Floresville Electric Light & Power System (FELPS) employees raise a flag March 13 from the bucket truck of a beloved co-worker, Burton Shields, who lost his life in a car accident earlier that day. According to a report by the Texas Department of Public Safety (DPS) Highway Patrol, Shields was traveling south on U.S. 181 near Oak Fields Drive north of Floresville around 7 a.m. last Thursday when his car veered off the road and struck a tree. Matthew A. We are reposting this episode in honor of Veteran's Day!  On this week's episode of Next Level Minds I had the opportunity to sit down with Rick Yarosh. He is is a Retired US Army Sergeant turned husband and father of two. He served in Iraq for nine months before being severely injured by an IED while on patrol in Abu Ghraib on September 1, 2006. Rick suffered 2nd and 3rd-degree burns on over 60% of his body, had his right leg amputated below the knee, lost both ears, nose, multiple fingers, and most of the function in both hands. He spent half a year in the Brooke Army Medical Center in San Antonio, Texas, recovering. For his heroic service, Rick received the purple heart and was inducted into the National Wrestling Hall of Fame. Since then, he has spoken to millions from all walks of life, including sports teams, schools, churches, the military, non-profits, and even on Fox News! His message of overcoming adversity and turning a negative situation into a positive one has made Rick one of the world's HOPE experts. Gillian Schmitz, MD, FACEP is a board certified Emergency Physician and Associate Professor in the Department of Military and Emergency Medicine at USUHS. She works clinically in the Emergency Department and serves as the Vice Chair of Education at Brooke Army Medical Center in San Antonio, TX. Federal Veterans Affairs resources are being used to serve illegal aliens, says Army veteran Derick Carver. “Essentially, what's happening is, the VA was caught moving processors from the VA to support [Immigration and Customs Enforcement Health Services Corps] processing detainee medicals,” he says.  “We're talking over 1.1 million people,” said Carver, who currently serves as a graduate fellow in the Allison Center for National Security at The Heritage Foundation.  “We're talking hundreds of thousands of initial medical exams, with follow-up exams, hundreds of thousands of prescriptions being filled, dental appointments, vision appointments,” he added. “I mean, the care that the detainees are getting is better than a lot of what the veterans are getting and without … the hoops to jump through, because they just have access to this once they're detained.”  After being severely injured in Afghanistan in 2010, losing his left leg, part of his right leg, and a number of fingers, Carver became acutely aware of the issues with VA hospitals.  “I've stopped receiving care at VA [hospitals] because of just how negligent they are at times, especially with someone with my injuries and the levels of injuries that I have. It's just not feasible to receive proper care at a VA, or even at a private hospital for that matter,” Carver said. Instead, the veteran said he visits either Walter Reed Army National Military Medical Center in Bethesda, Maryland, or Brooke Army Medical Center at Fort Sam Houston in Texas—neither is a VA hospital—to receive care.  Carver joins “The Daily Signal Podcast” to share his personal story, discuss needed reforms for VA hospitals, and explain what steps should be taken to ensure U.S. veterans are receiving the physical, mental, and emotional care they need and deserve.  Enjoy the show!

Wall Street Journal reporter Evan Gershkovich, former US Marine Paul Whelan and Russian-American journalist Alsu Kurmasheva have landed in the United States, following the biggest prisoner exchange between Russia and the West since the Cold War.  The returnees were greeted with tears and embraces by their joyous families at Joint Base Andrews in Maryland, where President Joe Biden and Vice President Kamala Harris were also on the tarmac to welcome them after their plane touched down some 20 minutes before midnight on Thursday.  Whelan, who was first to exit the plane, gave a salute before shaking hands with the US leaders, followed by Gershkovich, who embraced both Harris and Biden before hugging his ecstatic parents. Kurmasheva rushed into the arms of her husband and daughters after greeting Harris and Biden.  Gershkovich went quickly to address media gathered at the tarmac, telling his Wall Street Journal colleagues the words they had been waiting more than a year to hear. “I'm home,” he said.  Biden observed the emotional family reunions, then removed his pin and placed it on Whelan's lapel as Harris looked on.  “It feels wonderful. Long time in coming. I was absolutely convinced we could get it done. I meant it when I said – alliances make a difference. They stepped up and took a chance for us and it mattered a lot,” Biden told reporters, referring to the extensive diplomatic effort that resulted in the swap.  The return marks the conclusion of what Biden called a “brutal ordeal” for the three Americans and their families following their detentions in Russia, where prisoners are often kept in harrowing conditions.  Whelan, 54, had spent almost six years in Russian prisons after his arrest in Moscow in December 2018, while Gershkovich, 32, had been detained for more than a year. Both were sentenced on espionage charges and declared wrongfully detained by the US State Department.  Kurmasheva, a 47-year-old journalist for the US-backed Radio Free Europe/Radio Liberty, was detained in October 2023 and found guilty of spreading false information about the Russian army – charges her family and employer deny.  Their release – and that of Washington Post contributor and permanent US resident Vladimir Kara-Murza, 42 – was orchestrated as part of a complex prisoner swap between Russia, the US and other Western nations involving 24 detainees in total.  Sixteen people were released from Russian custody on Thursday, including five Germans and seven Russian citizens who were political prisoners in their own country. Moscow in return got former high-ranking FSB colonel Vadim Krasikov, as well as several individuals accused of spying or cybercrime.  In a statement earlier Thursday, Biden hailed the agreement as a “feat of diplomacy,” in collaboration with US allies, including Germany, Poland, Norway, and Slovenia.  “Some of these women and men have been unjustly held for years. All have endured unimaginable suffering and uncertainty. Today, their agony is over,” said Biden, who hosted the families of Gershkovich, Whelan, Kurmasheva and Kara-Murza at the White House.  Whelan, Gershkovich and Kurmasheva flew roughly 10 hours from Ankara, Turkey, on top of a three hour and 45 minute flight from Moscow to Ankara.  They were accompanied on their return plane by multiple medics, a psychologist and US government officials including representatives from the National Security Council and State Department, a US official told CNN.  All three and their family members are expected to continue on to San Antonio where they will be taken to Brooke Army Medical Center to undergo medical evaluation and receive whatever care they need for as long as they need, the US official said.  This is typical protocol for wrongfully detained Americans who return home. WNBA player Brittney Griner also went to the center right after she was released from Russian detention in December 2022.  The returnees will also have the option to participate in a Defense Department program called PISA (Post Isolation Support Activities), which is meant to help them readjust to being back in the United States. Stellate Ganglion Block for PTSDIndividuals may develop PTSD (also called Post Traumatic Stress Injury) from a single or repeated traumatic events. When trauma causes a persistent injury pattern to the nervous system, it can become locked into a permanent protective mode, even in the absence of on-going threat. This can cause a variety of symptoms including:Being "on-guard" all the timeConstant anxiety and fearIntolerance of being around groups of peopleNightmaresIsolation from others physically and emotionallyDifficulty progressing in therapy because talking about the trauma makes you too anxiousWhat is a stellate ganglion block?The sympathetic nervous system (the "fight or flight" part of the nervous system) is organized in front of the spinal column. Along the spine there are nerve bundles referred to as ganglion. Several ganglia are located in the neck, including one in the lower neck called the stellate ganglion. These nerves are special for their connection to brain regions which store and control emotional memories. Injection of local anesthetic around these nerve bundles interrupts the nervous system's constant exaggerated response to emotional trauma. You can think of it like rebooting your computer.How is a stellate ganglion block done?For decades, the procedure was done by feeling for a particular bone in the front of the spine, pushing the carotid artery toward the middle with one hand, and inserting a needle until it contacted bone prior to injection. Later, the procedure was improved by the use of x-ray to verify the correct level of the spine. However, use of x-ray does not allow visualization of important nerves and arteries in the neck.Ultrasound imaging and needle guidance is the gold standard for accuracy and safety, and the only technique Dr. Vogel has used over the last 7 years. In 2009 Levi Rodgers, a coveted Special Forces Green Beret, was on deployment in Afghanistan when the car he and the four men on his team were in was struck by an IED. The vehicle was shattered and everyone in the car died that day, except for Levi. Levi would go on to Brooke Army Medical Center in San Antonio, Texas, where he would later meet Benjamin, to undergo an extensive recovery process -- from both severe physical wounds and mental turmoil. Though he faced the darkest of days, Levi clung to resilience, committed to honoring the legacy of his fellow soldiers whose lives were lost that day in Afghanistan. Today, Levi owns a successful real estate business, cherishing the people in his life, and creating a legacy that will last a lifetime. Bio: Dr. Legault is a cornea and refractive surgeon in San Antonio, TX. Dr. Legault was fellowship trained at Duke Eye Center. He received his medical degree from the Uniformed Services University of the Health Sciences and completed ophthalmology residency at Brooke Army Medical Center in San Antonio, where he has held positions of chief of refractive surgery and ophthalmology residency program director. Currently, Dr. Legault serves as the Virtual Medical Center Director for the United States Department of Defense. Dr. Legault has published numerous articles in sports ophthalmology, conducting research to improve eye care for athletes involved with basketball, mixed-martial arts, and the military. Dr. Legault is an active board member for the American society of ophthalmic trauma.

TOM MURPHY - Founder Tom Murphy is the proud husband and father of four living in the charming City of Saint Albans, VT. Born and raised in Philadelphia, PA where his parents ran a small shelter and recovery home for the hopeless in their own house, Tom has gone on to take this mantra of helping and caring for others in need his life's mission.   Tom graduated from Cooperstown High School and then became an All-American at the University of Brockport, taking second place at the 1997 NCAA Division III Wrestling Championships. Tom spent the next 16 years advancing through the railroad industry, eventually becoming a Corporate Director for the world's largest short line regional railroad company, RailAmerica. He ran their National Rail Traffic Control Center until stepping down and pursuing his full-time passion of being a Superhero. On top of his “mild-manner” role in the railroad, Tom has gone on to become the successful entrepreneur of multiple small-town businesses, namely one of Northern Vermont's premiere restaurants and bars – Twiggs: An American Gastropub and The Clothier. Tom is also a former professional mixed martial artist with a record of 8-0, competing in both amateur and professional fights, including on the popular UFC Ultimate Fighter Season 2. In June of 2004, Rick entered the US Army.  Four months later he graduated from Basic and Advanced Individual Training at Fort Knox, Kentucky as a U. S. Calvary Scout.  After graduation he was sent to Fort Hood, Texas for further training in operating and gunning a Bradley Fighting Vehicle.  In December of 2005 Rick was deployed as part of Operation Iraqi Freedom to Baghdad, Iraq.   On September 1, 2006, Rick was part of a routine military mission in the Abu Ghraib region of Iraq.  While on the mission, the Bradley was hit with an IED and the vehicle was engulfed in flames.  Within 40 hours after the explosion, Rick and his two comrades arrived at the military burn unit at Brooke Army Medical Center in San Antonio, Texas.  Sergeant Luis Montes, who was the commander of the Bradley, passed away on Sep 7.   Now retired from the Army, Rick gives inspirational talks to anyone who would like to hear his story.  His story, although tragic in parts, is not a story of tragedy.  It is one of being able to get through anything with a positive attitude.  Most importantly, Rick can honestly say he is happier than he has ever been in his entire life.  Find out more at https://www.rickyarosh.net/

How do you learn to love yourself when everything changes in an instant and the world you know is gone? Rick Yarosh was deployed to Iraq in December 2005. On Sept. 1, 2006, he was severely injured by an improvised explosive device (IED) in Abu Ghraib. For half a year, Rick recovered at Brooke Army Medical Center in San Antonio, TX, fighting through 2nd and 3rd degree burns on more than 60% of his body. His right leg was amputated below the knee. He also lost: both ears, his nose, multiple fingers, and most of the function in his hands. Rick says the only reason he would change that fateful moment is if he could bring back his brothers in arms that he lost in combat: Sgt. Luis Montes and SFC Anthony Venetz. Rick is a husband, father of two and retired U.S. Army sergeant. He is also an expert in HOPE (Hold On, Possibilities Exist!). Rick has toured the country as a motivational speaker and inspired millions of people from all walks of life. BG Teyhen graduated with honors as a distinguished graduate from the U.S. Army War College with a master's degree in Strategic Studies.Prior to assuming command of the Brooke Army Medical Center, BG Teyhen previously served as the Deputy Chief of Staff (Support, G-1/4/6) at the Office of the Surgeon General in Falls Church, Virginia. The basics of endovascular surgery share very little with the basics of open surgery and getting up to speed can be a big challenge.    Dr. Caravalho received his undergraduate degree in mathematics from Gonzaga University and his medical degree from the Uniformed Service University of the Health Sciences. He completed an internal medicine residency and nuclear medicine and cardiology fellowships.    Dr. Caravalho served in several clinical roles and operationally as a medical officer in the 1st Battalion, 1st Special Forces Group; Joint Special Operations Command; 75th Ranger Regiment; U.S. Army Special Operations Command; and XVIII Airborne Corps. He also commanded the 28th Combat Support Hospital. He deployed in support of Operation Iraqi Freedom and served as the senior medical officer for both Multi-National Force-Iraq and Multi-National Corps-Iraq. He also has been the Commander of Brooke Army Medical Center, Walter Reed Army Medical Center, both the Army's Northern and Southern Regional Commands, and the Army Medical Research and Materiel Command (MRMC).     MG(R) Caravalho's culminating assignments on Active Duty were as Army Deputy Surgeon General and then as Joint Staff Surgeon, where he served as the chief medical advisor to the Chairman of the Joint Chiefs of Staff.  He currently serves as President and CEO of the Henry M. Jackson Foundation for the Advancement of Military Medicine (HJF).     In this episode, Dr. Caravalho talks about his extensive experience in operational medicine in the Special Forces and in the 75th Ranger Regiment and how that prepared him to serve at the highest strategic levels in Military Medicine.  He reflects on his many commands and the lessons he's learned during his distinguished career.  He describes the importance of research within Military Medicine and the incredible accomplishments of the MRMC.  Dr. Caravalho provides a unique perspective of how Military Medicine needs to be able to function at the highest levels in a Joint Force environment where individual Services can't afford to operate within silos.     Dr. Caravalho also describes the mission and purpose of HJF and how the organization has been able to support Military Medicine in performance improvement, research, and innovation in ways that supplement the efforts of the Services. Surgical training is evolving, and with advancing surgical techniques, the traditional “time-served” model of residency may not best serve the needs of our learners or patients.  In this episode, we discuss recent efforts at the Indiana University surgical education program to implement a comprehensive laparoscopic cholecystectomy that utilizes all of the best practices of competency-based education.  We're joined by first author Dr. Betsy Huffman, along with her mentors Drs.
