than simply redirected. This approach addresses a critical security vulnerability where sensitive information like API tokens could be intercepted during initial connection attempts, even before a secure redirect could occur.The decision stems from a critical observation that initial plaintext HTTP requests can expose sensitive data to network intermediaries, including internet service providers, Wi-Fi hotspot providers, and potential malicious actors. By closing HTTP ports entirely, Cloudflare prevents the transport layer connection from being established, effectively blocking any potential data exposure before it can occur.Notably, the company plans to extend this feature to its customers, allowing them to opt-in to HTTPS-only traffic for their websites by the last quarter of 2025. OutlineChapter 14- Hypovolemic States- Etiology - True volume depletion occurs when fluid is lost from from the extracellular fluid at a rate exceeding intake - Can come the GI tract - Lungs - Urine - Sequestration in the body in a “third space” that is not in equilibrium with the extracellular fluid. - When losses occur two responses ameliorate them - Our intake of Na and fluid is way above basal needs - This is not the case with anorexia or vomiting - The kidney responds by minimizing further urinary losses - This adaptive response is why diuretics do not cause progressive volume depletion - Initial volume loss stimulates RAAS, and possibly other compensatory mechanisms, resulting increased proximal and collecting tubule Na reabsorption. - This balances the diuretic effect resulting in a new steady state in 1-2weeks - New steady state means Na in = Na out - GI Losses - Stomach, pancreas, GB, and intestines secretes 3-6 liters a day. - Almost all is reabsorbed with only loss of 100-200 ml in stool a day - Volume depletion can result from surgical drainage or failure of reabsorption - Acid base disturbances with GI losses - Stomach losses cause metabolic alkalosis - Intestinal, pancreatic and biliary secretions are alkalotic so losing them causes metabolic acidosis - Fistulas, laxative abuse, diarrhea, ostomies, tube drainage - High content of potassium so associated with hypokalemia - [This is a mistake for stomach losses] - Bleeding from the GI tract can also cause volume depletion - No electrolyte disorders from this unless lactic acidosis - Renal losses - 130-180 liters filtered every day - 98-99% reabsorbed - Urine output of 1-2 liters - A small 1-2% decrease in reabsorption can lead to 2-4 liter increase in Na and Water excretion - 4 liters of urine output is the goal of therapeutic diuresis which means a reduction of fluid reabsorption of only 2% - Diuretics - Osmotic diuretics - Severe hyperglycemia can contribute to a fluid deficit of 8-10 Iiters - CKD with GFR < 25 are poor Na conservers - Obligate sodium losses of 10 to 40 mEq/day - Normal people can reduce obligate Na losses down to 5 mEq/day - Usually not a problem because most people eat way more than 10-40 mEq of Na a day. - Salt wasting nephropathies - Water losses of 2 liters a day - 100 mEq of Na a day - Tubular and interstitial diseases - Medullary cystic kidney - Mechanism - Increased urea can be an osmotic diuretic - Damage to tubular epithelium can make it aldo resistant - Inability to shut off natriuretic hormone (ANP?) - The decreased nephro number means they need to be able to decrease sodium reabsorption per nephron. This may not be able to be shut down acutely. - Experiment, salt wasters can stay in balance if sodium intake is slowly decreased. (Think weeks) - Talks about post obstruction diuresis - Says it is usually appropriate rather than inappropriate physiology. - Usually catch up solute and water clearance after releasing obstruction - Recommends 50-75/hr of half normal saline - Talks briefly about DI - Skin and respiratory losses - 700-1000 ml of water lost daily by evaporation, insensible losses (not sweat) - Can rise to 1-2 liters per hour in dry hot climate - 30-50 mEq/L Na - Thirst is primary compensation for this - Sweat sodium losses can result in hypovolemia - Burns and exudative skin losses changes the nature of fluid losses resulting in fluid losses more similar to plasma with a variable amount of protein - Bronchorrhea - Sequestration into a third space - Volume Deficiency produced by the loss of interstitial and intravascular fluid into a third space that is not in equilibrium with the extracellular fluid. - Hip fracture 1500-2000 into tissues adjacent to fxr - Intestinal obstruction, severe pancreatitis, crush injury, bleeding, peritonitis, obstruction of a major venous system - Difference between 3rd space and cirrhosis ascities - Rate of accumulation, if the rate is slow enough there is time for renal sodium and water compensation to maintain balance. - So cirrhotics get edema from salt retension and do not act as hypovolemia - Hemodynamic response to volume depletion - Initial volume deficit reduced venous return to heart - Detected by cardiopulmonary receptors in atria and pulmonary veins leading to sympathetic vasoconstriction in skin and skeletal muscle. - More marked depletion will result in decreased cardiac output and decrease in BP - This drop in BP is now detected by carotid and aortic arch baroreceptors resulting in splanchnic and renal circulation vasoconstriction - This maintains cardiac and cerebral circulation - Returns BP toward normal - Increase in BP due to increased venous return - Increased cardiac contractility and heart rate - Increased vascular resistance - Sympathetic tone - Renin leading to Ang2 - These can compensate for 500 ml of blood loss (10%) - Unless there is autonomic dysfunction - With 16-25% loss this will not compensate for BP when patient upright - Postural dizziness - Symptoms - Three sets of symptoms can occur in hypovolemic patients - Those related to the manner in which the fluid loss occurs - Vomiting - Diarrhea - Polyuria - Those due to volume depletion - Those due to the electrode and acid base disorders that can accompany volume depletion - The symptoms of volume depletion are primarily related to the decrease in tissue perfusion - Early symptoms - Lassitude - Fatiguability - Thirst - Muscle cramps - Postural dizziness - As it gets more severe - Abdominal pain - Chest pain - Lethargy - Confusion - Symptomatic hypovolemia is most common with isosmotic Na and water depletion - In contrast pure water loss, causes hypernatremia, which results in movement of water from the intracellular compartment to the extracellular compartment, so that 2/3s of volume loss comes from the intracellular compartment, which minimizes the decrease in perfusion - Electrolyte disorders and symptoms - Muscle weakness from hypokalemia - Polyuria/poly dips is from hyperglycemia and hypokalemia - Lethargy, confusion, Seizures, coma from hyponatremia, hypernatremia, hyperglycemia - Extreme salt craving is unique to adrenal insufficiency - Eating salt off hands ref 18 - Evaluation of the hypovolemic patient - Know that if the losses are insensible then the sodium should rise - Volume depletion refers to extracellular volume depletion of any cause, while dehydration refers to the presence of hypernatremia due to pure water loss. Such patients are also hypovolemic. - Physical exam is insensitive and nonspecific - Finding most sensitive and specific finding for bleeding is postural changes in blood pressure - I don't find this very specific at all! - Recommends laboratory confirmation regardless of physical exam - Skin and mucous membranes - Should return too shape quickly - Elastic property is called Turgur - Not reliable is patients older than 55 to 60 - Dry axilla - Dry mucus membranes - Dark skin in Addison's disease Frim increased ACTH - Arterial BP - As volume goes down so does arterial BP - Marked fluid loss leads to quiet korotkoff signs - Interpret BP in terms of the patients “normal BP” - Venous pressure - Best done by looking at the JVP - Right atrial and left atrial pressure - LV EDP is RAP + 5 mmHg - Be careful if valvular disease, right heart failure, cor pulmonare, - Figure 14-2 - Shock - 30% blood loss - Lab Data - Urine Na concentration - Should be less than 25 mmol/L, can go as low as 1 mmol/L - Metabolic alkalosis can throw this off - Look to the urine chloride - Figure 14-3 - Renal artery stenosis can throw this off - FENa - Mentions that it doesn't work so well at high GFR - Urine osmolality - Indicates ADH - Volume depletion often associated with urine osm > 450 - Impaired by - Renal disease - Osmotic diuretic - Diuretics - DI - Mentions that severe volume depletion and hypokalemia impairs urea retension in renal medulla - Points out that isotonic urine does not rule out hypovolemia - Mentions specific gravity - BUN and Cr concentration - Normal ratio is 10:1 - Volume depletion this goes to 20:1 - Serum Na - Talks about diarrhea - Difference between secretory diarrhea which is isotonic and just causes hypovolemia - And osmotic which results in a lower electrolyte content and development of hypernatremia - Talks about hyperglycemia - Also can cause the sodium to rise from the low electrolyte content of the urine - But the pseudohyponatraemia can protect against this - Plasma potassium - Treatment - Both oral and IV treatment can be used for volume replacement - The goal of therapy are to restore normovolemia - And to correct associated acid-base and electrolyte disorders - Oral Therapy - Usually can be accomplished with increased water and dietary sodium - May use salt tablets - Glucose often added to resuscitation fluids - Provides calories - Promotes intestinal Na reabsorption since there is coupled Na and Glucose similar to that seen in the proximal tubule - Rice based solutions provide more calories and amino acids which also promote sodium reabsorption - 80g/L of glucose with rice vs 20 g/L with glucose alone - IV therapy - Dextrose solutions - Physiologically equivalent to water - For correcting hypernatremia - For covering insensible losses - Watch for hyperglycemia - Footnote warns against giving sterile water - Saline solutions - Most hypovolemic patients have a water and a sodium deficit - Isotonic saline has a Na concentration of 154, similar to that of plasma see page 000 - Half-isotonic saline is equivalent to 550 ml of isotonic saline and 500 of free water. Is that a typo? - 3% is a liter of hypertonic saline and 359 extra mEq of Na - Dextrose in saline solutions - Give a small amount of calories, otherwise useless - Alkalinizing solutions - 7.5% NaHCO3 in 50 ml ampules 44 mEq of Na and 44 mEq of HCO3 - Treat metabolic acidosis or hyperkalemia - Why 44 mEq and not 50? - Do not give with calcium will form insoluble CaCO3 - Polyionic solutions - Ringers contains physiologic K and Ca - Lactated Ringers adds 28 mEq of lactate - Spreads myth of LR in lactic acidosis - Potassium chloride - Available as 2 mEq/mL - Do not give as a bolus as it can cause fatal hyperkalemia - Plasma volume expanders - Albumin, polygelastins, hetastarch are restricted to vascular space - 25% albumin can pull fluid into the vascular space - 25% albumin is an albumin concentration of 25 g/dL compare to physiologic 4 g/dL - Says it pulls in several times its own volume - 5% albumin is like giving plasma - Blood - Which fluid? - Look at osmolality, give hypotonic fluids to people with high osmolality - Must include all electrolytes - Example of adding 77 mEw of K to 0.45 NS and making it isotonic - DI can be replaced with dextrose solutions, pure water deficit - Case 14-3 - Diarrhea with metabolic acidosis - He chooses 0.25 NS with 44 mEq of NaCl and 44 NaHCO3 - Talks about blood and trauma - Some studies advocate delaying saline until penetrating trauma is corrected APR about to. Ca flows out too leading to vasodilation - Glyburide is an K-ATP channel inhibitor (?) caused increased vasoconstriction and BP - Pluggin of capillaries by neutrophils - Cerebral ischemia - Increased NO generation - Which Fluids? - Think of what is lost and replace that. - Bleeding think blood - Raise the hct but not above 35 - Acellular blood substitutes, looked bad at the time of this writing - Di aspirin cross linked hemoglobin had increased 2 and 28 day mortality vs saline - Colloids sound great but they fail in RCTs - SAFE - FEAST - Points out that saline replaces the interstitial losses why do we think those losses are unimportant - Pulmonary circulation issue - Pulmonary circulation is more leaky so oncotic pressure less effective there - Talks about the lungs be naturally protected from pulmonary edema - Rate of fluid - 1-2 liters in first hour - Suggests CVP or capillary wedge pressure during resuscitation - No refs in the rate of fluid administration section - Lactic acidosis - Points out that HCO can impair lactate utilization - Also states that arterial pH does not point out what is happening at the tissue level.
