Yesterday Health Minister Mark Butler was forced to clarify that there will still be some patients who are charged a gap fee at the GP. Meanwhile at a state level, the LNP government are scrambling to redraw Queensland's hospital building plan after another scathing review. Australian Medical Association of Queensland President Dr Nick Yim told Luke Bradnam on 4BC Breakfast, "when the government did first make their announcement at the very start of the campaign... we've always said that it's quite unlikely that all GPs or 90 per cent of GPs will be bulk billing." "When we surveyed our members, they've told us that if they choose to universally bulk bill, that's actually going to be a loss of 20 to 30 per cent of their revenue." In this episode, Dr Choong-Siew Yong and Associate Professor Jeffrey Looi discuss the benefits and drawbacks of telepsychiatry and digital mental health delivery for general psychiatric care and specifically for children and adolescents. The pre- and post-pandemic approaches are discussed, including the emergency response for younger people. The conversation also examines the benefits and risks of telepsychiatry, including equitable digital technology access and technical challenges.  It also covers the use of telepsychiatry in rural and remote settings, and to allow videoconferencing when patients and their families are in different locations.  The discussion also explores new business models of telepsychiatry and the associated benefits and risks. Dr Choong-Siew Yong is a child and adolescent psychiatrist in Sydney, NSW. He has long-standing interests in medical workforce and education, practitioner regulation, doctors' health and mental health policy. He has served on the executive committees of the Australian Medical Association, NSW Medical Board and Medical Council, and the NSW Doctors' Health Advisory Service. For the RANZCP, he has served as the Chair of the Committee for Education Evaluation, Monitoring and Reporting and Continuing Medical Education (now known as the Committee for Continuing Professional Development). He is the current Chair of the Member Wellbeing Subcommittee. He talks about his interest in diet, how he discovered this link, his experience with patients, and, later, his role as an advocate in the public health space, the push back that inevitably comes from this and his overall passion and mission in the space.Dr James Muecke AM is an Adelaide-based ophthalmologist. He graduated with Honors from Adelaide University Medical School in 1988. Following his internship, James lived and worked as a volunteer doctor in Kenya in 1989. After completing ophthalmology training in Adelaide in 1995, James worked as an eye surgeon in Jerusalem for 12 months. He undertook subspecialty training in eye cancer in London and then returned to Adelaide in 1998, where he has been a Visiting Consultant and Senior Lecturer at Royal Adelaide and Women's & Children's Hospitals.James has taught the diagnosis and management of eye cancer in ten countries in Asia. He founded not-for-profit organization Sight For All in 2008, turning his boundless energy into a fight against blindness in the Aboriginal and mainstream communities of Australia and some of the poorest countries of the world. It's said that the world can be divided into shame based and guilt based cultures, with the west sitting firmly in the guilt side. This is because the west has prioritised this idea of the individual, with our sense of right and wrong a matter between ourself and whatever higher power we ascribe to – whether it's our god, the state laws, or our conscious itself. In the west, guilt is seen as an appropriate and even productive emotion, with shame as the corrosive cousin. But in shame based cultures, which really make up most of the world, its shame, not guilt, that does the heavy lifting. And given most of us in the west live in rich multicultural communities, it's important to understand shame if we're going to understand what motivates the people around us. In this conversation, we talk with a leading world expert about the deeply corrosive aspects of shame, and how to release its grip on us. We also speak with a leading psychiatrist who's written a book in defence of shame, to see what benefits shame can bring. We'll also open the lens a little wider, looking at how shame is used by conservatives, liberals, but more interestingly by the progressive left, which challenges us to be careful not to shame people's sexual expressions, weight, or in fact anything that's seen as an authentic version of oneself. Whilst at the same time, the left has very successfully used public shaming, including through cancel culture, as a way to regulate what can and can't be said.
