    Bryant Giles DO is a senior Ophthalmology Resident and he never intended to be an ophthalmologist. In this series we are going to discuss how plans change and how life leads us to opportunities that we would never have considered without the serendipity of life. Bryant focuses on his specialty but along the way we discuss life and different universities and how it all comes together. We also discuss Utah which is a place that everyone should experience. It’s a Specialty Spotlight series but its valuable for any medical student, premed or older physicians who want to find something different because they are not fulfilled in what they are currently doing. I also asked Bryant about his lifestyle in the military. He assured me that he is “doing fine on his current pay.” Ad reiterated the truism of living like a resident after residency as a means of finding a stable financial position. I also got a lot of questions from various people about when they can get vaccinated for SARS CoV2. So I thought it reasonable to clarify where we are right now.     Dawn Graham PhD is back for the last episode of this series on Psychology and its role in caring for patients. We talk about how students are primed to live in the future and as a result we are not teaching our students to be in “the present”. That has direct implications for our patients and the quality of care that we give them. We discuss the how “the next thing”, pursued from the silo of your own home, without true interpersonal communication, is affecting our psyches, our ability to have good in-person dialogue and ultimately suicide and burnout rates. Its convicting. You’ll notice that we discussed doing an episode on the social determinants of health after Week 6 last years because “my bandwidth was going to expand…” well that seems pretty funny now.     Well, because the nation has lost its mind, I think it is worthwhile to spend a little time getting you prepared for the discussion with Dawn Graham, PhD. I am concerned about how a contingent of this nation is scaring the grandmas.     What about really sick kids? Since we are in the age of the COVID we need to understand that children can have some very serious , albeit thankfully very rare, complications with a SARS CoV2 infection. But that said the American Academy of Pediatrics is strongly behind reopening schools because of all of the other positive impacts on children that schools have on them. Further the issue of dealing with hospitalized children and those with serious critical illness touches any rational person deeply. How do pediatricians deal with such things?     So here he is, after years of trying to find a pediatrician to opine about the specialty, Neil Copeland, MD; pediatrician. Neil is the first purposeful pediatrician on Rotations. If I sound gleeful it’s because I am. Pediatricians are notoriously hard to get to commit to interviews… John does a really good job of sorting out the process of getting through a pediatric residency but his comments are germane to any medical specialty. He also gives a great intro to the things that pediatricians do and the things that all good physicians should be doing prior to referring to a pediatrician. That makes this interview highly relevant to Family Physicians as well. Also, I hope you understand that the term “veterinary medicine” is an inside joke among physicians. It has nothing to do with our general love for managing children as patients. Babies can’t talk and so… …sometimes I find myself wondering why I have to explain this but we live in a hypersensitive time so there is the disclaimer. I also imagine that I will ruffle feathers with my initial comments about disaster management but I will ask that you indulge me. A lot of "expert" opinions get shot out into social media and misinform people who look at physicians as “experts” in things. That creates chaos and is not aligned with the calling of academics. I hope that a lot of young physicians consider going into administrative medicine after they get a decade or so of experience in actual clinical practice. You have to understand tactics and operations to become an effective strategic leader. The only practical way to gain that understanding is to spend a long-time practicing medicine. I would encourage you to register and take courses with FEMA’s Emergency Management Institute.     Trauma Informed Care Episode 2

    Lori Brown LISW-S, LICDC and Jennifer Schwirian LISW-S are back. This week on Rotations we continue ou r discussion about Trauma Informed Care. ACES is used as a predictive tool for future health issues. It is not a tool to identify mental or behavioral health problems and it was not designed to identify people who are somehow damaged. It is a tool that allows clinicians to help people understand what their long-term health risks may be. ACES is not a tool to look at social-economic status of childhood. We also discuss a little of the history of ACES and how Dr. Felliti moved from bariatric treatment and patients who dropped out of his program to identifying childhood trauma and correlating it with the propensity of overweight patients to leave bariatric treatment. It’s a good illustration of how humans are an interconnected system of mind, body and spirit and that injury in one system can reflect itself in problems in one of the other systems. It’s also a very curious story about the confirmation bias inherent in medicine; something that can create a protective conservative position or actually interfere with the progress of medical science. It also gets into the very prevalent issue of childhood sexual trauma and the hidden nature of child abuse and its potential to harm its victims well into adulthood.     Trauma Informed Care Episode 1

    Lori Brown LISW-S, LICDC and Jennifer Schwirian LISW-S are social workers. They are Licensed Clinical Social workers and they work with a population of patients who have significant mental illness. As part of their work they have developed a deep understanding of trauma. Not the crash your car into a bridge piling trauma, but trauma emotional trauma that occurs in childhood from neglect, un-met needs and other emotional insults that can significantly shape behavior in adulthood. In this series we are going to look at the topic of Trauma Informed Care. This is a topic that is controversial. There are some people who think that trauma in this context is overplayed. There are others that don’t think enough attention is being paid to the impact of childhood and developmental trauma on adult behavior and mental health.     Intro to Trauma

    I decided to give you a little teaser before I launch into trauma this week. When we speak of trauma for the next 3 weeks on Rotations we are not going to be thinking of the 70 MPH into a bridge abutment with a Lifeflight from the scene to the Level 1 facility for emergency care kind of trauma. We are going to discuss something else. Trauma in the context of behavioral health concerns itself with things that happen in childhood and how they shape a person into adulthood. It is controversial and some people behind it get pretty passionate. But I will be honest, until I started learning about it, I wasn't all to entertaining of its impact. But after giving it a think I actually asked one of our psychologists to lead his class for our medical students during Veterans Health Week and discuss the topic.     Melissa Thomas PhD, Health Care and the Amish, Episode 3

    Wow, I took a hiatus…well not really. In this 3rd and final segment with Melissa Thomas, PhD we start with mindfulness. That’s apropos because the Amish deal with time in a different fashion than do the “English”. BTW, I am “mindfully” giving you an extra-long episode… Dr. Thomas also clearly lets us know that she can “unplug” with the Amish. She leaves the phone in the car. That’s an important concept in an age when more and more people seem to be incapable of actually being without their phone for more than 5 minutes. The other thing we discuss is assumptions. For the Amish, their concept of what is “healthy” may be a different concept from that of “English” society. Healthy for the Amish may mean “I can provide for my family.”. Amish patients may not even ask a question thinking a physician’s time is valuable and if there was something to say, the doctor would tell them. Well we find out that time is again a critical factor and Western needs for “getting it done” might not be the best medicine. It might be letting a patient have some time to sort out how disease impacts their lives. It’s also challenging to the average American that thinks that you can’t have free market health care to hear that the Amish will shop around for open heart surgery. They will ask questions to save money. Bottom line the Amish remind all of us that we have a fiduciary duty to our patients. They deserve to know what they are paying for what they are getting.     The continuation of our interview with Melissa Thomas PhD and her experiences with the Amish people of Ohio and studying their health care. Melissa has 20+ years of working with the Amish of Ohio and she has gathered so many experiences that she has also published an extremely unique boom of photographs that the Amish gave her permission to create to help people see into their lives.     Sorry, but I do like reggae. Soooo, I gave you an ample dose of it. Frankly with all of the current kerfluffle I think a good chunk of one drop rhythm will make almost anyone happy…and cousin, we need some happy right now. So, there you are. In this series we find out that would-be meteorologists can fall into things like health care disparity research in Amish communities in Geauga and Holmes County, Ohio. Melissa Thomas PhD has a doctorate in science (public health) and is super smart and as non-threatening as anyone you will ever meet. She is just a very pleasant person and she has a very sophisticated sense of humor. That means that you can have a real conversation with her without feeling like you need a vacation afterward. Melissa has a heart for the Amish. Specifically understanding breast cancer in their communities and helping them not die from a disease that has a pretty good survival rate if patients just know what to do. She works tirelessly at trying to help a substantial portion of the population of Ohio. The talk is definitely about the Amish but it is also about cultural competency and how health care folks can move into and out of cultures that are very different from their own and not do damage in the process. She has a lot of wisdom to share and we hope you find something new to think about.     In this episode we continue with the discussion of moral injury. I can boil it down to a single concept: If you are thinking that you are unsure of whether or not you can continue to remain moral and ethical in your current practice, you have moral injury. The ideas in this episode are tough. How do we educate medical students to be aware of this concept? How do we empower physicians to deal with it? How do we work around administrations that are indifferent and only serve a spreadsheet? How do we work with sympathetic administrators who actually do want a healthy physician and clinician group but are constrained by insurance companies and regulations in their options to work toward that? The cynic will shout “Nationalize all Health Care”. But here is a news flash, The VA is federalized medicine and moral injury is not a stranger in the VA. Large bureaucracies don’t work well toward the goal of building familiarity and common understanding of local group needs. This is a very tough topic. We end with a discussion on physician unions. The dialogue doesn’t make that a panacea either. Private practice folks cannot unionize for some reason but employed physicians can unionize. But keep in mind, what does a strike look like? The reality is that like air traffic controllers, you can have a union but do you strike when it means you are not caring for patients?     This series is a little different. Obviously, a pulmonologist has something to say about COVID-19 these days. So, John and I decided to break it up. Episode 1 will deal with becoming a pulmonologist and what that is like. Episode 2 is all about COVID-19 and what it does what it does. Basically, what do we know about this disease as of 8 June 2020. Episode 3 is the catchall of questions and how the incredible blessing of the internet is also a major headache for people trying to care for the critically ill and also prevent people from actually getting COVID-19 and dying because someone thinks a mask is not useful. Dr. Bishara is also an officer in the United States Air Force and has been a principle advisor for public health policy within the West Virginia National Guard. He is about as humble as they come, and he deals with the sickest of sick people. He is someone that is approachable on any level and he loves what he does, mucus and all.     Sooooo...we are just gonna dive in. Some of you may remember our discussion with Pamela Wible MD about physician suicide. It was compelling. Well suicides don’t happen in a vacuum and so speaking about things that might drive a physician to despair is a pretty important. So we are gonna discuss moral injury. You can google that and get a definition for moral injury but I think that it is better to just get a couple of the folks who are deeply involved in this topic and trying to raise awareness of it.     I am about to release a very compelling series of episodes on Moral Injury in Health Care workers. I was prompted to do this because of the work of Pamela Wibel MD and other sources that we have spoken with over the years about physician suicide. My work with public health response during COVID-19 has made me all the more sensitive to this issue. Then, 2 weeks ago, a young man that I knew pretty well took his own life. It had a far reaching impact and so moral injury just hit very close to home. Because I am releasing this during the massive response to the death of George Floyd I felt that I needed to put this series into context. Nobody other than a sociopath thinks the death of another person is a good thing and nobody concerned about the health of our nation can casually ignore a death that occurs during a police arrest but I hope that in this intro to other things you understand why I am not going to join into the legion of voices commenting about those events. I am going to address something that I can speak to with some authority and that is why I felt the need to address questions that will arise when this goes out.     We continue with the interview (actually 2) the long-lost interview, with Dr. Todd Jamrose, Psychiatrist extraordinaire, D & D Player and BlueJackets Hockey Fan. We hit on clinical operations and the spider sense of physicians caring for patients. If they look sick they are sick. If they don’t look sick but your gut is saying that they are; they are. We get into the STAR*D study and talk about SSRIs and compliance and actually telling your patients about what you are prescribing for them. Adherence versus compliance is a distinction that all physicians should fully understand. It takes time, trust and patience to develop that with your patients. So hopefully as you listen you pick up some pearls and perhaps learn a little more about why you might want to pick psychiatry as a specialty if you happen to be a medical student. There is a lot medicine in psychiatry, especially when you start dealing with the anti-psychotics. Modern psychiatry, well-practiced, saves a lot of live and restores quality of life to people who suffer desperately with disordered or confused thoughts and feelings. Producers note: I am editing this just 5 days after finding the body of a friend and fellow soldier that decided to end his life. I’ll have more to say about this but not in this episode. Suicide is a terrible thing and I beg of you, if you are contemplating it, understand that you will leave more sadness than you can imagine. It is never neat. It is never clean. You can’t solve your problems by killing yourself; you’ll just break the hearts of those around you who will spend the rest of their lives wondering, why you didn’t reach out and allow them to be a friend.     So this is the long lost Dr. Jamrose series. Basically we recorded this well over a year ago and Todd Jamrose was terrific about spending time speaking with us about why psychiatry matters. We examine all sorts of things in this series about psychopharmacology, clinical psychiatry, acute in-patient psychiatry, VA Health Care and all manner of other things.     The final part of our great visit with Dr. David Morton. In this episode we discuss the nature of editing and working with social media for education. We talk about Google and YouTube as vehicles for information exchange. Then the wonk factor about education talk. Does the future of medical education come down to just 5 or 6 really excellent content producers that are paid a license fee by the schools? Well before you get worried, David doesn’t quite see the future looking like this. Our segment on MOOCs really got me rethinking about the actual value of the anatomy lab. It is well known that I hated anatomy but the introduction of ultrasound made it palatable for me and understandable. This is why we need faculty and this is why we need faculty in person and on site, but the nature of the faculty needed will change and it favors those who can correlate their instruction with clinical application. I would also mention that David gets a little Zen about the whole experience of anatomy but his points are extremely well taken. He changed my mind as to why I should actually be thankful for all those formalin fumes. We discuss the myriad of buzzwords and new shiny objects that educators come back from conferences with and how they are meant to solve all problems and generally miss the fundamental point of education. This segment at around 18:00 minutes is probably the best of the segment for anyone aspiring to be a great educator.     Part 2 of the series where we were able to dialogue with David Morton, PhD of the University of Utah. David has a little online presence thing called The Noted Anatomist. Basically, 115,000 people, we can surmise that most of them are medical students, have decided that David’s approach to teaching anatomy is worthy of their time. So, knowing this I figured that it was time to track David down and discuss this whole deal. In this segment we talk about the nature of modern medical education, textbooks, costs, how students now consume medical education and how The Noted Anatomist actually makes his content. How do Medical Universities manage this change in educational approach? What do the good universities do? The Universities who value creativity in faculty and are willing to push the boundaries of what constitutes a “traditional” approach to medical education? Is scholarly work just journals and textbooks or does audiovisual content posted through social media constitute a new way of doing this? The Noted Anatomist is INTERNATIONALLY affecting.     In this series we dialogue with David Morton, PhD of the University of Utah. David has a little online presence thing called The Noted Anatomist. Basically, 115,000 people, we can surmise that most of them are medical students, have decided that David’s approach to teaching anatomy is worthy of their time. So knowing this I figured that it was time to track David down and discuss this whole deal. Although I interviewed David this past winter (January 2020) in Salt Lake, since then COVID-19 has become a highly disruptive influence to the process of medical education. But in a fit of premonition, David and I decided to discuss what the internet has done for education. In short, the brave new world is the domain of the content creators and the better content that you can create, the more value students place in watching it. David was not happy with how he was taught, and just so that we are clear here, he could have gone to medical school, but he decided to be an anatomist instead and he was determined to develop pre-study materials that would make his instruction in anatomy all the more meaningful to the students. So, in this first part of the series we talk about his back ground and how he evolved as an online educator. The interview took place in his office at The “U” and I am not kidding when I tell you that the place was like a bunker.     Nisarg Bakshi DO Episode 2

    Our second episode with Nisarg Bakshi (now DO) OMS IV and cofounder of Rotations. We pick up where we left last week with Nisarg as he talks about moving on to begin his career as a pediatrician at Cook County Hospital in Chicago. What’s involved in a big move to a big city with an entirely different set of medical issues than most small parts of American face? Well Nisarg tells us. In addition, he talks about his strategy (or lack thereof) in picking a residency location (we think he just turned it into a chance to see friends and parts of the country where he could find good food). We discuss McDonalds in India and how India is dealing with COVID-19. Nisarg also discussed the cost of doing his interviews and also the challenges in dealing with all of this during the worst pandemic faced for 100 years. It’s a good interview and a time to catch up with one of Rotation’s co-founders. Finally, we get a commitment from him to get some serious interviews done up in Chicago after his first year is under his belt.     So I have been very busy. First of all OUHCOM has been sterilized. I mean tape sealing the doors and the word STERILIZED on them. No monkey business. And then I have been running around like a 1 armed paper hanger trying to cover the military public health response and to add to all of that, I still am seeing patients in the clinic. But today, after hours of briefings and policy memos and the nugwork of being a temporarily important person in a military response to a wicked little virus, I decided to do a podcast. SO I fetched all of my gear, moved it to my sound LIVE home office and decided to call my two second year OMS 2 students, Harsh and Adam. Now you need to give Harsh and Adam some serious props because they are on home confinement under State Order by Governor Dewine and they are still studying and working to be good doctors. On my “interview” (really a quiz) they did okay. Look, I am not being a Chicken Little. I see all the data on this every day. If you don’t follow the rules, either you or someone you love will get very sick and possibly die. Legit. Straight up. Full Stop. So listen as I review this wee little wicked thing called SARS CoV-2 and all that you need to know about beating it. Here’s the deal, if you are one of those people who gets mild to no symptoms from COVID-19 and you don’t self- isolate and engage in proper hygiene, you will kills someone. Sorry but that is the fact. So try and follow as I take Harsh and Adam on a rapid spin up of this thing and along the way remind them that they will get through this and be great doctors at the end of the day.     Hey, right up front, forgive me for the clipping. I had just gotten the Rodecaster and so levels were an issue. Dr. Jason McMullan has an incredible path through medicine. He started as a Paramedic and now works at University of Cincinnati as an Emergency Room Physician and a director of the pre-hospital fellowship at University of Cincinnati. He is widely published, He’s an editor on the New England Journal of Medicine Journal Watch and has some ties to NIH and Department of Defense as well. In this series we are going to talk about Pre-Hospital Medicine fellowships. There are over 40 programs in the country and they have the potential of greatly improving the nation’s ability to respond int eh pre-hospital realm by educating physicians about this very acute and important part of emergency medicine and first response. Jason deserves serious props because he agreed to do this interview on Memorial Day of 2019 and then was patient enough to wait over half a year for us to get it edited and out the door for you to listen to.     Our last segment with Nigel Allsopp. What a great series it has been. In this segment we will continue with Nigel and the history of working animals throughout conflict and natural disasters. As you might recall, Nigel is the President of the Australian War Animal Memorial Association and a very connected person in the world of working animals. We discuss PTSD (PTS for veterans who don’t like the term disorder) and the nature of the human canine bond in PTS service. It’s a really interesting look at the depth of connection that human handlers develop with their canine. What happens to the dogs after their service? How do they find homes? Do they all find homes? A lot of discussion about how we look after these animals when their working life is completed. Its compelling and something that some listeners may want to look at as far as finding an older but well-trained furry buddy. We also discuss the issues surrounding the care of these animals. Currently, retired service animals are not covered by pensions or care funds. This is a big issue. A lot of these dogs have medical needs incurred as a result of service, including saving people’s lives. So if you want a non-political cause, start writing your congressional representation and asking for funding for retired dogs. Get the VA to hire some veternarians. Some of his stories are a bit troubling, specifically because of a historic tendency for war canines to be left behind after the humans decide that it is time to quit and go home.     So why is Rotations talking about dogs? The reason is simple, dogs are cool. Super cool. Every day you are dependent upon dogs. They work with US Customs to inspect cargo going around the world, they work with police officers around the world, they are part of every major military in the world, they assist people, they serve as our pets and they help some of us heal. Two years ago we started work on a project called Dogs of War and we shot helf of that film in Tennessee at a wonderful place called Smoky Mountain Service Dogs. Then things stalled. Competing issues with our day jobs working at Ohio University and generally a lack of access to the population of the other half of our film put us in a serious hold. So what to do? It struck us that if we were going to look at Service Animals then it would be very helpful for our audience to understand “not service animals”. So we embarked on a journey to find out where dogs are used that are not “service animal jobs”. Through a very serendipitous group of events we ran into Nigel Allsopp. If you have ever encountered someone who is a walking museum of the history of something then you have met Nigel. In Nigel’s case, his mental museum is the history of working military and police animals. These are WORKING dogs, not Service Dogs. And for that reason we wanted to introduce all of you to Nigel. He is an incredibly humble, positive and pleasant man. In this first episode we start learning about just what working animals and dogs in general are and are not. It is a fascinating story of the interrelationship between some very unique animals who work and serve humans every day.
