Die Transplantation gesunder Organe, insbesondere der Nieren, zur Therapie unheilbarer Erkrankungen stellt eine der bedeutendsten Errungenschaften der modernen Medizin dar. Die zur Vermeidung der Organabstoßung erforderliche langzeitige Immunsuppression bein-haltet jedoch auch ein gegenüber der Normalbevölkerung erhöhtes Risiko für die Entstehung maligner Erkrankungen, so vor allem von Plattenepithelkarzinomen der Haut und malignen Lymphomen. Auffällig ist weiterhin auch die vermehrte Aggressivität dieser Malignomerkran-kungen, die sich in rascher Progredienz und Metastasierung und damit verbundener erhöhter Letalität äußert. Als mögliche Ursache dieser Phänomene kommen eine erhöhte Mutagen-sensitivität der DNS, wie auch eine reduzierte Reparaturkapazität der DNS-Reparaturmechanismen durch die chronische Behandlung mit immunsuppressiven Medika-menten in Betracht. Ziel dieser Arbeit sollte es daher sein, anhand der etablierten Verfahren Comet Assay und Mikrokerntest, Aussagen über die Reparaturkapazität und die Genomstabilität bei nieren-transplantierten Patienten zu ermöglichen. Untersucht wurde ein Kollektiv von 40 nierentransplantierten Patienten, darunter 21 Männer und 19 Frauen, denen ein Kontrollkollektiv von ebenfalls 21 Männern und 19 Frauen zum Vergleich gegenübergestellt wurde. Als zu untersuchendes Zellmaterial dienten Lymphozy-ten des peripheren Blutes. Der Comet Assay wurde als quantitatives Verfahren zur Bestimmung fremdstoffinduzierter DNS-Schäden an einzelnen, nicht proliferierenden Zellen eingesetzt. Die Untersuchung von DNS-Reparaturereignissen über einen definierten Zeitraum wurde hierdurch ermöglicht. Nach Induktion eines DNS-Schadens durch Zugabe eines mutagenen Agens wurden über einen Zeitraum von 60 Minuten Reparaturereignisse an der DNS von Lymphozyten quantitativ erfasst. Der Mikrokerntest wurde als quantitatives Verfahren zur Untersuchung erhöhter Genomin-stabilität sowohl aufgrund individueller Disposition als auch als Folge der Einwirkung muta-gener Substanzen verwendet. Eine erhöhte Genominstabilität, wie sie in der Bildung von Mikrokernen zum Ausdruck kommt, kann ihre Ursache sowohl in einer erhöhten Mutagen-sensitivität als auch in einem Defizit der DNS-Reparaturmechanismen haben. In der vorliegenden Arbeit wurde der Mikrokerntest an unbehandelten Lymphozyten der Pa-tienten- und der Kontrollgruppe durchgeführt, um Aussagen über die spontane Brüchigkeit des Genoms in beiden Gruppen zu erlauben. Die Ergebnisse der mit der Methode des Comet Assays durchgeführten Lymphozytenrepara-turversuche zeigten bei Nierentransplantierten weder eine erhöhte Sensitivität gegenüber Mutagenen noch eine dem Normalkollektiv gegenüber reduzierte DNS-Reparaturkapazität. Vielmehr wurden beim Vergleich der Mediane der DNS-Migrationen mitunter signifikant ver-minderte Reparaturaktivitäten der Lymphozyten-DNS des Kontrollkollektivs beobachtet. Beim Vergleich der Mittelwerte dagegen ergab sich zu den meisten Versuchszeitpunkten kein sig-nifikanter Unterschied zwischen Nierentransplantierten und Kontrollpersonen. Bei der Durchführung des Mikrokerntests wurden bei Nierentransplantierten wie auch bei Kontrollpersonen keine signifikanten Unterschiede der Mikrokernfrequenzen verzeichnet. Bei den weiblichen Studienteilnehmern fanden sich insgesamt signifikant höhere Mikrokernraten als bei den männlichen, unabhängig davon, ob diese zum Patienten- oder Kontrollkollektiv gehörten. Somit konnte die Hypothese, dass bei Nierentransplantierten mit einer erhöhten Sensitivität der DNS gegenüber schädigenden Substanzen oder mit einer Reduktion der DNS-Reparaturfähigkeit zu rechnen sei, durch die Ergebnisse dieser Studie nicht gestützt werden. Da es sich bei der Karzinogenese jedoch um einen komplexen Prozess handelt, bei dem zahlreiche Faktoren beteiligt sind, kommt auch in Zukunft der Forschung nach möglichen Ursachen große Bedeutung zu. Nur durch Kenntnis der beteiligten Mechanismen und durch die Einschränkung der damit verbundenen Risiken ist ein verbessertes Langzeitüberleben organtransplantierter Patienten möglich.
