In today's episode, Kristina shares her breech birth story following a long journey marked by cycle irregularities, miscarriage, and difficulty conceiving. At 40 weeks, she learned that her baby was breech and was scheduled for a C-section, despite having been told throughout pregnancy that the baby was head down.Unwilling to move forward with surgery, Kristina sought out an alternative and ultimately found a physician in Connecticut willing to support a hospital-based vaginal breech birth. She describes the pressure she faced from both her care team and loved ones, the process of transferring care late in pregnancy, and the decisions she made to stay aligned with her instincts. This episode offers a detailed look at late breech diagnosis, informed refusal, and the realities of navigating hospital-based breech birth. Kristina also shares what she would do differently and what this experience taught her about trust, decision-making, and preparation.#257 | Labor & Delivery Nurses' Roundtable: How Their Hands are Tied to Doctors' OrdersSend us Fan Mail Cozyearth: Use code DTB for 20% offNeeded

We shifted away from our prepared movie review plans due to last minute circumstances preventing our Cinema/Canada Correspondant, Cole, from coming on the show today. Also, Josh slept extremely poorly the night before. We still managed to have a grand old time talking about the sick, sad, beautiful, lovely, stupid, fascinating world via this week's Newz avalanches. AI in medicine (and a CEO of a murderous health insurance corporation signals that they still fear Luigian justice). Trump in war and neurocognitive failure/demonic possession. Israel in pure evil. Septuagesima is the Church's merciful call to begin preparing for the Lenten journey, reminding believers that earthly life is a temporary exile and a pilgrimage back to the heavenly Jerusalem. Drawing on biblical symbolism of the number seventy and St. Gregory the Great's interpretation of the laborers in the vineyard, the message emphasizes that we live in the “eleventh hour,” yet are promised the same reward of eternal life by God's generosity rather than our merit. Pre-Lent is described as a motherly warning to simplify life, practice temperance, and prepare through fasting, prayer, repentance, forgiveness, and renewed spiritual discipline. Holding together sorrow for sin and hopeful joy in Christ's healing, the sermon urges the faithful to respond with humility and urgency, remembering that time is short and the Master still calls all into His vineyard of repentance and resurrection hope.
God surely has reasons for waiting to give you the breakthrough you have sought for decades. Keep asking in hope.

Salman Rushdie, longtime friend of 92NY's storied literary community, returns to our stage for a reading and conversation with Daniel Kehlmann about The Eleventh Hour — a supremely inventive new collection about survival, death, and what comes into focus at the proverbial eleventh hour of life. Awarded the Best of Booker for his seminal debut, Midnight's Children — honoring it as the most accomplished novel to ever receive the prestigious literary prize — Salman Rushdie has been probing the depths of identity, history, and mortality to stunning effect for 45 years. In The Eleventh Hour, he turns his extraordinary imagination to life's final act with a quintet of stories that span the three countries in which he has made his work — India, England, and America. Do we accommodate ourselves to death, or rail against it? Do we spend our "eleventh hour" in serenity or in rage? Spanning private tragedy, national calamity, political parable, and simmering mystery, Rushdie's new stories further cement him as one of the boldest writers of our time. Join the internationally renowned, award-winning author for an unforgettable evening of reading and conversation in celebration of survival, freedom of expression, his new collection, and his extraordinary contributions to world literature.

My guest is Dr. Sunita Puri, a palliative-care physician and author of That Good Night: Life and Medicine in the Eleventh Hour. We talk about what it really means to care for patients when cure is no longer the goal, why our medical system resists honest conversations about death, and how clarity and compassion can coexist at the end of life. Topics we cover: • What palliative care really provides (beyond hospice) • Why "more treatment" ≠ "more life" • Prognosis, probabilities, and telling the truth kindly • How families can ask the right questions • Documentation that matters (and what to avoid) • The moral distress of clinicians • Cultural/faith factors that shape decisions • Dignity, autonomy, and realistic hope Guest Bio: Dr. Sunita Puri is an Associate Professor of Medicine at the University of California, Irvine School of Medicine, where she is the Director of the Inpatient Palliative Care Service. She has written for The New Yorker, The New York Times, and The Atlantic, among other publications. She is the author of That Good Night: Life and Medicine in the Eleventh Hour, a critically acclaimed literary memoir examining her journey to the practice of palliative medicine, and her quest to help patients and families redefine what it means to live and die well in the face of serious illness.
