Michael Beinhorn co-founded the New York musical collective Material in 1979 with Bill Laswell. Since then, he has collaborated with and produced a diverse range of recording artists including The Red Hot Chili Peppers, Soundgarden, Soul Asylum, Hole, Aerosmith, Herbie Hancock, Brian Eno, Ozzy Osbourne, Marilyn Manson, Social Distortion, Korn and Mew. Michael's combined worldwide record sales currently total over 45 million. He has received numerous Grammy nominations, including "Producer Of The Year" in 1998. He is one of the only music producers in history to have 2 separate recordings debut in the Billboard Top Ten in the same week (Marilyn Manson- Mechanical Animals- #1; Celebrity Skin- Hole- #9). In 2015, Michael wrote a book called "Unlocking Creativity", which outlines his philosophy regarding the creative process. In 2019, he started the world's first completely remote service for music production and artist development. Roy L Hales/Cortes Currents - The Audobin Society has been holding Christmas Bird Counts across North America for the past 125 years. On Cortes Island it is co-sponsored by the Cortes Island Museum and Birds Canada. Thirty-nine walkers, cyclists, boaters, and people in cars participated in Cortes Islands 2024 Christmas Bird Count. This was the third time that more than 4,000 birds have been counted since the islands first Christmas Bird Count in 2001. A new record was set: 4,545 Birds were listed. Donna Collins explained, "We had more birders out, more areas covered and lots of birds counted." The most numerous species were 1,057 Surf Scoters. There were also three unusual sightings. This is the first time Ring-billed gulls have been listed in a Christmas count. Laurel Bohart counted 32. Turkey vultures are not usually seen this time of year. One was seen during the week in 2014, but it did not make it into the official count. Sierra Sullivan saw a Turkey vulture this year. Michael Sullivan saw three American coots. Laurel Bohart and Donna Collins agreed to describe their experience of the day. Laurel Bohart: "Donna showed up at my place in Squirrel Cove. I had already looked at the birds on my feeder, which were maybe 12 Juncos. We went directly to the dump to check on birds there and spotted 6 Ravens. Of course, they're always there when the dump is open because of food. Along with them, there were some wrens." Donna Collins: "We were having a hard time because we could hear them, but couldn't see them." Laurel Bohart: "It's hard to see them because they were lurking in the bush. We spent some time just on the driveway, in and out of the Recycling Centre. Unfortunately my binoculars aren't very good and her eyes are better than mine and sharper. So she was just describing what they might be. We had the bird book with us and were frantically looking through the pages." "We saw a whole cloud of Pine Siskins." Cortes Currents: Can either of you recognize the birds by their sounds? Laurel Bohart: "A Wren's like ‘chik chik, chik chik, chik chik, chiche, chik, chik, chiche, chik, chik.' That's the alarm call." "The other calls were more a mingled sound, kind of a high pitched ‘we wi wi wi wi wi.'” Cortes Currents: What was that? Laurel Bohart: "That would be birds higher up in the trees and no, I couldn't really tell which ones they were. I could hear them, although my hearing aid is not perfect." "The adventure came first of all, when we went down to the waterfront by the Squirrel Cove store." "I have never seen so many gulls in my life. The gulls were Glaucus-winged Gulls mostly. There was one Mew gull, one Bonaparte's and a bunch of Ring billed gulls, all splashing very happily right around, right in the creek's mouth and making a great deal of noise." "What was happening was the gush of water out of the Basil Creek estuary was fanning farther out into the ocean. Behind that fan, where all the birds were in front, there were sea lions or seals. We had a line of a couple dozen Bufflehead ducks cleaning their feathers in the fresh water. There were Goldeneye ducks, I think there was four. There was a whole line of Surf Scoters, at least a dozen." "What caught our attention was the crows between there and the Klahoose First Nation." Cortes Currents: In the final tally it says you saw 89 crows. Do you have any explanation for that number?

Gibt es eine Klasse, die politisch ungebildet, aber leicht verführbar ist? Gerade in den Analysen über die Wählerschaft von Rechtspopulisten wie Donald Trump tauchen Überlegungen wieder auf, die ins 19. Jahrhundert zurückreichen und vor allem bei Karl Marx und Friedrich Engels anzutreffen sind. Mit „Lumpenproletarier“ sind jene gemeint, die nicht für den revolutionären Arbeitskampf zu gewinnen sind, sondern häufig in Passivität verharren oder zu konterrevolutionären Kräften neigen. Marx und Engels spalteten mit dem stets pejorativ gebrauchten Begriff all diejenigen vom Proletariat ab, die einer Revolution und der Organisation der Arbeiterschaft im Wege standen. Mitunter griff man dabei auf bürgerliche Moralkategorien zurück und schrieb jenen, die in Lumpen gehen bzw. solche sind, lasterhafte Eigenschaften zu. Durch diese Abwertung versuchte man, den von Bürgerlichen ebenfalls despektierlich verwendeten Begriff Proletarier aufzuwerten und zur Bildung einer revolutionären Klasse beizutragen. Sam Newman, Mike Sheahan and Don Scott - 'You Cannot Be Serious'
Mark Alexander Jackson (born 30 August 1959) is a former Australian rules footballer who played for the Melbourne Football Club, St Kilda Football Club and Geelong Football Club in the Victorian Football League (VFL) and for the South Fremantle Football Club in the West Australian Football League (WAFL). Known as "Jacko", Jackson's colourful and enigmatic personality often resulted in clashes with officials and teammates, which tended to overshadow the fact that he was also a capable full forward. Following his football career, Jackson became notable for several television appearances, including commercials for Jenny Craig, Energizer and Nutri-Grain, along with several feature films. Jackson has also written an autobiography, Dumb Like a Fox, which was released in 1986. Jackson was born at the Royal Woman's Hospital as one of six children to George Jackson and his wife Frances, and grew up in the eastern Melbourne suburb of Nunawading. Football career South Fremantle & Richmond (1979–1980) After playing colts and reserves football for Richmond in 1977 and 1978, Jackson started his senior career in the West Australian Football League, spending the 1979 season with South Fremantle. Coach Mal Brown employed Jackson as a forward to protect Ray Bauskis, a skillful but lightweight full-forward. Jackson kicked 53 goals in 22 games and Bauskis 66 in 17 games. South Fremantle finished second in 1979. The night before the second semi-final, Jackson returned to Melbourne after being told that the other players had voted him out of the team. He consequently missed out on playing in the 1979 grand final, which saw the then largest crowd in West Australian football history. Jackson rejoined Richmond for the 1980 season. However, with Michael Roach and Brian Taylor at the club, there was no room for another full forward. Jackson spent the entire 1980 season playing in the reserves and kicked 131 goals. Melbourne (1981–1982) Jackson moved to Melbourne for the 1981 season, coached by Ron Barassi. Jackson performed a handstand in front of the Hawks full back, Kelvin Moore, reportedly after Moore had told him he "wouldn't be a full forward while his arse pointed to the floor". Jackson on Open Mike said it was not pointed at Moore, rather it was directed at umpire Glenn James after he disputed a decision. Jackson kicked 76 goals in each of his two years with the Demons, leading the goal kicking in 1981 and one goal less than Gerard Healy in 1982. St Kilda (1983) In 1983, Jackson joined St Kilda on a three-year contract believed to have been worth $40,000 per year. He played in the first nine matches (in which the Saints lost the first eight matches in a row) and kicked 40 goals, including 10 in Round 5 against Sydney. However, controversy reared its head again when he was relegated to the reserves for "disciplinary reasons" in Round 10, and then returned for the Round 11 game against Collingwood, where he was held to one goal in a high-scoring 16-point loss. Jackson was again dropped to the reserves, and suffered a bruised chest during the Sunday game. On the Tuesday following that game, the Saints had told Jackson his services were no longer required. It was later revealed that, among other things, he had played a dangerous prank on club legend Trevor Barker by placing a brick behind the brake pedal in his car. He had also placed a lit cigarette butt in the pocket of club chairman Lindsay Fox in the social club. Jackson spent the remainder of the year playing for the Melbourne Harlequins rugby side. However, his 41 goals were still enough to be the Saints' leading goal kicker for the 1983 season. Geelong (1984–1986) 1984 saw Jackson return to the VFL with Geelong. He led their goal kicking in 1984 with 74. In total during his time at Geelong, Jackson scored 115 goals in 31 games. Geelong started 1985 poorly with one win in the first four rounds but improved with five wins in the next six rounds to be in fifth position. But after losing to Fitzroy in Round 11 they would spend the remainder of the season hovering just outside the Top Five, eventually finishing sixth on the ladder. The Round 11 match against Hawthorn at Princess Park was overshadowed by various spiteful incidents, including Leigh Mattews king-hit on Geelong midfielder Neville Bruns behind play, leaving Bruns with a broken jaw. Although this incident was not reported at the time, video footage of the incident resulted in Matthews being charged with assault by Victoria Police and subsequently deregistered by the VFL for four matches. Jackson kicked four goals for the game to be the Cats' main scorer for the day, but was reported four times during the match: by boundary umpire Gower, boundary umpire O'Leary, goal umpire Bill Pryde and field umpire Ian Robinson for allegedly striking Gary Ayres in the final quarter; by goal umpire Pryde for allegedly striking Chris Langford on two separate occasions during the final quarter; by field umpire Robinson for allegedly striking Chris Mew in the final quarter; and by field umpire Robinson for allegedly striking Chris Langford in the final quarter. At the VFL Tribunal hearing on the Monday following the game, Jackson was suspended for a total of eight matches, stemming from outcomes of three of the charges: He pleaded guilty to the charge from four umpires of striking Ayres (two-match suspension). He pleaded not guilty to striking Mew and was severely reprimanded The third charge of striking Langford by goal umpire Pryde was upheld (two-match suspension) He pleaded guilty to the fourth charge of striking Langford (four-match suspension). After kicking six goals in the first round of 1986 and two in the second round, he retired after his omission from the Geelong senior side in the following round. He ended his VFL career with 308 goals from 82 games, leading his club's goalkicking on 3 occasions. He holds the record for the most consecutive games from debut with at least one goal, with 79 games. His first and only goalless game in his career was in his third-to-last game in Round 21, 1985 against Richmond at VFL Park. Jackson gained a controversial reputation for his on-field antics and was regularly reported: twice at Melbourne and four times at Geelong, including an eight-match suspension following a fight against Hawthorn. Later years An autobiography of Jackson's football career was published in 1986 and titled Jacko, Dumb Like a Fox, written with the assistance of Melbourne journalist Jon Anderson. Later in 1986 he played a few games for Brunswick Football Club in the Victorian Football Association (VFA) First Division, the first of which against Sandringham drew a crowd of nearly 15,000 to Gillon Oval; he was sacked from Brunswick in July after missing training. Jackson returned to South Fremantle for the 1987 WAFL season and kicked 45 goals from the opening 10 games. This included nine goals against West Perth in round three. South Fremantle won its opening three games of the season, but then went on an 18-match losing streak to win the wooden spoon – the club's first since 1972. Jackson walked out on the club after round 10, where the Bulldogs lost to West Perth by a league-record 210 points. Despite only playing half the season, he still finished as the club's leading goalkicker. Later in 1987, he went to Queensland and played a game for QAFL club Kedron; he was paid a large fee of $2,000 per game, but his presence drew a large crowd which earned more than $12,000 for the club. He had intended to play more games for Kedron, but was suspended for unbecoming conduct after dropping his shorts several times during the match. Singing career Following his retirement from football, Jackson used his fame and popularity to launch a singing career. His first single, "I'm an Individual" was a hit on the Australian singles chart. A second single, "My Brain Hurts" was not so successful. A 1991 release, "You Can Do This", also failed to make an impression. Jackson combined a rap-like delivery with lyrics based on Australian comedy and larrikinism. Acting career After his singing career ended, Jackson began appearing in advertisements, the most successful of which was his role in Energizer battery commercials during the late 1980s and early 1990s. The commercials ended with a manic Jackson yelling "Get Energizer. It'll surprise you! Oi!". These commercials were shown extensively in the United States, even though few people knew of Jackson, during a period of high American interest in things Australian in the wake of the Crocodile Dundee films. This American infatuation with Australian culture is referenced in The Simpsons episode "Bart vs. Australia" (1995), in which Jackson makes a cameo promoting Energizer. He was the brand's last human spokesman before the emergence of the Energizer Bunny. There was also a 10 in 1 "Oi! Jacko Gym" action figure toy that could talk, do push-ups, lift weights and ride a skateboard—all battery operated, with Jackson wearing the battery company logo on his singlet. He was also linked with Nutri-Grain amongst other companies, and for a time worked as a professional actor for commercials. Jackson has appeared in various television sitcoms and movies—one of the most notable being as survival expert "Jetto" in the short-lived American action-adventure series The Highwayman (1988)—as well as being on talkback radio and in various children's programs and talk shows. During 2005 Jackson embarked on a tour with author and renowned criminal Mark "Chopper" Read. In 2014, Jackson was featured on 7mate's Bogan Hunters as a celebrity judge. Boxing Jackson also appeared in a televised Australian celebrity boxing match in 2002 in which he went up against Australian former rugby league centre Ma. Meninga. Jackson was soundly defeated. Jackson had previously beaten Essendon tough man Ron Andrews in a points decision on 10 December 1984 in a six-round boxing match at the Perth Entertainment Centre.

On Episode 216 of Jake's Happy Nostalgia Show, we interview Nigel Plaskitt! Nigel is a British puppeteer and actor who is best known for his many contributions to The Muppets and The Jim Henson Company. He puppeteered on the special The Tale of the Bunny Picnic and a number of films, including Labyrinth, The Muppet Christmas Carol, Muppet Treasure Island and Muppets Most Wanted! In television, he performed Mew in The Secret Life of Toys and a wide variety of characters in Mopatop's Shop and Bunnytown. As a voice actor, he lent his voice to Tungar the Tiger in the Sesame Street outreach spin-off Panwapa and narrated Rev. W. Awdry's letter at the beginning of a number of episodes in Thomas & Friends. Some of Nigel's on-camera acting work includes playing Mason in the 1976 film Spy Story, Corporal Klein in Owner Occupied, Unstoffe in Doctor Who and many more!

OutlineChapter 14- Hypovolemic States- Etiology - True volume depletion occurs when fluid is lost from from the extracellular fluid at a rate exceeding intake - Can come the GI tract - Lungs - Urine - Sequestration in the body in a “third space” that is not in equilibrium with the extracellular fluid. - When losses occur two responses ameliorate them - Our intake of Na and fluid is way above basal needs - This is not the case with anorexia or vomiting - The kidney responds by minimizing further urinary losses - This adaptive response is why diuretics do not cause progressive volume depletion - Initial volume loss stimulates RAAS, and possibly other compensatory mechanisms, resulting increased proximal and collecting tubule Na reabsorption. - This balances the diuretic effect resulting in a new steady state in 1-2weeks - New steady state means Na in = Na out - GI Losses - Stomach, pancreas, GB, and intestines secretes 3-6 liters a day. - Almost all is reabsorbed with only loss of 100-200 ml in stool a day - Volume depletion can result from surgical drainage or failure of reabsorption - Acid base disturbances with GI losses - Stomach losses cause metabolic alkalosis - Intestinal, pancreatic and biliary secretions are alkalotic so losing them causes metabolic acidosis - Fistulas, laxative abuse, diarrhea, ostomies, tube drainage - High content of potassium so associated with hypokalemia - [This is a mistake for stomach losses] - Bleeding from the GI tract can also cause volume depletion - No electrolyte disorders from this unless lactic acidosis - Renal losses - 130-180 liters filtered every day - 98-99% reabsorbed - Urine output of 1-2 liters - A small 1-2% decrease in reabsorption can lead to 2-4 liter increase in Na and Water excretion - 4 liters of urine output is the goal of therapeutic diuresis which means a reduction of fluid reabsorption of only 2% - Diuretics - Osmotic diuretics - Severe hyperglycemia can contribute to a fluid deficit of 8-10 Iiters - CKD with GFR < 25 are poor Na conservers - Obligate sodium losses of 10 to 40 mEq/day - Normal people can reduce obligate Na losses down to 5 mEq/day - Usually not a problem because most people eat way more than 10-40 mEq of Na a day. - Salt wasting nephropathies - Water losses of 2 liters a day - 100 mEq of Na a day - Tubular and interstitial diseases - Medullary cystic kidney - Mechanism - Increased urea can be an osmotic diuretic - Damage to tubular epithelium can make it aldo resistant - Inability to shut off natriuretic hormone (ANP?) - The decreased nephro number means they need to be able to decrease sodium reabsorption per nephron. This may not be able to be shut down acutely. - Experiment, salt wasters can stay in balance if sodium intake is slowly decreased. (Think weeks) - Talks about post obstruction diuresis - Says it is usually appropriate rather than inappropriate physiology. - Usually catch up solute and water clearance after releasing obstruction - Recommends 50-75/hr of half normal saline - Talks briefly about DI - Skin and respiratory losses - 700-1000 ml of water lost daily by evaporation, insensible losses (not sweat) - Can rise to 1-2 liters per hour in dry hot climate - 30-50 mEq/L Na - Thirst is primary compensation for this - Sweat sodium losses can result in hypovolemia - Burns and exudative skin losses changes the nature of fluid losses resulting in fluid losses more similar to plasma with a variable amount of protein - Bronchorrhea - Sequestration into a third space - Volume Deficiency produced by the loss of interstitial and intravascular fluid into a third space that is not in equilibrium with the extracellular fluid. - Hip fracture 1500-2000 into tissues adjacent to fxr - Intestinal obstruction, severe pancreatitis, crush injury, bleeding, peritonitis, obstruction of a major venous system - Difference between 3rd space and cirrhosis ascities - Rate of accumulation, if the rate is slow enough there is time for renal sodium and water compensation to maintain balance. - So cirrhotics get edema from salt retension and do not act as hypovolemia - Hemodynamic response to volume depletion - Initial volume deficit reduced venous return to heart - Detected by cardiopulmonary receptors in atria and pulmonary veins leading to sympathetic vasoconstriction in skin and skeletal muscle. - More marked depletion will result in decreased cardiac output and decrease in BP - This drop in BP is now detected by carotid and aortic arch baroreceptors resulting in splanchnic and renal circulation vasoconstriction - This maintains cardiac and cerebral circulation - Returns BP toward normal - Increase in BP due to increased venous return - Increased cardiac contractility and heart rate - Increased vascular resistance - Sympathetic tone - Renin leading to Ang2 - These can compensate for 500 ml of blood loss (10%) - Unless there is autonomic dysfunction - With 16-25% loss this will not compensate for BP when patient upright - Postural dizziness - Symptoms - Three sets of symptoms can occur in hypovolemic patients - Those related to the manner in which the fluid loss occurs - Vomiting - Diarrhea - Polyuria - Those due to volume depletion - Those due to the electrode and acid base disorders that can accompany volume depletion - The symptoms of volume depletion are primarily related to the decrease in tissue perfusion - Early symptoms - Lassitude - Fatiguability - Thirst - Muscle cramps - Postural dizziness - As it gets more severe - Abdominal pain - Chest pain - Lethargy - Confusion - Symptomatic hypovolemia is most common with isosmotic Na and water depletion - In contrast pure water loss, causes hypernatremia, which results in movement of water from the intracellular compartment to the extracellular compartment, so that 2/3s of volume loss comes from the intracellular compartment, which minimizes the decrease in perfusion - Electrolyte disorders and symptoms - Muscle weakness from hypokalemia - Polyuria/poly dips is from hyperglycemia and hypokalemia - Lethargy, confusion, Seizures, coma from hyponatremia, hypernatremia, hyperglycemia - Extreme salt craving is unique to adrenal insufficiency - Eating salt off hands ref 18 - Evaluation of the hypovolemic patient - Know that if the losses are insensible then the sodium should rise - Volume depletion refers to extracellular volume depletion of any cause, while dehydration refers to the presence of hypernatremia due to pure water loss. Such patients are also hypovolemic. - Physical exam is insensitive and nonspecific - Finding most sensitive and specific finding for bleeding is postural changes in blood pressure - I don't find this very specific at all! - Recommends laboratory confirmation regardless of physical exam - Skin and mucous membranes - Should return too shape quickly - Elastic property is called Turgur - Not reliable is patients older than 55 to 60 - Dry axilla - Dry mucus membranes - Dark skin in Addison's disease Frim increased ACTH - Arterial BP - As volume goes down so does arterial BP - Marked fluid loss leads to quiet korotkoff signs - Interpret BP in terms of the patients “normal BP” - Venous pressure - Best done by looking at the JVP - Right atrial and left atrial pressure - LV EDP is RAP + 5 mmHg - Be careful if valvular disease, right heart failure, cor pulmonare, - Figure 14-2 - Shock - 30% blood loss - Lab Data - Urine Na concentration - Should be less than 25 mmol/L, can go as low as 1 mmol/L - Metabolic alkalosis can throw this off - Look to the urine chloride - Figure 14-3 - Renal artery stenosis can throw this off - FENa - Mentions that it doesn't work so well at high GFR - Urine osmolality - Indicates ADH - Volume depletion often associated with urine osm > 450 - Impaired by - Renal disease - Osmotic diuretic - Diuretics - DI - Mentions that severe volume depletion and hypokalemia impairs urea retension in renal medulla - Points out that isotonic urine does not rule out hypovolemia - Mentions specific gravity - BUN and Cr concentration - Normal ratio is 10:1 - Volume depletion this goes to 20:1 - Serum Na - Talks about diarrhea - Difference between secretory diarrhea which is isotonic and just causes hypovolemia - And osmotic which results in a lower electrolyte content and development of hypernatremia - Talks about hyperglycemia - Also can cause the sodium to rise from the low electrolyte content of the urine - But the pseudohyponatraemia can protect against this - Plasma potassium - Treatment - Both oral and IV treatment can be used for volume replacement - The goal of therapy are to restore normovolemia - And to correct associated acid-base and electrolyte disorders - Oral Therapy - Usually can be accomplished with increased water and dietary sodium - May use salt tablets - Glucose often added to resuscitation fluids - Provides calories - Promotes intestinal Na reabsorption since there is coupled Na and Glucose similar to that seen in the proximal tubule - Rice based solutions provide more calories and amino acids which also promote sodium reabsorption - 80g/L of glucose with rice vs 20 g/L with glucose alone - IV therapy - Dextrose solutions - Physiologically equivalent to water - For correcting hypernatremia - For covering insensible losses - Watch for hyperglycemia - Footnote warns against giving sterile water - Saline solutions - Most hypovolemic patients have a water and a sodium deficit - Isotonic saline has a Na concentration of 154, similar to that of plasma see page 000 - Half-isotonic saline is equivalent to 550 ml of isotonic saline and 500 of free water. Is that a typo? - 3% is a liter of hypertonic saline and 359 extra mEq of Na - Dextrose in saline solutions - Give a small amount of calories, otherwise useless - Alkalinizing solutions - 7.5% NaHCO3 in 50 ml ampules 44 mEq of Na and 44 mEq of HCO3 - Treat metabolic acidosis or hyperkalemia - Why 44 mEq and not 50? - Do not give with calcium will form insoluble CaCO3 - Polyionic solutions - Ringers contains physiologic K and Ca - Lactated Ringers adds 28 mEq of lactate - Spreads myth of LR in lactic acidosis - Potassium chloride - Available as 2 mEq/mL - Do not give as a bolus as it can cause fatal hyperkalemia - Plasma volume expanders - Albumin, polygelastins, hetastarch are restricted to vascular space - 25% albumin can pull fluid into the vascular space - 25% albumin is an albumin concentration of 25 g/dL compare to physiologic 4 g/dL - Says it pulls in several times its own volume - 5% albumin is like giving plasma - Blood - Which fluid? - Look at osmolality, give hypotonic fluids to people with high osmolality - Must include all electrolytes - Example of adding 77 mEw of K to 0.45 NS and making it isotonic - DI can be replaced with dextrose solutions, pure water deficit - Case 14-3 - Diarrhea with metabolic acidosis - He chooses 0.25 NS with 44 mEq of NaCl and 44 NaHCO3 - Talks about blood and trauma - Some studies advocate delaying saline until penetrating trauma is corrected APR about to. Keep BP low to prevent bleeding. Worry about diluting coagulation factors - Only do this if the OR is quickly available - Volume deficit - Provides formula for water deficit and sodium deficit - Do not work for isotonic losses - Provides a table to adjust fluid loss based on changes in Hgb or HCTZ - Says difficult to estimate it from lab findings and calculations - Follow serial exams - Serial urine Na - Rate of replacement - Goal is not to give fluid but to induce a positive balance - Suggests 50-100 ml/hr over what is coming out of the body - Urine - Insensibles 30-50 - Diarrhea - Tubes - Hypovolemic shock - Due to bleeding - Sequesting in third space - Why shock? - Progressive volume depletion leads to - Increased sympathetic NS - Increased Ang 2 - Initially this maintains BP, cerebral and coronary circulation - But this can decrease splanchnic, renal and mucocutaneous perfusion - This leads to lactic acicosis - This can result in intracellular contents moving into circulation or translocation of gut bacteria - Early therapy to prevent irreversible shock - In dogs need to treat with in 2 hours - In humans may need more than 4 hours - Irreversible shock associated with pooling of blood in capillaries - Vasomotor paralysis - Hyperpolarization of vascular smooth muscle as depletion of ATP allows K to flowing out from K channels opening. Ca flows out too leading to vasodilation - Glyburide is an K-ATP channel inhibitor (?) caused increased vasoconstriction and BP - Pluggin of capillaries by neutrophils - Cerebral ischemia - Increased NO generation - Which Fluids? - Think of what is lost and replace that. - Bleeding think blood - Raise the hct but not above 35 - Acellular blood substitutes, looked bad at the time of this writing - Di aspirin cross linked hemoglobin had increased 2 and 28 day mortality vs saline - Colloids sound great but they fail in RCTs - SAFE - FEAST - Points out that saline replaces the interstitial losses why do we think those losses are unimportant - Pulmonary circulation issue - Pulmonary circulation is more leaky so oncotic pressure less effective there - Talks about the lungs be naturally protected from pulmonary edema - Rate of fluid - 1-2 liters in first hour - Suggests CVP or capillary wedge pressure during resuscitation - No refs in the rate of fluid administration section - Lactic acidosis - Points out that HCO can impair lactate utilization - Also states that arterial pH does not point out what is happening at the tissue level.
