Clinical Challenges in Emergency General Surgery: C Diff - When to Pull the Operative Trigger

It's hospital day five. The patient looked better yesterday… but now she's hypotensive, on vasopressors, acidotic, and spiraling toward multi-organ failure. The CT scan doesn't show perforation or megacolon, but your gut tells you this is going south. Do you keep pushing medical therapy… or is it time to operate?Join Drs. Rushabh Dev, Jeffrey Coughenour, Kevin Bartow, Raymond Okeke, and Desra Fletcher from the Emergency General Surgery team in Tiger Country at Mizzou as they tackle one of the deadliest and most challenging diseases acute care surgeons face: fulminant Clostridioides difficile infection. In this Clinical Challenges episode, the panel discusses diagnostic stewardship, ASCRS recommendations, timing of operative intervention and technique, subtotal colectomy versus diverting loop ileostomy with lavage, and physiology that should push surgeons toward definitive source control. Through a real-world high-risk case vignette, the team explores the hardest question in emergency general surgery: when to stop hoping medical therapy will work and pull the operative trigger.Hosts Dr. Rushabh Dev FACS (Moderator, Surgical Attending) – Assistant Professor of Surgery, Associate PD ACS & SCCM Fellowship, SICU Medical Director, Lieutenant Commander United States Navy Reserve  Dr. Jeffery Coughenour FACS (Surgical Attending) – Professor of Surgery and Emergency Medicine, Trauma Medical Director at the University of Missouri SOM Dr. Kevin Bartow FACS (Surgical Attending) –Professor of Surgery, Minimally Invasive Surgeon and General Surgery. Department of General Surgery at the University of Missouri SOM Raymond Okeke – Acute Care Surgery/Surgical Critical Care Fellow, University of Missouri School of Medicine  Desra Fletcher – PGY 3 General Surgery Resident, University of Missouri School of Medicine  Learning ObjectivesBy the end of this episode, listeners should be able to: Define the spectrum of Clostridioides difficile infection (CDI), including non-severe, severe, and fulminant disease, and recognize the physiologic implications of fulminant colitis.  Review contemporary diagnostic stewardship for CDI, including appropriate stool testing, pitfalls of PCR/NAAT interpretation, and the role and limitations of CT imaging in fulminant disease.  Describe evidence-based medical management of fulminant CDI, including high-dose enteral vancomycin, intravenous metronidazole, rectal vancomycin for ileus, and principles of antimicrobial stewardship.  Recognize the high-risk clinical features that should prompt urgent surgical evaluation, including worsening shock, vasopressor dependence, lactate elevation, organ failure, and evolving abdominal exam findings.  Discuss the operative indications and timing for surgery in fulminant CDI and understand why delayed intervention contributes to mortality.  Compare subtotal colectomy with end ileostomy versus diverting loop ileostomy with antegrade lavage, including current evidence, patient selection, limitations of the literature, and ASCRS recommendations.  Review practical operative strategies for subtotal colectomy in unstable patients, including damage-control principles and common technical pitfalls.  Apply clinical reasoning to a complex, high-risk case of fulminant CDI in a patient with decompensated cirrhosis, septic shock, and multi-organ dysfunction.  References ASCRS Clinical Practice Guidelines for *Clostridioides difficile* Infection (2021)  Surgical Management of *Clostridium difficile* Colitis — Neal et al., 2011 (Loop Ileostomy + Lavage Protocol)  Clinical Practice Guidelines for *Clostridioides difficile* Infection in Adults and Children (IDSA/SHEA, 2021 Update)  Adjunctive Hyperbaric Oxygen and Surgical Outcomes in Necrotizing Soft Tissue Infections (Background discussion of severe infection physiology)  Total Abdominal Colectomy Versus Diverting Loop Ileostomy for Fulminant CDI — Systematic Review & Meta-analysis  Current Status of Surgical Therapy for Fulminant *Clostridioides difficile* Colitis  Behind the Knife Episode 648 – Emergency General Surgery Journal Review: *Clostridioides difficile* Infection  Bottom line: Fulminant C. diff is one of the few EGS diseases where the hardest decision is not what operation to perform — it's recognizing when medical therapy has failed before the patient becomes unsalvageable. Please visit https://behindtheknife.org to access other high-yield surgical education podcasts, videos and more.  If you liked this episode, check out our recent episodes here: https://behindtheknife.org/listenBehind the Knife Premium: https://behindtheknife.org/premiumOral Board Review: https://behindtheknife.org/oral-boardOral Board Simulator: https://behindtheknife.org/oral-board/simulatorGeneral Surgery Oral Board Review Course: https://behindtheknife.org/premium/general-surgery-oral-board-reviewTrauma Surgery Video Atlas: https://behindtheknife.org/premium/trauma-surgery-video-atlasDominate Surgery: A High-Yield Guide to Your Surgery Clerkship: https://behindtheknife.org/premium/dominate-surgery-a-high-yield-guide-to-your-surgery-clerkshipDominate Surgery for APPs: A High-Yield Guide to Your Surgery Rotation: https://behindtheknife.org/premium/dominate-surgery-for-apps-a-high-yield-guide-to-your-surgery-rotationVascular Surgery Oral Board Review Course: https://behindtheknife.org/premium/vascular-surgery-oral-board-reviewColorectal Surgery Oral Board Review Course: https://behindtheknife.org/premium/colorectal-surgery-oral-board-reviewSurgical Oncology Oral Board Review Course: https://behindtheknife.org/premium/surgical-oncology-oral-board-reviewCardiothoracic Oral Board Review Course: https://behindtheknife.org/premium/cardiothoracic-surgery-oral-board-reviewDownload our App:Apple App Store: https://apps.apple.com/us/app/behind-the-knife/id1672420049Android/Google Play: https://play.google.com/store/apps/details?id=com.btk.app&hl=en_US

Episode 406 of The VentureFizz Podcast features Bill Simmons, serial entrepreneur and Co-Founder of Orbit.me and DataXu. I'm going to use the cliché: Bill actually is a rocket scientist. His background is in aerospace engineering, he holds a PhD from MIT, and he worked on 13 space missions. In addition, he was part of a major Government competition for simulating options for space travel to Mars. His team simulated 35 billion possible options to generate 1,100 different Mars missions that were all feasible. This groundbreaking technology that leverage Big Data, which we now recognize as AI and machine learning, launched his first company, DataXu, in 2008. DataXu became a pioneer in the programmatic ad platform category and raised over $87M in funding. The company scaled as a major player in the Boston tech scene, and was acquired by Roku in 2019. Now, Bill is tackling a challenge we all likely face with his new startup, Orbit.me. Information is scattered across texts, multiple email inboxes, LinkedIn, WhatsApp, and social apps—it's impossible to keep track of what matters. Orbit.me is a perfect use case for AI, organizing your scattered messages into “Orbits” which are dedicated spaces built around the real contexts of your life, like parenting, work, or other important matters. Google's AI is poised to wipe out half of news publishers' traffic, but Barry Adams isn't here to panic—he's here to bring the hard truths (with a side of hope). On this week's EDGE of the Web, we dive deep into how Google's relentless AI advancements, from overviews to the new AI mode, threaten the very foundation of news publishing. Join host Erin Sparks and News SEO specialist Barry Adams as they break down the seismic shifts hitting news publishers: AI overviews stealing clicks, Top Stories boxes protecting (for now), and why AI mode is a technological hand grenade waiting to go off. Barry shares what's working, what's not, and why keeping up with Google's user experience data from the Chrome browser is harder than ever. We cover how publishers can future-proof themselves—think personalization, paywalls, and bundling—before Google fully flips the AI switch. Side quest: Ever wondered how to train your audience to pay for news in a world where everything's free (thanks, Google)? Barry makes a compelling case for why quality, unique journalism is more vital than ever—even if it means surviving on half your pie. In this episode of Gangland Wire, retired Kansas City Intelligence detective Gary Jenkins sits down with veteran Chicago journalist Chuck Goudie, whose decades of reporting have made him one of Chicago's most respected voices on organized crime. A fan of the show asked for more Chicago stories—and this conversation delivers. We dive into the legacy of the Spilotro family, sparked by the recent passing of John Spilotro, brother of the infamous Las Vegas mob figure Tony Spilotro. Chuck shares his reflections on how the Outfit has evolved, from its heyday of dominance in gambling, loansharking, and union racketeering to its much smaller—yet still persistent—presence today. Together, we revisit the Outfit's historic ties to the Teamsters, the Strawman trials, and the legendary names like Anthony Accardo who shaped Chicago's mob identity. Chuck solves a mystery and provides the name of the man who killed Sam Giancana. Chuck also offers personal insights into how mob families navigated the push and pull of blood ties, with some members rising into notoriety while others tried to lead straight lives under the shadow of organized crime. Our conversation shifts to Chuck's recent investigative work on the disappearance of Jimmy Hoffa, exploring the long-standing theories and mob connections that keep the story alive decades later. This episode blends history, reflection, and storytelling—offering both an inside look at Chicago's Outfit and a reminder of why these stories still captivate us today. 1:02 The Legacy of John Drummond 4:11 Current Status of the Outfit 7:28 The Last of the Spilotro Family 10:02 Family Dynamics of the Spilotros 13:18 Frank Calabrese's Las Vegas Fame 13:25 Giancana's Murder Investigation and who did it 18:18 Surveillance in the Giancana Case 22:03 The Straw Man Trials 25:40 Ken Eto's Gangland Story 27:52 Investigating Jimmy Hoffa's Disappearance 31:03 Closing Thoughts with Chuck Goudie Subscribe to Gangland Wire wherever you get your podcasts, and join us each week as we uncover the stories buried beneath the headlines—and the bodies. Good to be back here in the [0:02] studio of Gangland Wire. This is Gary Jenkins, retired Kansas City Police Intelligence Unit detective here in Kansas City. And, you know, guys, I have, I was talking with a fan not too long ago from Chicago, I think on the, maybe the Facebook group, and he said, you need to do more Chicago stories. And I had to admit, I hadn't done that many Chicago stories. I got caught up in New York a lot, It seemed like, and anyhow, we're back to Chicago and another guy's mentioned to another guy and we were talking and, and somebody said, I don't remember who, maybe that original fan said you need to get Chuck Goudie on there. He's been doing a lot of reporting on the outfit over the years. And I didn't really know who Chuck was. So I started searching. He did a recent story about the death of the last spilotro brother, John spilotro. So I thought, man, this is, this is it. This is what I got to do. So welcome, Chuck Goudie from Chicago. Well, it's quite an introduction. Some might call it a eulogy, but thankfully that's not what he does. [1:02] Really? Now, I think I told you earlier, you know, last time I interviewed a Chicago newsman, it was John Bulldog Drummond. Bulldog Drummond, I tell you what, he was the dean of Chicago newsmen, television newsmen, when it came to reporting on the mob. There's no doubt about it.

With jury selection beginning July 30th, 2025, we explore the challenges of finding impartial jurors in one of the most publicized cases in recent memory, despite the large Ada County jury pool. This episode analyzes the strengths and weaknesses each side brings to trial, from the prosecution's seemingly overwhelming DNA and digital evidence to the defense's constitutional challenges and alternate theories. We examine the key factors that will likely determine the trial's outcome, including the genetic genealogy evidence facing its first major courtroom test and the interpretation of complex digital forensics. The episode explores the lasting impact this case has already had on criminal justice, from the use of emerging forensic technologies to the challenges of ensuring fair trials in the social media age. We also reflect on the permanent changes to the University of Idaho and Moscow community, and consider how this case will influence law enforcement, legal education, and criminal procedure for years to come.
